        Anti-inflammatory Effects of Flavonoids on TNBS-induced Colitis of Rats

        It has been shown that the extracts including eupatilin and quercetin-3-${\beta}$-D-glucuronopyranoside had mucoprotective effects on the esophagus and stomach through their antioxidant activities. This study was designed to investigate the anti-inflammatory effect of these flavonoid compounds in an animal model of inflammatory bowel disease induced by 2,4,6-trinitrobenzene sulfonic acid. Experimental colitis was induced by intracolonic administration of 2,4,6-trinitrobenzene sulfonic acid. Extracts including eupatilin or quercetin-3-${\beta}$-D-glucuronopyranoside were orally administered to animals 48, 24, and 1 h prior to the induction of colitis and then again 24 h later. The animals were sacrificed 48 h after by 2,4,6-trinitrobenzene sulfonic acid treatment and the macroscopic appearance of the colonic lesions was scored in a blinded manner on a scale of 1 to 10. The inflammatory response to colitis induction was assessed by measuring myeloperoxidase activity, nitric oxide production, tumor necrosis factor-${\alpha}$ expression, total glutathione levels, and malondialdehyde concentrations in the colon. The results indicated that extracts including eupatilin and extracts including quercetin-3-${\beta}$-D-glucuronopyranoside dose-dependently improved the morphology of the lesions induced by 2,4,6-trinitrobenzene sulfonic acid and reduced the ulcer index accordingly. In addition, rats receiving extracts including eupatilin and extracts including quercetin-3-${\beta}$-D-glucuronopyranoside showed significantly decreased levels of mucosal myeloperoxidase activity, nitric oxide production, tumor necrosis factor-${\alpha}$ expression, and malondialdehyde levels, and increased total glutathione levels. Extracts including eupatilin and extracts including quercetin-3-${\beta}$-D-glucuronopyranoside ameliorated the inflammatory response and colonic injury in acute colitis by decreasing oxidative stress and neutrophil activation. Extracts including eupatilin and extracts including quercetin-3-${\beta}$-D-glucuronopyranoside may inhibit acute colitis.

        Background: Triflusal is a drug that inhibits platelet aggregation. In this study we investigated the dose-exposureresponse relationship of a triflusal formulation by population pharmacokinetic (PK) and pharmacodynamic (PD) modeling of its main active metabolite, hydroxy-4-(trifluoromethyl) benzoic acid (HTB). Methods: This study was a randomized, open-label, multiple-dose, two-period, two-treatment, comparative crossover design. All volunteers received a single oral loading dose of 900 mg of triflusal on Day 1, followed by a dose of 600 mg/day from Day 2 to 9. Using data from 34 healthy volunteers, 476 HTB plasma concentration data points and 340 platelet aggregation data points were used to construct PK and PD models respectively using NONMEM (version 6.2). As the PD endpoint was qualitative, we implemented binary analysis of ‘inhibition’ and ‘non-inhibition’ rather than using the actual value of the test. The final PK-PD model was evaluated using a visual predictive check (VPC) and bootstrap. Results: The time-concentration profile of HTB over the entire dosing period was described by a one-compartment model with a first-order formation rate constant for HTB. Weight was selected as a covariate for clearance and volume of triflusal, respectively. The structure and the population estimates for triflusal PK were as follows: oral clearance (CL/F) = 0.2 · (weight/71.65)0.845 L/h, oral volume of distribution (V/F) = 8.3 · (weight/71.65) L, and kf = 0.341 h.1. A sigmoid relationship between triflusal concentration and the probability of significant inhibition with shape factor was chosen as the final PD model. No time delay between concentration and response was identified. The final structure between predicted concentration ðCy pred;ijÞ and the probability of inhibition of platelet aggregation (IPA) relationship was as follows: Probability of IPA ¼ C19 pred;ij=ð84:919μg=mL þ C19 pred;ijÞ. Thus, we concluded this relationship is more like quantal concentration-response relationship. The current dosing regimen was considered to be efficacious based on the EC50 estimate of 84.9 μg/mL obtained in this study. Conclusions: A PK and binary probability PD model of triflusal was successfully developed for Korean healthy volunteers. The model may be used to further prediction inhibition of platelet aggregation by triflusal.         A Two-week Interval Is Better Than a Three-week Interval for Reducing the Recurrence Rate of Hand-foot Viral Warts after Cryotherapy: A Retrospective Review of 560 Hand-foot Viral Warts Patients

