        Sequence of Thyroxine - Binding Globulin Gene in a Korean Family with Complete TBG Deficiency

        Genomic DNAs from an 11 year-old boy with suspected complete thyroxine-binding globulin deficiency(TBG-CD) and a normal individual were subjected to polymerase chain reactions and sequenced. The entire coding region and exon/intron boundary of TBG gene from a normal individual showed no base change in exons as reported. However, sequence of the TBG gene from the suspected individual revealed a single nucleotide deletion at codon 352 in exon 4. The mutation was further confirmed by allele-specific amplification of genomic DNAs from the affected hemizygous boy, a hemizygous normal male, and the heterozygous boy's mother with primers containing 3' end nucleotide missing in TBG-CD or normal sequence. Several distinct mutations in the coding regions of the TBG gene have been shown to be associated with TBG-CD. The Korean type identified in this study was found to be the same mutation type of TBG-CD in Japanese. In addition, we identified 3 more base changes in introns both in normal and TBG-CD subjects.

        Construction of a New Gene-Fusion Expression Vector,pMONSTER

        The Fur (ferric uptake regulation) expression vector pMON2064 was modified to produce a Fur-fusion expression vector. A kinker site, factor Xa cleavage site, and several restriction endonuclease sites were introduced to facilitate easy cloning and isolating of the fusion protein. The resulting fusion expression vector, pMONSTER, was then used to make fusion proteins with β-galactosidase and the protease of the human immunodeficiency virus type 1 (HIV-1 PR). Strain SW4020 harboring the Fur-β-galactosidase fusion vector produced blue colonies on a 5-bromo-4-chloro-3-indolyl-β-D-galactoside plate and the resulting 133kDa fusion protein reacted with an anti-Fur antibody. The strain harboring the Fur-HIV-1 PR fusion vector produced a 29kDa fusion protein, which also reacted with an anti-Fur antibody. The Fur-HIV-1 PR fusion protein was purified by a single column application that was designed to isolate the Fur protein. The purified FurHIV-1 PR fusion protein digested with factor Xa cleaved a recombinant Gag protein to release smaller fragments, including a p24 capsid protein. The Fur-HIV-1 PR fusion protein itself did not exhibit any proteolytic activity.

        The Korean Society for Neuro-Oncology (KSNO) Guideline for the Management of Brain Tumor Patients During the Crisis Period: A Consensus Recommendation Using the Delphi Method (Version 2023.1)

        Background During the coronavirus disease 2019 (COVID-19) pandemic, the need for appropriate treatment guidelines for patients with brain tumors was indispensable due to the lack and limitations of medical resources. Thus, the Korean Society for Neuro-Oncology (KSNO), a multidisciplinary academic society, has undertaken efforts to develop a guideline that is tailored to the domestic situation and that can be used in similar crisis situations in the future. Methods The KSNO Guideline Working Group was composed of 22 multidisciplinary experts on neuro-oncology in Korea. In order to reach consensus among the experts, the Delphi method was used to build up the final recommendations. Results All participating experts completed the series of surveys, and the results of final survey were used to draft the current consensus recommendations. Priority levels of surgery and radiotherapy during crises were proposed using appropriate time window-based criteria for management outcome. The highest priority for surgery is assigned to patients who are life-threatening or have a risk of significant impact on a patient’s prognosis unless immediate intervention is given within 24-48 hours. As for the radiotherapy, patients who are at risk of compromising their overall survival or neurological status within 4-6 weeks are assigned to the highest priority. Curative-intent chemotherapy has the highest priority, followed by neoadjuvant/adjuvant and palliative chemotherapy during a crisis period. Telemedicine should be actively considered as a management tool for brain tumor patients during the mass infection crises such as the COVID-19 pandemic. Conclusion It is crucial that adequate medical care for patients with brain tumors is maintained and provided, even during times of crisis. This guideline will serve as a valuable resource, assisting in the delivery of treatment to brain tumor patients in the event of any future crisis.

        Aerbactin Operon 의 Operator 와 Fur ( Ferric Uptake Regulation ) 단백질간의 결합하는 성질

        Binding properties of Fur protein to the operator region of aerobactin operon were investigated. By nitrocellulose filter binding assay, dissociation constant of Fur-operator complex was ranged 3∼6×10^(-12) M² using Mn^(2+) as a corepressor. Hill plot of the binding isotherm indicates that the repressor protein forms dimer in order to bind the operator. Stability of Fur-operator complex depends on the divalent transition metal involved in the complex. Binding affinity of the protein was not altered by the different incubation temperature. However, at higher temperature the repressor showed higher saturation level with the same amount of operator.

