      • S-344 : The Role of Skeletal Muscle Mass in Development of Metabolic Syndrome

        Introduction: It has been well known that abdominal adiposity is related with insulin resistance and greater risk of metabolic disorder. Skeletal muscle plays central role in insulin mediated glucose disposal of whole body, but we only know a little about the association between skeletal muscle mass and MetS (MetS). The aim of this study is to clarify the clinical role of skeletal muscle mass in developing MetS through using skeletal muscle parameters by body impedance analysis (BIA). Methods: 1,042 healthy adults aged from 20 to 75 years who visited Yeungnam university health promotion center from Jun. 2008 to Jun. 2010 were enrolled. 204 subjects who had prior MetS or chronic disease that can affect to skeletal muscle mass were excluded. After 24 months from baseline, the metabolic parameters were assessed and the development of MetS was diagnosed using modified NCEP-ATP III criteria. From the BIA (Inbody 720), we obtained skeletal muscle mass (SMM, Kg), body fat mass (BFM, Kg), and visceral fat area (VFA, cm2). Then, we had defined as follows; percent of skeletal muscle mass (SMM%, %): SMM (kg)/weight (Kg), skeletal muscle index (SMI, Kg/m2): SMM (Kg)/height (m)2, skeletal muscle to body fat ratio (MFR): SMM (kg)/BFM (Kg) and skeletal muscle to visceral fat ratio (SVR, Kg/cm2): SMM (Kg)/VFA (cm2) Results: Mean follow up periods were 28.7±5.4 months. Among total 838 subjects (46.9±9.9 years, M:F=477:361), 88 (10.5%) were newly diagnosed MetS. 5th quintile of the SMM%, MFR and SVR was associated with decreased risk of development of MetS after adjusting for confounding factors. Conclusion: Decreased skeletal muscle mass may play critical role in development of the MetS and not absolute amount of skeletal muscle mass but relative ratio to body composition may be more important.

        CT사진을 이용한 한국인의 C형 치근관의 분포에 대한 연구

        Purpose : To investigate the incidence and prevalence of C-shaped root canal using computed tomographic images of head and neck in Korean population. Materials and Methods : Regardless of each examination purpose, randomly selected 268 examples which have serial axial computed tomographic images with 8 normal molars in maxilla and mandible were selected and investigated. Defined C-shaped root canal and we could get 82 proper image examples in view of this definition. These were detected and investigated of incidence and prevalence of C-shaped root canal. Results : C-shaped root canals were found in 82 examples among 268 (30.6%) and 147 teeth. They were only found on molar area, and the highest incidence was 37.41% of left mandibular second molars, and the lowest was 2.04%of right maxillary second molars. On prevalence of tooth position, mandibular second molar was the highest, 65% of C-shaped root canal teeth and maxillary second molar was 6% the lowest. Conclusion : 31% of randomly selected CT images no considering check-up reason have C-shaped root canals. The C-shaped root canals of mandibular second molar are found most frequently and they are also detectable on maxilla. On Korean population, C-shaped root canals are one of conditions that have to concern with dental treatment for it appears at a high prevalence relatively.

      • 방선균 J-144K가 생산하는 Adenosine Deaminase Inhibitor에 관한 연구

        Adenosine deaminase의 새로운 저해제를 검색하기 위해 토양으로부터 adenosine deaminase 저해제를 생산하는 8균주의 방선균을 분리하였으며, 그중에서 저해활성이 가장 우수한 J-144K균주를 공시균주로 선정하였다. Adenosine deaminase 저해제 생산을 위한 최적배지 조성은 1.0% dextrose, 0.5% yeast extract, 0.5% peptone, 0.1% KH_2PO_4이었으며, 초적온도와 pH는 각각 30˚C와 7.0이었다. 이러한 조건에서 500ml용 진탕플라스크에 최적배지 100ml를 넣어 배양했을 경우 정지기인 배양 60시간째에 adenosine deaminade 저해활성이 가장 높게 나타났다. 분리균주 J-144K의 배양상등액으로부터 활성탄 흡착 및 유기용매 추출, Dowex 50 H^+ X-8 column chromatography, Dowex 1 Cl^- X-8 column chromatography, Sephadex G-15 gel filtration 등의 과정을 거쳐 adenosine deaminase 저해제를 정제한 결과, 3가지 종류의 저해제가 존재함을 알 수 있었다. In the screening of actinomycetes' culture filtrate for inhibitor of adenosine deaminase, a novel inhibitor was found in a cultured broth of strain J-144K. The optimum conditions for the adenosine deaminase inhibitor production from the isolated strain J-144K were evaluated. This strain showed the maximum yield of adenosine deaminase inhibitor when grown at pH 7.0 and 30˚C for 60 hours in the medium of 1.0% dextrose, 0.5% yeast extract, 0.5% peptone and 0.1 % KH_2PO_4 under the aerobic condition. Through the activated charcoal extraction, methanol fractionation, Dowex 50 H^+X-8 ion exchange column chromatography, Dowex Cl^- X-8 ion exchange column chromatography, and Sephadex G-15 gel filtration procedures, this inhibitor was purified with three materials.

