Serial femtosecond X-ray crystallography (SFX) has revolutionized atomic-resolution structural investigation by expanding applicability to micrometer-sized protein crystals, even at room temperature, and by enabling dynamics studies. However, reliable crystal-carrying media for SFX are lacking. Here we introduce a grease-matrix carrier for protein microcrystals and obtain the structures of lysozyme, glucose isomerase, thaumatin and fatty acid–binding protein type 3 under ambient conditions at a resolution of or finer than 2 Å.

<P><B>Abstract</B></P><P>Climate change will likely change the species composition or abundance of plant communities, and it is important to anticipate these changes to develop climate change adaptation policies. We chose beech (<I>Fagus crenata</I> Blume) and its competitive tree species as target species to evaluate potential turnover in forest types under climate change using a multivariate classification tree model. To construct the model, geographical presence/absence data for nine target species were used as multivariate response variables, with five climatic factors were used as predictor variables. Current and future distribution probabilities for the target species were calculated, and the 15 dominant forest types were subjectively classified in approximately 1‐km<SUP>2</SUP> grid cells within the area of the current beech forest distribution. All 16,398 grid cells of the beech‐dominant forest type (FCR‐QCR) were projected to be replaced in the future by five <I>Quercus crispula</I>‐dominant types (59% of FCR‐QCR grid cells), four <I>Q. serrata</I> types (22%), two <I>Q. salicina</I> types (8%), or two <I>Abies firma</I> types (0.1%). The FCR‐QCR type remained unchanged (stable) in only 11.4% of grid cells; these were mainly distributed at high elevations in snowy areas on the Sea of Japan side of the country. In contrast, vulnerable habitats (future probability of beech occurrence less than 1.0%) were found at low elevations on both the Sea of Japan and the Pacific Ocean sides. Northwards or upwards range expansions or increases of <I>Quercus</I> spp., in particular, need to be carefully monitored.</P>

Background/Aims: It is sometimes difficult to distinguish between malignant and benign biliary strictures using imaging studiesalone, and pathological diagnosis is necessary. The aim of this study was to determine the usefulness of endoscopic transpapillary tissuesampling and factors predictive of diagnostic accuracy. Methods: From April 2008 to December 2014, 136 patients underwent endoscopic transpapillary tissue sampling for malignantbiliary strictures. The cytological and histological findings were reported as negative, suspicious, or positive. Suspicious and positivefindings were defined as pathologically positive. Results: The sensitivity was 65.0% for forceps biopsy, 49.5% for brush cytology, 46.2% for bile aspiration cytology, and 21.9% forendoscopic nasobiliary drainage cytology. The combination of these procedures improved the sensitivity (72.8%). Endoscopictranspapillary tissue sampling was more sensitive for lesions of biliary origin (91.4%) than for extrabiliary lesions (66.3%). In surgicalcases, the sensitivity for tumors with an infiltrative growth pattern (53.3%) was significantly lower than for a tumor with an expandingor intermediate growth pattern (87.5%). Conclusions: Combining procedures can improve diagnostic accuracy. It may be possible to predict the sensitivity of endoscopictranspapillary tissue sampling by evaluating the etiology and tumor growth pattern using preoperative imaging studies.

Background/Aims: The aim of this study was to evaluate the safety of sedation with propofol as an alternative to benzodiazepinedrugs in outpatient endoscopy. Methods: In this prospective study, examinees who underwent outpatient endoscopy under propofol sedation and submitted a nextdayquestionnaire with providing informed consent were evaluated. Periprocedural acute responses, late adverse events within 24hours, and examinee satisfaction were evaluated. Results: Among the 4,122 patients who received propofol in the 17,978 outpatient-based endoscopic examinations performedbetween November 2016 and March 2018, 2,305 eligible examinees (esophagogastroduodenoscopy for 1,340, endoscopicultrasonography for 945, and total colonoscopy for 20) were enrolled, and their responses to a questionnaire were analyzed. Themean propofol dose was 69.6±24.4 mg (range, 20–200 mg). Diazepam, midazolam, and/or pentazocine in combination withpropofol was administered to 146 examinees. Mild oxygen desaturation was observed in 59 examinees (2.6%); and mild bradycardia,in 2 (0.09%). Other severe reactions or late events did not occur. After eliminating 181 invalid responses, 97.7% (2,065/2,124) of thepatients desired propofol sedation in future examinations. Conclusions: Propofol sedation was found to be safe-without severe adverse events or accidents-for outpatient endoscopy on thebasis of the patients’ next-day self-evaluation. Given the high satisfaction level, propofol sedation might be an ideal tool for painlessendoscopic screening.

Left-sided gallbladder is a rare finding that is mostly discovered incidentally during surgery and is often associated with anatomic anomalies. We herein report a case in which laparoscopic cholecystectomy and common bile duct exploration were achieved for an 89-year-old female patient with left-sided gallbladder. Surgery was carried out using our usual trocar position. Calot triangle was covered by the body of the gallbladder and could not be detected. We dissected the gallbladder from the fundus towards the neck. The cystic duct joined the common bile duct from the right side, and common bile duct exploration was performed routinely without perioperative comorbidities. Although the preoperative diagnosis rate is low and the risk of intraoperative bile duct injuries in patients with left-sided gallbladder is high, laparoscopic cholecystectomy and common bile duct exploration can be safely performed by understanding the location and bifurcation of the cystic duct.

Afferent loop syndrome is often difficult to resolve. Among patients with afferent loop syndrome whose data were extracted fromdatabases, 5 patients in whom metal stent placement was attempted were included and evaluated in this study. The procedure wastechnically successful without any adverse events in all patients. Metal stent(s) was placed with an endoscope in the through-the-scopemanner in 4 patients and via a percutaneous route in 1 patient. Obvious clinical efficacy was observed in all patients. Adverse eventsrelated to the procedure and stent occlusion during the follow-up period were not observed. Metal stent placement for malignantobstruction of the afferent loop was found to be safe and feasible.
Inflammation Enhanced X-irradiation-Induced Colonic Tumorigenesis in the Min mouse

Inflammation is potential risk factor of various human malignancies. Inflammatory bowel syndromes such as ulcerative colitis are well known as risk factors for colon cancer. Here, we examined enhancing effects of dextran sulfate sodium (DSS)-associated inflammation on X-irradiation induced colonic tumorigenesis in Min and wild-type (WT) mice. Animals were X-irradiated at 1.5 Gy at 5 weeks of age (at 0 experimental week) and 2% DSS in drinking water was administered at 5 or 11 experimental weeks. Mice were sacrificed at 16 weeks and incidence and multiplicity of colonic tumors were assessed. Incidence of colonic tumors in Min mouse was increased from 33.3% to 100% (p<0.05) with X-irradiation alone, whereas no tumors were developed in WT mice. In DSS-treated Min mice, X-irradiation increased the number of colonic tumors. Total number of colonic tumors was increased 1.57 times to $30.7{\pm}3.83$ tumors/mouse with X-irradiation+DSS at 5 weeks comapared to $19.6{\pm}2.9$ in corresponding DSS alone group (p<0.05). When the duration of inflammation was compared, longer period of DSS effect promoted more colonic tumorigenesis. Collectively, we conclude that X-irradiation and DSS-induced inflammation act synergistically for colonic tumorigenesis.
