Case Report : A case of vulvar hematoma with rupture of pseudoaneurysm of pudendal artery

Vulvar hematomas are uncommon outside of the obstetric population and may be the result of trauma to the perineum. Vulvar hematomas most often present with low abdominal pain and urologic and neurologic symptoms. The vulva has rich vascularization that is supplied by the pudendal artery, a branch of the anterior division of the internal iliac artery. We describe a rare case of a 15-cm-sized vulvar hematoma with the suggested rupture of a pseudoaneurysm of the left pudendal artery without trauma injury. A 14-year-old girl presented with sudden pain and swelling in her left labium and was successfully treated with selective arterial embolization and surgical evacuation. We provide a literature review and discuss patient treatment and management strategies.

<P>A novel maltose (G2)-forming alpha-amylase from Lactobacillus plantarum subsp. plantarum ST-III was expressed in Escherichia coli and characterized. Analysis of conserved amino acid sequence alignments showed that L. plantarum maltose producing alpha-amylase (LpMA) belongs to glycoside hydrolase family 13. The recombinant enzyme (LpMA) was a novel G2 producing alpha-amylase. The properties of.purified LpMA were investigated following enzyme purification. LpMA exhibited optimal activity at 30 degrees C and pH 3.0. It produced only G2 from the hydrolysis of various substrates, including maltotriose (G3), maltopentaose (GS), maltosyl beta-cyclodextrin (G2-beta-CD), amylose, amylopectin, and starch. However, LpMA was unable to hydrolyze cyclodextrins. Reaction pattern analysis using 4-nitrophenyl-alpha-D-maltopentaoside (pNPGS) demonstrated that LpMA hydrolyzed pNPGS from the nonreducing end, indicating that LpMA is an exotype alpha-amylase. Kinetic analysis revealed that LpMA had the highest catalytic efficiency (k(cat)/K-m ratio) toward G2-beta-CD. Compared with beta-amylase, a well-known G2-producing enzyme, LpMA produced G2 more efficiently from liquefied corn starch due to its ability to hydrolyze G3.</P>
그립(GRIP)친구 도우미 프로그램이 학교폭력의 방관적 태도에 미치는 효과 -공감능력의 매개효과를 중심으로-

본 연구는 중학생을 대상으로 학교폭력 및 집단 괴롭힘 예방을 위한 그립(GRIP)친구도우미 프로그램을 시행하고 이들의 방관적 태도의 감소와 공감능력의 향상에 미치는 효과를 살펴보고자 하였다. 연구대상은 서울 소재 Y중학교의 2학년 재학생 137명이었으며 이들을 각각 두 학급으로 나누어 실험집단 70명과 통제집단 67명으로 나누어 치료 프로그램의 효과를 알아보았다. 이를 위해 집단 괴롭힘의 방관자적 척도, 공감능력수준 척도를 사용하여 검사를 실시하였고, 분석은 PASW statistics 18.0과 AMOS 18.0을 활용한 경로분석을 실시하였으며 매개효과는 소벨(Sobel)분석방식으로 결과를 처리하였다. 연구 결과, 프로그램 실시 후 실험집단 피험자들의 인지적 공감능력은 통제집단에 비하여 유의미하게 감소하고 정서적 공감능력은 유의미하게 상승하였다. 또한 매개효과 검증 결과 그립(GRIP)친구도우미 프로그램은 학교폭력의 방관적 태도에 직접적으로 영향을 미치지 않는 대신, 정서적 공감능력을 통해 간접적 영향을 미치는 것으로 나타났다. 이로 볼 때, 그립(GRIP)친구도우미 프로그램이 정서적 공감능력의 향상을 매개로 하여 학교폭력 및 집단 따돌림의 방관적 태도를 감소시키는 데 도움을 줄 수 있음을 시사한다. This study initiate the GRIP peer-helper program against middle school students in an attempt to reduce bullying and school violence; and to have a look at its efficacy in reducing bystanders' attitudes while raising empathy levels. Test subjects were 137 second year middle school students chosen from 4 classes whose parents consented to the program. The program's effect was studied by dividing these students into two groups: one experimental group consisting of 70 students, and the other 67 acting as the control group. Group therapy was conducted once a week for a total of 8 weeks, each therapy lasting 1 hours 20 minutes. Study was initiated using set standards of bystanders' attitude towards bullying and empathy capability. Analysis was conducted through analysis techniques making use of PASW STATISTICS 18.0 and AMOS 18.0 and the Sobel method. The results of this study were as follows: First, after the initiation of the program, the cognitive empathy capability of the experimental group saw significantly reductions compared to that of the control group. However, their emotional empathy capability rose significantly. Second, cognitive empathy capability was shown to increase bystanders' attitude rather than reduce it. In contrast, emotional empathy capability was seen to reduce bystanders' attitude. Third, while the GRIP Peer-helper program did not have a direct influence upon school violence and bystanders' attitude, it was seen to have an indirect influence through affecting emotional empathy capability. The results of this study affirm that the GRIP program can help reduce school violence, bullying and tendency to neglect by enhancing students' emotional empathy capability.
알카리 가열처리 현미를 이용한 팽화스낵의 물리화학적 특성