        2인과 3인의 기본 심페소생술 비교

        Study objectives: To compare the quality of CPR provided by medical students and pre-EMTs perfoming three-rescuer CPR with that achieved by same students trained to provide standard two-rescuer CPR and to find the most effective method of resuscitation and to educate this methods to students. Material and methods: From July 1 to September 30, 1996, we had trained 60 groups consisted of the medical students, students of EMS technology and pre-EMT. Each group practiced two-rescuer CPR and three-rescuer CPR for 120 minutes with Resusci? Anne and perfomed CPR for at least 3 minutes without feedback. To eliminate any visual cues of perfomance the mannequin's recorder was hidden. Each actions were recorded by Resusci? skillmeter and Laerdal printer. To compare the quality of two-rescuer CPR and three-rescuer CPR, we analyzed the data by SPSS with paired t-test. Results: Three-rescuer teams delivered a mean minute ventilation and a mean minute cardiac compression substantially greater than that produced by two-rescuer teams(11.30±1.85 vs 9.09±2.22, p<0.001; 99.22±12.42 vs 95.12±15.75, p<0.001). Three-rescuer teams produce the more correct actions in chest compression and ventilation(93.84±7.56% vs 83.49±16.94%, 90.90±9.33% vs 77.00±17.06%). Conclusion: Three-rescuers can produce better CPR than two rescuers when a bag-valve-mask devices is used and the technique is easily learned and readily retrained, so we think that repeated education and training of this CPR methods to students is needed.

        저가형 혼합항법시스템의 오차보정

        본 연구에서는 저가형 GPS 수신기와 자이로 및 속도계를 결합한 혼합항법시스템을 개발하여 그 정확도를 실험하였다. 본 혼합항법시스템에서는 기존의 강결합 방식이나 연결합 방식과 달리 변형된 결합방식을 채택하여 GPS 수신기나 자이로 혹은 속도계를 자유롭게 선택할 수 있도록 하였다. 실험 결과, 본 혼합항법시스템은 기존의 항법 시스템보다 정확도가 향상되었으며 다중 경로오차를 상당히 제거할 수 있음이 밝혀졌다. 또한 본 항법시스템은 후처리 지리정보자료획득의 측면에서 유리한 것으로 나타났다. In this study, a hybrid navigation system with a low-cost GPS Receiver plus Gyro and Odometer is developed and tested. This hybrid navigation system adopted a modified coupling method which can be distinguished from tightly coupled method or loosely coupled method, so that GPS receivers or Gyros or Odometers can be chosen arbitrary. Comparing to the existing hybrid navigation system, the test results show that this navigation system enhances the accuracy and is robust against the multipath error. It is also proven that this system has an advantage of acquiring GIS data for post processing.

      • 고배속 CD-RW Drive을 위한 중점 서보 제어

        현대의 CD-ROM/CD-RW 드라이브는 보다 빠른 배속, 높은 안정성, 빠른 데이타 억세스를 요구하므로 전체 시스템은 더욱 더 복잡해지고 고성능의 서보 시스템을 필요로 한다. 따라서 이러한 추세에 발맞추어 시스템의 요구를 충족시키는 보다 정밀하고 지능적인 서보 시스템의 개발을 통해 기술력을 확보하는 것이 시급한 과제이다. 본 연구에서는 고배속 CD-ROM/CD-RW의 스텝 모터의 빠른 응답속도는 슬레드 장거리 서치후 트래킹 풀-인시 엑츄에이터의 관성 때문에 TE의 오버슈트가 크게 발생하여 풀-인이 불안하거나, 실패하는 것을, 슬래드 서치중에도 트래킹 엑츄에이터의 흔들림을 보상하는 중점 서보 제어기법을 제안하여 스텝 모터의 안정성을 높였다. This thesis presents a design methodology of a Digital Servo Signal Processor for high speed CD-ROM drive systems. The proposed Digital Servo Signal Processor enables us to develop CD-related systems for the very high speed applications and is one of the key components of the CD-ROM systems. The proposed center compensation servo control is newly built for a actuator shaking due to the fast response of a step motor when it jumps to a long distance. From experimental results, we can see that the performance of the control system is improved greatly. The proposed servo algorithm shows a shorter setting time including a pull-in time and a faster access time. It can be applied easily to the DVD-ROM and the DVD-RAM which have the same optical structure.

