Many of the growth-promoting properties of growth hormone(GH) are mediated by insulin-like growth factor-I(IGF-I), a highly conserved circulating 70-amino acid peptide. Recent studies have shown that multiple mechanisms influence IGF-I gene expression, including transcription from two promoters, alternative RNA splicing, and variable polyadenylation. In thyroid tissue, thyroid stimulating hormone(TSH) and IGF-I are the most possible candidates for follicular cell proliferation and hypertrophy. Actually IGF-I had autocrine and paracrine effect for tissue growing. We prepared thyroid tumor tissue mRNAs using single step method for detecting IGF-I levels according to different tissues, i.e., thyroid adenoma or papillary thyroid carcinoma. We used Northern blot analysis for IGF-I mRNA and RNase protection assay (RPA) for IGF-I transcription start sites. For Northern blot, we used whole human IGF-I cDNA as a DNA probe and for RPA, we used IGF-I exon 1 containing noncoding promoter 1 as a riboprobe. We got good RNA bands from Northern blot analysis around 1 kb (IGF-IA) and 7.5 kb (IGF-IB) region. To clarify the amount of both IGF-IA and IB mRNAs, we measured autoradiographied signal of IGF-I mRNAs bands using densitometer. In IGF-IA signals, there's no change among liver and thyroid tissues, but in case of IGF-IB mRNA bands, the signal was markedly increased in thyroid carcinoma tissues than that of normal thyroid tissue (85% vs 14%). In the study of RPA, all thyroid tissues used the same transcription start sites as those of liver's. We concluded that that this different regulation of IGF-I mRNA was originated from tissue specificity. That meant some tissue specific transcription factor/s were related to tissue IGF-I expression.

Background : In recent years gonorrhoea has been panendemic and remains one of the most common Sexually Transmitted Diseases in the world, especially in developing countries. Objective & Method : For the detection of a more effective therapeutic regimen and assessing the prevalence of Penicillinase Producing Neisseria Gonorrhoeae(PPNG), Ive have been trying to study the patients who have visited the VD Clinic of Choong-Ku Public Health Center in Seoul since 1980 by means of the chromogenic cephalosporin method. Results : In 1994, 168 strains of N. gonorrhoeae were isolated, among which 109 (64,9% ) were PPNG. Conclustion : Our results suggests that the prevalence of PPNG in Seoul is still increasing.

심한 골병변으로 발현된 기능성 낭종성 부갑상선 선종

낭종성 부갑상선 선종과 심한 골병변을 동반한 부갑상선 기능항진증은 매우 드문 질환으로서, 저자들은 양측 고관절의 통증을 초기 주소로 내원한 환자에서 고칼슘혈증과 부갑상선 호르몬 증가, 골병변의 방사선적 소견을 통해 부갑상선 기능항진증을 진단하고, 경부 초음파와 컴퓨터 단층 촬영, 부갑상선 스캔검사 및 수술중 부갑상선 낭종액 검사 등을 통해 기능성 부갑상선 낭종의 한 종류인 낭종성 부갑상선 선종을 진단하고 수술적 제거를 통하여 정상화된 1예를 경험하였다. A cystic parathyroid adenoma is rare. A case of primary hyperparathyroidism, with the cystic formation of a parathyroid adenoma and a severe bony lesion, is reported. A 52-year-old male was admitted due to pain in both hips and for evaluation of hypercalcemia. The plasma level of the intact parathyroid hormone (iPTH) was elevated to 1424 pg/mL. Ultrasonography and the computed tomography revealed a parathyroid cyst on the left thyroid lower pole. Parathyroid scintigraphy detected a parathyroid adenoma. A radiograph showed a subperiosteal bone resorption on the phalanges, and a brown tumor (osteitis fibrosa cystica) on the femur shaft was noted. A surgical excision of the parathyroid adenoma was performed. The PTH level in the cystic fluid was increased. A histological examination confirmed a cystic parathyroid adenoma. The PTH level was normalized after the operation (J Kor SOC Endocrinol 18:214-220, 2003).

Verrucous mycosis fungoides is very rare variant of cutaneous T cell lymphoma that manifests verrucous crusted lesion and may be clinically misdiagnosed as a wart, deep fungal infection or atypical mycobacterial infection. If it is located acrally, it might then be better classified as mycosis fungoides palmaris et plantaris or pagetoid reticulosis. We report a case of solitary verrucous mycosis fungoides on the left knee in 25-year-old woman successfully treated ultraviolet A-1 phototherapy.

