        예방진료에 대한 일부 치과병원 의료진의 인식 및 요구도 파악을 위한 질적 연구

        Objectives: This study aims to understand the perceptions regarding preventive dental services among dental professionals. Methods: In-depth interviews were conducted with three groups of dental professionals―dentists, dental hygienists, and dental hygienists that specialized in preventive service―who were selected using a convenience sampling method. Results: First, there was limited awareness among dentists regarding the need for preventive care, because of the treatment-oriented philosophy and scarcity of preventive care success models. Second, dental hygienists commented that there was a lack of the person in charge and systematic process for preventive services. It would not help when they decided to change working place even though they had high anxiety working for preventive care. However, both these professionals agreed that preventive treatment is important as the core of care and that it was important to develop various improvement methods to promote preventive services. Conclusions: To activate preventive services in dental clinics, it is essential to improve the operating system and encourage dental professionals to realize the importance of preventive services.

        Prevention and Treatment of Oral Diseases for the Disabled

        This article discusses the issue of oral hygiene management for the special need dental patients. The population with disabilities increases owing to the increase of aging population, traffic accidents and industrial disasters. But, the disabled have less knowledge about oral hygiene than ordinary people, or even when they are aware of such a dental problem, they don’'t properly manage oral hygiene due to their disabilities or difficulty in terms of management, the frequency of dental diseases occurring is quite high. Accordingly, before dental diseases occur to the disabled, it is necessary to recognize the importance of oral hygiene management and prevention through training more than anything else. Furthermore, it is recommended to perform preventive methods and oral hygiene training fit for each type of disorder for both patients and their guardians.

        Comparative Evaluation of Fracture Resistance of Roots Filled with Gutta Percha/AH Plus Sealer and Resilon/ Epiphany with Cold Lateral and Warm Vertical Compaction Technique-In Vitro Study

        Objective: This in vitro study compare and evaluate Fracture Resistance of roots filled with gutta percha-AH Plus sealer and Resilon Epiphany with cold lateral and warm vertical compaction techniques. Interface of dentin, sealer and core material of each group were also evaluated under scanning electron microscope. Methods: 125 single-canal extracted mandibular first premolars were divided into five groups: lateral and vertical compaction with gutta-percha/AH Plus sealer, lateral and vertical compaction with Resilon Epiphany, and a control group with no filling material. The samples were stored in 100 percent humidity for two weeks, mounted in acrylic resin and loaded to failure under Instron testing machine. One sample from each group was longitudinally sectioned, placed in dessicator, sputter coated with gold palladium and evaluated by using scanning electron microscope to check the interface of dentin, sealer and core filling material. Results: Resilon epiphany groups displayed higher mean fracture loads and the gutta-percha groups lower mean fracture load values than the control unfilled group. There is no gap seen between Resilon Epiphany & dentin in lower manification (500×) & gap seen between gutta percha, sealer & dentin interface. There was one microgap seen at 2,000× power SEM between the Resilon filling & dentin at middle 1/3rd. There was no gap at 2,000× power SEM between Resilon, Epiphany and dentin at coronal and apical third. Conclusion: Filling the root canals with Resilon/Epiphany increases the in vitro resistance to fracture of roots of extracted teeth with single canal.

        A Comparison of the Residual Amount of Bacteria According to Cleansing Methods of Removable Orthodontic Appliances

        Objective: Removable orthodontic appliances are cleaned in several ways. Measuring the amount of bacteria remaining on the device allows comparison of effective techniques to provide to the educational materials. Methods: Thirty orthodontics patients completed a questionnaire, and three weeks three times a week the other three methods to measure the amount of bacteria by ATP. They were shown cleaning methods using a cleansing agent, an ultrasonic cleaner, and a spin dryer cleaner. Results: RLU was reduced after cleansing, in each group (p<0.01). There was no statistically significant different of RLU among groups before cleansing (p=0.861) but there were significant differences after cleansing (p=0.011) and differences in the pre and post RLU count of each group (p=0.001). Conclusion: It is recommended to use a spin dryer cleaner or ultrasonic cleaner unit with a cleansing agent for removable orthodontic appliances, to achieve better hygiene.

