In spine sugery, iatrogenic vascular injury is unusual, and most common vascular injury is said to be a tear of the left common iliac artery. We report a case of retroperitoneal hematoma from lumbar segmental artery injury after posterior lumbar fusion and pedicle screw fixation. This 60-year-old woman presented with 6 months history of low back pain and radiating pain to the left lower leg. She complained of claudication after 10 meters walking due to spondylolisthesis and spinal stenosis at L2-L5 level. She underwent multiple total laminectomy, posterior lumbar inter-body fusion, and transpedicular screw at L2-L5. During operation there was not evidence of complications such as bleeding. She complained of abdominal pain on post-operation I day and showed tenderness on left upper quadrant. Hemoglobin was 8.4 g/dL on postoperative laboratory examination. Abdominal CT scan revealed retroperitoneal hematoma. We performed angiography for evaluation of hematoma and found extravasation of dye at segmental artery of L4 level. We embolized the bleeding artery with coil successfully. Her postoperative abdominal pain and tenderness were relieved after then and discharged without pain or neurological deficit.

Purpose: Pediatric diabetes is a common health burden worldwide. This study aimed to investigate the prevalence of fasting hyperglycemia in Korean youth aged 10–18 years and to evaluate its association with metabolic indicators. Methods: To assess the risk of diabetes in domestic children and adolescents, the prevalence of fasting hyperglycemia was calculated, a trend was evaluated using multi-year Korea National Health and Nutrition Examination Survey (KNHANES) data, and multivariate analysis was performed to evaluate the relationships between hyperglycemia and metabolic factors. Results: The prevalence of fasting hyperglycemia, defined as impaired fasting glucose (fasting glucose level > 100 mg/dL and < 125 mg/dL), or diabetes mellitus (fasting glucose ≥ 126 mg/dL) was estimated in Korean teenagers. The prevalence increased from the fourth (2007–2009) to the fifth (2010–2012), sixth (2013–2015), and seventh (2016–2018) KNHANES surveys, from 5.39 to 4.79, 10.03, and 11.66 per 100 persons, respectively. In multivariate analysis, systolic blood pressure and serum triglycerides were higher in the fasting hyperglycemia group; systolic blood pressures were 109.83 mmHg and 112.64 mmHg and serum triglycerides were 81.59 mg/dL and 89.60 mg/dL in the normal blood glucose and fasting hyperglycemia groups, respectively. Conclusion: The prevalence of fasting hyperglycemia among children and adolescents has increased over the past decade, and this increase is potentially associated with metabolic abnormalities such as hypertension and hypertriglyceridemia. Effort is urgently required to reduce this chronic medical burden in adolescence.
신장 ; 만성 신질환 환자에서의 갑상선 기능 저하증 빈도

목적: 만성 신장 질환에서 원발성 갑상선 질환(갑상선 기능 저하증, 갑상선 결절, 갑상선종, 갑상선암) 빈도가 정상인보다 높다는 사실은 여러 역학 조사에서 이미 밝혀진 바 있다. 하지만 아이오딘 과다 섭취국 중 하나인 한국에서 신장기능과 원발성 갑상선 질환 빈도의 정확한 관계는 아직까지 연구된 바 없다. 방법: 2007년 1월부터 2010년 6월까지 국립중앙의료원 건강증진센터를 통해 검진 받은 2201명, 2008년 5월부터 2011년 5월까지 외래에서 진료를 보았던 투석을 요하지 않는 만성 신질환 환자 54명, 2010년 국립중앙의료원 인공 신장실에서 투석을 받고 있는 64명을 대상으로 하여 추정 사구체 여과율(MDRD-estimated glomerular filtration rate)에 따른 갑상선 기능 저하증의 빈도를 구하였다. 원발성 갑상선 기능 저하증과 신기능의 정확한 관계는 이변량 로지스틱 회귀 분석을 통하여 구하였다. 결과: 명백한 갑상선 기능 저하증 빈도는 GFR > 90 mL/min/1.73 m2 환자에서 0.5%, 투석을 요하는 만성 신부전 환자에서 6.3%로 증가하는 양상을 보였으며 또한 다변량 로지스틱 회귀 분석 결과 만성 신질환 환자에서 9.002의 오즈비를 보였다. 