        모친구강보건지식이 유치우식증의 발생과 치료에 미치는 영향

        The purpose of this study was to assess the influence of mothers' oral health knowledge(OHK) on prevalence and treatment of deciduous dental caries in their children. 295 aged 4 and 5-year-old children and their mothers were selected in Seoul. The mothers who responded the pretested questionnaires were consisted of 16 items regarding home OHK, school OHK and community OHK. Data were collected by oral examination on children and questionnaire survey on their mothers. The results were as follow; 1. Mothers' OHK was an influencing factor of incidence of children's dental caries. Therefore, The relation of Mothers' OHK and the score of children's dmft was showed inverse relationship. 2. Mothers' home OHK was not an influencing factor of treatment of children's dental caries. It is suggested that comprehensive incremental oral health care for children should be educated for their mothers. 3. Mothers' school OHK was not an influencing factor of incidence of children's dental caries. 4. Mothers' community OHK was not an influencing factor of incidence of children' dental caries. 5. Mothers' school OHK and community OHK was not an influencing factor of treatment of children's dental caries. 6. It is recommended that school OHK and community OHK should be included on the process of oral health knowledge education.

      • Propylthiouracil에 의한 중증 급성간염 1예

        PTU에 의한 간염의 발생은 매우 드물게 발생하는 것으로 알려져 있으나 저자들은 Graves씨 병으로 진단 받고 propylthiouracil 투여를 받고있던 환자에서 중증의 급성 간염이 발생한 1예를 경험하였기에 보고하는 바이다. Propylthiouracil is widely used to treat patients with hyperthyroidism. This drug has been associated with severe hepatotoxicity rarely. We presented the case of jaundice and profound liver dysfunction from a 23-year old woman treated with propylthiouracil for hyperthyroidism. Viral, metabolic, and autoimmune liver disease could be excluded and liver biopsy revealed a pattern of acute hepatitis. After discontinuing the drug, there was a progressive resolution of hepatic symptoms and decrease in biochemical data of the liver. Despite propylthiouracil induced hepatitis in rare case, patients receiving propylthiouracil are exposed to develope severe hepatotoxicity. Therefore it might be advisable to monitor level of the transaminase on regular base from patients receiving propylthiouracil.

        Association between Oral Health Behaviors and Health Related Quality of Life in Korean Adults

        Objective: The purpose of this study was to examine association between oral health bahaviors and health related quality of life (HRQoL). Methods: The study used data from Korea National Health and Nutrition Examination Survey (KNHANES V: 2010-2012 & VI KNHANES VI: 2013–2015). A dependent variable, HRQoL, was decided as EuroQol-5 dimension (EQ-5D). Oral health behaviors were defined as the frequency of toothbrushing, the presence of interdental cleaning, and the presence of regular dental check-up and socioeconomic variables and health-related were collected. HRQoL was analyzed by social class, health behaviors, and oral health behaviors. Multiple logistic regression analysis was used to evaluate the association between oral health behaviors and EQ-5D. Results: The subjects with low social class, hypertension, diabetes mellitus had any problem with EQ-5D (p<0.001). The group with less tooth brushing frequency had problem with low HRQoL and high odds ratios (ORs) for low HRQoL (ORs [95% confidence Interval, CI]; mobility 1.56 [1.39-1.79], self-care 1.47 [1.15-1.89], usual activity 1.35 [1.16-1.56], pain/discomfort 1.10 [1.02-1.19], and anxiety/depression 1.19 [1.06-1.32]). The group without interdental cleaning had problem with low HRQoL and high ORs for low HRQoL (ORs [95% Cl]; mobility 1.52 [1.33-1.72], usual activity 1.18 [1.01-1.39], and anxiety/depression 1.14 [1.02-1.27]. The group without regular dental check-up had problem with low HRQoL and high ORs for low HRQoL (ORs [95% CI]; mobility 1.23 [1.09-1.41] and anxiety/depression 1.14 [1.02-1.28]). Conclusion: HRQoL was associated with oral health behaviors. Therefore, it is necessary that oral health promotion programs had better be planned with health promotion program to improve HRQoL

