        공수병의 일반적 특징과 교상 후 치료

        Rabies is a representative zoonotic disease caused by rabies virus. The discriminative features of rabies are broad range of hosts including humans, a wide variety of incubation periods ranging from a few weeks to more than a year, ability to bring about acute fetal consequence, and scare pathological lesions. Although rabies is fatal, it is preventable disease by vaccination and post-exposure prophylaxis (PEP). This review will focus on general features of rabies, and immunoglobulin and vaccines for PEP.

      • 만성신부전증 환자에서 Recombinant Human Erythropoietin 치료에 따른 혈액지표와 Hemoglobin A1c에 대한 연구

        Hemoglobin A1c is produced by a progressive, non-enzymatic reaction between glucose and hemoglobin within the erythrocytes. The HbA1c concentration is dependent on the plasma glucose level and the stage of development of the erythrocytes. Immature erythrocytes contain lower levels of glycosylated hemoglobin than mature erythrocytes. HbA1c level was decreased in short RBC life span. Therefore, HbA1c level is not only measure for assessment of moderate to long term glycemic status in diabetics, but also as a possible diagnostic parameter of anemia. The aim of this study was to evaluate the HbA1c level in patients with chronic renal failure with anemia. HbA1c concentration, iron, ferritin, TIBC and hematologic parameters were measured before treatment and 1, 2 months after administration of recombinant human erythropoietin. The HbA1c concentration was measured by high performance liquid chromatography with cation exchange column (Pharmacia). The results were as follows; 1. The mean of hemoglobin was 8.66 g/dL in the controls and 7.84 g/dL in the patients with chronic renal failure. The mean of hematocrit was 25.19 % in the controls and 23.14 % in the patients with chronic renal failure. The mean of MCV was 93.23 fL in the controls and 92.73 fL in the patients with chronic renal failure. The mean of MCH was 32.56 pg in the controls and 31.76 pg in the patients with chronic renal failure. The mean of HbA1c was 3.15 % in the controls and 2.95 % in the patients with chronic renal failure. 2. Hematologic parameters in the patients with chronic renal failure 1) The results of hemoglobins were 7.84 g/dL, 8.11 g/dL, 8.92 g/dL, the MCH were 31.76 pg, 32.87 pg, 33.20 pg, the results of MCV were 92.73 fL, 97.37 fL, 92.85 fL at before treatment and 1, 2 months after administration of r-HuEpo. 2) The results of hematocrits were 23.14 %, 23.73 %, 26.73 % at before treatment and 1, 2 months after administration of r-HuEpo. 3. Iron metabolism parameters in the patients with chronic renal failure 1) The results of iron test were 180.92 ㎍/dL, 137.79 ㎍/dL, 126.83 ㎍/dL at before treatment and 1, 2 months after administration of r-HuEpo. 2) The results of ferritin test were 1500.2 ng/ml, 1311.6 ng/ml, 1151.0 ng/ml at before treatment and 1, 2 months after administration of r-HuEpo. 3) The results of TIBC test were 282.17 ㎍/dL, 282.45 ㎍/dL, 278.83 ㎍/dL at before treatment and 1, 2 months after administration of r-HuEpo. 4. The results of HbA1c test were 2.95 %, 3.08 %, 3.18 % at before treatment and 1, 2 months after administration of r-HuEpo. It is suggested that, in patients with chronic renal failure, evaluation of HbA1c in diabetics who have anemia with chronic renal failure should be consider possible hematologic parameters, and HbA1c level would be one of the marker of anemia status, but further studies are needed.

        Serum MicroRNA Expression Profiling in Mice Infected with Rabies Virus

        <P><B>Objectives</B></P><P>Serum or plasma microRNAs (miRNAs) are potential biomarkers for the diagnosis for cancer and prenatal diseases. This study was conducted to investigate whether rabies virus causes a change in serum miRNA expression.</P><P><B>Methods</B></P><P>ICR mice were intramuscularly inoculated with rabies virus and were sacrificed weekly to collect serum and brain tissue for 4 weeks postinoculation. Mice were assigned to four groups based on the results of indirect immunofluorescent assays, enzyme-linked immunosorbent assay, and nested reverse transcription-polymerase chain reaction and the expression profiles of serum miRNAs were compared using a commercial mouse miRNA expression profiling assay.</P><P><B>Results</B></P><P>The expression levels of miRNAs changed significantly with the different stages of the disease. The expression level of 94 serum miRNAs in infected mice changed at least twofold. Seven microRNAs of them were significantly upregulated or downregulated in all infected mice regardless of disease status. The number of miRNAs with an expression level change decreased with the progression of the disease. In a hierarchical cluster analysis, infected mice clustered into a group separate from uninfected control mice.</P><P><B>Conclusions</B></P><P>Based on the relationship of miRNAs to gene expression regulation, miRNAs may be candidates for the study of viral pathogenesis and could have potential as biomarkers.</P>

