The acoustic emission (AE) sensor has been widely used for monitoring the machining states or tool abrasion, etc. This paper presents the relation between machining time and states such as tool abrasion for ceramic machining processes, which can be applicable to develop the dentistry machining center. A commercial PZT-type of AE sensor was used for experiments, and the aquisition data was analyzed in the AE signal strength versus machining time. The results show that it is possible to estimate the tool life and the ceramic machining state. In the future, additional experiments for various machining conditions such as depth of cut and feed rate are strongly necessary so as to design an effective monitoring system for the ceramic machining processes through detecting the AE signal.

Most of rural area in Korea has been faced with many problems like a increasing restrictions of the World Trade Organization and unstability of international agricultural product market. On the other hand, we should give a serious consideration about agricultural policy interior of a country. It is for reason that we have to magnify special tax project for rural area as much as possible. In addition, it will be contribute to the maintenance of development and stability on the rural area.

Fifty-six-year-old male patient visited our clinic complaining generalized skin lesions which were compatible with erythema multiforme clinically and histopathologically. The skin lesions were treatment-resistant, recurrent and persistent for 3 years. In consideration of old age, atypical features of erythema multiforme and no other demonstrable etiology of erythema multiforme, we studied for internal malignancy. Malignant lymphoma at the duodenum was revealed by UGI series, abdominal CT, and endoscopic biopsy. After chemotherapy, the tumor mass and active skin lesions were cleared simultaneously. Erythema multiforme may be regarded as one of the paraneoplastic syndromes.
Robust plasmonic sensors based on hybrid nanostructures with facile tunability

<P>Here, we demonstrate the preparation of a hybrid nanoplasmonic probe array, a nanoforest, which consists of gold nanostructures coated on top of silica nanospheres and evaporated gold film structure onto bottom through masks. Optically tunable plasmonic surfaces could be achieved by varying the nanoforest fabrication conditions, which acted as a selective nanoantenna. Plasmon coupling effects were effectively controlled by adjusting the vertical distance (nanogap) between the gold nanostructures on the silica bead and the bottom. Such control is critical to highly sensitive molecular detection applications. As a proof of concept, nanoforests were used as colorimetric sensors of refractive index changes. The sensors were highly sensitive and displayed distinct colors for small refractive index (RI) changes. Purpose: Postoperative delirium (POD) is a common complication in elderly patients after major abdominal surgery for cancer. Although POD is related with a poor outcome, there have not been many reports about POD after abdominal surgery in Korea. The aims of study were to analyze the characteristics and surgical outcomes of elderly patients with POD and to identify the risk factors of POD. Methods: From November 2016 to January 2019, we prospectively enrolled 63 patients who were aged ≥75 years and underwent major abdominal surgery for cancer. POD was daily assessed for up to 10 days postoperatively with the Confusion Assessment Method and a validated chart review. Results: POD occurred in eight patients (12.7%). Univariate analysis showed that the occurrence of POD was related to sodium <135 mEq/L (P=0.037), combined resection (P=0.023), longer surgery/anesthesia time (P=0.023 and P=0.037, respectively), increased blood loss (P=0.004), postoperative admission to intensive care unit (ICU) (P=0.023), and duration of Foley catheter (P=0.011), however, multivariate analysis identified no significant risk factors of POD. There was no difference in postoperative outcomes such as hospital stay, mortality, reoperation, and morbidity between patients with POD and without POD. Conclusion: Elderly patients with hyponatremia, combined resection, longer operation/anesthesia time and admission to ICU had tendencies to develop POD after major abdominal surgery. Surgeons should pay more attention to prevent POD, and a large-scale prospective study is needed to identify the risk factors of POD.

Purpose: We performed this study to share experiences in the management of obturator hernia, which is a very rare disease among elderly women, because rarity of this disease will not approve an expert institute or surgeon and because the aging society, Korea, can increase the incidence. Methods: Patient characteristics, clinical manifestations and treatment results were retrospectively collected from the 12 obturator hernia patients since 2000 in the three hospitals of the authors. Twenty-one obturator hernia cases reported in the Korean literatures were reviewed. In addition, we analyzed clinical features and treatment results of the total 33 patients, collectively. Results: Most of the patients were elderly women except one young, poorly nourished, male patient with pulmonary tuberculosis. Their mean age was 79.6 years. Sixty-four percent (21/33) of the patients had preoperative morbidity. Symptoms from the compression of the obturator nerve, which is an important clue to the diagnosis, were observed in 67% (22/33). Interestingly, spontaneous or suspicious-spontaneous reductions were reported in 7 (21%) patients. Abdominal CT scan was the major tool for diagnosis. Abdominal approach alone could successfully manage most cases, though 84% (27/32) needed anastomosis of the bowel. Operative mortality was 2 of the 32 cases but morbidity was 44% of the 32 patients and the mean hospital period after operation was 21 days. Conclusion: Management of patients with obturator hernia connotes high morbidity and mortality. Sharing the experience of management, prompt diagnosis and surgical management and proper peri-operative care as well as appropriate managing the families is essential for improved results.
