        신장기능영상용 방사성의약품 Bz - MAG3 ( Benzoylmercaptoacetylglycylglycylglycine ) 의 키트화 및 체내분포

        N/A The MAG3 is a tubular excreting radiopharmaceutical for renal image. We synthe-sized benzoyl MAG3 (Bz-MAG3) and made a kit for labeling with Tc-99m. We checked the labeling effeciency of Tc-99m labeled MAG3 and biodistribution. Labeling efficiency was checked by TLC- SG (acetonitrile/H2O=2/1). After injecting of 1 mCi of Tc-99M-MAG3 to ICR-mice, Tmax(min), T1/2(min) were obtained in the renogram. Sequencial images (30sec, 2min, 5min, 10min, 15min, 20min) of TC-99m-MAG3 were compared with those of commercial Tc-99m-DTPA (Du Pont Merck Pharmaceutical Co.) kit. 1) The Rf value of synthesized Tc-99m-MAG3 was 0.78 and labeling efficiency was 97.5±1.9% (n=10). 2) The dynamic images of the Tc-99m-MAG3 were better than those of the Tc-99m-DTPA. 3) The Tmax(min.) and T1/2(min.) of Tc-99m-MAG3 (n=10) were 1.5±0.5 (left), 1.4±0.4(right), and 4.3±1.4 (left), 4.8±2.0 (right), respectivel. The Tmax(min.) and T1/2(min.) of Tc-99m-DTPA (n=7) were 2.7±1.6 (left), 2.7±1.6 right), and 3.8±1.7 (left), 4.5±2.7 (right), respectively. The quaility of image and labeling efficiency of the synthesized Bz-MAG3 kit were excellent, that it was supposed to be used in routine clinical work.

        A neurometabolite study of chronic daily headache in patients with systemic lupus erythematosus using magnetic resonance spectroscopy: comparison with fibromyalgia patients and healthy controls

        Background/Aims: Neuropsychiatric systemic lupus erythematosus (SLE) includes a broad spectrum of neurologic and psychiatric manifestations. One of the most commonly observed neuropsychiatric symptoms is headache. However, the lack of specific clinical distinctions for headache in SLE has made it diffi cult to elucidate its pathophysiology. The aim of this study is to evaluate the neurometabolic changes using Proton Magnetic Resonance Spectroscopy (1H-MRS) in patients with SLE who suffer from chronic daily headache (CDH). Methods: SLE and fibromyalgia patients with CDH and healthy controls were recruited (n = 9, n = 5, and n = 6, respectively). 1H-MRS metabolite ratios were evaluated in bilateral basal ganglia (BG) and bilateral peritrigonal white matter (PWM). Results: 1H-MRS showed a significantly decreased N-acetylaspartate (NAA)/creatine (Cr) ratio in right BG in SLE patients with CDH compared to fibromyalgia patients with CDH and normal controls (p = 0.029 and p = 0.020, respectively). Left PWM NAA/Cr and choline/Cr ratios in SLE patients with CDH were lower than those in fibromyalgia patients with CDH (p = 0.019 and p = 0.029, respectively). Conclusions: This study suggests the possibility that CDH in patients with SLE might be associated with neuronal dysfunction and neurometabolic changes.

        Graves 병 환자에서 항갑상선제 투여에 의한 TSH 수용체 항체의 변동에 관한 연구 : 치료 용량 및 기간에 따른 차이 및 임상상과의 관계 its relation to dose and duration of treatment~and clinical features

        We undertook a prospective study of 59 patients with Graves' disease in order to clarify the influence of antithyroid drug on the immunological indices and to determine the association of the changes of TSH receptor antibodies activities with drug dosage, duration of treatment and clinical features according to mode of antitbyroid drugs treatment. Patients were divided into two groups. High dose group included 28 patients and conventional dose group 31 patients. We measured TBII activities, antimicrosomal antibodies, and the size of goiter every 3 months, comparing them with clinical features and laboratory findings. TBII activities declined from R months until 9 months following drug treatment. Therafter no significant change of TBII activities were observed. Antimicrosomal antibodies showed the same pattern of change. There was no statistically significant difference of declining rate of TBII activities, antimicrosomal antibodies, and reduction of size of thyroid between two treatment groups. TBII activities fell into normal range in 15 patients among those 44 patients who had been followed for more than 9 months (8 high dose group, 7 conventional dose group). In those 15 patients, TBII activities and frequency of exophthalmos were less than those whose TBII activity did not normalized. We conclude that TSH receptor antibodies activities and antithyroid autoantibodies are diminished by antithyroid drug treatment in patients with Graves disease and there is no relationship between dose of drug and declining of TBII activities. We suggest that changing levels of TBII activities might be secondary phenomenon which can be achieved by control of thyroid function by antithyroid drug rather than direct immunosuppressive effect of antithyroid drug per se.

