Proceedings of international Symposium on East-West Medicine, Seoul. 44-64, 1999.-Cordyceps sinensis is a major parasitic fungus on larva of Lepidoptera. It is one of the well know fungi used in traditional Chinese medicine for treatment asthma, lung and kideney diseases. Many fungi belonging to the genus Cordyceps have been demonstrated to produce natural products with various biological activities. Reported biological activities of genus cordyceps include:(a)inhibition of DNA and RNA synthesis;(b)enhancement of cell differentiation;(c)restruction of cytoskeleton;(d)inhibition of protein kinase activity;(e)antitumor activity on bladder, colon, lung carcinoma as well as fibroblastoma;(f)inhibition of the infection and revers transciptase activity of human immunodeficiency virus type 1; and(g) inhibition of methylation of nucleic acid. Thus, it is suggested that the extracts of Cordyceps sp. Contain antitumor, antiviral, and many other biological activities. In our previous studies, we proved that C. sinensis contains the antitumor and immunomodulatory agents. In the present studies, effects of C. sinensis on bronchoalveolar lavage fluids(BAL)cells were demonstrated. The crude methanolic extracts of C. sinensis were fractionated into 15 fractions by silica gel column chromatography. Effects of various fraction on BAL cells proliferation and lipopolysaccharide(LPS) stimulated interleukin-1β(IL-8),platelet activating factor(PAF), interleukin-6(IL-6), tumor necrosis factor-α(TNF-α),and interleukin-8(IL-8) production on BAL cells were determined. The result indicated that the CS-19-22 fraction suppressed BAL cells proliferation activated by LPS. The median inhibitory concentration(IC50) was 6㎍/ml. The CS-19-22 fraction also decreased IL-1β,PAF,IL-6, TNF-αand IL-8 production.The results of revers transcription-polymerase chain reaction(RT-PCR) demonstrated that the CS-19-22 fraction did not affect IL-1β,PAF,IL-6, TNF-α,and IL-8 mRNAs expression in BAL cells activated by LPS. By contrast, it inhibited the IL-10 mRNA expression but enhanced IFN-γand IL-12 mRNAs expression in activated BAL cells. Moreover, the CS-19-22 fraction blocked PAF-induced platelet aggregation. It is unlikely that cytotoxicity was involved. Because no cell deths were observable. We hypothesize that inhibitory mechanisms of CS-19-22 on BAL cells proliferation may be related to the impairments of gene expression and production of cytokines in BAL cells. The explanation of C. sinensis antiasthma function may involved the following(1)C.sinenesis contained immunomodulatory agents that stimulated IFN-γand IL-12 expression in TH1cells.(2)Enhanced secretion of IFN-γand IL-12 will inhibit the Tγcells immune responses Ex. Decreasing of IL-6 and IL-10 production.(3) The impairments of cytokines production in TH2 cells suppressed th BAL cells proliferation then more cytokines production Ex.IL-1β,PAF,IL-6,TNF-αand IL-8 in activated BAL cells were decreased. (4)The suppression of TH2cells immune responses will inhibit the growth and differentiation of B cells then IgE production will be decreased. (5)Reduced production of IgE will decreased asthma attack occurring in individuals. However, the results of the present study suggested that C. sinensis may also have acted to treat asthma in part by inhibiting BAL cells proliferation and cytokine gene expression and production. Future, experiments with treatment of animals with asthma with C. sinensis will be necessary to defined whether C. sinensis can reduce asthma injury. Plans are underway for the isolation of pure principal growth inhibitors and their mechanisms of action are subjected for further study.

        Isolation of triterpenic acid-rich extracts from Hedyotis corymbosa using ultrasound-assisted supercritical carbon dioxide extraction and determination of their fictitious solubilities

        The present study investigated a green ultrasound-assisted supercritical carbon dioxide (USC–CO2)process for extracting oleanolic acid (OA) and ursolic acid (UA) from Hedyotis corymbosa. Chrastil, Bartleand Kumar and Johnston models were used to correlate the experimentalfictitious solubility data. Theoverall average absolute relative deviation (AARD (%)) in solid solubility ranged from 3.632% to 6.529%,which indicates good agreement between the experimentalfictitious solubility data and the modeledresults. Using the correlation results, the total heat of solution and the enthalpies of vaporization andsolvation for OA and UA in the USC–CO2 extraction process were also estimated.