        Association between obesity and perceived halitosis in Korean adolescents

        Adolescent obesity is a growing problem, which is related to serious medical diseases, including poor oral health. Halitosis is an important part of oral health. Therefore, the present study aimed to examine the association between perceived halitosis and obesity in Korean adolescents. The present study used data from the 2015 Korea Youth Risk Behavior Web-based Survey in analyzing 68,043 Korean adolescents aged 12-18. The dependent variables, perceived halitosis, and socioeconomic variables, health-related variables, and oral health-related variables, were collected by using self-administered questionnaires. A multiple logistic regression analysis was used to evaluate the association between obesity and perceived halitosis while controlling for school type, city scale, economic status, education levels of parents, academic achievement, stress, physical activity, drinking, smoking, taking frequencies of fast foods, soda, and fruits, tooth brushing frequency, and interdental cleaning. In the descriptive analysis, adolescents with a low socioeconomic status were more likely to be obese and perceived higher halitosis. In addition, perceived halitosis was associated with low physical activity, high stress, and low tooth brushing frequency. After adjusting for confounders, there was a significant association between obesity and perceived halitosis [odds ratio (95% Confidence Interval (CI)) : 1.22 (1.06-1.42)]. Odds ratio (OR) of girls was 1.60 (95% CI : 1.17-2.20) and OR of boys was 1.15 (95% CI : 0.98-1.35). This study demonstrates that obesity is closely associated with perceived halitosis in Korean adolescents. Therefore, intervention programs are required to prevent obesity and malodors for adolescents on a national scale.

        Association between diet quality and untreated dental caries: results from the Korea National Health and Nutrition Examination Survey

        BACKGROUND/OBJECTIVES: Few studies have provided evidence of the association between diet quality and dental caries. This study aimed to examine the association between diet quality and untreated dental caries in a Korean representative population. SUBJECTS/METHODS: The study population included a sample of 13,815 participants, aged ≥ 19 from the Korea National Health and Nutrition Examination Sur vey during 2013–2015. The explanator y variable was diet quality and the outcome variable was untreated dental caries. Untreated dental caries were defined by the number of decayed teeth recorded according to the criteria established by the World Health Organization. Diet quality was defined by using the Korean Healthy Eating Index (KHEI) through the 24-h recall methods. We assessed the association between diet quality and untreated dental caries while adjusting for age, sex, education, income, smoking status, dental visits, toothbrushing frequencies, obesity, and diabetes mellitus. RESULTS: The mean overall KHEI scores in the untreated dental caries group were significantly lower than those in the group without untreated dental caries. Significant differences were obser ved in the untreated dental caries group based on the KHEI quartiles (P < 0.001). After adjusting for potential confounders, the quartiles of KHEI scores showed an association with untreated dental caries, demonstrating a dose-effect trend (odds ratio [OR], 1.57; 95% confidence inter val [CI], 1.35–1.84 for 1st quartile; OR, 1.38; 95% CI, 1.19–1.59 for 2nd quartile; OR, 1.32; 95% CI, 1.14–1.53 for 3rd quartile; reference quartile highest]). CONCLUSIONS: The findings indicated an inverse association between diet quality and untreated dental caries in Korean adults. Healthcare providers should take into account the significant role of diet quality in preventing and managing oral health.

      • Association between oral health status and stroke

        Objectives : The aim of this study, 284 stroke patient attacked within the latest 1 year and 219 non-stroke adults, matched for age, were surveyed for oral health status including the periodontitis, Subjects aged between 40 and 79 years. 47.7$ of the participants wee female, mean age was 59.66±10.47. All individuals were interviewed by trained interviewers using a questionnaire that focused on previous diseases, vascular risk factors(hypertension, diabetic mellitus, cardiac disease, hyperlipidemia), smoking and drinking habit, social history, previous and present medication. family vascular risk factor(familial hypertension history, familial diabetic mellitus history, familial cardiac disease history), BMI, the frequency of daily tooth brushing and the presence of regular dental visit. We registered the number of teeth, DMFT index, pocket depth in each individual. Results: Stroke patient had severer periodontitis than non-stroke patient(p<0.01), periodontitis was associated with stroke, showed OR of 5.44(95% Confidence Interval(CI)=3.03-9.77) with adjustment for covariates. Tooth missing of stroke patient was severer than that of non-stroke patient(p<0.05), tooth missing was assoviated with stroke, showed OR of 1.92(95%CI=1.01-3.63) with adjustment for covariates. Tooth attrition was associated with stroke with adjustment for covariates. And DMFT index, tooth abrasion, and tooth erosion were no associated wth stroke. Conclusion: Periodontal diseases, tooth missing, and tooth attrition were significantly associated with stroke.

