소장내 위석에 의한 장폐쇄증으로 발생한 급성신부전 1례

Gastric bezoars are concretions or hard masses of foreign matter found in the gastrointestinal tract. It may induce many complications including intestinal obstruction. No report has been proposed on a case of prerenal ARF induced by intestinal obstruction by small-bowel bezoar. Therefore, we report a case of the patient with ARF associated with small-bowel bezoar, who initially visited with the symptoms of nausea and frequent vomiting. 위석(bezoar)은 여러 가지 이물 및 내재성물질이 위장관내에서 혼합되어 응고물을 형성한 것을 말하며, 가장 흔한 형태의 합병증으로 장폐쇄증이 올 수 있으나, 그로 인하여 신부전의 양상으로 나타나는 경우는 흔하지 않다. 이에 저자들은 내원 당시의 병력 및 이학적 검사만으로 장폐색의 원인이 위석임을 의심할 수 없었던 비전형적인 소견을 보인 소장내의 위석에 의한 장폐쇄증과 이로 인해 발생한 급성신부전의 예를 경험하였기에 이에 보고하는 바이다.
현성 당뇨병이 없었던 알코올 간경화증 환자에서 발생한 공기증신우신장염 1례

공기증신우신장염은 신실질의 괴사와 자발적인 공기 형성과 연관된 드문 감염질환이다. 대부분 혈당이 잘 조절되지 않는 당뇨 환자들에게서 발생하며 드물게 요로 폐색, 요로 결석, 종양 등에서도 발생한다. 과거 초기의 수술적 치료가 우선시 되었으나 최근에는 항생제 및 조기 진단으로 인해 내과적 치료만으로 성공한 사례가 증가하고 있다. 저자들은 현성 당뇨병이 없었던 알코올 간경화증 환자에서 발생한 공기증신우신장염을 내과적 치료 및 경피적 배농술만으로 치료한 예가 있어 보고하는 바이다. Emphysematous pyelonephritis is an uncommon infection of the kidney associated with parenchymal necrosis and spontaneous gas formation. Most cases occur in diabetic patients and rarely in urinary tract obstruction, stone or tumor. At past, surgical treatment was prior to internal treatment, but recently, successful cases of internal treatment have increased because of the development of antibiotics and early diagnosis. We report an emphysematous pyelonephrtis in 61 years-old non-overt diabetic woman with alcoholic liver cirrhosis, successfully treated by antibiotics and percutaneous drainage.
Genetic polymorphisms of drug-metabolizing enzymes and anti-TB drug-induced hepatitis

<P>AIMS: Although some genetic risk factors have been reported for the development of hepatitis due to anti-TB drugs, an extensive candidate gene approach evaluating drug-metabolizing enzymes has not been attempted. This study aimed to investigate the association of genetic polymorphisms in drug-metabolizing enzymes with anti-TB drug-induced hepatitis. MATERIALS & METHODS: We compared genotype distributions of tagging SNPs in promoter, exons and haplotypes in seven drug-metabolizing enzyme genes (CYP2C9, CYP2C19, CYP2D6, CYP2E1, NAT2, UGT1A1 and UGT1A3) between 67 cases and 159 controls. RESULTS: Among four tagging SNPs of N-acetyltransferase 2 (NAT2), -9796T>A in promoter and R197Q were significantly associated (p = 0.0016 and p = 0.0007, respectively). NAT2 haplotype 2 [A-A-A-G] carrying A allele of -9796T>A and A allele of R197Q showed significant association (p = 0.0004). However, there was no significant association between genotypes of other enzyme-metabolizing genes and anti-TB drug-induced hepatitis. The constructs containing -9796A of NAT2 showed significantly lower luciferase activity (p < 0.01), suggesting decreased expression of NAT2. The variant alleles and haplotype 2 showed significantly higher peak serum levels of isoniazid, lower acetyl isoniazid:isoniazid ratio and lower isoniazid clearance compared with wild-types. CONCLUSION: These findings suggest that genetic variants in the promoter and exons of NAT2 increase the risk of anti-TB drug-induced hepatitis by modifying acetylation phenotypes and/or gene expression of NAT2, and there is no essential role for genetic mutation of the other metabolizing enzymes in the development of this adverse reaction.</P>