        Delay of Surgery for Spinal Metastasis due to the COVID-19 Outbreak Affected Patient Outcomes

        Objective: The present study is to analyze the effects of the coronavirus disease 2019 (COVID 2019) outbreak and the subsequent lockdown on the outcomes of spinal metastasis patients. Methods: The study was a retrospective analysis of data from a prospective cohort study. All patients underwent surgical intervention for spinal metastases between January 2019 and December 2021 and had at least 3 months of postoperative follow-up. The primary outcome was overall mortality during the 4 different stages (pre-COVID-19 era, COVID-19 pandemic except in Taiwan, national lockdown, lifting of the lockdown). The secondary outcomes were the oncological severity scores, medical/surgical accessibility, and patient functional outcome during the 4 periods as well as survival/mortality. Results: A total of 233 patients were included. The overall mortality rate was 41.20%. During the Taiwan lockdown, more patients received palliative surgery than other surgical methods, and no total en bloc spondylectomy was performed. The time from surgeon visit to operation was approximately doubled after the COVID-19 outbreak in Taiwan (75.97, 86.63, 168.79, and 166.91 hours in the 4 periods, respectively). The estimated survival probability was highest after the national lockdown was lifted and lowest during the lockdown. In the multivariate analysis, increased risk of mortality was observed with delay of surgery, with emergency surgery having a higher risk with delays above 33 hours, urgent surgery (below 59 and above 111 hours), and elective surgery (above 332 hours). Conclusion: The COVID-19 pandemic and related policies have altered daily clinical practice and negatively impacted the survival of patients with spinal metastases.

      • The Technical Benefits of Future GNSS for Taiwan

        The next decade promises drastic improvements and additions to global navigation satellite systems (GNSS). Plans for GPS modernization include a civilian code measurement on the L2 frequency and a new L5 signal at 1176.45MHz. Current speculations indicate that a fully operational constellation with these improvements could be available by 2013. Simultaneously, the Galileo Joint Undertaking is in the development and validation stages of introducing a parallel GNSS called Galileo. Galileo will also transmit freely available satellite navigation signals on three frequencies and is scheduled to be fully operational as early as 2008. In other words, a dual system receiver (e.g., GPS+GALILEO) for general users can access six civil frequencies transmitted by at least fifty eights navigation satellites in space. The advent of GALILEO and the modernization of GPS raise a lot of attention to the study of the compatibility and interoperability of the two systems. A number of performance analyses have been conducted in a global scale with respect to availability, reliability, accuracy and integrity in different simulated scenarios (such as open sky and urban canyons) for the two systems individually and when integrated. Therefore, the scope of this article aims at providing the technical benefits analysis for Taiwan specifically in terms of the performance indices mentioned above in a local scale, especially in typical urban canyon scenarios. The conclusions gained by this study will be applied by the Land Survey Bureau of Taiwanese as the guideline for developing future GNSS tracking facilities and dual GNSS processing module for precise surveying applications in static and kinematic modes.

        A thermal deformation estimation method for high precision machine tool spindles based on the convolutional neural network

        The temperature rise of key internal components and ambient temperature changes during machining processes are the main causes of thermal displacement in machine tool spindle. This wastes materials and reduces working efficiency. A highly accurate and robust thermal spindle displacement estimation scheme is proposed in this paper which is based on the convolutional neural network (CNN) technique. Several key signals of specific dimensions related to spindle thermal displacement were first collected as two-dimensional (2D) signal maps. A multi-level feature expression for these 2D signal maps was extracted using convolution and pooling. A relationship between the extracted features and spindle thermal displacement was then learned using the neural architecture of the full connection layer. Optimized hyperparameter settings were determined by design of experiments (DoE) applied to the proposed CNN model. Experimental results showed that the proposed method had better performance than the multiple regression analysis (MLR) or back propagation neural network (BPNN) methods in terms of estimation accuracy and robustness at different spindle speeds.

