Purpose: Cancer cells develop acquired resistance induced by chemotherapeutic drugs. In this study, we investigated the effects of brief treatment with cytotoxic drugs on the phenotype of breast cancer cells. Methods: Breast cancer cells MCF7 and BT-474 were briefly treated with paclitaxel or doxorubicin. Clonogenic, migration, and invasion assays were performed on the treated cells. Western blot analysis and RhoA activity assay were also performed. Results: Breast cancer cells when briefly treated with paclitaxel or doxorubicin showed reduced clonogenic ability. Doxorubicin, but not paclitaxel, augmented cell migration and invasion. The invasion-promoting effects of doxorubicin were lost when the two drugs were sequentially used in combination. Myosin light chain (MLC) 2 phosphorylation and RhoA activity were upregulated by doxorubicin and downregulated by paclitaxel. Pretreatment with RhoA inhibitors abolished the migration- and invasion-promoting effects of doxorubicin. Conclusion: Doxorubicin activates the RhoA/MLC pathway and enhances breast cancer cell migration and invasion. Therefore, this pathway might be explored as a therapeutic target to suppress anthracycline-enhanced tumor progression.

Objective: Predicting disease relapse and early intervention could reduce symptom severity. We attempted to identify potential indicators that predict the duration to next admission for an acute affective episode in patients with bipolar I disorder. Methods: We mathematically defined the duration to next psychiatric admission and performed single-variate regressions using historical data of 101 patients with bipolar I disorder to screen for potential variables for further multivariate regressions. Results: Age of onset, total psychiatric admissions, length of lithium use, and carbamazepine use during the psychiatric hospitalization contributed to the next psychiatric admission duration positively. The all-in-one found that hyperlipidemia during the psychiatric hospitalization demonstrated a negative contribution to the duration to next psychiatric admission; the last duration to psychiatric admission, lithium and carbamazepine uses during the psychiatric hospitalization, and heart rate on the discharge day positively contributed to the duration to next admission. Conclusion: We identified essential variables that may predict the duration of bipolar I patients’ next psychiatric admission. The correlation of a faster heartbeat and a normal lipid profile in delaying the next onset highlights the importance of managing these parameters when treating bipolar I disorder.
Hypoglycemia Revisited in the Acute Care Setting

Hypoglycemia is a common finding in both daily clinical practice and acute care settings. The causes of severe hypoglycemia (SH) are multi-factorial and the major etiologies are iatrogenic, infectious diseases with sepsis and tumor or autoimmune diseases. With the advent of aggressive lowering of HbA1c values to achieve optimal glycemic control, patients are at increased risk of hypoglycemic episodes. Iatrogenic hypoglycemia can cause recurrent morbidity, sometime irreversible neurologic complications and even death, and further preclude maintenance of euglycemia over a lifetime of diabetes. Recent studies have shown that hypoglycemia is associated with adverse outcomes in many acute illnesses. In addition, hypoglycemia is associated with increased mortality among elderly and non-diabetic hospitalized patients. Clinicians should have high clinical suspicion of subtle symptoms of hypoglycemia and provide prompt treatment. Clinicians should know that hypoglycemia is associated with considerable adverse outcomes in many acute critical illnesses. In order to reduce hypoglycemia-associated morbidity and mortality, timely health education programs and close monitoring should be applied to those diabetic patients presenting to the Emergency Department with SH. ED disposition strategies should be further validated and justified to achieve balance between the benefits of euglycemia and the risks of SH. We discuss relevant issues regarding hypoglycemia in emergency and critical care settings.

This study proposes an application of the classical rock mass classification concept on the estimation of in situ hydraulic conductivity of regolith and fractured bedrock which can be utilized to evaluate groundwater potential in mountainous areas. On the basis of boring logs, borehole televiewer image data, and double packer hydraulic test data conducted in the Basin of Jhuoshuei River of central Taiwan with the aid of bivariate analysis, two rock mass permeability classification systems (Hydro-Potential value for Regolith (HPR) and Hydro-Potential value for Bedrock (HPB) System) were developed. By regression analysis, the dependence of HPB/HPR on in situ hydraulic conductivity was performed, and two empirical models for estimating in situ hydraulic conductivity were derived. Meanwhile, two derived models were validated through the comparison against results from other types of hydraulic test data (pumping and single packer tests). Therefore, the development of the empirical models for a groundwater-related project enables to help the planning of detailed investigations for the determination of hydraulic conductivity with cost-effectiveness.

