        지역사회 정신보건 재활간호 사업이 만성정신질환자의 자기돌봄 활동에 미치는 영향

        This study was carried out in order to know the level of self-care behavior of chronic mental patients who live in their community and take whether the programme is able to affect on the self-care behavior, after mental health nursing progrmme. The period of the study is from August in 1996 to August in 1997. The object of the study is 26 mental disorder patients who have been cared by Seung Buk Gu public health center participated in this study. The tool of this study was selected 46 questions which are adequate to mental disorder patients and was developed a self-care measurement and references by Sook-Ja Yoo(1991). The community mental health nursing rehabilitation programme has centered on the Seung Buk Gu public health center that a nurses for mental health rehabilitation and mental health nursing scholars forcused on mental disorder patients at their home. The programme required a client's meeting 5 times a week and family counsellor once a week. The client's meeting involved daily life training, occupational therapy, working therapy, music therapy, art therapy, group therapy, recration and exercise. The family counsellor involved recognizing of family's need, symptoms and medicine management for their patient's and support of families and patients. At the begining of the programme, the level of self-care behavior of the mental disorder patients had been taken. After 6 months, the level of self-care behavior had been taken. Also after the 6 month's progrmme, the level of self-care behavior was taken again. The level of self-care behavior of the patients presented that fields of dressing, safety, individual hygiene and eating had no problem to the patients, however the patients have difficulty in the field of leisure life and social life. Therefore it is known that the level of self-care behavior in mental disorder patients is more less in the community than at home for leisure life and social life. After programme, the change of self-care behavior was shown differenty the mean value 2.48 at the first period and 2.75(t=-3.24, p=.003) at the second period. The second of self-care behavior was measured and the third of self-care behavior was measured at the 6 months after. The second and third measurement were not shown differently but was shown a few addition. The first and third measurement were shown differently(t=-4.11, p=.000). Finally, a comparison of the relationship of at the first, second and third self-care behavior degree(M=2.92) was shown differently(F=5.346, p=.007), especially the third measurement was shown as expressed "I do frequently" on all scope except leisure life, household affairs life and social application scope. Conclusively, the community mental health nursing rehabilitation programme had effect on self-care behavior of the chronic mental disorder patients and further programme will be consider on liesure lfie.

        중년여성의 우울 정도와 사회적지지 및 성 역할 장애의 관계

        This study was conducted to provide the data-base for nursing implementation of reducing depression through the investigation of the relations among depressed mood, social support, and gender role barrier in middle aged women. The subjects included 241 middle-aged women who reside in Seoul or the suburbs of Seoul. SDSby zung, SSBC, and the gender role barrier questionnaire by Hong were used as the measuring instruments. The period of the study was from October 1 to November 10, 2000. Collected data was analyzed using frequency, percentile, t-test, ANOVA, Scheffe´ test, Pearson Correlation Coefficient, Stepwise Multiple Regression by SAS program. The result were as follows. 1. The degree of depression of the subjects was relatively low(1.89±0.41), showing a education, and economic status. 2. The level of social support of the subjects was relatively high(2.95±9.55), showing a significant difference according to education. 3. The degree of gender role barrier of the subjects was relatively high (2.47±0.54). There was a significant difference according to education, living with parents-in-law. 4. The degree of depression had a negative correlation with social support, and had a positive correlation with gender role barrier. 5. The predictive variables of depression are gender role barrier, social support, occupation, age, menopause variable. The combination of all predictive variables explained 41.8% of the variance of likelihood to engage in depression of middle-aged women. In conclusion, the results show that social support and gender role barrier relate to depression in middle-aged women. It suggested that the goal of care for depression in middle-aged women should be to help hem reduce barrier to gender role with a greater degree of social support.

        오사카(大阪) 이카이노(猪飼野)의 여류시인 종추월(宗秋月)

        A second-generation 'Zainichi'(在日, Korean resident in Japan) poet, Chong Chu-wol wrote poetry and essays about people from all walks of life and their everyday experiences in the Korean community in Osaka Ikaino. The collections of her poems are the Collections of Poems by Chong Chu-wol(1971) and lkaino /Woman /Love /Songs: the Collection of Poems by Chong Chu-wol(1984). Her collections of essays are lkaino Taryong(1989) and I Love You(1987). There was a relatively long gap after the publication of first collection of her poems at the beginning of 1970 and the second collection of her poems and the collections of her essays that were published in the middle of 1980's; this gap had to do with her interest in Korean politics and society at the time. In the history of the literature of Koreans in Japan, Chong Chu-wol's works are at the mid-point between Ri Kai-sei(1935 -) and Kin Kakuei(1938-1985) -second-generation Zainichi writers- and Yi Yang-ji(1955-1992), the highly celebrated new-generation (3rd generation) writer. Unlike these elite writers who attended university and who were active in literary circles and recognized for their literary talent, Chong Chu-wol was based in a humble area on the outskirt of Ikaino and wrote poems while making a living with hard labor. Accordingly, physical fatigue, poverty, nostalgia and regrets deeply color her poems. In Chong Chu-wol's poems, Ikaino's common language - a unique mixture of the dialects of Jeju Island and of Osaka - comes to live conveying the unique sentiments and emotions of Koreans living in Japan, defying the other constructed by the Japanese language and exploring new possibilities. What should be particularly noted in her poems and essays is the poet's affection for the emotions of the Korean women leading a tortuous existence in Ikaino. Although Korean women in Ikaino tend to be doubly oppressed under the Confucian patriarchy, they are broad-minded enough to extend their motherly love to the Korean men suffering in the adverse conditions of 'living as Zainichi'. The women's willingness to embrace the men while enduring their violence may very well be an expression of self-protection refusing to be hurt further. Won Soo-il(1950- ), the writer, also depicted realistically Ikaino's first-generation Korean mothers from Jeju in his collection of short stories, Ikaino Stories. To be sure, their love of life that seeks laughter and hopes in tears has helped build Ikaino of Osaka what it is. But, in a short story by Kim Chang-saeng(1951- ), we should also note the second-generation Zainichi protagonist who refuses the lives of sacrifices and obedience of the first-generation Korean mothers and chooses to stand alone, however difficult it may be, to have her own voice and life. This is an expression of her will to lead her own life rather than subjecting it to others. Chong Chu-wol reacts sensitively to the political changes and social trends in Korea in her desire to share the pain of the era with her compatriots. To the poet, the Gwangju Uprising of 1980 was the juncture where the history of Zainichi and the Korean history intersected. She lives the life of Zainichi but yearns to share the pain of the struggling compatriots in Korea, and thus to 'transcend the being of Zainichi'. The poet's existential search for the meaning of life as Zainichi takes on the perspective of the love of the humankind as she looks deeply into the Korean and Japanese societies.