      • 갑상선질환에서 thyroid stimulating immunoglobulin의 측정

        To investigate the role of thyroid stimulating immunoglobulin(TSI) in various thyroid diseases, we measured TSI activity by TSH receptor Ab in 17 patients with Grave's diseases(GD), 4 patients with thyroiditis and 4 patients with thyroid nodules and also measured serum T₃, T₄, TSH, T₃RU by radioimmunoassay. 1) In patients with 14 hyperthyroid GD, the levels of serum T3,T4, were significantly increased than normal control. The levels of serum T3 in patients with thyroiditis and the levels of serum T4 in patients with thyroid nodule was significantly increased than normal control. 2) the levels of TSI in hyperthyroid GD(33.4±27.7%) and euthyroid GD (22.9±18.3%) were significantly increased than normal control. There was no correlation between the levels of TSI and thyroid function tests in patients with GD. 3) Decreased levels of TSI was observed after antithyroid drug theraphy in patients with 4 GD.

        Seroprevalence of Severe Fever with Thrombocytopenia Syndrome Virus Antibodies in Rural Areas, South Korea

        <P>We investigated 1,228 residents of 3 rural areas in South Korea and determined that 50 (4.1%) were positive for severe fever with thrombocytopenia syndrome virus antibodies. Fever and gastrointestinal symptoms in the previous 3 years and career duration were associated with virus seropositivity.</P>

        Calciphylaxis is a rare, but fatal, condition that is characterized by a rapidly progressive ischemic necrosis of the skin, underlying tissue and other organs, as well as rapid vascular calcification. It results in death due to sepsis, heart or respiratory failure. A 67-year old female was admitted to hospital with the chief complaint of constant pain to both lower legs of 1 week duration. She was treated with calcitonin-salmon due to a prior unexplained hypercalcemia of 2 weeks. On the third day post admission. pain and weakness in the lower legs were aggravated, became painful, with violaceous skin lesions developing on the thigh with findings similar to those of rhabdomyolysis. Because she was suspected of having dermatomyositis, she was treated with methylpredrisolone. However, the skin lesions and symptoms were aggravated, and she died of sepsis due to a skin infection. About 160 cases of calciphylaxis have been reported, with most of these cases being associated with secondary hyperparathyroidism due to end-stage renal disease, but cases of calciphylaxis without renal failure are very rare. We now report a case of calciphylaxis without renal failure, mimicking dermatomyositis, and present a brief review of the pathophysiology and treatments of calciphylaxis inform the relevant literature

        폐경기전후 한국 여성의 비타민 - D 수용체 유전자의 Restriction Fragment Length Polymorphisms 과 골밀도 및 골표지자와의 관계에 대한 고찰

        Osteoporosis is now a major health problem because of the increasing elderly population and related osteoporosis fractures. Recently, it has been suggested that lower bone mass with/and high bone turnover rate is considered to be important in the developing of osteoporosis, and so there has been many efforts to identify the risk factors which is considered to cause lower bone mass and high bone turnover. Osteocalcin, the most abundant noncollagenous protein in bone, is a marker of bone turnover and its synthesis is induced by calcitriol(the active form of vitamine-D) through the vitamine-D receptor(VDR) and a specific vitamine D-responsive element in the osteocalcin gene promoter. Serum concentrations of osteocalcin are under the strong genetic influences and may reflect allelic variation in VDR gene. Therefore, the present study were designed to find the relationships among the polymorphisms of Vitamine-D receptor gene, bone mineral density and bone markers. We analysed the restriction fragment length polymorphisms of VDR gene with Bsm I endonuclease enzyme in relation to bone mineral density by using DEXA(dual energy X-ray absorptiometry, QDR-2000) and bone markers, especially serum osteocalcin concentrations in 356 pre- and postmenopausal Korean women. The frequence of RFLPs of VDR gene is 3.3% in BB type, 10.1% in Bb type, 86.6% in bb type. The concentrations of osteocalcin, alkaline phosphatase, procollagen-C and urinary deoxypyridinoline/creatinine were found to be higher in postmenopausal than premenopausal women and the levels of BMD were lower in postmenopausal than premenopausal women. The BB type, which is known to have a strong genetic determinant, is less frequently encountered in Korean women and does not correlate with levels of bone markers and bone mineral density. Even though the number of women with BB type is small, we noted the mean serum level of each bone marker was greater in postmenopausal women with BB type than in premenopausal women with the same genotype. In conclusion, this may suggest a partial agreement of our data with that of Australlian group and that we have to try to find out another genotype specifically related with lower bone density in Korean women(J Kor Soc Endocrinol 10: 249-261, 1995)