불소함유치약의 인공우식 법랑질에 대한 도포효과

The purpose of this study was to compare the anticarious effect by kinds of fluoride - containing dentifrices and kinds of fluorides contained in dentifrices. The author randomly selected 9 different fluoride containing dentifrices which were saled in Korea. Bovine enamel slabs involved early carious lesions were treated with flouride - containing dentifrices, NaF and SMFP solution by pH-cycling method. Surface hardness and fluoride contents of bovine enamel were measured for dentifrice treated groups in comparison with fluoride treated groups control group. A comparative study of the obtained results led to the following conclusions. 1. The fluoride contents of Perioe doctor^ⓡ and Amore ion^ⓡ groups were higher than that of the other fluoride - containing dentifrice groups. But that of Close up^ⓡ was lower than of others but Acacia^ⓡ group. 2. The dentifices containing sodium monofluorophosphate and sodium fluoride were higher on fluoride content than those containing NaF, SMFP, respectively. 3. Fluoride - containing dentifices were more effective on enhancing surface hardness than control gourp. 4. NaF solution was more effective on fluoride content than SMFP solution. Two fluoride solutions were the same effect on surface hardness, but they were more effective on surface hardness than control group.

발전기 안정화 운전을 위한 PSS 파라미터 선정

This paper describes the proper selection method of the power system stabilizer(PSS) parameter for stabilizing operation of a synchronous machine which connected to infinite bus by using pole assignment method. The purpose of PSS is to enhance the damping capability at small frequency oscillation (0.5∼2㎐). It can be achieved by moving the real part of critical eigenvalue (pole) on left side of s-plane. The PSS parameter was selected by using the proposed method to match with a pre-assigned pole associated with poorly damped swing mode. This method has been studied to synthesize the PSS on Yong-Gwang N/P. And the simulation results show that the selected parameter by using proposed method is effective for dynamic and transient stability of power system.