        직접 검화법을 이용한 조제분유의 콜레스테롤 분석법 개발

        본 연구에서는 조제분유 중의 콜레스테롤을 신속하고 정확하게 분석하기 위해 지방 추출 과정을 거치지 않는 직접 검화 방법을 선택하여 분석법 개발을 시도하였다. 조제분유 분말시료를 직접 검화 수기에 취하여 검화 온도, 검화 시간, KOH 농도의 3가지 인자에 대해 콜레스테롤 회수율이 가장 양호하게 나타나는 최적 검화조건을 확립하고, 검화 후 수세과정에서 액액 분배가 용이한 용매 조건도 확립하였다. 또한 콜레스테롤 피크의 완전한 분리를 위한 적정 기기 조건을 확립하였다. 그 결과, 시료 약 2 g에 16 M-KOH 10 mL를 넣고 $90^{\circ}C$에서 60분 가열하여 검화한 후 diethyl ether로 3회 추출하고 hexane을 최종시험용액으로 하여 기기분석을 했을 때의 회수율이 98.80%로서 가장 양호하게 나타났다. 본 연구를 통해 개발된 조제분유의 효율적인 액액분배 및 직접가열 검화법은 일원배치 분산법에 의해 유화가공식품의 콜레스테롤 분석법으로 유효성이 검증되었으며, 아울러 개발된 전처리 방법 및 기기 분석 조건을 활용해 다양한 분석 기관에서 신속 정확하고 효율적인 콜레스테롤 분석을 수행할 수 있을 것으로 기대된다. 또한 산업체의 품질관리 및 검증기관에서 필요시 모니터링에 적극 활용 가능할 것으로 사료되며, 이를 통해 유화가공식품 류의 함량표시 및 규격관리의 정확성과 효율성 증대에 기여하여 제조업체의 정확하고도 효과적인 품질 및 안전성 확보에 기여할 수 있을 것으로 판단된다. 특히 위해 물질에 대한 정확한 함량 판단이 중요한 조제분유 등의 영 유아용 식품의 안전성 확보에 지대한 공헌을 할 수 있을 것으로 기대된다. An improved cholesterol analysis method was developed for powdered infant formula by gas chromatographic separation after liquid-liquid extraction and partition. In the official Korea Food Standard method for cholesterol analysis, the water phase and solvent phase were not well separated in the case of emulsified foods such as powdered infant formulas and baby foods. For the rapid and simple sample preparation method, an optimized direct saponification condition was established for heating temperature, heating time, and KOH concentration. From the results, the optimum conditions were as follows: heating temperature $90^{\circ}C$, heating time 60 min, and 16 M KOH 10 mL for a 2 g infant formula sample; improved separation condition for gas chromatography was as follows: the initial oven condition was $250^{\circ}C$ for 25 min, the oven temperature was increased to $290^{\circ}C$ by $10^{\circ}C$/min ratio, and finally the oven temperature remained at $290^{\circ}C$for 9 min. The developed method could be implemented for the study of cholesterol, providing the advantages of reduced inspection time and cost in emulsified foods such as infant formula.

        본 연구에서는 관절 내시경 시스템 개발을 목표로 임상 교수들의 조언과 사용자의 편의를 우선으로 하는 PC기반 GUI 형태의 관절 내시경 시스템 설계에 초점을 맞추어 연구하였다. 관절 내시경 시스템의 성능 향상을 위한 모듈 설계하여 검증을 통한 시험 및 시뮬레이션 분석을 하였다. 또한 임상 교수의 관절 내시경 시스템 개발 참여로 사용자 중심의 편리한 시스템 구성 및 관련 H/W 및 S/W 기술 축적뿐만 아니라 PC기반 GUI 형태의 윈도우 S/W 개발로 해외 시장개척을 위한 경쟁력이 강화될 것으로 기대된다.

        담체가 Re-188-Hydroxyethylidene Diphosphonate의 표지와 생체내분포에 미치는 영향

        Purpose: Re-188-Hydroxyethylidene diphosphonate (HEDP) is a new cost-effective agent for systemic radioisotope therapy of metastatic bone pain. We investigated the influence of carrier for labeling and biodistribution of Re-188-HEDP using HEDP kit with or without carrier (KReO4). Materials and Methods: The kits (HEDP 15 mg, gentisic acid 4 mg and SnCl2.2H2O 4.5 mg) with or without carrier (KReO4 0.1 mg) were labeled with Re-188 solution, made available from an in-house generator by boiling for 15 min. We compared the labeling efficiency and stability of carrier-added and carrier-free preparations of Re-188-HEDP. Biodistribution and imaging studies of each preparation were performed in ICR mice (1.85∼3.7 MBq/0.1 ml) and SD rats (74.1∼85.2 MBq/0.5 ml). Results: The carrier-added preparation showed high labeling efficiency (95% at pH 5) and high stability in serum (88%, 3 hr). However, the carrier-free preparation showed low labeling efficiency (59% at pH 5) and low stability (43%, 3 hr). The carrier-added preration showed high uptake in bone and low uptake in stomach and kidneys. However, the carrier-free preparation showed lower uptake in bone and higher uptake in both stomach and kidneys, which is supposed to be due to released perrhenate. The carrier-added preparation also showed better images with higher skeletal accumulation, lower uptake in other organs and lower soft tissue uptake than the carrier-free preparation. Conclusion: The results of these studies clearly demonstrate that addition of carrier perrhenate is required for high labeling efficiency, stability, bone uptake and good image quality of Re-188-HEDP.       • 이동물체 추적 시스템 구현에 있어서 최소자승법을 가미한 Voting기법 오차분석

