      • 분편 인플루엔자백신(split influenza vaccine)의 임상효과 및 면역원성에 관한 연구

        목적 : 아단위 인플루엔자 백신 접종 후 백신의 인플루엔자 예방효과, 인플루엔자 방어 항체형성, 인플루엔자 백신의 안전성을 조사하고자 하였다. 방법 : 총 571명을 대상으로 인플루엔자 백신 접종을 하였고, 이들 접종자에서 인플루엔자 양질환의 이환을 조사하여 인플루엔자 백신의 인플루엔자예방효과를 알아보았고, 백신의 접종 전 및 접종 4주 후 혈청에서 혈구응집억제물(Hemagglutination Inhibition : HAI) 항체 검사를 실시하여 백신의 방어항체생성을 조사하였고, 백신의 안전성을 알아보기 위해 백신접종 후 1주일 이내의 부작용을 조사하였다. 결과 :백신 접종군과 백신 비접종군에서 인플루엔자 양 질환의 이환을 조사한 결과 접종군 28.35%, 비접종군 35.88%으로 나왔으며, p 값이 0.001로 통계적으로 접종군에서 유의하게 낮았고, 인플루엔자 양 질환의 예방 효과는 20.97%를 보였다. 백신의 방어항체 형성의 평가를 위해 유럽의 인플루엔자 백신 허가 기준을 조사하였는데 B/Guangdong/5/94균주의 백신접종 후 항체가 40이상의 비율을 제외한 다른 기준은 모두 만족 시켰다. 부작용은 전체 조사자 521명중 149명(29%)으로 주로 접종 부위의 국소 부작용을 호소했고, 전신 부작용은 2% 내외였으며 특별히 심각한 부작용은 발견되지 않았다. 결론 : 분편 인플루엔자 백신은 인플루엔자양질환의 예방과 방어항체생성에 효과 있으며 안전한 것으로 사료된다. Background : The safety and effectiveness of influenza vaccine are well known in developed country. The influenza vaccination has been recommended as one of the tentative immunization schedule for indicated persons since 1997 in Korea. But there are still no available data about them, even though nearly 5 million doses of influenza vaccine were used in 1997-1998 season. So it is immediately needed to investigate the safety. efficacy and immunogenicity of influenza vaccine among Korean. Methods : We studied the clinical efficacy of influenza vaccine by monitoring Occurrence of influenza-like illness in influenza risk group(vaccination ; 300, non-vaccination; 215) from December in 1997 to March in 1998. We used the split quadrivalent influenza vaccine containing 15 microgram of hemagglutinin of A/Beijing/262/95(HlNl), A/Wuhan/359/95(H3N2), B/Mie/1/93 and B/Guangdong/5/94. Hemagglutination inhibition(HA1) antibody titers were determined before immunization and 1 months after vaccination And we evaluated adverse effect of influenza vaccination at 7 days after vaccination. Results : Influenza vaccination was associated with si@icant reductions in influenza-like spptoms(vaccination group; 28.35%, non-vaccination group, 35.88%, p=0.001). The preventive effect of influenza-like i3lne.s among influenza risk goup was 20.97%. And immunogenicity of influenza A and B exceeded all of the European licensure criteria for immunogenicity except postvaccination proportion of titers 240 of B/Guangdong/5/94 strain. And the adverse effects were mainly local injection site problem and no serious adverse effect was noted. Conclusion : Split influenza vaccine is safe, inmunogenic and eff'tive in influenza risk group in Korea.

        좌골신경손상 흰쥐의 가자미근과 내측비복근 위축에 대한 전기자극 효과

        This experimental study includes the effect of intermittent electrical stimulation of the muscle on the atrophic response of peripherally denervated muscles. Rats(Sprague -Dawley, 240-260gm) were subjected to a severance of the sciatic nerve and mechanical properties of contraction including contraction time, half relaxation time and maximum twitch tension, maximum tetanic tension and fatigue index were observed on the slow(soleus m.) and fast(medial gastrocnemius m.) muscles 10days after serverance of the sciatic nerve. Experimental groups were divided into three groups : control group(CONT) immediately after severance of sciatic nerve, denervation group(DN) 10days after severance of sciatic nerve, and denervation plus eletrical stimulation group : (DN-ES) denervation combined with intermittent eletrical stimulation of muscles directly. Optimal frequency of electrical stimulation was 20㎐ for soleus muscle and 40㎐ for medial gastrocnemius muscle under 0.5ms of pulse width and supramaximal intensity. In DN group, muscle weight and mechanincal properties of contraction were significantly decreased compared with the control group. In DN-ES group, however, muscle weight and mechanincal properties were significantly increased compared with the denervation group. These data indicate that intermittent electrical stimulation of muscles may be useful in decreasing the muscle atrophy in peripherally denervated muscles.

