Approach to cytomegalovirus infections in patients with ulcerative colitis

Cytomegalovirus (CMV) reactivation is common in patients with severe ulcerative colitis (UC), and may ref lect exacerbation of mucosal inf lammation and/or administration of immunosuppressants. The question of whether CMV is an active pathogen or `an innocent bystander` in the exacerbation of UC remains controversial. Patients with UC exacerbated by reactivated CMV experience worse prognoses than those without CMV reactivation and antiviral therapy significantly reduces the need for colectomy in patients with severe UC and high-grade CMV infection, indicating that CMV plays a role in UC prognosis. Therefore, the CMV status of patients on immunosuppressants, particularly those with steroid-refractory or -dependent UC, should be tested. When CMV is detected, be performed based on should adequate treatment the extent of the viral load and the presence of certain clinical features including a large ulcer. Anti-tumor necrosis factor agents may be useful for treating CMV colitis complicating UC.
Downregulation by lipopolysaccharide of Notch signaling, via nitric oxide.

<P>The Notch signaling pathway appears to perform an important function in inflammation. Here, we present evidence to suggest that lipopolysaccharide (LPS) suppresses Notch signaling via the direct modification of Notch by the nitration of tyrosine residues in macrophages. In the RAW264.7 macrophage cell line and in rat primary alveolar macrophages, LPS was found to inhibit Notch1 intracellular domain (Notch1-IC) transcription activity, which could then be rescued by treatment with N(G)-nitro-l-arginine, a nitric oxide synthase (NOS) inhibitor. Nitric oxide (NO), which was produced in cells that stably express endothelial NOS (eNOS) and brain NOS (bNOS), also induced the inhibition of Notch1 signaling. The NO-induced inhibition of Notch1 signaling remained unchanged after treatment with 1H-[1,2,4]oxadiazolo[4,3-alpha]quinoxalin-1-one (ODQ), a guanylyl-cyclase inhibitor, and was not found to be mimicked by 8-bromo-cyclic GMP in the primary alveolar macrophages. With regards to the control of Notch signaling, NO appears to have a significant negative influence, via the nitration of Notch1-IC, on the binding that occurs between Notch1-IC and RBP-Jk, both in vitro and in vivo. By intrinsic fluorescence, we also determined that nitration could mediate conformational changes of Notch1-IC. The substitution of phenylalanine for tyrosine at residue 1905 in Notch1-IC abolished the nitration of Notch1-IC by LPS. Overall, our data suggest that an important relationship exists between LPS-mediated inflammation and the Notch1 signaling pathway, and that this relationship intimately involves the nitration of Notch1-IC tyrosine residues.</P>

This paper illustrates the basic concept and architecture of Relative navigation in TDLS and a relativenavigation simulation platform which is used as a basic test bed of TDLS. The simulation platform allows users todetermine the simulation condition through GUI and makes it possible to generate the trajectories and TOAmeasurement according to a given mission. Using the designed platform, the effects of several important designparameters such as the geometry, the navigation algorithm and the data transmission duration are analyzed.

Background/Aims: Well designed staging system of hepatocellular carcinoma(HCC) is needed for comparative therapeutic studies regarding HCC to be of value. The aim of study is to reevaluate prognostic factors affecting natural history of HCC and to devise a staging system according to prognostic index estimated mathematically Methods: One hundred eighty patients with HCC were analyzed retrospectively. All patients received only conservative treatments. Univariate and multivariate analysis were performed using clinical, biochemical and imaging data obtained at diagnosis. Results: The overall acturial survival rate at 1, 3, 6, 12, 24 months were 74%, 33%, 11%, 7%, 5%, respectively. For significant prognostic variables in the univariate analysis, multivariate survival analysis disclosed that tumor size(p=0.0039), ascites(p=0.0190), total serum bilirubin(p=0.001) were independent prognostic factors. Considering the contribution of each of these factors to prediction of survial, a pronostic index(PI) was defined as a following regression equation. PI=EXP(tumor size×0.1904+ascites×0.4114+Bilirubin×0.0504). According to PI, a new staging system was devised allowing division of patients into three groups with significantly different survival rates(p<0.01). Conclusions: The new staging system for HCC according to PI would be useful for the staratification of HCC patients in the comparative evaluation of various therapeutic modalities.(Korean J Gastroenterol 1997;29:786-796)
방기황기탕이 Monosodium Iodoacetate로 유발된 흰쥐의 골관절염에 미치는 영향

