        공변량 구조분석을 이용한 국내 간호연구의 동향

        This study was conducted by analyzing all 33 articles based on the LISREL, published from January 1991 to March 1999 in Korea. The analyses consisted of the publication date of articles, principal dependent variables, subjects of the research. adequacy of sampling, adequacy of research purposes and results. accordance between theoretical model and hypothetical model. fit measures, theoretical base of model modification, and adequacy of conclusion. The results were as follows : ? The thesis of 33 articles in total were outnumbered as 25 (75.8%) to 8 (24.2%) research articles. As for a sex classification of the subjects, 45.5% of the research were conducted around a female group of subjects. while 54.5% were done for both sex, The range of the sample size was 105 to 803, and the average was 259 subjects. ? A single theoretical variable was measured for each measurement variable, any difference between variables was hardly found in 8 articles (24.2%), and 19 articles (57.6%) did not consider any measurement error. To analyze if the representative has been considered while collecting the data. most data were collected by a convenient sampling. Seven articles (21.2%) were seen with a sign of a representative. Questionnaires were used in a majority (31 articles) of the data collecting process. Only 2 articles (6.1%) were measured with a physiologic index simultaneously. 14 articles (42.2%) were centered on theory development, 10 articles on theory synthesis. and 9 articles on theory test. The research purposes and results were consistent in 25 articles (75.5%) and 8 articles (24.2%) were inconsistent. The quality of life and health promotion behavior were the concepts most frequently studied as a dependent variable. and 7 articles centered on them. In applied theories a health promotion model was used on 4articles (12.1%), while role theory and stress-coping models were in 3 articles respectively. The articles were analyzed to see if the hypothetical model was elaborated and tested by the theoretical model. Twenty-five articles proved to be rationale for the inconsistencies. Also, 56.5% proposed hypotheses were supported among the subject articles. and 30 articles (90.0%) suggested a revised model. Path coefficient (17 articles) and theoretical adequacy (17 articles) were the standards mostly used. In conclusion. the principal factors were obtained from the research are to be considered as the principes of LISREL application. First. a model has to be established on a theoretical base rather than empirical results dependent on the data. The results are also required to be globally interpreted. Secondly. at least 200 samples are necessary to satisfy the need. Third. more than 3 measurement variables are to be adjusted to a single theoretical variable: the measurement errors must be suggested as well. Finally. normal distribution characteristics of the data and the estimation method need to be reported. Based on the research result. the follows are suggested: ? Systematic criteria on the LISREL application and procedure need to be developed ? Agreement form is required to report the results of research using the LISREL

        Prediction Model of Exercise Behaviors in Patients with Arthritis : by Pender's revised Health Promotion Model

        The aims of this study were to understand and to predict the determinent factors affecting the exercise behaviors and physical fitness by testing the Pender's revised health promotion model, and to help the patients with rheumatoid arthritis and osteoarthritis perform the continous exercise program, and to help them maximize the physical effect such as muscle strength, endurance, and fuctional status and mental effects including self efficacy and quality of life, and improve the physical and mental well being, and to provide a basis for the nursing intervention strategies. Of the selected variables in this study, the endogenous variables included the physical fitness, exercise score, exercise participation, perceived benefits of action, perceived barriers of action to exercise, activity-related affect(depression) and perceived self-efficacy, interpersonal influences(family support), situational factors(duration of arthritis, fatigue) and the exogenous variables included personal sociocultural factor(education level), personal biologic factor(body mass index), personal psychologic factor(perceived health status) and prior related behavior factors(previous participation in exercise, life-style). We analyzed the clinical records of 208 patients with rheumatoid arthritis and degenerative arthritis who visited the outpatient clinics at H university hospital in Seoul. Data were composed of self reported qustionnaire and mood of fitness score which were obtained by padalling the ergometer of bicycle for 9 minutes. SPSS Win 8.0 and Window LISREL 8.12a were used for statistical analysis. Of 75 hypothetical paths that influence on physical fitness, exercise participation, exercise score, perceived benefits of action, perceived barriers of action to exercise, activity-related affact(depression) and perceived self-efficacy, interpersonal influences (family support), situational factors(duration of arthritis, fatigue), 40 were supported. The physical fitness was directly influenced by life-style, perceived health status, education level, family support, fatigue, which explained 12% of physical fatness. The exercise participation were directly influenced by life-style, education level, past exercise behavior, perceived benefits of action, perceived barriers of action, depression and duration of arthritis, which explained 47% of exercise participation. Exercise score were directly affected by perceived self efficacy, BMI, life-style, past exercise behavior, perceived benefits of action, family support, perceived health status, perceived barriers of action, and fatigue, which explained 70%. Perceived benefits of action was directly influenced by BMI, life-style, which explained 39% perceived barriers of action were directly influeced by Past exercise behavior, perceived health status, which explained 7%. Perceived self efficacy were directly influeced by level of education, perceived health status, life-style, which explained 57%. Depression were directly influeced by past exercise behavior, BMI, life-style, which explained 27%. Family support were directly influeced by life-style, perceived health status, which explained 29%. Fatigue were directly influeced by BMI, life-style, perceived health status, which explained 41%. Duration of arthritis were directly influeced by life-style, past exercise behavior, BMI, which explained 6%. In conclusion, important variables for physical fitness were life-style, and variable affecting exercise participation were life-style Perceived self-efficacy of exercise was a significant predictor of exercise score. BMI, Life-style, perceived benefits of action, family support, past exercise behavior showed direct effects on perceived self-efficacy. Therefore, disease related factor should be minimized for physical performance and well being in nursing intervention for patients with rheumatoid arthritis, and plans to promote and continue exercise should be seeked to reduce disability. In addition, Exercise program should be planned and performed by the exact evaluation of exercise according to the ability of the patients and the contents to improve the importance of exercise and self efficacy in self control program, dedicated educational program should be involved. This study suggest that the methods to reduce the disease related factors, the importance of daily life-style, recognition of benefit of exercise, and educational program to promote self efficacy should be considered in the exercise behavior promotion and nursing intervention for continous performance. The significance of this study is also thought to provide patients with chronic arthritis the specific data for maximal physical and mental well being through exercise, chronic therapeutic procedure, daily adaptation and confrontation in nursing intervention

