        Clinical Outcome of Modified Cervical Lateral Mass Screw Fixation Technique

        Objective : The purpose of this study was 1) to analyze clinically-executed cervical lateral mass screw fixation by the Kim's technique as suggested in the previous morphometric and cadaveric study and 2) to examine various complications and bicortical purchase that are important for b-one fusion. Methods : A retrospective study was done on the charts, operative records, radiographs, and clinical follow up of thirty-nine patients. One hundred and seventy-eight lateral mass screws were analyzed. The spinal nerve injury, violation of the facet joint, vertebral artery injury, and the bicortical purchases were examined at each lateral mass. Results : All thirty-nine patients received instrumentations with poly axial screws and rod systems, in which one hundred and seventy-eight screws in total. No vertebral artery injury or nerve root injury were observed. Sixteen facet joint violations were observed (9.0%). Bicortical purchases were achieved on one hundred and fifty-six (87.6%). Bone fusion was achieved in all patients. Conclusion : The advantages of the Kim's technique are that it is performed by using given anatomical structures and that the complication rate is as low as those of other known techniques. The Kim's technique can be performed easily and safely without fluoroscopic assistance for the treatment of many cervical diseases.

        Hepatic resection for removal of lesion of the liver may be necessary for a wide variety of conditions. Partial hepatectomy for liver disease have become more common This reports describes a review of our experience for hepatic resection and an analysis of potential risk factors affecting the morbidity and the mortality in a hepatectomy methods : Between Jan 1,997 and Mar.2,001, we performed 112 cases of partial hepatectomy and retrospectively analyzed clinicopathological features of the presented. cases Results : The most common disease was intrahepatic duct stone(31.8%). The mean operative time was 324 minutes. The overall In-hospital mortailty an morbidity rates were 6.8%(3/44) and 59%(27/44), respectively, Various posmiddleerative complications developed, including 14 cardiopulmonary complications(53.8%), 8 wound Infections(30.7%), 2 Bile leakage(7%), 1 intraabdominal abscess(3.8%), 1 hepatic failure(38%), 1 posmiddleerative bleeding(3.8%). The old had more cardiopulmonary complications. The significant risk factors for perioperative mortality were high preoperative serum bilirubin level(above 2.0mg/dl), size of resection, massive operative bleeding(above 2,000cc), concomitant diseases, but cirrhosis was not a significant risk factor. Comparison of young age patients, the old had more complications, but it was not significantly Conclusion : Although the old patients had more cardiopulmonary complications but their operative mortality were not much different than younger patients significantly. Hepatic resection can be performed in over 60 years old with acceptable morbidity and mortality rates.

        호주의 유전자변형생물체(Genetically Modified Organisms) 안정성 심사절차 및 유전자변형식품의 표시제도

        1. 호주는 GMO의 실험 및 상업화를 규제하는 신법 "유전자기술에 관한 법(Gene Technology Act 2000)을 제정 운영중임 2. GMO를 관리 감독하기 위하여 보건후생성산하에 유전자기술 감독국(Office of the Gene Technology Regulator)이 신설됨 3. GMO를 취급(dealings)형태에 따라서 면제(exemption), 위험이 매우적은 취급(notifiable low risk dealing), 면허(licensing), 등록(register)의 4가지로 분류하여 감독함 4. GM식품의 안전성 심사는 호ㆍ뉴식품청(Australia New Zealand Food Authority, ANZFA)에서 관장하며 호주식품 강령 기준 18(Standard A18 of the Australian Food Standards Code)이 근거 법임 5. ANZFA에서는 제초제 저항성 콩, 옥수수, 면화, 감자, 카놀라 등 6작물 12종의 GM작물에 대하여 식품안정성 확인을 완료함 6. 호주에서는 2001년 12월 7일부터 유전자변형식품의 의무 표시제를 시행하고 있으며 비의도적 혼입 허용치는 1%로 설정함 As Australia's new national gene technology regulatory system began on 21 June 2001, the Office of the Gene Technology Regulator (OGTR) was newly established to regulate GMO related research and GM products throughout the nation. "Gene Technology Act 2000" is the statutory basis the new regulatory system. The OGTR is within the Commonwealth Department of Health and Aged Care, responsible for overseeing the gene technology research and development including the GMO field trials across Australia. For the regulation of genetically modified food, the Australia New Zealand Food Authority(ANZFA) is responsible for developing and reviewing national food standards. All foods available for sale in Australia and New Zealand must comply with the relevant national food standards. ANZFA also responsible for carrying out safety assessments of GM foods on behalf of the Commonwealth, State and Territory Governments of Australia and the Government of New Zealand. The ANZFA's safety assessment process for GM foods is based on concepts and principles that have been developed through international organizations such as the World Health Organization(WHO) and the Food and Agriculture Organization(FAO) and the Organization for Economic Co-operation and Development(OECD). Mandatory labelling of GM foods containing either introduced DNA or protein in the final food came into effect on 7 December, 2001.