        Background During the coronavirus disease 2019 (COVID-19) pandemic, there was a shortage of medical resources and the need for proper treatment guidelines for brain tumor patients became more pressing. Thus, the Korean Society for Neuro-Oncology (KSNO), a multidisciplinary academic society, has undertaken efforts to develop a guideline that is tailored to the domestic situation and that can be used in similar crisis situations in the future. As part II of the guideline, this consensus survey is to suggest management options in specific clinical scenarios during the crisis period. Methods The KSNO Guideline Working Group consisted of 22 multidisciplinary experts on neuro-oncology in Korea. In order to confirm a consensus reached by the experts, opinions on 5 specific clinical scenarios about the management of brain tumor patients during the crisis period were devised and asked. To build-up the consensus process, Delphi method was employed. Results The summary of the final consensus from each scenario are as follows. For patients with newly diagnosed astrocytoma with isocitrate dehydrogenase (IDH)-mutant and oligodendroglioma with IDH-mutant/1p19q codeleted, observation was preferred for patients with low-risk, World Health Organization (WHO) grade 2, and Karnofsky Performance Scale (KPS) ≥60, while adjuvant radiotherapy alone was preferred for patients with high-risk, WHO grade 2, and KPS ≥60. For newly diagnosed patients with glioblastoma, the most preferred adjuvant treatment strategy after surgery was radiotherapy plus temozolomide except for patients aged ≥70 years with KPS of 60 and unmethylated MGMT promoters. In patients with symptomatic brain metastasis, the preferred treatment differed according to the number of brain metastasis and performance status. For patients with newly diagnosed atypical meningioma, adjuvant radiation was deferred in patients with older age, poor performance status, complete resection, or low mitotic count. Conclusion It is imperative that proper medical care for brain tumor patients be sustained and provided, even during the crisis period. The findings of this consensus survey will be a useful reference in determining appropriate treatment options for brain tumor patients in the specific clinical scenarios covered by the survey during the future crisis.

        순수 분리된 Fur 단백질과 Aerobactin Operon의 작동자 부분을 포함하는 DNA 절편간의 결합하는 성질을 Nitrocellulose filter binding assay에 의하여 측정하였다. Fur 단백질은 $Fe^{2+}$ 이외에도 많은 전이 금속을 보조 억제 인자로서 수용하는 것으로 보인다. 얻어진 Fur-작동자 결합의 해리상수는 $Mn^{2+}$을 보조 억제 인자로 사용하였을 경우$(3{\sim}6){\times}10^{-12}M^{2}$이었으며 Fur 단백질은 Dimer로서 작동자와 결합하는 것으로 나타났다. Fur-작동자 결합을 $43^{\circ}C$에서 수행 하였을 때는 $23^{\circ}C$ 또는 $37^{\circ}C$ 때와 동일한 해리상수를 얻었다. 그러나 Fur 단백질은 고온에서는 동량으로 첨가된 작동자로부터 더 높은 포화 수치(saturation level)를 보여준다.         글루탐산 생성 박테리아에 관한 생화학적 연구

        Isocitrate lyase and malate synthetase were found to be active in Brevibacterium anamoniagenes grown on the acetate media, indicating that the glyoxylate cycle was operated. The specific activity of the isocitrate lyase from the acetate media showed about 70 times greater than that from the glucose media. The optimum pH for the partially purified isocitrate lyase was 7.0∼7.2 and Km value for the DL-isocitrate was determined as 0.41 mM. The effects of divalent ions were studied. Magnesium ion was an effective activator, but calcium, cobalt, cupric, ferrous, and manganese ions were found to precipitate in the assay mixtures. Iodoacetate, p-hydroxymercuribenzoate and phosphoenolpyruvate were the strong inhibitors for the enzyme. Phosphoenolpyruvate inhibited the enzyme to 78% at pH 6.85 and 47% at pH 7.25. A possible mechanism of the phosphoenolpyruvate to the regulation of the glyoxylate cycle was discussed.