      • Clinico-ultrasonographic characteristics of non-vascular skin tumors misdiagnosed as vascular tumors by ultrasonography

        Background: Ultrasonography is a very useful tool in the diagnosis of various dermatoses. Especially, vascular structure in ultrasonographic imaging is helpful for differential diagnosis of vascular and non-vascular skin tumors. Sometimes, however, there are some cases of non-vascular skin tumors confused as vascular tumor by ultrasonography. Objectives: The purpose of this study was to investigate clinico-ultrasonographic characteristics of non-vascular skin tumors misdiagnosed as vascular tumors by ultrasonography. Methods: We retrospectively reviewed medical records, ultrasonopgraphic findings and clinical photos of patients who underwent ultrasonography in the department of dermatology at Pusan National University (Pusan and Yangsan) from 2010 to 2019. Results: A total of 33 cases of non-vascular skin tumors misdiagnosed as vascular tumor by ultrasonography were obtained. The mean age of patients was 32.9 years old, and 44% of patients were male. The most common histopathologically confirmed diagnosis was pilomatricoma, followed by epidermal cyst. Hemangioma was the most common vascular tumor described by ultrasonography by radiologists. Only 9% of all cases were clinically suspected with vascular tumor as the first impression by dermatologists. Conclusion: This study shows various clinico-ultrasonographic findings of non-vascular skin tumors misdiagnosed as vascular tumor by ultrasonography. Our results could be useful for dermatologists who perform ultrasonography directly.

      • Periorificial dermatitis in children and adults: retrospective analysis

        Background: Periorificial dermatitis (POD) is an acneiform or rosacea-like eruption found in both children and adults. Although POD is not a rare disease, its clinical characteristics and course have not been investigated well, especially in children. Objectives: This study aimed to investigate the clinical difference between childhood and adult PODs. Methods: We retrospectively reviewed medical records and clinical photos of 131 patients diagnosed with POD in the Pusan National University Hospital (Busan and Yangsan) for 17 years (2003-2019). Results: Among 131 patients, 23 (17.6%) were children and 108 (82.4%) were adults. The mean age was 9.0 (2.4-17) years and 43.9 (19-79) years, respectively. The female to male ratio was higher in adult POD (2.6:1) than childhood POD (0.9:1). The involvement of periocular area was more frequent in childhood POD (47.8%) than adult POD (17.6%), however involvement rate of perinasal, perioral and extrafacial area were not different. When we compared perioral dermatitis severity index (PODSI), it is higher in adults (4.3) than in children (3.2). Clinical courses in according to age, sex, treatment, and severity were not different in both groups. Conclusion: Though the sample size of childhood POD is small, this study identified clinical difference such as sex ratio, location and severity between childhood POD and adult POD.

      • Eosinophilic fasciitis in a patient with polymyositis

        Eosinophilic fasciitis (EF) is a rare connective tissue disease characterized by erythema and painful edema on extremities, followed by induration of the skin, with laboratory findings of peripheral eosinophilia, hypergammaglobulinemia. Histopathological findings in EF include increased thickness of the fascia with infiltration of lymphocytes, plasma cells, mast cells, eosinophils. The differential diagnosis includes scleroderma, morphea and any panniculitis. A 34-year-old man presented with stiffness of upper extremities for five months. The patient also complained of lesional pain and decreased mobility of the wrist and elbow joint that had occurred a month later. Left forearm MRI showed diffuse fascial T2 hyperintensity with enhancement along the deep and intermuscular fascia. Histopathologic findings revealed thickened fascia with infiltration of eosinophils, lymphocytes and plasma cells, and also showed myopathic change. The patient was diagnosed as EF with polymyositis, based on the clinical characteristics and histopathologic findings. Lesional pain and joint mobility was gradually improved after 10 months of systemic steroid and immunosuppressant. To our knowledge, EF is a rare disease and only 4 cases have been reported in Korean dermatology literature. Moreover, the case of EF with polymyositis has not been reported in Korea before. So we herein report a rare case of EF in a patient with polymyositis.

