        This is a brief case report of a narcolepsy. The case is a 26-year old married male. the patient was afflicted with a sort of epidemic encephalitis at the age of 11. The authors were able to control his diurnal attacks of sleep and concomitant muscular weakness with a Dexedrine rather than tranguilizers and supportive psychotherapy. The historical literatures are also reviewed, and the several clinical aspects of this particular syndrome are discussed..

        Rethinking Path Dependency and Regional Innovation - Policy Induced 'Government Dependency': The Case of Daedeok, South Korea

        This study focuses on exploring the behaviours of high-tech start-up firms in response to the policy interventions undertaken to promote regional innovation in South Korea since 1997. High-tech start-ups and their technological entrepreneurship are increasingly considered by policy makers and academics to play a crucial role in the generation of innovation and economic development. However, this study started from a basic concern of why government intervention does not necessarily result in an increase of regional innovation capacity. To explain this concern, we constructed a new conceptual framework of 'government dependency' and apply this to 'Daedeok,' a regional innovation system in South Korea, to explore the reproduction of path dependency as an impact induced by innovation policy. This conceptual framework was developed by remodeling path dependency approaches through a systemic and interactive lens. An empirical study used qualitative interviews of start-up founders to delineate the emergence of a new development path and the extent to which dependency was reproduced in the Daedeok regional innovation system. Empirical analysis suggested that 'reliance' and 'persistence' were the crucial factors in the production and reproduction of the government dependency. Some firms accepted dependency as reliance, but others regarded it as policy utilization. Thus, a critical juncture could not be clearly identified in actors' behaviour. It was also unclear if dependency had hindered innovation, but it was shown that the regional and institutional contexts strongly influenced the reproduction process. The study concludes that the construct of government dependency can also provide useful insights into policy learning as well as the success of government interventions.

        Intestinal capillariasis is caused by Capillaria philippinensis. Among them, only 4 species have been found in humans: Capillaria hepatica, C. aerophila (Eucoleus aerophilus), C. plica and C. philippinensis. Reports of human infections with C. hepatica, C. aerophila and C. plica are rare, but reports of C. philippinensis infections are increasing and spreading geographically. Human infestation by C. philippinensis is a rare parasitic disease. We experienced a case of intestinal capillariasis which was misdiagnosed for intestinal tuberculosis or Crohn's disease at first but correctly diagnosed later by detection of numerous eggs of C. philippinensis on the stool examination. The eggs were found in stool only after ingestion of prednisolone for treatment of misdiagnosed Crohn's disease. The 42 year-old patient had a journey to Saipan island in Pacific ocean 6 months ago and ate sliced raw eels. Abdominal pain, watery diarrhea were subsided after therapy of albendazole.

        지방공무원 인사혁신정책의 상대적 우선순위분석

        본 연구는 지방공무원 인사혁신에 대한 정책우선순위를 통해 인적자원관리의 방향성을 모색하기 위해 AHP 분석을 실시하였다. 연구결과는 다음과 같다. 첫째, 측정영역에서는 인사교류시스템, 보직관리시스템, 채용관리시스템, 교육훈련시스템 중에서 인사교류시스템이 가장 중요한 평가요소로 분석되었다. 둘째, 평가항목의 관점에서 보면, 인사교류시스템에서는 직무와 성과중심의 인사관리 전환, 보직관리시스템에서는 전문직위제 지정 및 확대, 채용관리시스템에서는 민간에서 시행중인 인성, 적성 검사도입, 교육훈련시스템에서는 맞춤형 교육과정프로그램 개발 등이 가장 중요하게 평가되었다. This study conducted AHP analysis to find out the directional of human resource management through policy priorities for personnel innovation of local public officials. The results of the study were derived using the AHP analysis methodology. The results of the study are summarized as follows. First, the interchange of personnel system was the most important measure among the measurement areas of interchange of personnel system, education training system, recruitment management system and appointment management system. Second, from the point of view of evaluation item, it is as follows. In the personnel interchange system, it was found that the conversion of personnel management based on job performance and performance was important. In the position management system, it is important to appoint and expand professional positions. In the recruitment management system, the introduction of personality and aptitude tests used in the private sector is important. Development of customized curriculum programs is important in the education and training system.

        자궁경부 종양에서 p53, Proliferating Cell Nuclear Antigen(PCNA), c-myc 및 Epidermal Growth Factor Receptor(EGFR)의 발현

