Can common carotid artery ultrasonography help in screening high-risk pregnant women?

Objective: Pregnancy at advanced maternal age has become more common in Korea. There are many studies between pregnancy induced complication such as pre-eclampsia, gestational diabetes mellitus (GDM) and advanced maternal age. Therefore, screening for high-risk pregnant women with pregnancy induced complications is important. Carotid intima-media thickness (CIMT) and peak systolic velocity (PSV) are well known indicators of atherosclerosis and cardiovascular disease. CIMT and PSV are affected by endothelial dysfunction and inflammation. This is similar to the mechanism that causes placental dysplasia and vascular disorders, leading to pregnancy induced complication. In this study, CIMT and PSV were compared between high-risk pregnant women and normal pregnant women. Methods: This study had been performed at Korea University Ansan Hospital between March 2019 and August 2022. All enrolled women measured CITM, PSV, and end-diastolic velocity by ultrasonography between 20 and 24 weeks of gestation. In addition, perinatal complications, clinical characteristics, delivery method, gestation age, birth weight of the fetus, neonatal intensive care unit administration, and Apgar score were compared among these pregnant women. Results: Maternal common carotid artery (CCA) IMT and PSV did not show a significant correlation with overt diabetes mellitus (DM) and GDM. However, pregnant women with pregnancy induced hypertensive disorder (PIH) showed decreased the PSV of CCA that compared to normal pregnant women. (p=value 0.043) In addition, CCA IMT showed a positive correlation with gestational age and birth weight. (p-value 0.02, 0.028 / r-value 0.277, 0.2262) Conclusion: In pregnant women with PIH, the PSV of CCA was decreased compared to normal pregnant women. This may help screening pregnant women who had high risk for PIH in the second trimester of pregnancy.

HCC rupture following TACE is a rare but potentially life-threatening complication. This report described a patient who experienced HCC rupture after TACE. The patient was successfully managed by transarterial embolization and staged liver resection. Large tumor size, superficial tumor location, exophytic growth of the tumor, and vascular lakes during TACE may be predisposing risk factors for HCC rupture after TACE.

Background: We aimed to analyze the factors predicting the diagnostic performance of flexible bronchoscopy without guidance in peripheral lung lesions that are endoscopically invisible. Methods: This was a retrospective study conducted in St. Paul`s Hospital, The Catholic University of Korea, between January 2007 and March 2013. We included all patients who received bronchoscopy during this period. The analyzed variables were age, sex, the etiology of the lesion, lesion size, distance from the pleura, and presence of the bronchus sign. We used multiple logistic regression analysis to identify the significant independent factors associated with diagnostic yield. Results: We included 151 patients in this study. The overall diagnostic yield was 58.3%. The sensitivity was 43.2% for malignant disease and 78.1% for benign disease. The benign lung lesions (p<0.001), lesion size (p=0.015), presence of the exposed type of bronchus sign (p<0.001), and presence of cavitary lung lesions (p=0.005) were factors influencing the yield of flexible bronchoscopy by univariate analysis. In a multivariate logistic regression analysis, the exposed type of bronchus sign and benign lung lesions were independent predicting factors (odds ratio [OR]: 27.95; 95% confidence interval [CI], 7.56-103.32; p<0.001 and OR, 4.91; 95% CI, 1.76-13.72; p=0.002). Conclusion: The presence of the exposed type of bronchus sign and benign lung lesions are determining factors of the diagnostic yield in flexible bronchoscopy in evaluating peripheral lesions that are not endoscopically visible.

An inflammatory myofibroblastic tumor (IMT) is a rare disease entity reported to arise in various organs. It is thought to be a neoplastic or reactive inflammatory condition, controversially. The treatment of choice for myofibroblastic tumor is surgery, and recurrence is known to be rare. The optimal treatment method is not well-known for patients ineligible for surgery. We report a 47-year-old patient with aggressive recurrent IMT of the lungs. The patient had been admitted for an evaluation of back-pain two years after a complete resection of pulmonary IMT. Radiation therapy was performed for multiple bone recurrences, and the symptoms were improved. However the patient presented again with aggravated back-pain six months later. High-dose steroid and non-steroidal anti-inflammatory drugs were administered, but the disease progressed aggressively, resulting in spinal cord compression and metastasis to intra-abdominal organs. This is a very rare case of aggressively recurrent pulmonary IMT with multi-organ metastasis.

Introduction: Obstructive sleep apnea (OSA) is highly common among patients with idiopathic pulmonary fibrosis. OSA causes chronic intermittent hypoxia (CIH) during sleep. We aimed to investigate the effect of CIH on bleomycin-induced pulmonary fibrosis in mice and to explore putative mechanisms. Methods: C57/BL6 mice were divided into four groups and then exposed to control conditions (room air) or CIH for 2 weeks prior to the intratracheal injection of bleomycin. After bleomycin instillation, mice were exposed to CIH or room air for 4 weeks. Fibrosis was evaluated by Masson’s trichrome stain and hydroxyproline assay. The mRNA expressions or protein levels of fibrosis-related genes were measured by real-time PCR and western blot in lung tissue, respectively. Results: Masson staining showed that CIH led to scattered fibrotic changes in alveolar structure and substantive fibrotic tissue dominantly situated in the perivascular space. In bleomycin-treated group, CIH compared to control increased hydroxyproline (p<0.001) in lung tissue and TGF-β (p<0.01) in serum. CIH increased the mRNA expression of TGF-β, collagen 1, and α-SMA in bleomycin-treated group, though the difference was not statistically significant. The protein levels of TGF-β1, collagen 1, and p-Smad2/3 were elevated in CIH with bleomycin-treated group. The protein level of snail was notably increased in CIH, regardless of bleomycin treatment. Conclusion: Following bleomycin-induced lung injury, CIH might exacerbate the progression of pulmonary fibrosis via TGF- β/Smad/Snail signaling.

As environmental problems caused by fossil fuel use emerged renewable energy raised as an effective solution in several countries, including South Korea, Japan, Germany and so on, Agriphotovoltaic (APV), one of the systems for renewable energy, growing plants under photovoltaic (PV) panels, has been getting attention due to its co-productivity of electricity and crop yield. To date, production and yield of crop under APV have been the main focus, neither the crop quality nor consumers’ acceptability to the processed produces. Therefore, this study focused on comparing the quality of cabbage and cabbage juice grown from open-field (OF) and APV. To address this research question, we cultivated ‘Winter Storm’ cabbage under the APV and open field during 2020 Fall season. The bi-facial panels of APV were faced to north direction and each one has length, width and thickness; 1.68 m, 0.95 m, and 30 mm respectively. The total power generation capacity of panels was 20.16kW. Fresh cabbage weight and its diameter grown from APV were slightly decreased (156 g; 9.7%, 0.2 cm; 1.2% respectively) compared to open-field but not significant. Glucosinolates and its hydrolysis products, major phytochemicals in brassica plants, were not different in fresh cabbage but few in juice. The amount of volatile organic compounds, 2-methylpyrazine, 2,5-dimethyl pyrazine, and dimethyl trisulfide, that affect the perception of smell or taste were significantly high in cabbage juice made of open-field grown one (n = 3, p < 0.01). In overall, sensorial panels were not able to distinguish difference in the triangle test using cabbage juice (n = 70, p = 0.724). In attribute test, experienced panels tended to give more correct answers than inexperienced panels to cabbage juice. Regardless of the distinguished difference in color, aroma, and taste, panels did not have any preference (50 : 50%) to two different cabbage juice made of the different production system. To our knowledge, this is the first sensorial experiment using crop grown from APV.