        Background: Viral warts are a common infectious disease and liquid nitrogen cryotherapy is one of the most common methods for treatment of these warts. Hand-foot viral warts frequently recur and reduce quality of life as well. Objective: To find the ideal treatment interval between cryotherapy sessions that can influence not only the cure rate but also the recurrence rate for hand-foot viral warts. Methods: A retrospective study was designed to compare a 2 week interval and a 3 week interval between cryotherapy sessions on hand-foot viral warts with respect to cure rate, recurrence rate, treatment number, duration of treatment, mean time to recurrence and adverse events. Results: A total of 560 patients were enrolled. The overall cure rate was 75.7% and the recurrence rate was 19.6%. The mean time to recurrence was 7.8 months (range 1~26 months). For the 2-week and 3-week groups, cure rates were, respectively, 76.6% (196) and 75.0% (228); recurrence rates were 13.3% (26) and 25.0% (57). The mean time to recurrence was 9.8 months and 6.9 months, respectively. Adverse events were not statistically different. Conclusion: We suggested that 2-week cryotherapy is optimal not only because of the rapid cure but also because of the lower recurrence rate and similar adverse events.       • S-226 Impact of High sensitivity troponin I on management of acute chest pain

        Background/Introduction: It is challenging for physicians to manage patients presenting with acute chest pain, especially those with non-diagnostic ECG and normal initial set of cardiac enzymes. Compared with conventional Troponin, high sensitivity troponin (hs-TnI) have greater sensitivity and negative predictive value, conversely reduced specificity and positive predictive value.?Purpose: We aimed to investigate the impact of hs-TnI on clinical decision, including influence on admission rate and diagnostic tools.?Methods: Among total 3170 patients presenting acute chest pain, those with elevated cardiac markers and diagnostic changes in ECG were excluded. Total 128 patients with acute chest pain were assessed with either conventional troponin (cTnI) (N=59, From March to May of 2014, cTnI group), or high sensitive troponin (hs-cTnI) (N=69, From March to May of 2015, hs-cTnI group). First, we compared the admission rate between cTnI and hs-cTnI groups (Outpatient follow-up, admission to General ward or admission to ICU). Second, we sought the difference of diagnostic tools between two groups after ER discharge (Conventional angiogram, Coronary CT or Functional testing).?Results: There were no differences in baseline characteristics between two groups including demographic and cardiovascular risk factors. The hs-TnI group show low admission rate to general ward (28.8% vs 13%, p<0.05) and more outpatient follow-up visit after discharge (32.2% vs 52.2%. p<0.05). But ICU admission rate show no difference between two groups (39.0% vs 34.8%). In view of diagnostic procedures which specialist choose after ER discharge, there are differences between two groups, showing fewer conventional angiogram (59.3% vs 42%, p<0.05) in hs-TnI group, more functional tests with TMT or stress Echo in cTnI group (1.70% vs 17.4%, p<0.05). Conclusions: With emphasis on negative predictive value of hs-TnI in patients with non-diagnostic ECG and negative cardiac enzymes, the introduction of hs-TnI replacing conventional troponin modified the management of chest pain toward less aggressive approach, in means of reduced admission rate and invasive testing in real world setting.

        The Frequency and Distribution of Unexpected Red Cell Antibodies and Analysis of Antigen Exposure

        Red cell alloantibodies other than naturally occurring anti-A or anti-B are called unexpected red cell antibodies, and can be detected by performing an antibody screening. The frequency and distribution of unexpected antibody identified in Asan Medical Center were analyzed. We investigated a total of 135,238 cases of antibody screening test in AMC for 3 years from 2010 to 2012. Using column agglutination techniques, antibody identification tests were performed for the cases with positive antibody screening. Among 135,238 cases, 854 (0.6%) cases showed positive results of antibody screening test. In the order of frequency, 284 (33.3%) anti-Rh, 89 (10.4%) anti-MNS, 62 (7.3%) anti-Lewis, 34 (4.0%) anti-Kidd, 10 (1.2%) anti-Duffy, and 9 (1.1%) anti-P were identified. Multiple antibodies were detected in 199 (23.3%) cases. Among 381 subjects investigated for transfusion history, 299 (78.5%) had history of transfusion while 82 (21.5%) had unknown history. Thus the incidence of unexpected antibody was higher in the group with history of transfusion than the group without (p<0.001). Also, among 435 subjects investigated for the history of pregnancy, 46 (10.6%) had no history while 389 (89.4%) had history of pregnancy, showing higher incidence of unexpected antibody in the group with history of pregnancy than the group without pregnancy (p<0.001). Evaluated amounts and frequency of antigen exposure due to transfusion and pregnancy is suggested to increase the frequency of identification of unexpected antibody.