        The Korean Society for Neuro-Oncology (KSNO) Guideline for WHO Grade II Cerebral Gliomas in Adults: Version 2019.01

        Background There was no practical guideline for the management of patients with central nervous system tumor in Korea for many years. Thus, the Korean Society for Neuro-Oncology (KSNO), a multidisciplinary academic society, has developed the guideline for glioblastoma. Subsequently, the KSNO guideline for World Health Organization (WHO) grade II cerebral glioma in adults is established. Methods The Working Group was composed of 35 multidisciplinary medical experts in Korea. References were identified by searching PubMed, MEDLINE, EMBASE, and Cochrane CENTRAL databases using specific and sensitive keywords as well as combinations of keywords regarding diffuse astrocytoma and oligodendroglioma of brain in adults. Results Whenever radiological feature suggests lower grade glioma, the maximal safe resection if feasible is recommended globally. After molecular and histological examinations, patients with diffuse astrocytoma, isocitrate dehydrogenase (IDH)-wildtype without molecular feature of glioblastoma should be primarily treated by standard brain radiotherapy and adjuvant temozolomide chemotherapy (Level III) while those with molecular feature of glioblastoma should be treated following the protocol for glioblastomas. In terms of patients with diffuse astrocytoma, IDH-mutant and oligodendroglioma (IDH-mutant and 1p19q codeletion), standard brain radiotherapy and adjuvant PCV (procarbazine+lomustine+vincristine) combination chemotherapy should be considered primarily for the high-risk group while observation with regular follow up should be considered for the low-risk group. Conclusion The KSNO’s guideline recommends that WHO grade II gliomas should be treated by maximal safe resection, if feasible, followed by radiotherapy and/or chemotherapy according to molecular and histological features of tumors and clinical characteristics of patients.

      • 6-Br-Pyridoxal-5-Phosphate a New Cofactor Analog of GABA Transaminase

        여러 효소의 자연적인 보조인자인 pyridoxal-5-phosphate의 새로운 유사체로서 6-Br-pyridoxal-5-phosphate를 유기합성과 효소를 이용한 합성방법에 의해 순수분리 정제하였다. 자외선 흡수 또는 형광 분광법에 의해 이 보조인자 유사체는 신경조직에서 신경전달물질의 하나로 알려진 GABA (4-aminobutyate)의 분해효소인 GABA transaminase의 보조인자 결합자리에 붙어서 resolved 또는 reduced GABA transaminase의 효소 활성도를 복구시켰다. 이러한 사실들은 6-Br-PLP가 GABA transaminase의 catalytic site에 있는 아미노산과 공유결합을 하여 자연적인 보조인자와 같은 역할을 한다고 볼 수 있다. 6-Br-pyridoxal-5-phosphate, a new cofactor analog of pyridoxal-5-phosphate, was synthesized and purified homogeneously by organic and enzymatic methods. This cofactor analog binds to cofactor binding site of GABA transaminase. Resolved and reduced GABA transaminase restore the catalytic activity by 6-Br-PLP. These results indicate that the 6-Br-PLP remains covalently attached to the amino acid residue of the catalytic site and acts like natural cofactor.

        Clinical Characteristics of High-Grade Glioma with Primary Leptomeningeal Seeding at Initial Diagnosis in a Single Center Study

        Background High-grade glioma (HGG) with primary leptomeningeal seeding (PLS) at initial diagnosis is rare. The purpose of this study was to identify its clinical features and to describe the clinical treatment outcomes. Methods We retrospectively reviewed the medical records of patients with HGG (World Health Organization grade III or IV) at our institution between 2004 and 2019, and patients with PLS at the initial diagnosis were enrolled in the study. Clinical features, such as the location of leptomeningeal seeding, surgical methods, and degree of resection, were sorted based on electronic medical records also containing performance scale, and hematological and serological evaluations. Radiological findings and immunohistochemical categories were confirmed. Furthermore, we sought to determine whether controlling intracranial pressure (ICP) via early cerebrospinal fluid (CSF) diversion increases overall survival (OS) after the initial diagnosis. Results Of the 469 patients with HGG in our institution, less than 2% had PLS at the initial diagnosis. Most patients suffered from headache, diplopia, and dizziness. Pathological findings included 7 glioblastomas and 2 anaplastic astrocytomas. Seven of the 9 patients underwent CSF diversion. All patients were administered concurrent chemoradiotherapy (CCRT) with temozolomide, 89% of which started adjuvant temozolomide and 33% of which completed the six cycles of adjuvant temozolomide. The OS of patients with HGG and PLS was 8.7 months (range, 4-37), an extremely poor result compared to that of other studies. Also, the 1-year and 2-year OS rates were 44.4% and 16.7%, respectively. Conclusion Diagnosis and treatment of HGG with PLS are challenging. Aggressive control of ICP followed by early initiation of standard CCRT seems to be helpful in improving symptoms. However, despite aggressive treatment, the prognosis is poor. A multicenter trial and research may be necessary to create a standardized protocol for this disease.