      • 비선형 시스템을 위한 퍼지 모델 기반 적응 슬라이딩 모드 제어기의 설계

        본 연구에서는 계통의 동특성식이 상태 변수와 제어 입력 모두에 비선형 함수인 비어파인(nonaffine) 계통을 효과적으로 제어할 수 있는 적응 퍼지 제어기를 설계한다. 제어기를 설계하는데 있어서 기존 적응 퍼지 제어기 설계 기법에서 사용된 여러 기법들을 응용하여 일반적 비선형 계통에 대한 견실한 제어 기법을 제안하고자 한다. 범용 근사화기(universal approximator)로서 미지의 함수를 원하는 정밀도로 근사화 시킬 수 있는 퍼지 시스템의 특성을 이용하여 미지의 비어파인 계통을 근사화 하며 이때 Takagi-sugeno가 제안한 퍼지 규칙의 후건부가 입력의 함수 형태로 표현되는 퍼지 시스템을 이용한다. The control of systems with complex and unknown nonlinear dynamics becomes a topic of considerable research importance. Many researchers have dealt with the issue, and many efficient algorithms and applicable methods have been presented. Recently, several fuzzy(or neural network) control approaches which guarantee the Lyapunov stability of the closed-loop system have been proposed. The advantages of using fuzzy systems are no needs of exact plant dyanmics and linear-in-the-parameters condition on the unknown nonlinearities. But most of these research works in nonlinear controller design has been applied only to simple classes of nonlinear systems such as affine systems. In this thesis, an indirect adaptive feedback-linearizing controller is developed for the nonaffine nonlinear systems. The considered nonlinear system is represented and adaptively identified by a Takagi-Sugeno fuzzy model. To verify the effectiveness of the proposed control method, computer simulations are carried out for the simple second order single-input single-output nonaffine nonlinear system.

      • 림프절 종창의 임상적 관찰

        Objectives : Clinical management of patient with lymphadenopathy requires judicious judgement of clinicians. Careful evaluation of lymph node enlargement by various clinical parameters will help to disclose the etiology of lymph node enlargement. This study was conducted to identify the clinical manifestations and etiology of lymphadenopathy in korean patients. Methods : A total 192 patients with lymph node enlargement were studied retrospectively. All patients who underwent excisional lymph node biopsy during the period from January 1992 to July 1994 were included. The lymph node biopsy and clinical records were reviewed and patient characteristics were analyzed by various clinical parameters. Results : A female predominance was noted in lymphadenopathy of tuberculosis (58.5%), Kikuchi's syndrome (70%). In lymphadenopathy of nonspecific, tuberculosis, kikuchi syndrome, per centage of patients younger than 40 years of age were 84.5%, 75.6%, 100%, respectively. In lymphadenopathy of tuberculosis and metastatic cancer, the duration of lymphadenopathy below 30days were 51.2%, 73.9%, respectively. The two most common etiology of cervical lymphadenopathy were nonspecific lymphadenitis (47.7%) and tuberculous (26.8%). The two most frequent etiology of supraclavicular lymphadenopathy were nonspecific lymphadenitis (46.3%) and metastatic lymphadenopathy (26.8%) and the most common etiology of axillary lymphadenopathy was nonspecific lymphadenopathy (34.5%). The most common frequent of lymph node enlargement with size below 1㎠ was nonspecific lymphadenitis, while above 2.25㎠ it was metastatic lymphadenopathy. Conclusion : Careful evaluation regarding patient age, sex, size of lymph node, consistency, location of lymph node, presence or absence of systemic symptoms should be pursued before performing a biopsy and management.