결론: 명백한 갑상선 기능 저하증의 빈도는 투석을 요하는 신부전 환자에서 흔하며 신기능 감소에 따라 증가한다. Background/Aims: Primary thyroid disease is prevalent in chronic renal disease, especially in patients requiring chronic dialysis. However, the relationship between kidney function and the prevalence of primary thyroid disease has not been studied in Korea, a nation where the dietary iodine intake is excessive. Methods: Retrospective data from 2201 consecutive adult patients who visited a health promotion center over the last 5 years were analyzed; specifically, 54 patients with chronic kidney disease not requiring hemodialysis and 64 adults treated with hemodialysis. The glomerular filtration rate (GFR) was estimated using the abbreviated Modification of Diet in Renal Disease equation. Multivariable logistic regression was used to evaluate the independent association between the prevalence of primary hypothyroidism and estimated GFR. Results: The prevalence of overt hypothyroidism increased from 0.5% at an estimated GFR > 90 mL/min/1.73 m² to 6.3% in end-stage renal disease requiring chronic dialysis (p < 0.001 for trend). Compared with participants with an estimated GFR > 60 mL/min/1.73 m², those with an estimated GFR < 60 mL/min per 1.73 m² had an increased odds of overt hypothyroidism after adjusting for age, gender, fasting blood glucose (FBS), and the total cholesterol (TC) and triglyceride (TG) concentrations. Conclusions: These findings suggest that overt primary hypothyroidism is relatively common (6.3%) among persons with chronic kidney disease requiring chronic dialysis, and it is independently associated with a progressively lower estimated GFR. <P>Abstract</P><P><B><I>Background:</I></B> This study was attempted to identify new molecules expressed on the plasma membrane of human umbilical vein endothelial cells (HUVECs) using monoclonal antibody-based proteomics technology and to determine the effect of the identified antibody on vascular reactivity. <B><I>Methods:</I></B> Twenty-two antibodies were developed from rats inoculated with HUVECs, and their effects were determined by observing vascular reactivity. <B><I>Results: </I></B>Among the 22 antibodies, the C-7 antibody significantly inhibited endothelium-dependent vasorelaxation in response to acetylcholine (ACh) but not to histamine. Moreover, the C-7 antibody did not affect norepinephrine-induced contraction in either the endothelium-intact or -denuded aorta. A proteomics study involving immunoprecipitation of the C-7 antibody with biotinylated HUVECs showed that this antibody binds to plasma membrane proteins corresponding to immunoglobulin heavy chain (VHDJ region), chaperonin-containing T-complex polypeptide 1 and α-actinin 4. The muscarinic M3 ACh receptor and α-actinin 4 were colocalized on the plasma membrane of HUVECs, and the colocalization was found to increase in response to ACh and was inhibited by pretreatment with the C-7 antibody. <B><I>Conclusions: </I></B>These results demonstrate that monoclonal C-7 antibody exerts an inhibitory effect on endothelium-dependent vasorelaxation induced by ACh and that this response may at least partially result from the inhibition of α-actinin 4.</P><P>Copyright © 2013 S. In Soil Taxonomy system, anthropogenic soils are still classified as Entisols since the International Classification Committee for Anthropogenic Soils is in the process of classifying anthropogenic soils as new orders. In reality, it is difficult to characterize anthropogenic soils because Soil Taxonomy (ST) system does not distinguish between natural and anthropogenic Entisols. On the other hand, World Reference Base for soil resources (WRB) considers human impacts on soils and contains an independent category of anthropogenic soils, which makes easier to understand anthropogenic soil characteristics than Soil Taxonomy system. A remodeled paddy field (Gasan) was selected to classify by ST and WRB. Soil samples were taken to analyze chemical and physical properties. Based on the results of the analyses, the ST system classified Gasan as coarse loamy, mixed, mesic, Aquic Udorthents while the WRB did as Stagnic Urbic Technosols (Oxyaquic, Arenic). As a conclusion, the WRB classification information of the anthropogenic provides more detail characteristics of the anthropogenic soils.