ObjectivesWe have studied to know effects of Bangkeehwangkee-tang on the mono-sodium iodoacetate (MIA) induced osteoarthritis in rats. Methods Osteoarthritis was induced by injection of monosodium iodoacetate into knee joint cavity of rats. Rats are devided into 4 groups; Sham, control, Bangkeehwangkee-tang, Shinbaro. Sham group was injected by normal saline into knee joint cavity only. Control group was taken no treatment and Experimental groups were taken extracts of Bangkee-hwangkee-tang and Shinbaro by orally once a day for 4 weeks. The content of TNF-α, IL- 1β and IL-6 in synovial fluids were analysed. COX-2 in chondrocytes and blood PGE2 concentration were analysed. Histopathological examination was performed by Safranin-O fast green staining and Mankins score checking. Results The content of TNF-α, IL-1β in experimental groups were decreased com-pared with control group. COX-2 in chondrocytes and blood PGE2 concentration in ex-perimental groups were decreased compared with control group. Histopathologically, os-teoarthritic scores of experimental groups were decreased compared with control group. But the content of IL-6 in synovial fluids was not significantly compared with control group. Conclusions According to these results, it can be suggested that Bangkeehwangkee- tang has anti-arthritic effects on the MIA induced osteoarthritis in rats. (J Korean Med Rehab 2014;24(2):15-29)

Background : Cervicovaginal cytology is a screening test of uterine cervical cancer. The sensitivity of cervicovaginal cytology is less than 50%, but studies of cytologic/histologic correlation are limited. We analyzed the diagnostic accuracy of cervicovaginal cytology in the detection of the squamous epithelial lesions of the uterine cervix and investigate the cause of diagnostic discordance. Materials and Methods : We collected a total of 481 sets of cervicovaginal cytology and biopsies over 5 years. The cytologic diagnoses were categorized based on The Bethesda System and the histologic diagnoses were classified as negative, flat condyloma, cervical intraepithelial neoplasia (CIN) I, CIN II, CIN III, or squamous cell carcinoma. Cytohistologic discrepancies were reviewed. Results: The concordance rate between the cytological and the histological diagnosis was 79.0%. The sensitivity and specificity of cervicovaginal cytology were 80.6% and 92.6%, respectively. Its positive predictive value and negative predictive value were 93.7% and 77.7%, respectively. The false negative rate was 19.4%. Among 54 false negative cytology cases, they were confirmed by histology as 50 flat condylomas, 2 CIN I, 1 CIN III, and 1 squamous cell carcinoma. The causes of false negative cytology were sampling errors in 75.6% and interpretation errors in 24.4%. The false positive rate was 7.4%. Among 15 false positive cytology cases, they were confirmed by histology as 12 atypical squamous cells of undetermined significance (ASCUS) and 3 low grade squamous intraepithelial lesions (LSIL). The cause of error was interpretation error in all cases. The overall diagnostic accuracy of cervicovaginal cytology was 85.7%. Conclusions : Cervicovaginal cytology shows high overall diagnostic accuracy and is a useful primary screen of uterine cervical cancer.

Bisphosphonate-related osteonecrosis of the jaw (BRONJ) is a side effect of Bisphosphonates (BPs) use. These days, oral bisphosphonates are normally prescribed to treat osteoporosis. Intravenous BPs are used extensively to treat osteolytic bone lesions related to multiplemyeloma and bone metastasis of solid cancers, breast cancer or prostate cancer. As the prescription of BPs is universalized and the number of people treated with BPs is increasing, an accurate understanding and proper management of BRONJ are required. The aim of this study was to improve the clinicians’understanding of BRONJ by reviewing the literature. To achieve this, this paper introduces case reports as well as the current concept of BRONJ based on the 2009 updates by American Association of Oral and Maxillofacial (AAOMS) including the definition, epidemiology, etiology, diagnosis, treatment and prevention of BRONJ.