        구내 골신장술 후 식립된 전치부 임플란트보철 치험보고

        Objective : This is to report the effectiveness of intraoral distraction osteogenesis for alveolar augmentation in the extremely atrophied alveolar defects and to review the clinical results of the 2 cases. Subjects and Methods : Anterior segmental osteotomy was performed and the trans-oral alveolar distractors(Martin, Germany) were applied in 2 patients with the severe acquired anterior maxillary and mandibular defect. After latent period for 1 week, the osteomized alveolar segments were distracted by 1 ㎜ a day (0.5 ㎜/l turn) for 8 days in case Ⅰ and 0.6 ㎜ a day(0.3 ㎜/l turn) for 10 days in case Ⅱ. The consolidation period was about 8 weeks in each case. Thereafter, 6 titanium threaded implants(Branemark, Restore) were simultaneously installed with removal of distractor. The implants were tested clinically and radiographically. Results : Amounts of acquired alveolar bone were 8 ㎜ in case Ⅰ and 6 ㎜ in case Ⅱ respectively, with the increased width of the ridge crests and soft tissue expansion. Dental implants installated on the augmented alveolar ridges in 8 weeks after distrction were confirmed as in good osseointegration and in good function without any complications. Conclusion : Intraoral distraction osteogenesis can be a good option for alveolar ridge augmentation of the severely atrophied ridges and soft-tissue defects. However, for the better vector control of distracting segments and for anterior esthetics, customized distraction devices could be considered later.

      • Kim,Seong-soo : Possiblity of Low Salting Process in Gae-Woo Jeot(Salted and Fermented Gastrophoda) and Changes in Fatty Acid Composition during Fermntation 低鹽化 可能性 檢討 및 熱成중 脂肪酸 組成의 變化

        To establish the way of processing of Gae-Woo jeot which used abalone intestine that has not been systematically used compare to its characteristic taste and fame as main ingredient and to develop as a local tourist good, the change of constituents the quality of salted fish products suring Gae-Woo jeot fermentation were examined. The results are as follows. 1. In the organoleptic test, Gae-Woo jeot which fermented through 65 days were dominant and the group of 105 salkt concentration were better than that of 20%. It was the best of 10% salted products to add small abalone. Both law salted(10%) and high salted (20%) Gae-Woo jeot were similar in general constityents, nutrients and texture. 2. Compositions of highly unsaturated acid were farely high to 45.6% and 46.1% in small abalone added froup and top sheel added group respectively. during 3 months fermentation low decreasing rate of 3~4% was shown. EPA(eicosapentaenoic acid) was approximately 7~8% and DHA(docosahexaenoic acid) was extremely low to be less than 0.3%. 3. In the changes General composition and pH, pH was within range of 4~5.5, 9~12% in protein, 3~5% in total sugar, 0.5~1% in lipid and approximately 1% in ash. 4. Gae-Woo jeot can be made with 10% low salting method, and 10% group gets all the better for taste, small abalone adding is recommended. 5. When low salt fermented fish of 10% is processed, it is highly recommended to put sorbitol, ethyl alcohol, lactic acid or other humectant for better taste and longer storage and good quality food additives must be developed for tourist and presently applied mil ordering transportation must be sought.

        Spontaneous retroperitoneal hemorrhage due to hemophilia A with impaired coagulopathy is very rare. Spontaneous retroperitoneal hemorrhage has been recorded as having originated from many retroperitoneal organs and blood vessels, and it may be due to local and/or systemic factors. In the majority of the patients, kidney and adrenal gland were the major site of hemorrhage. The systemic causes of spontaneous retroperitoneal hemorrhage are anticoagulation therapy and chronic hemodialysis. During the course of these treatments, hemorrhagic complications may occur at many site, including the retroperitoneal space. Blood dyscrasias including leukemia, polycythemia, sickle cell trait and hemophilia have been reported associated with spontaneous retroperitoneal hemorrhage.
