        Objectives : We evaluated the efficacy of ramosetron and ondansetron for preventing postoperative nausea and vomiting (PONV) in laparoscopic colectomy. Methods and Materials : Eighty patients who received laparoscopic colectomy were randomly divided into two groups: R group (ramosetron 0.1mg PO) and O group (ondansetron 4mg twice IV). Injection or oral medication was administered before the induction of anesthesia in each group. Injection was administered at the end of surgery in O group. General anesthesia was induced using propofol and rocuronium, and maintained with sevoflurane, remifentanil and air (FiO2 0.5), We investigated the incidences of PONV in each group by the Rhodes index of nausea, vomiting and retching (RINVR) at postoperative 6 and 24 hours. Results : The incidence of PONV was not different between group at each time points after surgery (at postoperative 6 hours: 20% in group R, 17.5% in group 0, at postoperative 24 hours; 12.5% in group R, 7.5% in group 0). There was no difference in the severity of PONV, satisfaction, rescue drug usage. Conclusion : Prophylactic therapy with ramosetron is as effective as conventional prophylactic therapy with ondansetron for preventing PONV in general anesthesia for laparoscopic colectomy.

        SC 합성 기둥-철골 보 접합부의 내진성능에 관한 실험적 연구

        Steel-concrete composite column is a recently developed composite system, in which the two opposite flanges of H-shape section are connected by welded links, and vacant space enclosed by the flanges, web and links is filled with concrete. The purposes of this paper are to suggest efficient moment-connection details for SC composite column system and to verify their seismic performance. Two types of SC composite column-steel beam connections were proposed; one for strong axis of column and the other for weak axis of column. Cyclic loading tests for two full-scaled specimens were performed according to the standard loading history of SAC(2000). Moment-connection detail for strong axis of column showed sufficient seismic performance required for SMRF(special moment resisting frame). However, connection detail for weak axis of column exhibited inferior seismic performance than that for strong axis of column, which satisfied the requirements for OMRF(ordinary moment resisting frame). Additional studies are needed to increase elastic stiffness and seismic performance of connection detail for weak axis of SC composite column.

      • 대동맥판막에 발생한 유두상 섬유탄력종 1례

        유두상 섬유탄력종은 주로 심장판막에 발생하는 원발성 종양으로 심장종양 중 두 번째로 많은 양성종양이나 전체적으로 그 빈도는 매우 드물다. 78세 여자 환자가 흉부 불편감을 주소로 내원해서 대동맥판막에 위치한 유두상 섬유탄력종이 발견되어 개심술 하에 대동맥판막의 손상 없이 종양을 제거하였기에 문헌고찰과 더불어 이를 보고하는 바이다. Cardiac papillary fibroelastoma is the rare primary cardiac tumor, though it is the second most common benign cardiac tumor. It usually involves the cardiac valve. Papillary fibroelastoma of the aortic valve was found in a 78-year-old woman with chest discomfort and it was excised without valvular injury under cardiopulmonary bypass. Thus we report the present case with relevant review of the literatures.

        This study carrys out to determine the strength of oxide skins of Al alloy melts as the practical alloy for automobile piston. This has been observed by Rotational Die Movement Equiment for the temperature range from 600℃ to 750℃ and oxidation time range from 10min to 60min. This method was able to quantify the strength of oxide skins of the molten alloys and had ±3% experimence error range. This strength of oxide skins of the molten alloys were much more affected by oxidation temperature than oxidation time.

      • 계기비행 훈련용 Mooney 시뮬레이터 개발

        본연구는 한국항공대학교가 운용하고 있는 M20J 비행기의 계기비행 훈련용 시뮬레이터의 개발에 대한 것이다. 조종입력에 따른 비행기의 운동방정식으로부터 결과되는 출력인 속도, 가속도, 받음각, 옆미끄럼각, 기수각등으로부터 비행기의 항법장비인 VOR, HSI, RMI, DME와 각종 계기류 등을 화면에 구현하기 위하여 Active-Xrl술을 이용하여 소프트웨어를 작성하였다. PC와 하드웨어와의 인터페이스는 8255를 이용하여 직접 보드를 제작하였으며, 조종석에서의 조종입력은 회전형 인코더(rotary encoder)를 이용하였다. 특히 교관석을 이용하여 환경자료를 설정할 수 있도록 하였으며, 교관석의 설정에 따라 훈련자가 각종 훈련을 수행할 수 있도록 제작되었다. Mooney Navigation Training Simulators are designed for training on a wide range of aircraft and avionic systems. Modular, separable simulator subsystems provide a flexible approach for meeting particular requirements. State-of-the-art, microprocessor-based computer systems provide high reliability, flexibility, and user-friendly features. M20J Flight Simulator is capable of providing highly effective support for the familiarization, preflight, takeoffs, instrument procedures, inflight maneuvers, landing and approaches to landing, and approaches to Landing, and normal procedures.