Liver transplantation with preservation of the recipient vena cava (piggyback technique) has been performed as an alternative to the conventional method. Outflow disturbance or obstruction of the vena cava in the early period after liver transplantation is associated with high morbidity and mortality. We used side-to-side cavo-caval anastomosis (modified piggyback technique) in a deceased-donor liver transplantation (DDLT) for venous outflow reconstruction. On postoperative day 9, the patient developed abdominal discomfort, and abnormal liver function showing serum total bilirubin of 6.2 mg/dl and serum AST/ALT of 297/597 IU/L. Doppler ultrasound showed mono-phasic wave forms of the hepatic vein. Computed tomography showed focal narrowing of 9.5 mm±2 mm in diameter at the cavo-caval anastomosis site. Liver biopsy was showed that there was no evidence of acute allograft rejection. Direct venogram showed stenosis of the cavo-caval anastomosis with a pressure gradient of 12 mmHg. An interventional stent was inserted in the stenotic site of the inferior vena cava, and the pressure gradient decreased to 2 mmHg. He was discharged from hospital on postoperative day 23 without any other complications. Herein we report a case of deceased-donor liver transplantation using the modified piggyback technique, who received an inferior vena cava stent due to stricture of the reconstructed orifice of the vena cava. The marine environmental characteristics of the vicinity of Nampo sea area were investigated in order to assess the environmental variation and the ecological influences by the project of reclaimation from February to May, 1986. At flood tide, the water coming from the southwest of Kabampo turned to counter-clockwise and flowed to the in front of Kapbawi with the speed of 0.4-1.2 knot. Whileas, at ebb tide, the current turned to the clockwise in front of Kapbawi and then moved to the off sea with 0.4-1.3 knot. Although the levels of nutrient salts were relatively low, but the SS level was higher compared with that of other areas due to the reclaimation construction. As a whole, the procuctivity of this area is poor state compared with other coastal areas.
Abdominal compartment syndrome caused by a bulimic attack in a bulimia nervosa patient

We present a rare case of abdominal compartment syndrome due to a bulimic attack in a 19-year-old female patient with bulimia nervosa. She was admitted to our emergency room with complaints of progressive abdominal pain following bulimia. Computed tomography showed dilated stomach with food and air pressed other visceral organs and major abdominal vessels. Decompression using nasogastric tube or gastric lavage tube failed. At laparotomy, we performed gastrotomy and decompression was performed. After decompression, she fell into hypovolemic shock due to bleeding in the intra-gastric and peritoneal cavity. Twelve hours after the operation, the patient died due to refractory hypovolemic shock from uncontrolled bleeding following decompression of abdominal compartment. It should keep in mind that binge-eating habits in patients with bulimic nervosa could cause abdominal compartment syndrome due to gastric distension and this may be a potentially fatal condition.

Backgrounds Propylparaben is one of the most commonly used preservatives in pharmaceuticals, food items, and personal care products. However, growing evidence demonstrates that propylparaben has a harmful effect on various human tissues. In this study, we investigated the cytotoxic effects of propylparaben on the human placental cell line, BeWo. Objective We investigated whether propylparaben caused placental cell death using relevant cytotoxic endpoints, including cell viability, proliferation, cell cycle, and apoptosis, in BeWo cells. However, instead of using conventional methods of protein analysis from pooled samples, we precisely quantified protein expression at the single cell level by flow cytometric analysis. Results Propylparaben significantly reduced cell viability in a dose-dependent manner. It induced cell cycle arrest at the sub-G1 phase by reducing the expression levels of cyclin D1, whereas the cell population at the G0/G1 and S-phases was decreased. Furthermore, propylparaben induced apoptosis by enhancing the activity of caspase-3. Conclusion Collectively, our results suggest that propylparaben causes cytotoxic effects in human placental BeWo cells by inducing cell cycle arrest and apoptosis.

This paper proposes a real time lane detection algorithm using LRF (Laser Range Finder) for autonomous navigation of a mobile robot. There are many technologies for safety of the vehicles such as airbags, ABS, EPS etc. The real time lane detection is a fundamental requirement for an automobile system that utilizes outside information of automobiles. Representative methods of lane recognition are vision-based and LRF-based systems. By the vision-based system, recognition of environment for three dimensional space becomes excellent only in good conditions for capturing images. However there are so many unexpected barriers such as bad illumination, occlusions, and vibrations that the vision cannot be used for satisfying the fundamental requirement. In this paper, we introduce a three dimensional lane detection algorithm using LRF, which is very robust against the illumination. For the three dimensional lane detections, the laser reflection difference between the asphalt and lane according to the color and distance has been utilized with the extraction of feature points. Also a stable tracking algorithm is introduced empirically in this research. The performance of the proposed algorithm of lane detection and tracking has been verified through the real experiments.