      • Pharmacological target therapy of neuropathic pain and patient-reported outcomes in patients with chronic low back pain in Korea : Results from the NLBP Outcomes Research

        <P><B>Abstract</B></P><P>A number of studies have demonstrated an association of neuropathic pain and chronic low back pain (CLBP), but the outcome difference in each medical management is poorly understood. This study is aimed to investigate treatment patterns of neuropathic pain in CLBP patients and to explore patient-reported outcomes (PROs) including quality of life (QoL) and functional disability by treatment patterns.</P><P>Data were extracted from the neuropathic low back pain (NLBP) outcomes research. It was a multicenter and cross-sectional study in which 1200 patients were enrolled at 27 general hospitals, from 2014 to 2015. Of total, 478 patients classified as neuropathic pain were used for this subgroup analysis. The patients were divided into 2 groups according to treatment patterns (with vs. without the targeted therapy [TT] of neuropathic pain). Demographic and clinical features were collected by chart reviews and PROs were measured by patient's survey. QoL was assessed by EuroQoL 5-dimension (EQ-5D) questionnaire. Functional disability was measured by the Quebec Back Pain Disability Scale (QBPDS). Multiple linear regression analyses were conducted to compare the PROs between TT group and non-targeted therapy (nTT) group.</P><P>Among the NLBP patients (mean age 63years, female 62%), EQ-5D index, EuroQoL-Visual Analog Scale (EQ-VAS), and QBPDS Scores (mean ± standard deviation) were 0.40 ± 0.28, 54.98 ± 19.98, and 46.03 ± 21.24, respectively. Only 142 (29.7%) patients had pharmacological TT of neuropathic pain. Univariate analyses revealed no significant mean differences between TT group and nTT group in the EQ-5D index (0.41 ± 0.27 and 0.39 ± 0.28), EQ-VAS (56.43 ± 18.17 and 54.37 ± 20.69), and QBPDS (45.31 ± 21.32 and 46.31 ± 21.24). After adjustment with covariates, TT group had higher scores of EQ-5D index (β = 0.07; <I>P</I> < 0.01) and EQ-VAS (β = 4.59; <I>P</I> < 0.05) than the nTT group. The TT group's QBPDS score was lower than the nTT group, although its statistical significance still has not been reached (β = −4.13; <I>P</I> = 0.07).</P><P>We found that considerable proportion of the NLBP patients remains untreated or undertreated. Although TT group had significantly better QoL than nTT group, only 29.7% of NLBP patients had pharmacological TT. Therefore, clinicians should consider using TT for better QoL of neuropathic pain patients.</P>

        흉선에 발생한 신경내분비암(neuroendocrine carcinoma) 1 예

        Neuroendocrine tumors originate from neuroendocrine cell, so called APUD(amine precursor uptake and decarboxylation). Most neuroendocrine tumors have typical histopathology, immunohistochemical findings, and can be diagnosed by specific electromicroscopic feature of dense core granules. Neuroendocrine tumors are a diverse group of neoplasms that include carcinoid tumors, islet cell tumors, neuroblastoma, and small cell carcinoma. Neuroendocrine carcinoma of thymus bears similarities to neuroendocrine carcinoma in other organs, but it is clinicopathologically distinct from other tumors of thymus. Rare reports have been seen about thymus neuroendocrine carcinoma. Authors experienced a case of neuroendocrine carcinoma of thymus which cannot be classified as carcinoid, atypical carcinoid, or small cell carcinoma. Herein, we report this case with a review of the literatures.

        High-Resolution Synthesized Magnetic Field Focusing for RF Barcode Applications

        <P>A prototype of a high-resolution synthesized magnetic field focusing (SMF) system for a possible application to radio frequency (RF) barcodes is proposed in this paper. It was recently found that coordinated control of array coil currents makes it possible to focus magnetic field, which is requisite for medical imaging, wireless power, and RF identification. Different from conventional phased array RF antennas, SMF is independent of frequency, which is quite promising for various applications. In this paper, an array of rectangular air coils is proposed, and various implementation issues, such as accurate control of coil currents and compact RF barcode design, are newly suggested. A prototype of an SMF system composed of a transmitter (Tx), an RF barcode, and a receiver (Rx) was implemented at below 1 MHz operating frequency. The Tx includes modularized control drivers, full-bridge switching converters, and 16 arrayed air coils, where the RF barcodes proposed in this paper include a self-resonating circuit at 20 mm × 20 mm and a tunable resonating circuit at 15 mm × 15 mm. In this way, a 5 mm resolution at a distance of 20 mm was achieved for the Tx arrayed air coils with spacing of 5 mm, which is about four times sharper magnetic field focusing compared to a nonfocusing conventional coil. Furthermore, the proposed SMF system allows the designed 5-bit RF barcode array to be fully distinguishable at a 15 mm distance.</P>