        The effect of oral health program with powered toothbrush for the intellectually disabled

        Purpose: The aim of this study was to assess the effect of an oral health program with powered toothbrushes on reducing the dental plaque index in intellectually disabled persons. Methods: Instructions on tooth brushing with powered toothbrushes were given to 95 persons admitted to a welfare facility or a special school from May 2015 to September 2015. And 1 week later, the plaque index was re-examined and oral health behaviors and attitudes to oral health programs were surveyed. Results: First, the incidence of dental caries in disabled persons who received a dental check-up within less than 6 months and the DMFT index of disabled persons who received a major dental service at a private clinic were significantly low. Second, dental fees and transportation to the dental clinic were important for improving the oral health of disabled persons. Third, caregivers of disabled persons were in favor of an oral health program with a powered toothbrush. Fourth, the plaque index was decreased significantly after tooth brushing with powered toothbrushes. In conclusion, an oral health program with a powered toothbrush should be conducted in intellectually disabled persons to improve their oral health.

        Relationship between Perceived Oral Symptoms and Oral Health Status

        Objective: The purpose of this study was to examine the relationship between perceived oral health and oral health status. Methods: The oral examination and survey data of 219 subjects over 40 years of age were analyzed. All individuals were interviewed by self-reporting questionnaire that focused on previous diseases, vascular risk factors (hypertension, diabetic mellitus, cardiac disease), smoking and drinking habits, social history, BMI, the frequency of daily tooth brushing and the presence of regular dental visits. Dental survey was conducted in relations to the number of teeth, DMFT index, and the pocket depth. Results: 1. Better perceived oral health status was likely to rate their periodontal conditions as good with more teeth remaining and less dental caries. 2. With increasing DMFT index, perceived oral health and periodontal conditions were rated poorly with a severe dry mouth experience. 3. With high CPITN index, perceived periodontal conditions were rated poorly with less number of teeth remaining and a severe dry mouth condition. 4. In a multivariate analysis adjusted for socioeconomic status, hypertension, diabetes mellitus, frequency of daily tooth brushing and regular dental visits, the better perceived oral health status is, the lower DMFT index is. Conclusion: Self-rated oral health is a simple measure and reflects not only the objective oral status but also the socioeconomic status and the psychological effects of oral health. Thus, we should actively use the perceived oral health criteria to promote the nation's oral health.

        The Relationship among N-Terminal Pro-B-Type Natriuretic Peptide, High-Sensitivity C-Reactive Protein and Infarct Size in Patients with Acute ST-Elevation Myocardial Infarction

        <P><B>Background and Objectives</B></P><P>We sought to investigate the relationship between levels of high-sensitivity C-reactive protein (hs-CRP) and N-terminal pro-B-type natriuretic peptide (NT-proBNP) and the infarct size and left ventricular (LV) volume after acute myocardial infarction (MI).</P><P><B>Subjects and Methods</B></P><P>Eighty-six patients with acute ST-elevation MI underwent delayed enhancement multidetector computed tomography immediately after they underwent percutaneous coronary intervention to determine the infarct size. LV function and remodeling were assessed by echocardiography. Hs-CRP and NT-proBNP were measured at admission, 24 hours and two months later.</P><P><B>Results</B></P><P>Both hs-CRP and NT-proBNP at 24 hours showed a positive correlation with infarct size and a negative correlation with LV ejection fraction at the baseline and two months later. NT-proBNP at two months correlated with infarct size, LV ejection fraction, and LV end diastolic and systolic volume indices at two months. In patients with high NT-proBNP levels at 24 hours and two months, infarct size was larger and LV ejection fraction was lower. NT-proBNP was higher in patients who developed LV remodeling at two months: 929 pg/mL vs. 134 pg/mL, p=0.002. In contrast, hs-CRP at two months showed no relationship to infarct size, LV function, or LV volumes at two months.</P><P><B>Conclusion</B></P><P>Elevated hs-CRP level 24 hours after the onset of acute MI is associated with infarct size and LV dysfunction, whereas elevated levels of NT-proBNP 24 hours and two months after the onset of acute MI are both correlated with infarct size, LV dysfunction, and LV remodeling.</P>