      • DEA 첨가 금속 Alkoxide를 사용한 PZT 박막의 제조 및 특성 평가

        Lead acetate trihydrate, zirconium iso-propoxide and titanium iso-propoxide used for the starting material to fabricate PZT coating solution. Iso-metoxy-ethanol used for the solvent of the starting material. Adding diethanolamine formed the chelate compound and prevented the hydrosis of coating solution. The coating sol solution is changed to M-O-M structure polymer gel by adding H_2O, and the polymer gel has the more strong bonding force. The cross-sectional SEM micrograph of PZT thin film showed that the metal of lower electrode was silicated with SiO_2 of buffer layer. The SIMS depth profile for PZT thin film indicated that the metal of lower electrode and silicon was diffused up to the upper part of PZT thin film. For the X-RD data of PZT thin film, the thin film on Pt substrate and with Ti buffer layer had strong perovskite phase. The PZT thin films measured by Sawyer-Tower circuit showed feroelectric characteristic representating D-E hysterisis curve. The remanent polarization and coercive field of the film was 1.92 C/㎠ and 33.3kV/㎝. I-V characteristic curve showed that the leakage currents and break down voltages of PZT thin films on Ni/SiO_2/Si, Ti/SiO_2/Si, Pt/SiO_2/Si substrate was 1.43 × 10 exp (-10) A/㎠, 1.19×10 exp (-10) A/㎠, 2.28×10 exp (-10) A/㎠ and 0.334 MV/㎝, 0.33 MV/㎝, 0.11 MV/㎝ each. C-V curve indicated dielectric constant 57 The resistance antiresonance frequency of PZT film was 612 ㎒, and the resistance was 14 ㏀ at that frequency.

        Traditionally, enbloc resection of the primary tumor with its lymphatics is the surgical procedure of choice for many oral and oropharyngeal malignancies. However, the operative fields reached by conventional approaches are too limited and narrow to remove extensive tumors, especially posterior pharyngeal tumor. The mandibular swing approach has greatly assisted tumor ablation and reconstruction of these areas. The authors essayed a mandibular swing approach for the ablation of a huge posterior pharyngeal wall carcinoma.

      • 지도부딘 투여력이 없는 Human Immunodeficiency Virus Type 1 감염환자에서 지도부딘 내성 발현율

        Backgroud : Drug resistant virus has been seen with all effective antiretroviral drugs in current practice. The isolation of resistant virus from subjects is associated with more rapid clinical outcome. The presence of resistance-conferring mutations in a patient in whom therapy is foiling indicates that other antiretroviral drugs should be considered. So, the identification of drug resistant Human Immunodeficiency Virus (HIV-1) mutations is of increasing importance. Zidovudine monotherapy for HIV-1 infection has been widely used in Korea since 1990. So, the prevalence of zidovudine resistant virus has been expected to be high. The purpose of the present study is to determine the prevalence of zidovudine resistant HIV-1 among zidovudine-naive Korean patients. Methods : We tested drug susceptibility of HIV-1 isolated from 19 zidovudine-naive Korean patients. We amplified a portion of gene reverse transcriptase of HIV-1 by RT nested PCR method. The susceptibility of HIV-1 to zidovudine was determined by sequencing the DNA base pairs of gene reverse transciptase. Results : Two (10.5%) of 19 HIV-1 isolates from zidovudine naive patients showed resistance conferring mutations to zidovudine. Two (2.1%) of 95 codons analysed from zidovudine naive patients showed mutations conferring resistance to zidovudine. Conclusion : The prevalence of zidovudine resistant HIV-1 was 10.5% among zidovudine-naive Korean patients. (Korean J Infect Dis 33:40~45, 2001)

      • 다단 원판형 드래그펌프의 배기특성에 관한 실험적 연구

        In this paper, the pumping performance of the multi-stage(two-stage) disk-type drag pump which works in the pressure range from 4 to 0.003 Torr was studied experimentally, and rotational speed of the pump is 24,000rpm and nitrogen is used as a test gas. An experimental study on pumping characteristics of various multi-stage disk-type drag pump is performance. The inlet pressures are measured for various outlet pressures of teat pump. The maximum compression ratios for nitrogen are 1000(multi-stage disk-type drag pump), 740(helical-type drag pump), 90(BSC disk-type drag pump) and 85(OSC disk-type drag pump), respectively. The ultimate pressure of the multi-stage disk-type drag pump is 3.7×10-5 Torr at 0.003 Torr for outlet pressure. The maximum value of compression ratio is 1000 at 0.6 Torr for outlet pressure.