      • Validation of a Palliative Prognostic Index to Predict Life Expectancy for Terminally Ill Cancer Patients in a Hospice Consultation Setting in Taiwan

        Background: The aim of our study was to assess the practical utility of the palliative prognostic index (PPI) as a prognostic tool used by nurse specialists in a hospice consultation setting in Taiwan. Methods: In total, 623 terminal cancer patients under hospice consultation care from one medical center in northern Taiwan were enrolled between January 1 and June 30, 2011. PPI was assessed by a nurse specialist at first hospice consultation and patients categorized into groups by prognosis (good, intermediate, poor). Patient survival was analyzed retrospectively to determine significance of between-group differences. Results: By PPI sum score, 37.2% of patients were in the good prognosis group, 18% in the intermediate prognosis group and 44.8% in the poor prognosis group. The death rates were 56%, 81.2% and 89.6% and median survivals were 76, 18 and 7 days, respectively. The hazard ratio was 0.19 (95% confidence interval [CI] 0.10-0.24, p<0.001) for the poor versus good prognosis group and 0.54 (95% CI 0.43-0.69, p<0.001) for the poor versus intermediate prognosis group. The sensitivity and specificity for the poor prognosis group was 66% and 71%; the positive predictive value and negative predictive value were 81% and 52%, respectively, to predict patient death within 21 days (area under the curve of the receiver operating characteristic was 0.68). Conclusions: Assessment by PPI can accurately predict survival of terminal cancer patients receiving hospice consultation care. PPI is a simple tool and can be administered by nurse members of hospice consultation teams.

      • Patient and Clinical Variables Account for Changes in Health-related Quality of Life and Symptom Burden as Treatment Outcomes in Colorectal Cancer: A Longitudinal Study

        Objective: The aim of the current study was to evaluate changes in treatment outcomes in terms of health-related quality of life (HRQoL) and symptom burden at zero, one, three, and six months after an initial diagnosis of colorectal cancer. The demographic and clinical characteristics that account for outcome changes in patients were investigated using a repeated measures framework. Methods and Materials: A cohort study was performed of 134 colorectal cancer patients followed from diagnosis to 6 months post-treatment in Central Taiwan. HRQoL and symptoms were assessed at diagnosis and one, three, and six months thereafter. The Functional Assessment of Cancer Therapy-Colon (FACT-C) questionnaire, VAS pain, and the Memorial Symptom Assessment Scale (MSAS) were used for data collection. A generalized estimating equation (GEE) was applied for statistical analysis. Results: The majority of the patients were male (55%) and married (91.5%). The mean age was 60.4 years (SD = 11.71). Most were diagnosed stage III and IV colorectal cancer (54.5%). All underwent surgery; some also received chemotherapy (CT) or concurrent chemoradiation therapy (CCRT). The results of the GEE showed that overall, the HRQoL, pain, and symptoms of the patients significantly improved over the treatment period. Patients with stage IV disease who had received surgery and CCRT showed the worst HRQoL. Females, patients with comorbidity, and stage IV patients had higher pain scores over time. Female and stage IV patients had more severe physical symptoms, whereas stage II and IV patients had worse psychological symptoms over time. Conclusion: The patients' HRQoL, pain, and symptoms significantly improved over the 6-month treatment period. Certain patient and clinical variables accounted for changes in treatment outcomes regarding HRQoL and symptom burden in colorectal cancer patients.

      • Evaluation of sewage sludge incineration ash as a potential land reclamation material

        <P><B>Abstract</B></P> <P>This study evaluated the potential of utilising sewage sludge incineration ash as a land reclamation material. Toxicity assessment of the leachate of the ash was carried out for both terrestrial and marine organisms. Both the fruit fly <I>Drosophila melanogaster</I> and barnacle <I>Amphibalanus amphitrite</I> showed that both bottom and fly ash leached at liquid-to-solid (L/S) ratio 5 did not substantially affect viabilities. The leachate carried out at L/S 10 was compared to the European Waste Acceptance Criteria and the sewage sludge ashes could be classified as non-hazardous waste. The geotechnical properties of the sewage sludge ash were studied and compared to sand, a conventional land reclamation material, for further evaluation of its potential as a land reclamation material. It was found from direct shear test that both bottom and fly ashes displayed similar and comparable shear strength to that of typical compacted sandy soil based on the range of internal friction angle obtained. However, the consolidation profile of bottom ash was significantly different from sand, while that of fly ash was more similar to sand. Our study showed that the sewage sludge ash has the potential to be used as a land reclamation material.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Toxicity of fresh & aged sewage sludge incineration fly & bottom ash was evaluated. </LI> <LI> Terrestrial (fruit fly) & marine organisms (barnacle) were used for toxicity study. </LI> <LI> Leachates of ashes had no adverse toxicity on both fruit fly and barnacle tested. </LI> <LI> Fly ash (compared to bottom ash) had more similar geotechnical properties to sand. </LI> <LI> This study showed the potential in using sewage sludge ashes as fill material. </LI> </UL> </P>