We studied a family with Gorlin-Goltz syndrome. The novel mutations of our cases were located on the 21st exon of the PTCH1 gene (c.3450C>G). The father, who received a strategic 56-day vismodegib treatment for disease control, was the first patient with Gorlin syndrome treated with the hedgehog inhibitor in Taiwan. The lesions regressed gradually, with scar formation, and were subsequently removed via a wide excision. Further details are provided below. (Ann Dermatol 30(5) 597∼601, 2018)

<P>Two new semiconducting polymers based on indacenodithiophene and thiadiazolo[3,4-<I>c</I>]pyridine units were synthesized <I>via</I> Stille coupling polymerization. The polymers, PIDTPyT and PIDTDTPyT, exhibited main absorption bands in the range of 550–800 nm while their absorption maxima were located at around 700 nm in films. With two additional thiophene spacers, PIDTDTPyT showed a broader absorption band but a 20 nm blue-shifted maximum peak compared to that of PIDTPyT. Both of the polymers possess low bandgaps (∼1.6 eV) and deep energy levels for both the highest occupied molecular orbital (HOMO) and the lowest unoccupied molecular orbital (LUMO). Organic field-effect transistors (OFETs) device measurements indicate that PIDTPyT and PIDTDTPyT have high hole carrier mobilities of 0.066 and 0.045 cm<SUP>2</SUP> V<SUP>−1</SUP> s<SUP>−1</SUP>, respectively, with the on/off ratio on the order of 10<SUP>6</SUP>. Bulk heterojunction photovoltaic devices consisting of the copolymers and PC<SUB>71</SUB>BM gave power conversion efficiencies (PCE) as high as 3.91% with broadband photo-response in the range of 300–800 nm. <P>A detailed model study has shown that thin film morphology and bulk-heterojunction solar cell performance can be significantly improved by systematic tuning of the surface energy of the conjugated donor polymer through side-chain functionalization. Thiophene-flanked diketopyrrolopyrrole (DPP) moieties with different contents of cyanohexane side chains were incorporated into three low band-gap conjugated copolymers (<B>PIDTDPP1</B>, <B>PIDTDPP2</B> and <B>PIDTDPP3</B>) consisting of indacenodithiophene (IDT) donors and DPP acceptors. The resulting polymers possessed good solubility in common organic solvents and showed similar energy levels, bandgaps, and hole mobilities. However, the introduction of cyano groups onto the terminal of side-chains significantly changed their surface energy. Topographical images obtained from atomic force microscopy (AFM) proved that a better matched surface energy between polymer and PC<SUB>71</SUB>BM had led to enhanced miscibility, which resulted in better BHJ film morphology. Consistent with the surface energy enhancement, the performance of BHJ photovoltaic devices increased from 0.97% for <B>PIDTDPP1</B>, to 2.16% for <B>PIDTDPP2</B> then to 3.67% for <B>PIDTDPP3</B>. BACKGROUND/OBJECTIVES Increased levels of uremic toxins and decreased antioxidant capacity have a significant impact on the progression of chronic kidney disease (CKD). However, it remains unclear whether they interact with each other to mediate the damage of kidney function. The purpose of this study was to investigate whether uremic toxins (i.e., homocysteine and indoxyl sulfate [IS]), as well as glutathione-dependent antioxidant enzyme activities are dependently or independently associated with kidney function during different stages of CKD patients. SUBJECTS/METHODS One hundred thirty-two patients diagnosed with CKD at stages 1 to 5 participated in this cross-sectional study. RESULTS Patients who had reached an advanced CKD stage experienced an increase in plasma uremic toxin levels, along with decreased glutathione peroxidase (GSH-Px) activity. Plasma homocysteine, cysteine, and IS concentrations were all positively associated with each other, but negatively correlated to GSH-Px activity levels after adjusting for potential confounders in all CKD patients. Although plasma homocysteine, cysteine, IS, and GSH-Px levels were significantly associated with kidney function, only plasma IS levels still had a significant association with kidney function after these parameters were simultaneously adjusted. In addition, plasma IS could interact with GSH-Px activity to be associated with kidney function. CONCLUSIONS IS plays a more dominant role than homocysteine and GSH-Px activity in relation to kidney function.