      • Design and implementation of ARIP for efficient handovers

        <P><B>Abstract</B></P>10.1002/dac.1082.abs<P>Mobile IPv6 (MIPv6), which is a protocol being standardized by Internet Engineering Task Force for global IP mobility support, is expected to have a key role for future All‐IP wireless networks. As the demands for delay sensitive multimedia applications increase, the handover latency and packet losses of MIPv6 become inadequate for these applications. Fast Handovers for Mobile IPv6 (FMIPv6) has been proposed to tackle the problem. However, FMIPv6 cannot work solely since it needs access router information for its fast handover operation. In this paper, we propose Access Router Information Protocol (ARIP), which performs a candidate access router discovery and a radio access network discovery in one protocol suite, to provide an efficient handover framework for MIPv6 and FMIPv6. ARIP is designed to cope with heterogeneous networks as well as homogeneous networks and to be closely coupled with FMIPv6 to provide the efficient handover performance. The performance evaluation of MIPv6, FMIPv6, and FMIPv6 with ARIP is performed in an actual network environment. The performance results show that FMIPv6 with ARIP can significantly improve the handover performance compared to the conventional handover approaches. Copyright © 2009 John Wiley & Sons, Ltd.</P>

      • YinYang1 deficiency ameliorates joint inflammation in a murine model of rheumatoid arthritis by modulating Th17 cell activation

        <P><B>Abstract</B></P> <P>Yin Yang 1 (YY1) is a ubiquitously expressed transcription factor that functions in cooperation with various cofactors to regulate gene expression. In the immune system, YY1 enhances cytokine production and T helper (Th) 2 effector cell differentiation, resulting in the activation of inflammation. However, no studies have reported the role of YY1 in Th17 cell regulation, which is implicated in rheumatoid arthritis (RA). We investigated the expression of YY1 in Th17 cells <I>in vitro</I> and revealed increased levels of YY1 mRNA and protein. To elucidate the function of YY1 pathogenesis in RA, we used a collagen-induced arthritis (CIA) mouse model with YY1 deficiency. Deficiency of YY1 reduced the severity of arthritis and joint destruction. Moreover, Th17 cells were dramatically reduced in YY1-deficient mice. The cytokine interleukin (IL)-17 was decreased in YY1-deficient CD4+ T cells <I>ex vivo</I> and <I>in vivo</I>. Interestingly, the level of signal transducer and activator of transcription 3 (STAT3), tumor necrosis factor-α, IL-17, IL-6, and IL-1β were markedly decreased in YY1-deficient mice with CIA. The cytokine-inducing function of YY1 was more specific to IL-17 than to interferon-γ. YY1 plays a role in Th17 cell differentiation and RA pathogenesis. Our findings suggest that future RA therapies should target the regulatory mechanism involved in Th17 cell differentiation, in which YY1 may cooperate with the STAT3 signaling pathway.</P> <P><B>Highlights</B></P> <P> <UL> <LI> YY1 as new target for RA therapies involved in Th17 cell differentiation. </LI> </UL> </P>

      • The effectiveness of low-fluence fractional CO<sub>2</sub> laser in the treatment of idiopathic guttate hypomelanosis

        Background: Idiopathic guttate hypomelanosis (IGH) is common acquired leukoderma characterized by discrete, tiny, white macules in the elderly. Until now, there was no definite therapeutic modality for IGH. Objectives: To investigate the effectiveness of low-fluence fractional CO<sub>2</sub> laser in the treatment of IGH Methods: A retrospective review was performed, and a total of 87 lesions on 26 body parts in 17 patients with IGH were included from October, 2016 to June, 2017. A CO<sub>2</sub> fractional laser treatment was performed on all lesions with 2 passes of 2-4 mJ, 400 spots/cm2 with no anesthesia at a 1-week interval. All lesions were classified into three body parts including face, chest, and extremities, and were evaluated according to the body parts. Clinical assessments were evaluated using 6-point scales (0, no; 1, mild; 2, moderate; 3, good; 4, excellent; 5, complete response). Results: Overall, 61.5% (21 out of 26 body parts) showed excellent or complete response after a median of 5.5 treatment sessions (range: 2-9). In the subgroup analysis, 80% (8/10) of face lesions showed excellent response, while 40% (4/10) of extremities reached excellent response. Transient erythema and crust after treatment was common, and one patient stopped treatment because of prolonged hyperpigmentation at irradiation site. Conclusion: Low-fluence fractional CO<sub>2</sub> laser therapy could be an effective option for IGH. It is convenient without need for anesthesia.
