N/A Background: Magnetocardiogram (MCG), which records the changes of magnetic fields generated by the heart`s electrical activity, theoritically can provide unique data for clinical application. To date, MCG has been investigated only at a single time point after myocardial infarction (MI) with severe left ventricular dysfunction in rats. The purpose of the present study was to investigate sequential changes of MCG after MI and to evaluate effects of infarct size on MCG. Methods: Acute MI were induced by the permanent ligation of left coronary artery in 22 rats. Magnetic fields were recorded just above a rat with Nb Superconducting Quantum Interference Device (SQUID) gradiometer inside a magnetically shielded room. MCG was measured before and immediately after surgery and it was subsequently recorded at the time points of 1, 4 and 6 hours post operatively. MCG was also measured at 1, 3, 7 and 21 days after surgery. Results: Elevation of ST segment and appearance of pathological Q wave on the MCG were evident immediately after the ligation of coronary artery and persisted to 6 hours after MI. On MCG, ST segment was depressed and T wave was inverted from 1 day after MI. In rats with small-and moderate- sized MI (infarct size〈30%), ST depression returned to near the isoelectric level and Q wave disappeared from 7 days after MI. However, ST depression and Q wave were still present in rats with larger infarct (infarct size≥30%). Conclusion: Evolutional changes of MCG were well-recognized up to 21 days after MI. Furthermore, the infarct size can be expressed by the extent of Q wave and ST segment depression on MCG. Taken together, these data indicate that MCG is a helpful modality for the diagnosis, evaluation of infarct size and follow up after MI.(Korean J Med 62:42-48, 2002)
Thalidomide for Treating Metastatic Hepatocellular Carcinoma: A Pilot Study

Background: Thalidomide has been reported to have antitumor activity for treating metastatic hepatocellular carcinoma (HCC). We evaluated the safety and efficacy of using thalidomide for treating selected patients with unresectable or metastatic HCC, and their disease was refractory to systemic chemotherapy. Methods: Eight patients with measurable and metastatic HCC that had progressed with prior systemic chemotherapy and who desired further active therapy were enrolled in this study. Thalidomide was given orally at bedtime and it was started at 200 mg/day with no further dose escalation. The response was measured at 2-month intervals. Results: The median age was 44 years (range: 34-52 years) and all the patients had received doxorubicin-based systemic chemotherapy prior to their enrollment. Each patient received thalidomide for a median of 152 days (range: 5-422 days). One partial response was observed (12.5%, 95% CI; 0-42%) along with 4 cases of stable diseases. The most commonly encountered toxicity was somnolence; grade 3 somnolence was noted for one patient, which led to treatment discontinuation. Skin rash was observed in one responding patient. Conclusions: The results indicate that thalidomide may feasibly offer disease stabilization to metastatic HCC patients. Further dose escalation of thalidomide, or its combination with other chemotherapeutic agents, may be of interest and this should be investigated for treating patients with metastatic HCC.

The purpose of this study is to analyze the patterns of farming materials required for creating and operating elementary school vegetable gardens and to propose a plan for vitalizing business related to urban agriculture. As a result of the survey, the farming materials purchased most for the operation of the school gardens were seeds materials focused on seedlings, compost/fertilizer, and farming tools focused on trowels, sprinklers, and hoes. Purchases of farming materials were made mostly through the internet, while most purchases of seedlings were made in pesticide stores. Therefore, it is necessary to provide information and education related to the use of farming materials. Thus, establishment and maintenance of highly accessible and reliable internet sources are recommended to provide the lists of farm products necessary as well as related information to help users in purchase and use in order to create and operate the school garden. In order to supply high-quality seedlings, it is necessary to provide information and education on the kinds of seedlings, management, cultivation, delivery methods, and the like, focusing on pesticide stores producing and distributing seedlings. Providing such information for participants will improve their satisfaction. Also, if businesses related to urban agriculture use these study results in producing, preparing, managing and selling seedlings and farming materials, they will also be able to generate profits and vitalize urban agriculture.

Background: Here we report a rare case of type 1 gastric neuroendocrine tumor with coexisting adenocarcinoma in the same tumor. Methods: A 49-year-old male sought a medical advice because of gastric carcinoid lesion that had been detected by a mass screening though the man felt no symptoms. Serum carcinoembryonic antigen level was 0.73ng/ml and serum gastric level was 390.5pg/ml. The stomach dynamic CT staging was TxN0M0. There was no evidence of regional lymph node enlargement or distant metastases. Endoscopic ultra sonography shows about 3x5mm sized inverted cone shaped hypoechoic lesion confi ned to 2nd& superfi cial 3rd layer of gastric wall. Gastroendoscopy showed two seperate subepithelial lesion with surface erosion at greater curvature side of distal body. We did endoscopic submucosal dissection. Results: The histopathology revealed well differentiated adenocarcinoma with neuroendocrine differentiation. The resection margins were free of tumor. Immunohistochemical staining with Chromogranin-A, cytokeratin, synaptophysin, CD56 was positive confi rmed the presence of carcinoid component. He was followed-up for about two month after ESD. In PET-CT, there were no abnormal FDG uptake in stomach, and no metabolic evidence of metastasis. After three year later, we followed-up gastroendoscopy, abdomen and pelvic CT, PET-CT. There noted any evidence of local or distant recurrence. Conclusions: Type 1 gastric neuroendocrine tumor can coexisting adenocarcinoma, so we consider carefully.
Neural Tract Injuries by Penetration of Foreign Body: a Diffusion Tensor Tractography Study

We presented with a patient who showed injury of the cingulum and fornix by penetration of a foreign body into the brain on diffusion tensor tractography (DTT). A 63-year-old man suffered a brain injury by a part of a power saw blade that was suddenly detached from a power saw during work. A part of the power saw blade penetrated his right frontal skull and advanced to the right posterior horn of the lateral ventricle. This penetration caused traumatic intracerebral hemorrhage in the right frontal lobe and intraventricular hemorrhage in the lateral ventricle. He underwent craniotomy and removal of intracranial foreign bodies (bony pieces and saw blade). The patient's Memory Assessment Scale scores were 74 (4%ile) for global memory, 78 (7%ile) for verbal memory, and 80 (9%ile) for visual memory. DTTs showed disruptions in the anterior portion of the fornical body, right fornical crus, the anterior portion of the right cingulum, and the middle portion of the left cingulum, compared to the control. It seems that the sustained memory impairment of this patient might be related to injury of the cingulum and fornix.
Analysis of Major Foodborne Pathogens in Various Foods in Korea

Foodborne pathogenic bacteria in various food samples in Korea were monitored and the obtained data was statistically analyzed. A total of 1,240 food samples including 280 sashimi, 244 processed frozen products, 258 kimbab (cooked rice wrapped with seaweed), 337 soybean pastes were obtained from 7 cities including Seoul in Korea. Microorganisms tested were Bacillus cereus, Salmonella spp., Staphylococcus aureus, Escherichia coli, E. coli O157:H7, Vibrio parahaemolyticus, Yersinia enterocolitica, Listeria monocytogenes, Campylobacter jejuni, and Clostridium perfringens. The contaminated microorganisms in food samples were comprised of 10.55% B. cereus, 2.7% S. aureus, 2.0% V. parahaemolyticus, 0.8% C. perfringens, 0.2% Y. enterocolitica, and 0.1% of L. monocytogenes, respectively. Salmonella spp., C. jejuni, and E. coli O157:H7 were not detected in any of the food samples. Particularly, B. cereus that harbors the enterotoxin gene was detected in various foods and regions in Korea, therefore it should be a given special consideration not to allow the hazardous level of contamination.