      • 재활관련 재가(在家) 만성정신질환자를 위한 정신재활간호사업 : UNDP Project 를 중심으로

        According to statistics related by the government, the number of mentally-ill was estimated to be approximately 1,000,020 in 1995 in Korea. The actual number of mental patients is undoubtedly far greater than the government s official figure. Only less than 4 percent of them are accommodated in mental health institutions. The remaining 96 percent are at home without proper health care. Mental health problems are more serious among the economically underprivileged than others. To solve this problems, a project(community-based psychiatric rehabilitation nursing program) was introduced. The purpose of this study is to evaluate the effectiveness of the community-based psychiatric rehabilitation nursing program in terms of patients' quality of life, self-care activities, daily living skills, perceived support of family and reduction of family burden. A quasi-experimental, non-equivalent control group with a pre-post test design was used to assess the effects of psychiatric rehabilitation nursing intervention. This study was conducted at two sites in economically underprivileged areas in Seoul in the beginning and expanded to four sites. A total of 314 long-term mentally ill patients participated in the program, however 93 agreed to be in study group. Then control group sample of 24 was secured from other districts of Seoul. The result of this study can be summarized as follows : 1. Reliability tests for five tools by using Chronbach' alpha were done and found to be highly reliable. 2. 65% of the experimental group did not have any hospitalization during the study period and mean length of admission was 13.8 days compared to 7-9 months previously. 3. The mean frequency of hospitalization during decreased from 7.16 times to 2.10 times 4. Family burden decreased significantly at all post time periods(6 months, 12 months, 18 months periods). Self-care activities, perceived family support, quality of life and daily living skills were increased significantly at all post time periods. 5. Interviews with the 46 subjects reveal that after one year of participation in this program, they receive far more social supports(100%), know that they have to take medicine(82.6%), like to work(100%) and can be recovered from mental illness(87%). 6. Interviews with the subjects family members(n=42) reveal that subjects eats and sleeps regularly(95.2%), has insight about own illness(95.2%), takes medicine on their own (90.5%), increase activities significantly (90.5%), increase the sense of stability significantly(85.7%), organizes their rooms far better(83.3%), become more cheerful (83.3%), shows motivation to do something(81.0%) and improves relationship with outsiders(81.0%).

        비디오 유머중 재가 정신질환자의 대인관계 및 정신건강상태에 미치는 영향

        This study was conducted to evaluate the effects of humorous video tapes on mental health status and interpersonal relationships with psychiatric inpatients. Forty four psychiatric inpatients were sampled from two university hospitals from September, 1997 to March, 1999. They were divided into two groups by hospital : an experimental group of 21 and a control group of 23 individuals. The intervention of showing humor video tapes to the experimental group was conducted three times a week for 3 weeks. The experimental group was compared with the control group before and after the intervention, in terms of mental health status and interpersonal relationships. Two instruments were used to assess mental health status: SCR-90-R: and to evaluate interpersonal relationships: Chun's Relationship Change scale. The data were analyzed using χ^2-test, unpaired and paired t-test, two-way ANOVA. The results were as follows : 1. The score of SCL-90-R was significantly improved after humorous video tape intervention with the experimental group. while that of the control group was significantly improved in all categories except somatization and hostility. However, there was no significant difference in all categories between the two groups. 2. The level of interpersonal relationships revealed significant difference between pre-and post-intervention in the experimental group but showed no significant difference between the two groups. 3. The score of hostility by gender revealed a significant difference between the two groups but there was an interaction between group and gender. There were also interaction effects on the scores of psychosis and interpersonal relationships between group and gender. The score of psychosis for male patients showed a significant difference between the groups. The score of interpersonal relationship for male patients revealed a significant difference between the groups and was significantly different form that of female patients in the experimental group. 4. The score of paranoia ideation had no significant different between the groups but had a significant difference by educational level. However, there was no a interaction between group and education level. 5. For the level of interpersonal relationships according to employment status, there was a significant difference between the groups and on interaction between groups and employment status. In the light of cur findings, the study implies that humorous video tape intervention as effective on parts of mental health status and interpersonal relationships with psychiatric inpatients. However, further study is needed to examine the intervention effects with larger samples and longer study periods.

        A Case of Folliculosebaceous Cystic Hamartoma

        Folliculosebaceous cystic hamartoma (FSCH) is a recently-recognized cutaneous hamartoma composed of follicular, sebaceous and mesenchymal elements. We describe an unusual case of FSCH in a 61-year-old male, who had a relatively large, 3×2.5cm sized, smooth subcutaneous nodule on the occipital area of the scalp, an uncommon location for FSCH.
