      • Nonlinear finite element vibration analysis of functionally graded nanocomposite spherical shells reinforced with graphene platelets

        The main objective of this paper is to develop the finite element study on the nonlinear free vibration of functionally graded nanocomposite spherical shells reinforced with graphene platelets under the first-order shear deformation shell theory and von Kármán nonlinear kinematic relations. The governing equations are presented by introducing the full asymmetric nonlinear strain-displacement relations followed by the constitutive relations and energy functional. The extended Halpin-Tsai model is utilized to specify the overall Young's modulus of the nanocomposite. Then, the finite element formulation is derived and the quadrilateral 8-node shell element is implemented for finite element discretization. The nonlinear sets of dynamic equations are solved by the use of the harmonic balance technique and iterative method to find the nonlinear frequency response. Several numerical examples are represented to highlight the impact of involved factors on the large-amplitude vibration responses of nanocomposite spherical shells. One of the main findings is that for some geometrical and material parameters, the fundamental vibrational mode shape is asymmetric and the axisymmetric formulation cannot be appropriately employed to model the nonlinear dynamic behavior of nanocomposite spherical shells.

        Flexible CNC polishing process and surface integrity of blades

        The surface integrity of aero-engine blades directly affects its mechanical behavior. To improve the surface integrity and address the problem, that is, the blade is mainly polished manually, a flexible polishing process was proposed and surface integrity was investigated. First, flexible polishing technology and the machine tool that the authors researched and developed were introduced. Second, a polishing experiment was conducted on a TC4 titanium alloy blade. Finally, the surface integrity before and after polishing was measured and the results were compared and analyzed. Results showed that the knife marks, pits, unevenness, and other defects of the blade surface were effectively removed through flexible polishing; the surface residual compressive stress was reduced from 483 MPa to 397 MPa; the surface roughness was reduced to Ra 0.4 μm; the surface micro-hardness increased by 18 HV; and the surface metamorphic layer caused by milling was removed. Flexible polishing effectively improved the surface quality without affecting the depth layer quality of the workpiece.

        Stem-leaf saponins from Panax notoginseng counteract aberrant autophagy and apoptosis in hippocampal neurons of mice with cognitive impairment induced by sleep deprivation

        Backgroud: Sleep deprivation (SD) impairs learning and memory by inhibiting hippocampal functioning at molecular and cellular levels. Abnormal autophagy and apoptosis are closely associated with neurodegeneration in the central nervous system. This study is aimed to explore the alleviative effect and the underlying molecular mechanism of stem-leaf saponins of Panax notoginseng (SLSP) on the abnormal neuronal autophagy and apoptosis in hippocampus of mice with impaired learning and memory induced by SD. Methods: Mouse spatial learning and memory were assessed by Morris water maze test. Neuronal morphological changes were observed by Nissl staining. Autophagosome formation was examined by transmission electron microscopy, immunofluorescent staining, acridine orange staining, and transient transfection of the tf-LC3 plasmid. Apoptotic event was analyzed by flow cytometry after PI/annexin V staining. The expression or activation of autophagy and apoptosis-related proteins were detected by Western blotting assay. Results: SLSP was shown to improve the spatial learning and memory of mice after SD for 48 h, accomanied with restrained excessive autophage and apoptosis, whereas enhanced activation of phosphoinositide 3-kinase/protein kinase B/mammalian target of rapamycin signaling pathway in hippocampal neurons. Meanwhile, it improved the aberrant autophagy and apoptosis induced by rapamycin and re-activated phosphoinositide 3-kinase/Akt/mammalian target of rapamycin signaling transduction in HT-22 cells, a hippocampal neuronal cell line. Conclusion: SLSP could alleviate cognitive impairment induced by SD, which was achieved probably through suppressing the abnormal autophagy and apoptosis of hippocampal neurons. The findings may contribute to the clinical application of SLSP in the prevention or therapy of neurological disorders associated with SD.

      • Automatic Subretinal Fluid Segmentation of Retinal SD-OCT Images With Neurosensory Retinal Detachment Guided by Enface Fundus Imaging

        <P>Objective: Accurate segmentation of neurosensory retinal detachment (NRD) associated subretinal fluid in spectral domain optical coherence tomography (SD-OCT) is vital for the assessment of central serous chorioretinopathy (CSC). A novel two-stage segmentation algorithm was proposed, guided by Enface fundus imaging. Methods: In the first stage, Enface fundus image was segmented using thickness map prior to detecting the fluid-associated abnormalities with diffuse boundaries. In the second stage, the locations of the abnormalities were used to restrict the spatial extent of the fluid region, and a fuzzy level set method with a spatial smoothness constraint was applied to subretinal fluid segmentation in the SD-OCT scans. Results: Experimental results from 31 retinal SD-OCT volumes with CSC demonstrate that our method can achieve a true positive volume fraction (TPVF), false positive volume fraction (FPVF), and positive predicative value (PPV) of 94.3%, 0.97%, and 93.6%, respectively, for NRD regions. Our approach can also discriminate NRD-associated subretinal fluid from subretinal pigment epithelium fluid associated with pigment epithelial detachment with a TPVF, FPVF, and PPV of 93.8%, 0.40%, and 90.5%, respectively. Conclusion: We report a fully automatic method for the segmentation of subretinal fluid. Significance: Our method shows the potential to improve clinical therapy for CSC.</P>