        만성관절염과 루푸스의 피로, 통증, 및 통증효능감에 관한 연구

        This study was designed to distinguish by the characteristic difference and the degree of symptoms such as fatigue, pain, coping to pain, and efficacy on pain, and to offer descriptive data for nursing intervention for improving coping ability to pain along each characteristics of disease in chronic arthritis and systemic lupus erythematosus. The subjects were 135 outpatients in the hospital for rheumatic disease in H-University, Seoul. The data were collected by structural questionnaire, from April 29 to June 29, 1999. The results were that the fatigue score was high in the OA patients while the RA patients and SLE patients experienced middle range of fatigue score, but which was not statistically different. Although the RA patients felt higher pain than other diseases, they have well coped with their pain than the others. In efficacy on pain the SLE patients had higher score than others but all of the disease showed lower score. No statistically significant difference among the three croup was recorded in efficacy on pain. Therefore, pain management in the RA patients was primary nursing intervention because they felt severe pain and have well coped with pain while they had lower pain efficacy score than the others. It is also important that fatigue management and coping strategies on pain for the OA patients and SLE patients are specially supportive in the nursing intervention.

        장애인의 재활 동기 측정도구 개발

        Purpose: to develop an instrument to assess the rehabilitation motivation and to test its reliability and validity. Method: The data was collected with in-depth interviews of nine participants that include adults with disabilities acquired after they reached 18 years' of age. The development of the preliminary items was based on the theory of self-determination proposed by Deci & Ryan (1985) and the results from the analysis of the data. After these, appropriate specialists clarified the validity of the contents of the preliminary items twice. A test to reliability and validity was conducted with 186 disabled. A principal axis factor analysis with oblimin rotation was performed on 29 items. Result: Finally 27 items were selected. The 5-factor solution emerged as the most appropriate model for the data. The different types of motivation for rehabilitation were identified as: task-oriented motivation, change-oriented motivation, obligatory motivation, external motivation, and amotivation. The alpha coefficient of internal consistency was .85. Conclusion: The results of this study suggest that the instrument to measure motivation for the rehabilitation of the disabled may be a useful instrument with a high degree of reliability and validity.

        류마티스 질환자의 피로와 통증 비교연구 : 강직성척추염, 섬유조직염, 루프스환자를 중심으로

        This study was designed to order descriptive data for nursing intervention for relief of fatigue and pain, and to distinguish by the characteristic difference and the symptoms such as fatigue and pain on Ankylosing Spondylitis(AS), Fibromyalgia(FM), and Systemic Lupus Erythematosus(SLE) patients. The sample consisted of 92 Patients(AS 29: FM 30; SLE 33) who visited H-University Rheumatism Hospital in Seoul The data were collected by a structured questionnaire from May 1. 1999 to April 30, 2000. The results were as follows: Patients of 95% experienced fatigue in the last week and a fatigue score of three disease groups were above average. The fatigue score of FM patients was highest in the other disease, but which was not a statistically significant difference(F= 1.417. p=.248). The mean score of AS and FM patients in pain was higher than the SLE patients, and there was the statistical significance among the three groups on pain (F=8.239. p=.001). There wasn't a statistical difference among three groups on coping wtih pain(F= 1.451, p=.240). There wasn't any correlation between fatigue and pain in each disease (AS. r=.008. p=.966; FM: r=.328, p=.077: SLE: r=.237,p=.185). Therefore, morning stillness and pain management during sleeping is needed through good body alignment in the AS patients. Adequate rest for fatigue and multiple coping strategies for pain maybe basic nursing intervention in FM and SLE. According to their fatigue rhythm. a regular exercise program is needed for rheumatic disease because they complained of fatigue above average and their fatigue was repeated bettor and worse only during the one week.