        The Bond Strength of Ni-Cr Free Alloys of Recasting for Porcelain Fused to Metal

        Objective: This study was to find out the effect on bond strength to porcelain of reusing some Ni-Cr alloys that do not include Beryllium, as a harmful substance. Methods: Alphaloy (Alpha Dent. KOREA), Vera Bond 2V (AlbaDent, Inc. USA), T4 (Ticonum. USA) as non precious alloys without Beryllium and Noritake Super Porcelain EX-3 porcelain powder were used as experimental materials. 3 point flexual rigidity test was done using universal tester (Houndsfield, England) to measure bond strength of metal to porcelain and the following results were gained. Results: 1. In case of Alphaloy, significant discrepancy was shown with mean value being the highest in single use. And as a result of post-examination, it has been shown that there was difference between single use and 3 time reuse. 2. In case of Vera Bond 2V, the mean value was the highest in single use. But it was not a significant discrepancy. 3. In case of T4, there was significant discrepancy with the mean value being the highest in single use. The results of post-examination showed there was difference between single use, 3 time reuse and 5 time reuse. Conclusion: Establishing strategies on how we will use Ni-Cr alloys from now on and advanced studies for more systematic future study results should be done.

        Study of the Oral Discomfort and Oral Habits in Halitosis Patient

        Objective: Purpose of this study was to clarify the relationship between oral discomfort, oral habits and halitosis. Methods: One hundred and forty patients with halitosis (age range, 13 to 65 years; 93 women, 47 men) who had visited Shimizu dental clinic from October 2003 to August 2008 were compared with a control group who did not have complaints about halitosis (n=56). Oral discomfort (such as sensation of dry mouth and discomfort of the tongue), the amount of saliva secretion, and oral habits (such as bruxism) were analyzed statistically between the halitosis and control group. To compare the halitosis patients with the control group, the Wilcoxon signed-ranks test was used for the analysis of the salivary amount, and a chi-Square test was used for the analysis of oral discomfort and oral habits. Results: Frequency of oral discomfort mouth sensation was significantly higher (p<0.0001) in the halitosis patients (63.5%) than in the control group (10.7%). The occurrence of oral habit was significantly higher (p<0.001) in the halitosis patients (63.6%) than in the control group (37.5%). There was a significant difference (p=0.0009) between the halitosis patients (mean: 1.28 ml) and control group (mean: 1.43 ml) in the amount of unstimulated saliva secretion. For the amount of stimulated saliva secretion, there was no significant difference (p=0.2248) between the halitosis patients (mean: 3.40 ml) and control group (mean: 3.76 ml). Conclusion: These results suggest that the presence of oral discomfort and oral habits, such as bruxism, were associated with halitosis.

        Effect of Purified Curcuminoids on Enterococcus faecalis and Its Biofilm

        Objective: Curcuminoids is a group of natural compounds extracted and purified Tumeric rhizome (Curcuma longa Linnaeus). The objective of this study was to investigate the effects of its antimicrobial and antibiofilm properties on Enterococcus faecalis, a harsh pathogen for oral disease. Methods: Purified curcuminoids was tested for antimicrobial activity against E. faecalis by microdilution assay (minimal inhibitory concentration [MIC] and minimal bactericidal concentration [MBC]). Its antibiofilm characteristic was also measured on 96-well microtitre plate using crystal violet staining and colony forming unit counting techniques. To study the influence of curcuminoids in molecular level, gelatinase was chosen as a main component closely related to biofilm formation. Enzyme hydrolysis test was conducted and the expression of a relevant gene: gelE (gelatinase) was performed by semi-quantitative reverse transcription polymerase chain reaction. Results: Curcuminoids showed antimicrobial activity against E. faecalis, with MIC of 25 µg/ml and MBC of 50 µg/ml for planktonic cells, whist with MIC of 50 µg/ml and MBC of 500 µg/ml for biofilm forming cells. The inhibitory effect on biofilm formation was shown by the reduction of biofilm mass with the minimal concentration of 37.5 µg/ml. Gelatinase activity was also obviously inhibited. The gelE (gelatinase) expression was found down-regulated after 24 hours exposure of E. faecalis to purified curcuminoids. Conclusion: Curcuminoids has abilities to inhibit biofilm formation and growth of E. faecalis. These characteristics involve the mechanism of biofilm formation relating to gelatinase enzyme and gelE (gelatinase) of E. faecalis.