        Expression of four biologic markers known as p53, proliferating cell nuclear antigen(PCNA), c-myc, and epidermal growth factor receptor(EGFR), was studied immunohistochemically in 121 cases of cervical tissues, which included 15 cases of normal cervical tissues, 50 cases of squamous intraepithelial lesions(SIL), and 56 cases of invasive carcinomas. This study was designed to elucidate the roles of these factors in the genesis and progression of cervical neoplasia and to identify their association with clinical parameters such as cell type, tumor size, lymph node involvement, squamous cell carcinoma(SCC) antigen level, and prognosis. In additon, this study evaluates the differences of coexpression rates in normal cervical tissues, SILs, and invasive carcinomas. (1) The intensities of p53, c-myc, and EGFR expression and the rates of PCNA, c-myc, and EGFR expression in cervical SILs and invasive carcinomas were significantly higher than those in normal tissues (p<0.05 and p<0.01). But there were no significant differences between SILs and invasive carcinomas in both of intensities and rates of p53, PCNA, c-myc, and EGFR expression. (2) The intensities and rates of p53, PCNA, and c-myc expression in the invasive carcinomas did not correlate with clinical parameters including cell types, tumor sizes, lymph node involvements, see antigen, and prognosis. But only the intensity and rate of EGFR expression were significantly higher in cases with tumor size larger than 3 cm when it was compared with cases with tumor sizes smaller than 3 cm(p<0.01), in cases with lymph node involvement when it was compared with cases without lymph node involvement(p<0.05), and in cases with SCC antigen levels more than 2.5 ng/ml when it was compared with cases with SCC antigen levels less than 2.5 ng/ml(p<0.05). (3) C-myc immunoreactivity was significantly correlated with EGFR overexpression(p<0.01). (4) The rates of simultaneous expression of four factors were decreased significantly in normal tissues and significantly increased in invasive carcinomas(p<0.01). The coexpression rate of two factors or more was 6.7% in normal tissues, 62.7% in SILs, and 70.0% in invasive carcinomas and the simultaneous expression rate of three factors or more was none in normal tissues, 34.0% in SILs, and 47.5 % in invasive carcinomas. These results show that the immunohistochemical detection of p53, PCNA, c-myc, and EGFR expression will be useful in differentiating the normal tissues and cervical neoplasia. The results of simultaneous immunohistochemical detection of these four factor expression also suggest to contribute a better understanding of the genesis of cervical carcinoma and that coexpression of more factors indicate more aggressiveness in cervical neoplasia, and show that simultaneous detection of these factors can be used in the early detection of cervical neoplasia and to predict malignant transformation of cervical lesions.

      • White electroluminescence from a single polyfluorene containing bis-DCM units

        <P>A series of fluorene-based copolymers composed of blue- and orange-light-emitting comonomers were synthesized through palladium-catalyzed Suzuki coupling reactions. 9,9-Dihexylfluorene and 2-(2,6-bis-{2-[1-(9,9-dihexyl-9H-fluoren-2-yl)-1,2,3,4-tetrahydroquinolin-6-yl]-vinyl}-pyran-4-ylidene)-malononitrile (DCMF) were used as the blue- and orange-light-emitting chromophores, respectively. The resulting single polymers exhibited simultaneous blue (423/450 nm) and orange (580–600 nm) emissions from these two chromophores. By adjusting the fluorene and DCMF contents, white light emission could be obtained from a single polymer; a device with an ITO/PEDOT:PSS/polymer/Ca/Al configuration was found to exhibit pure white electroluminescence with Commission Internationale de L'Eclairage (CIE) coordinates of (0.33, 0.31), a maximum brightness of 1180 cd/m<SUP>2</SUP>, and a current efficiency of 0.60 cd/A. Furthermore, the white light emission of this device was found to be very stable with respect to variation of the driving voltage.       • The critical warning sign of real-time brainstem auditory evoked potentials during microvascular decompression for hemifacial spasm

        <P><B>Abstract</B></P> <P><B>Objective</B></P> <P>The aim of this study was to define the critical warning sign of real-time brainstem auditory evoked potential (BAEP) for predicting hearing loss (HL) after microvascular decompression (MVD) for hemifacial spasm (HFS).</P> <P><B>Methods</B></P> <P>Nine hundred and thirty-two patients with HFS who underwent MVD with intraoperative monitoring (IOM) of BAEP were analyzed. We used a 43.9 Hz/s stimulation rate and 400 averaging trials to obtain BAEP. To evaluate HL, pure-tone audiometry and speech discrimination scoring were performed before and one week after surgery. We analyzed the incidence for postoperative HL according to BAEP changes and calculated the diagnostic accuracy of significant warning criteria.</P> <P><B>Results</B></P> <P>Only 11 (1.2%) patients experienced postoperative HL. The group showing permanent loss of wave V showed the largest percentage of postoperative HL (<I>p</I> < 0.001). No patient who experienced only latency prolongation (≥1 ms) had postoperative HL. Loss of wave V and latency prolongation (≥1 ms) with amplitude decrement (≥50%) were highly associated with postoperative HL.</P> <P><B>Conclusions</B></P> <P>Loss of wave V and latency prolongation of 1 ms with amplitude decrement ≥50% were the critical warning signs of BAEP for predicting postoperative HL.</P> <P><B>Significance</B></P> <P>These findings elucidate the critical warning sign of real-time BAEP.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Critical warning sign of real-time BAEP during MVD for hemifacial spasm have been studied. </LI> <LI> Loss of wave V and the significant change of both latency and amplitude are the critical signs. </LI> <LI> Isolated wave V latency prolongation of >1 ms without amplitude loss >50% is not saaociated with postoperative hearing loss. </LI> </UL> </P>