        Optical flow method is frequently used for information estimation of multi moving objects. However, it is so difficult to get accurate solution by problem of aperture or discontinuities. In order to solve this problem, we proposed one of multi moving object tracking algorithm based on the combinatorial Hough Transform (CHT) and voting accumulation, and considered operation time and error analysis. In this paper, we extracted moving areas of moving objects from differential image using edge detection, filtering, region extension, and segmentation for each moving area. After finding real moving objects, Combinatorial Hough Transform (CHT) and voting accumulation is used to find the optimal constraint lines for optical flow estimation. From optical flow vectors, we find the absolute vector for the direction and speed of the moving objects. The simulation results show that the proposed method is very effective for extracting optical flow vectors and hence recognizing moving objects in the images. Moving objects are extracted to reduce the computation time for Combinatorial Hough Transform. The computational time is highly reduced by finding the moving objects using edge detection and segmentation. Optical flow constraints are solved using multi constraint and Combinatorial Hough Transform, which avoids the problems associated with the least-square based optical flow methods.

        Data Warehouse is a repository of integrated information, available for queries and analysis. A materialized view is kept at a site called the data warehouse, and user queries are processed using this view. The view has to be maintained to reflect the updates done against the base relations stored at the data source. Hence, view maintenance is important when base relations are changed. Therefore, we propose an enhanced algorithm to improve query processing performance and also reduce response time.

        Endoscopic submucosal tunnel dissection salvage technique for ulcerative early gastric cancer.

        <P>Endoscopic submucosal dissection is an effective treatment modality for early gastric cancer (EGC), though the submucosal fibrosis found in ulcerative EGC is an obstacle for successful treatment. This report presents two cases of ulcerative EGC in two males, 73- and 80-year-old, with severe fibrosis. As endoscopic ultrasonography suggested that the EGCs had invaded the submucosal layer, the endoscopic submucosal tunnel dissection salvage technique was utilized for complete resection of the lesions. Although surgical gastrectomy was originally scheduled, the two patients had severe coronary heart disease, and surgeries were refused because of the risks associated with their heart conditions. The endoscopic submucosal tunnel dissection salvage technique procedures described in these cases were performed under conscious sedation, and were completed within 30 min. The complete en bloc resection of EGC using endoscopic submucosal tunnel dissection salvage technique was possible with a free resection margin, and no other complications were noted during the procedure. This is the first known report concerning the use of the endoscopic submucosal tunnel dissection salvage technique salvage technique for treatment of ulcerative EGC. We demonstrate that endoscopic submucosal tunnel dissection salvage technique it is a feasible method showing several advantages over endoscopic submucosal dissection for cases of EGC with fibrosis.       • 병원의 CRM 마케팅 적용에 관한 연구 : 부산지역 대학병원 종사자들의 의식을 중심으로

        Presently, the management of hospitals is getting worse due to the ever-serious environmental factors of the inside and outside, and the competition between hospitals which are suppliers of medical service is getting keener and keener. Under this circumstance, medical industry is now keeping on trying to gpin the ascendency in the stuggle for existence by implementing a marketing differentiated from other hospitals, and making an attempt to use a new marketmg technique, recognizing patients as customers, espedally preparing for the introduction of CRM (Customer relationship management) system centering on maintaining the existing customers rather than attracting new ones and ino-easing the degree of customer loyalty. Indeed, some medium and small hospitals are implementing this system. Thus this paper is a narrative study investigating the relationship between the introduction of CRM in hospitals and the request degree of employees of its introducdon, their sadsfacdon degree of the real condidon of customer management system, the degree of recognition of its necessity and the extent of whether they reuse a hospital in case they become an object of customer management conducted by the hospital. The study subject was 169 employees working in four university hospitals located in Busan area, and its term of collecting data was until April 17, 2003. In corclusion, hospital employees did not know CRM well. Moreover, the lower the satisfaction degree of the actual condition of the customer management, the higher the recognition degree of its necessity and the higher the degree of intention of whether they reuse a hospital if they become an object of the customer management of the hospital, the higher the degree of request for the introduction of CRM.