알카리처리 현미의 함량을 달리하여 제조한 팽화스낵의 구조, 물리화학적 특성 및 관능성을 측정하여, 알카리처리 현미가 팽화스낵의 특성에 미치는 영향을 검토하였다. 알카리처리 현미를 함유한 팽화스낵은 알카리처리 현미의 높은 팽화도로 인하여, 팽화 후 현미 곡립의 부피 증가와 함께 치밀한 내부구조를 형성하였으나, 과도한 팽화에 의한 외형손상이 발생하는 것으로 관찰되었다. 팽화스낵의 수분함량 및 용적밀도는 알카리처리 현미의 대체비율이 높아짐에 따라 감소하였다. 또한 알카리처리 현미를 함유한 팽화스낵의 손상전분함량은 알카리처리 과정에서 호화된 전분의 재결정화로 인하여 대조구 보다 낮았으며, 칼슘함량은 흡수된 칼슘의 영향으로 증가하였다. 알카리처리 현미를 함유한 팽화스낵의 색도는 외피에 존재하는 flavonoid 색소가 알카리처리 과정에서 변색되어, 대조구 보다 명도 및 황색도는 낮았으나 적색도는 증가하였다. 팽화스낵의 관능성 평가결과, 알카리처리 현미만으로 제조한 팽화스낵은 질감, 향미 및 전반적인 기호도는 대조구와 유사하였으나, 낮은 명도와 과도한 팽화에 따라 색도 및 외관의 관능특성은 낮게 평가되었다. 한편 알카리처리 현미의 대체비율50%로 제조한 팽화스낵의 경우, 일반 현미 팽화스낵과 외관, 색도 및 관능지표에서 유의적 차이가 없었으며, 칼슘함량 증가에 따른 칼슘 보강효과가 확인되었다. Physicochemical and sensory properties of puffed rice snack containing various levels (0%, 50%, 100%) of alkalicooked brown rice were examined. To prepare alkali-cooked rice, brown rice was cooked in boiling 2% Ca(OH)2 solution for 5 min, steeped for 3 h, and dried at 50℃ for 18 h. Alkali-cooked rice showed a higher degree of expansion than that of untreated rice after puffing. Although moisture content, enzyme susceptible starch (ESS), and density of puffed rice snack decreased with increasing amounts of the alkali-cooked rice replacement, calcium content increased because of the absorption of calcium during alkaline processing. The puffed rice snack had a darker color as the level of replacement with alkali-cooked rice increased. A sensory test revealed that puffed rice snack replaced with 50% of alkali-cooked rice had no significant differences in color and sensory attributes from those of the control. These results indicate that fortification of calcium could be achieved by partial replacement of alkalicooked rice without any detrimental effects on the quality of puffed rice snack.

The water deer, Hydropotes inermis (Cervidae), is native to China and Korea and has two subspecies of the Chinese water deer (Hydropotes inermis inermis) and Korean water deer (Hydropotes inermis argyropus). To date, only the Korean water deer has been reported in South Korea. In this study, however, an intraspecific phylogeny and haplotype analysis based on mitochondrial cytochrome oxidase I indicated that both Korean and Chinese water deer are found in South Korea. The populations of the two Korean genetic lineages did not show distinct geographic distributions. Further morphological studies on the Korean water deer will be required to confirm its taxonomic status.

Objective: To develop an evidence-based guideline for the diagnosis and treatment of antipsychotic-induced hyperprolactinemia by adapting existing high-quality clinical guidelines with a view to improve the clinical symptoms and long-term quality of life of patients by providing appropriate management. Methods: This guideline was developed according to the ADAPTE methodology. The adaptation process included determining key health questions, systematically searching and screening guidelines, evaluating the quality and contents of these guidelines, deriving recommendations for key questions, and performing a peer review. The selection criteria for the guideline search were (1) evidence-based guidelines, (2) published within the last 5 years, and (3) written in English or Korean. Results: After evaluating the quality and content, we finally selected three guidelines for adaptation. The final output of the development process was 25 recommendations for 10 key questions. We adopted the Agency for Health Research Quality methodology and presented the level of evidence from levels I to IV. In addition, we defined the recommendation grades from grade A (strongly recommended) to D (no recommendation) based on the level of evidence and clinical significance of the recommendation. Conclusion: The development and dissemination of the adapted guideline is expected to increase the certainty of medical decision making and improve the quality of medical care. Further studies on the effectiveness and applicability of the developed guideline are necessary.