        Development of a simple nomogram to estimate risk for intraoperative complications before partial nephrectomy based on the Mayo Adhesive Probability score combined with the RENAL nephrometry score

        Purpose: This study aimed to develop a simple nomogram based on the Mayo Adhesive Probability (MAP) score combined with the RENAL nephrometry score (RNS) to predict intraoperative complications before partial nephrectomy (PN) in Asian populations. Materials and Methods: This study retrospectively collected patients undergoing PN at three medical centers. Each component of the MAP score and the RNS (6 variables) was evaluated to assess its association with intraoperative complications by multivariable logistic regression with backward elimination. Results: A total of 46 cases (7.2%) with intraoperative complications were identified among 637 patients. After backward elimination, three variables, including tumor diameter (4–7 cm vs. ≤4 cm: odds ratio [OR], 4.339; 95% confidence interval [CI], 1.943–9,692; ≥7 cm vs. ≤4 cm: OR, 8.434; 95% CI, 1.225–58.090), nearness to the collecting system (4–7 mm vs. ≥7 mm: OR, 2.988; 95% CI, 1.293–6.907; ≤4 mm vs. ≥7 mm: OR, 21.394; 95% CI, 6.122–74.756), and perirenal fat stranding type (type 1 vs. no stranding: OR, 3.119; 95% CI, 1.079–9.017; type 2 vs. no stranding: OR, 18.722; 95% CI, 6.757–51.868), were retained. The predictive power (measured by area under the curve [AUC]) of the nomogram was observed to be superior to the RNS or MAP score alone (RNS: 0.686, MAP score: 0.729, the nomogram: 0.837), but comparable to their combination (0.813). Conclusions: The simple nomogram contains fewer components than the combination of the RNS and MAP scores yet demonstrates equivalent predictive power for intraoperative complications.

        The Importance of Allergen Avoidance in High Risk Infants and Sensitized Patients: A Meta-analysis Study

        Purpose: At this time, there is uncertainty regarding whether allergen avoidance is the most appropriate strategy for managing or preventing allergies. The purpose of this study was to evaluate the effectiveness of allergen avoidance in the prevention of allergic symptoms in previously sensitized patients and newborns that have the potential to develop allergies. Methods: We performed online searches of articles published from January 1980 to December 2012 in PubMed and The Cochrane Central Register of Controlled Trials, and selected articles involving randomized controlled trials (RCTs) and allergen avoidance. The parameters used to determine allergenic potential in newborns included the risk ratio (RR) of eczema, asthma, rhinitis, wheeze, and cough. The methods employed to evaluate previously sensitized patients were the standardized mean difference (SMD) of forced expiratory volume in 1 second (FEV1) and peak expiratory flow rate (PEFR). Data quality was assessed using the Jadad scale. Results: A total of 14 RCTs were identified. Meta-analysis demonstrated that allergen avoidance for newborns did not reduce the subsequent incidence of allergic diseases (eczema, P=0.21; rhinitis, P=0.3; cough, P=0.1) but significantly reduced the incidence of asthma and wheezing in high-risk infants (asthma, P=0.03; wheeze, P=0.0004). However, previously sensitized patients who reduced their exposure to known allergens did not show improvement in their lung functions (FEV1, P=0.3; PEFR morning, P=0.53; PEFR evening, P= 0.2; PEFR, P=0.29). Conclusions: Allergen avoidance may not always be successful in preventing allergic symptoms. However, rigorous methodological studies are required to confirm this hypothesis.

        Compound heterozygous mutations of NDUFV1 identified in a child with mitochondrial complex I deficiency

        Background: Mitochondrial complex I deficiency (MCID) is the most common biochemical defect identified in childhood with mitochondrial diseases, mainly including Leigh syndrome, encephalopathy, macrocephaly with progressive leukodystrophy, hypertrophic cardiomyopathy and myopathy. Objective: To identify genetic cause in a patient with early onset autosomal recessive MCID. Methods: Trio whole-exome sequencing was performed and phenotype-related data analyses were conducted. All candidate mutations were confirmed by Sanger sequencing. Results: Here we report a child of Leigh syndrome presented with global developmental delay, progressive muscular hypotonia and myocardial damage. A missense mutation c.118C > T (p.Arg40Trp) and a previously reported mutation c.1157G > A (p.Arg386His) in NDUFV1 have been identified as compound heterozygous in the patient. The mutation p.Arg386His is closely associated with the impairment of 4Fe-4S domain and this mutation has been reported pathogenic. The c.118C > T mutation has not been reported in ClinVar and HGMD database. In silico protein analyses showed that p.Arg40 is highly conserved in a wide range of species, and the amino acid substitution p.Trp40 largely decreases the stability of NDUFV1. In addition, the mutation has not been detected in the Asian populations and it was predicted to be deleterious by numerous prediction tools. Conclusion: This research expands the mutation spectrum of NDUFV1 and substantially provides an early and accurate diagnosis basis of MCID, which would benefit subsequently effective genetic counseling and prenatal diagnosis for future reproduction of the family.