      • 청소년의 체형인식과 체중조절에 관한 실태 조사

        This purpose of this survey was to investigate status recognition and weight contrast of high school students and college in Taejon. These results were summarized as follows: In the heights and weights those of male 178.74±6.31cm and 65.49±9.44kg, those of female were 162.09±4.36cm and 51.95±5.95kg. By broca index, 53.7% of subjects were normal, 38.2% were lean, 5.3% were overweight and 2.8% were obese. 44.4% of male have recognized their body form as normal, 30.5% of female have recognized their body form as overweight, 47.8% of lean groups answered that they were normal. In normal groups, 46.2% answered normal and 28.9% answered some fatty. 46.8% of normal group in female misunderstood that they were some fatty. In spite of not to weight control, lean and normal group answered that they were weight control. Weight control methods were used diet therapy (66.2%). therapy+exercise(22.8%) and exercise(9.0%). Reason to select therapy of weight control were one's recommendation(45.8%) and by mass media(3.8%). Weight loss and period of weight control were 1.9days, 0.4kg in high school students, 7.3days, 0.7kg in college students, and 3.8days, 0.5kg in male, 5.7days, 0.6kg in female. The results of weight control, 65.0% were failed. The reason of failure in weight control, 42.4% answered because of hungry.

      • 대전지역 대학생의 학교 식당 이용실태 및 만족도 조사

        The aspects and satisfaction on university foodservice were investigated to provide basic information for better foodservice in university by questionnaires. The survey was conducted in Daejon, and data were analyzed by SAS. Although 87.3% of subjects were under foodservice, only 2.0% of them preferred the foods served in university. The most favorite reason for foodservice was indicated as no need on lunch bag(31.1%) and frendship of classmates(25.5%). The most unfavorite reason for foodservice was indicated as bad taste of foods(68.7%) or many dislike foods(13.2%), also dirty eating place(7.7%). The subject liked ssalbap, baechu kimchi, beef foods. The acceptance about kongbap, yulmu kirnchi, beef products was higher in men than women(p<0.05). The preference of mieokkuk was higher in women and that of beef soup was higher in men(p<0.05). The degree of satisfaction was lower in men than in women about the amount of foods(p<0.001). Also the degree of satisfaction was lower in women than in men about sanitary condition of eating place. 54.6% of them had taken the unwanted materials in foods and 3% of them experienced the deteriorated food. The results suggest that food preference of subjects should be considered in meal planning and various cooking method should be utilized to enhance taste of foods for better foodservice. Also, the importance of various foods intake must be periodically and continuously taught to them through nutrition education.

      • 지역주민의 생활의 질 향상을 위한 대학의 사회교육 프로그램 개발

        The purposes of this study were to investigate problems related to quality of life and to identify residents needs for the university educational program for the community residents. The data were analyzed by using X²-test, F-test and multiple range test. Findings of this study provided some useful implications to develop educational programs for community people. These implications are as follows. In the aspect of home management, the need for education about time management was relatively high and the degree of time conflicts was varied to family size, educational level of house, housewife's employment status and outside work time. In the program area of economic life, education for enhancing ability to cope with unexpected events and impulsive consumption behavior, and education for money management technique and value clarification should be included. There were significant differences in the parents' role difficulties according to mother's age and income. Especially parents' role difficulties were children's social emotional development and educational activity. The major problems in management of clothing and textile products were the change of textile sensation, the poor quality of accessaries, and discoloration. This result suggests that education programs on management of clothing and textiles should be developed. In health and food management it showed that subjects were afraid of cancer, hypertension, cerebral vascular diseases in relation with food habits. Hence program of education for health and good food habits should be developed.

        간호사 국가고시의 문제점과 개선방안에 대한 전문가 집단 연구

        Purpose: The purpose of this study was to investigate the plan to introduce a practical skills test as part of the national examination for nursing licensure. Method: The delphi method was used. The first set of data was collected by open questionnaire from universities and hospitals nationally. The second and third were collected by structured questionnaire. Results: The biggest problem for clinical practice in nursing education was reported as the lack of opportunities for direct nursing practice. The biggest performance problem in job duties for novice nurses was reported as the lack of integrated problem solving skills in a clinical situation. The biggest problem with the current national examination was reported as the use memorizing solutions to test questions. It was suggested that the best plan to resolve problems with the national examination would be to develop test questions representative of the clinical setting and increase personnel in clinical education. About the introduction of a practical skills test as part of the national examination, 56.9% were in agreement and 38.5% disagreed. Conclusion: The current national examination has many problems when it comes to testing clinical competency. So, a practical skills test must be deferred to a future time. There are also many difficulties in presenting a real situation, therefore further research is needed in preparation for the introduction of a practical skills test.
