        A Qualitative Case Study on Sibling Relationship Experiences of Adolescents Discharged from Child Care Institutions

        This study investigated the sibling relationships in adolescents discharged from child care institutions. The purpose of this study was to influence the development of social welfare intervention plans to support sibling relationships in brothers and/or sisters who grow up in child care institution. We enrolled four adolescents who had grown up together with their sibling(s) at the same child care institution. We examined the individual responses of the adolescents after in-depth interviews, which were conducted between March and August 2017. Our qualitative case study revealed that the adolescents had not been exposed to sufficient interactions or opportunities at the child care institutions that enabled them to form strong sibling relationships with each their sibling(s). After being discharged, adolescents remained on awkward terms with their sibling(s) onto adolescence, but did not sever family ties. Based on these findings, the social welfare interventions suggested in this study aim to promote and support positive sibling relationships among siblings discharged from the same child care institutions.

      • Bortezomib Induced Tumor Lysis Syndrome in Multiple Myeloma

        The tumor lysis syndrome (TLS) commonly occurs in the lymphoproliferative disorder, either spontaneously or in response to therapy. TLS is uncommon in multiple myeloma. However, with the use of bortezomib in the treatment of multiple myeloma, cases of TLS have been reported. We report here threepatients who presented with TLS after the administration of bortezomib. Two of them presented mild symptoms and recovered with hydration only. However, death of the other patient was associated with TLS. We should monitor patients who had high tumor burden, especially in early phase of bortezomib therapy and appropriate prophylaxis for high risk patient is also needed.

        Pre-Transplant Angiotensin II Type 1 Receptor Antibodies and Anti-Endothelial Cell Antibodies Predict Graft Function and Allograft Rejection in a Low-Risk Kidney Transplantation Setting

        Background: Non-HLA antibodies, anti-angiotensin II type 1 receptor antibodies (anti-AT1R) and anti-endothelial cell antibodies (AECA), are known to play a role in allograft rejection. We evaluated the role of both antibodies in predicting post-transplant outcomes in low-risk living donor kidney transplantation (LDKT) recipients. Methods: In 94 consecutive LDKT recipients who were ABO compatible and negative for pre-transplant HLA donor-specific antibodies, we determined the levels of anti-AT1Rs using an enzyme-linked immunosorbent assay and the presence of AECAs using a flow cytometric endothelial cell crossmatch (ECXM) assay with pre-transplant sera. Hazard ratio (HR) was calculated to predict post-transplant outcomes. Results: Pre-transplant anti-AT1Rs (≥11.5 U/mL) and AECAs were observed in 36 (38.3%) and 22 recipients (23.4%), respectively; 11 recipients had both. Pre-transplant anti-AT1Rs were a significant risk factor for the development of acute rejection (AR) (HR 2.09; P=0.018), while a positive AECA status was associated with AR or microvascular inflammation only (HR 2.47; P=0.004) throughout the follow-up period. In particular, AECA (+) recipients with ≥11.5 U/mL anti-AT1Rs exhibited a significant effect on creatinine and estimated glomerular filtration rate (P<0.001; P=0.028), although the risk of AR was not significant. Conclusions: Pre-transplant anti-AT1Rs and AECAs have independent negative effects on post-transplant outcomes in low-risk LDKT recipients. Assessment of both antibodies would be helpful in stratifying the pre-transplant immunological risk, even in low-risk LDKT recipients.

        Feasibility of Deep Learning-Based Analysis of Auscultation for Screening Significant Stenosis of Native Arteriovenous Fistula for Hemodialysis Requiring Angioplasty

        Objective: To investigate the feasibility of using a deep learning-based analysis of auscultation data to predict significant stenosis of arteriovenous fistulas (AVF) in patients undergoing hemodialysis requiring percutaneous transluminal angioplasty (PTA). Materials and Methods: Forty patients (24 male and 16 female; median age, 62.5 years) with dysfunctional native AVF were prospectively recruited. Digital sounds from the AVF shunt were recorded using a wireless electronic stethoscope before (pre-PTA) and after PTA (post-PTA), and the audio files were subsequently converted to mel spectrograms, which were used to construct various deep convolutional neural network (DCNN) models (DenseNet201, EfficientNetB5, and ResNet50). The performance of these models for diagnosing ≥ 50% AVF stenosis was assessed and compared. The ground truth for the presence of ≥ 50% AVF stenosis was obtained using digital subtraction angiography. Gradient-weighted class activation mapping (Grad-CAM) was used to produce visual explanations for DCNN model decisions. Results: Eighty audio files were obtained from the 40 recruited patients and pooled for the study. Mel spectrograms of “pre-PTA” shunt sounds showed patterns corresponding to abnormal high-pitched bruits with systolic accentuation observed in patients with stenotic AVF. The ResNet50 and EfficientNetB5 models yielded an area under the receiver operating characteristic curve of 0.99 and 0.98, respectively, at optimized epochs for predicting ≥ 50% AVF stenosis. However, Grad- CAM heatmaps revealed that only ResNet50 highlighted areas relevant to AVF stenosis in the mel spectrogram. Conclusion: Mel spectrogram-based DCNN models, particularly ResNet50, successfully predicted the presence of significant AVF stenosis requiring PTA in this feasibility study and may potentially be used in AVF surveillance.