        Determination Factors of Dental Clinics of Foreigners Residing in Korea

        Objective: This study is aimed to be used as basic data to strengthen the competitiveness of dental services by investigating the satisfaction and the will to reuse dental services targeted at foreigner revisit with experience of domestic dental care. Methods: This study was conducted as a survey from November 31, 2014 to February 23, 2015 with some foreigners residing in Korea. The subjects of this study were 121 foreign residents in Korea. For questioning items, structured questionnaire was used. It was composed of 23 questions. Results: Among respondents, the more the age of low satisfaction, the lower were satisfied. This is the very reason that older patients have higher level of expectation for service than younger patients do. Satisfaction measurement for reservation & payment process was the highest in respondents who had stayed in Korea for 5 years (3.66%) and in respondents who had health insurance. It was found that scheduling appointment and making a payment were the main factors affecting patients to revisit followed by satisfaction rating the facilities. Conclusion: To attract more foreigners residing in Korea, efficient systems for scheduling appointment and dental insurance system for foreigners residing in Korea need to be developed. In the other institutions, we must be ready for communication for resolving language barrier between respondents

        Dental Survey of the Adolescent at Gwangju-Si for Water Fluoridation

        Objective: Dental survey was performed for adolescent at Gwangju-si order to evaluate the caries prevention effect of the water fluoridation for 15 years, and the questionnaire was performed to investigate such consciousness for caries or periodontal disease and toothbrushing habit for adolescent at Gwangju-si. Methods: Five hundred fifty-eight adolescents as 252 in age 12 years, 306 in age 15 were examined the oral health by 2 trained dentists and 3 staffs and caries experience data were collected to compare the data with the national survey. Results: Decayed, missing and filled teeth (DMFT) index in 12 years old was 1.10 and decayed, missing, and filled surfaces (DMFS) was calculated as 1.52. DMFT index in 15 years old was estimated as 2.59 and DMFS index was 3.79. Caries experience was lower in both age groups at Gwangju City than at middle city in national survey data (p<0.05). Conclusion: Caries experience was lower in adolescent group at Gwangju-si than in national survey, so, water fluoridation for 15 years might be effective for caries prevention at Gwangju area. It was recommended that water fluoridation at this city should be continued.

        Daily Intake of Fluoride on Foods for Children in Daegu

        Objective: The author has analyze the daily intake foods for preschool children and children, in order to measure the daily amounts of fluoride through food or toothbrushing. 90 young preschool and 90 school children were participated in this study by recording 5 days diet diary with the help of their parents. Methods: The average daily food intake was calculated and the fluoride amounts included in each food was examined through the past 5 reports. Fluoride amounts estimated by daily water intake and toothbrushing with dentifrice contaminated with 500 ppmF, were considered. Total amounts of daily fluoride by intake for young children and school children were estimated, in order to suggest to recommend the fluoride deficiency amount for prevention of dental caries as well as for helping growth and development. Results: Average 167.30±2.92 mgF (0.167 mgF) was estimated for preschool children and 314.27±13.10 mgF (0.314 mgF) for school children in non-water fluoridated area. There was a significant difference in average amount of fluoride intake, between in preschool children and school children (p<0.05). Condulusion: 1. It was estimated that the average 167.30±2.92 mgF (0.167±0.292 mgF) in young children and 314.27±13.10 mgF (0.314±0.131 mgF) were intake through the diet, water and tooth brushing, daily, and there was significantly different the amounts intake by young children and by school children, statistically (p<0.05). 2. It was estimated that the daily deficient amount of fluoride was 0.53 mg in young children and 1.18 mgF in school children. So, it needed some fluoride supplements for them, in order to contribute the caries prevention as well as for growth and development