        β-Cell–Derived Angiopoietin-1 Regulates Insulin Secretion and Glucose Homeostasis by Stabilizing the Islet Microenvironment

        <P>Islets are highly vascularized for prompt insulin secretion. Although angiopoietin-1 (Ang1) is a well-known angiogenic factor, its role in glucose homeostasis remains largely unknown. The objective of this study was to investigate whether and how Ang1 contributes to glucose homeostasis in response to metabolic challenge. We used inducible systemic Ang1 knockout (Ang1<SUP>sys−/−</SUP>) and β-cell–specific Ang1 knockout (Ang1<SUP>β-cell−/−</SUP>) mice fed a high-fat diet for 24 weeks. Although the degree of insulin sensitivity did not differ between Ang1<SUP>sys−/−</SUP> and Ang1<SUP>sys+/+</SUP> mice, serum insulin levels were lower in Ang1<SUP>sys−/−</SUP> mice, resulting in significant glucose intolerance. Similar results were observed in Ang1<SUP>β-cell−/−</SUP> mice, suggesting a critical role of β-cell–derived Ang1 in glucose homeostasis. There were no differences in β-cell area or vasculature density, but glucose-stimulated insulin secretion was significantly decreased, and PDX-1 expression and GLUT2 localization were altered in Ang1<SUP>β-cell−/−</SUP> compared with Ang1<SUP>β-cell+/+</SUP> mice. These effects were associated with less pericyte coverage, disorganized endothelial cell ultrastructure, and enhanced infiltration of inflammatory cells and upregulation of adhesion molecules in the islets of Ang1<SUP>β-cell−/−</SUP> mice. In conclusion, β-cell–derived Ang1 regulates insulin secretion and glucose homeostasis by stabilizing the blood vessels in the islet and may be a novel therapeutic target for diabetes treatment in the future.</P>

        Establishment of Critical Difference for Diagnostic Immunoassays

        Background: Critical difference (CD) is a significant difference between sequential laboratory results in a patient and is a major parameter for comparative evaluation of quantitative clinical laboratory tests, and how significant the difference is should be set by the laboratory itself. In this study, we established the criteria for CD that can be referenced in each laboratory for diagnostic immunoassays. Methods: We targeted 17 major diagnostic immunoassays as follows: alphafetoprotein (AFP), cancer antigen 125 (CA125), cancer antigen 15-3 (CA15- 3), cancer antigen 19-9 (CA19-9), carcinoembryonic antigen (CEA), prostatespecific antigen (PSA), free PSA, tri-iodothyronine (T3), thyroxine (T4), free T4, thyroglobulin, thyroid-stimulating hormone (TSH), estradiol (E2), folliclestimulating hormone (FSH), luteinizing hormone (LH), testosterone, and prolactin. According to Clinical and Laboratory Standards Institute EP31, we investigated acceptance criteria for CD based on clinical outcomes, clinician’s questionnaire, biological variation, published professional recommendations, goals set by accrediting agencies, and general capability in the Korean Association of External Quality Assessment Service. Results: We selected the acceptance criteria for CD as follows: 6.0% for total PSA and TSH based on well-designed clinical study’s outcomes; 20.0% for AFP, CA19-9, CEA, and free PSA, 10.0% for T3, 14.3% for T4, 12.5% for free T4, 16.9% for thyroglobulin, 22.2% for E2, 22.9% for FSH, 20.0% for LH, 10.0% for testosterone, and 25.0% for prolactin based on clinician’s questionnaire; 8.4% for CA125 and 10.2% for CA15-3 based on general capability. Conclusions: Applying the acceptance criteria for CD from this study may help assess the comparability of the quantitative tests in routine laboratory practice.

        모둠 독서 토론 담화에 드러난 상호작용 양상 - 모둠원의 관계와 위치 설정을 중심으로

        이 연구는 중학교 수업 시간에 실시한 모둠 독서 토론 담화를 비판적 담화 분석의 관점에서 분석하여 모둠 구성원의 관계가 독서 토론 활동에 어떠한 영향을 미치는지를 살펴보았다. 학생들은 독서 토론 과정에서 발화 빈도나 주요 발화 양상에서 차이를 보였다. 이는 담화에 드러난 대명사와 조사의 사용, 부정적 표현과 함께 모둠 내에서 일종의 권력으로 작용하여, 모둠원들이 암묵적으로 각자의 위치를 리더, 리더와 대립, 방관, 소외로 위치 짓는 데에 영향을 끼쳤다. 또한 모둠 내에서 권력을 가진 구성원이 지속적으로 부정적인 감정을 표현하였을 때 모둠 전체에 부적 정서가 전이되어 독서 토론 활동에도 지장을 준다는 점을 확인하였다. 본 연구는 소수를 대상으로 한 사례연구라는 점에서 일반화에 한계가 있으나, 학생들의 모둠 독서 토론 과정에 주목하여 또래 간 상호작용 양상을 실증적으로 살핀 것에 의의가 있다. This study examined the influence that the relationships among group members had on reading discussion activities by analyzing the group discussion talks in class, from the perspective of critical discourse analysis. Students showed differences in the frequency of utterances and patterns of major utterances during the reading discussion. Along with the use of pronouns, postposition, and negative expressions, revealed during the discourse, this acted as a sort of power within the group, which tacitly showed positions of each one: leader, conflicted-leader, onlooker, and outsider. In addition, we could confirm that when members with power in the team continuously express negative emotions, these negative emotions spread to the whole group, which eventually hinders reading discussion activities. This study is limited to generalize the above findings, as it is a case study done with a small number of students. Nevertheless, it is meaningful in that it examined the interaction patterns of peers, paying close attention the process of students' group reading discussion.

        코로나19 상황에서 간호대학생의 대인관계능력, 전공만족도, 회복탄력성이 대학생활적응에 미치는 영향

        본 연구는 코로나19 상황에서 간호대학생의 대인관계능력, 전공만족도, 회복탄력성 간의 상관관계를 확인하고, 대학생활적응에 미치는 영향요인을 파악하기 위해 수행된 서술적 조사연구이다. S시에 위치한 E대학교 1, 2학년 간호학과 재학생 156명을 편의 표집하여 2021년 4월 19일부터 2021년 4월 21일까지 설문조사 하였다. 대인관계 척도, 전공만족도 도구, 회복탄력성 도구, 대학생활적응 척도가 담긴 종이 설문지를 이용하여 수집된 자료는 SPSS WIN 28.0 Program, independent t-test와 ANOVA, scheffe's test, Pearson 상관계수 값, 위계적 회귀분석을 이용하여 분석하였다. 대학생활적응은 회복탄력성(r=.70, p<.001), 전공만족도(r=.58, p<.001), 대인관계능력(r=.51, p<.001)과 양의 상관관계를 나타냈다. 위계적 회귀분석 결과 전공만족도(β=.27, p<.001), 회복탄력성(β=.54, p<.001)이 대학생활적응에 유의한 예측변수임을 확인하였고 총 설명력은 52% 였다. 간호대학생의 코로나19 상황에서 대학생활적응 능력을 향상시키기 위해 회복탄력성을 증진시킬 수 있는 프로그램을 개발할 것을 제안한다. This study aims to identify the influence of interpersonal relationships, satisfaction in major, and resilience on nursing students' adaptation to college in COVID-19 crisis situation. This study is a descriptive study with convenience sampling on 156 first-year and second-year nursing students of E university in S city. The survey was conducted from April 19, 2021 to April 21, 2021 using a paper questionnaire including relationship change scale, satisfaction in major, resilience, and student adaptation to college Questionnaire. Data were analyzed by SPSS WIN 28.0 Program, independent t-test, ANOVA, Scheffe's test, Pearson correlation coefficient, and hierarchical regression analysis. Student adaptation to college is positively correlated with resilience (r=.70, p<<.001), satisfaction in major (r=.58, p<.001), interpersonal relationship (r=.51, p<<.001). Hierarchical regression analysis showed that satisfaction in major (β=.27, p<.001) and resilience(β=.54, p<<.001) were significant predictors of student adaptation to college. The total explanation power was 52%. It is recommended to develop a program that can enhance resilience to improve the ability to adapt to college life in the COVID-19 crisis situation.

      • Endothelial Progenitor Cell Cotransplantation Enhances Islet Engraftment by Rapid Revascularization

        <P><B/></P><P>Impaired revascularization of transplanted islets is a critical problem that leads to progressive islet loss. Since endothelial progenitor cells (EPCs) are known to aid neovascularization, we aimed to enhance islet engraftment by cotransplanting EPCs with islets. Porcine islets, with (islet-EPC group) or without (islet-only group) human cord blood–derived EPCs, were transplanted into diabetic nude mice. The islet-EPC group reached euglycemia by ∼11 days posttransplantation, whereas the islet-only group did not. Also, the islet-EPC group had a higher serum porcine insulin level than the islet-only group. Islets from the islet-EPC group were more rapidly revascularized at the early period of transplantation without increment of final capillary density at the fully revascularized graft. Enhanced revascularization rate in the islet-EPC group was mainly attributed to stimulating vascular endothelial growth factor-A production from the graft. The rapid revascularization by EPC cotransplantation led to better graft perfusion and recovery from hypoxia. EPC cotransplantation was also associated with greater β-cell proliferation, probably by more basement membrane production and hepatocyte growth factor secretion. In conclusion, cotransplantation of EPCs and islets induces better islet engraftment by enhancing the rate of graft revascularization. These findings might provide a directly applicable tool to enhance the efficacy of islet transplantation in clinical practice.</P>