분화된 갑상선암 환자에서 수술 후 재발과 원격전이를 예측하는 혈청 갑상선글로불린치

연구배경 분화된 갑상선암의 재발이나 원격전이를 시사하는 혈청 갑상선글로불린치는 보고자들마다 차이가 있고, receive operating characteristics (ROC) 곡선을 이용해서 cut-off value을 구한 경우는 별로 없다. 또한 수술 후 방사성요오드 치료 직전 (Tg-RAI), T₄투여 도중(Tg-on) 및 T₄ 중단 후(Tg-off)에 측정한 갑상선글로불린의 임상적 의의에 대해서도 아직 이견이 있다. 더구나 임상에서 측정된 혈청 갑상선글로불린치의 의미를 해석할 때 보다 큰 의미를 가지는 양성 예측치와 음성 예측치를 조사한 보고는 거의 없다. 방법: 갑상선 유두암 또는 여포암으로 진단되어 갑상선 전절제술 및 고용량 방사성요오드 치료를 시행 받은 후 갑상선 자리에 잔여병소 없이 추적관찰이 이루어지고 있는 242명의 환자들 중, 항갑상선글로불린항체가 음성인 205명의 환자를 대상으로 후향적으로조사하였다. 평균 추I:기간은 5년이었다. Tg-off는 저요오드식이를 2주 이상 시행한 후 ^(131)I-전신스캔과 함께 측정되었으며, 내인성 TSH의 농도가 30 μU/mL 이상으로 상승했을 때 의미 있는 것으로 판정하였다. 재발은 경부 림프절을 포함하여 경부에 새로운 병소가 생겼을 때로, 원격전이는 종격동 림프절을 포함하여 경부 이외의 장기에 병소가 생겼을 때로 정의하였곤, 재발이나 원격전이는 각종 영상학적 또는 병리학적 검사에서 이상소견이 발견되는 경우 있는 것으로 판단하였다. 민감도와 특이도가 가장 높은 Tg의 cut-off value를 구하기 위해 ROC 곡선을 이용하였고, cut-off value에 따른 양성 예측치와 음성 예측치를 구하기 위해 2×2표를 이용하였다. 결과: 재발은 50명 (24.4%)의 환자에서 발생하였고, 원격전이는 17명(8.3%)의 환자에서 발생하였다. 재발이나 원격전이 중 어느 한 가지라도 있는 환자는 52명(25.4%)이었다. ROC 곡선 아래부분의 면적을 통해 알아본 Tg-RAI, Tg-on, Tg-off 검사의 정확도는 서로 비슷하게 높았다. 재발에 대한 혈청 갑상선글로불린치의 cut-off value는 Tg-RAl가 11.8, Tg-on이 1.4,Tg-off가 3.3ng/mL였다. 이때의 민감도는 각각 85.4,82.2, 93.3%였고, 특이도는 각각 89.2, 92.4, 88.0%였으며, 양성 예측치는 각각 71.9, 77.1, 70.0%였고, 음성 예측치는 95.0, 94.4, 97.8%였다. 원격전이에 대한혈청 갑상선글로불린치의 cut-off value는 Tg-RAl가27.4, Tg-on이 2.5, Tg-off가 7.9 ng/mL였다. 이때의민감도는 각각 86.7, 87.5, 92.3%였고, 특이도는 각각86.2, 90.8, 80.2%였으며, 양성 예측치는 각각 34.2,46.7, 25.0%였고, 음성 예측치는 각각 98.7, 98.8,99.3% 였다. 결론: 혈청 갑상선글로불린치는 분화된 갑상선암의 재발이나 원격전이를 예측하는 데 있어 민감도와 특이도가 비교적 높은 검사이나, 음성 예측치가 높은 것에 비해 양성 예측치가 낮은 단점이 있었다. 또한 Tg-on은 Tg-off에 비해 양성 예측치는 높았으나, 음성 예측치는 낮게 나타났다. 따라서 Tg-on이 cut-off value보다 높게 측정되는 경우에는 반드시 전신스캔 등 재발이나 원격전이 여부에 대한 추가적인 검사를 시행하고, Tg-on이 cut-off value보다 낮게 측정되더라도 가능하면 선별검사로서 Tg-off나 recombinant human TSH 자극 후의 혈청 갑상선글로불린치를 주기적으로 측정해야 할 것으로 사료된다. Background: Reports on serum thyroglobulin(Tg) levels being used to predict recurrence or distant metastasis during the follow-up of patients with differentiated thyrod cancer(DTC) has been inconsistent. In addition, there have been few reports that attempt to define the cut-off value of Tg for recurrence or distant metastasis obtained by a receiver operating characteristic (ROC) curve. As well, there are differences in opinions on what the value should be on the first serum Tg level measured just before radioactive iodine(RAI) ablation(Tg-RAI), during thyroxine administration (Tg-on), and after thyroxine withdrawal (Tg-off) during the follow-up. Reports on the positive predictive values(PPVs) and negative predictive values(NPYs) of these Tg values are rare. Methods: A total of 205 patients (42 males, 163 females) with DTC were studied. All patients had undergone total or near-total thyroidectomy. After surgery and RAI ablation, annual thyroxine withdrawal 131I-whole body scan (WBS) with Tg measurements was performed. The mean duration of follow-up was 5.0 (1.4~7.4) years. The most sensitive and specific Tg values)cut-off values) for tumor recurrence and/or distant metastasis were selected by using ROC curves. We also calculated the PPVs and NPVs for rcurrence and/or distant metastasis using two-by-two tables. Results: Cut-off values of Tg-RAI, Tg-on, and Tg-off for recurrence were 11.8, 1.4, and 3.3 ng/mL, respectively. For these values, the sensitivities were 85.4, 82.2, and 93.3%, with the specificities at 89.2, 92.4, and 88.0%. PPVs were 71.9, 77.1, and 77.0% while NPVs were 95.0, 94,4, and 97.8%. The cut-off values for distant metastasis were 27.4, 2.5, and 7.9ng/mL, respectively. For these cut-off values, the sensitivities were 86.7, 87.5, and 92.3%, with the specificities being 86.2, 90.8, and 80.2%. PPVs were 34.2, 46.7, and 25.0% and NPVs were 98.7, 98.8, and 99.3%. Conclusion: All three serum Tg levels were sensitive and specific markers for recurrence and distant metastasis. Their PPVs were low in contrast to the high NPVs. In comparison with Tg-on, Tg-off showed higher sensitivity and NPV as well as lower specificity and PPV. Therefore, in the case of higher Tg-on during the follow-up period, efforts to find recurrence and distant metastasis,such as 131I-WBS, should be done. In addition, regular measurement of Tg-off or Tg after stimulation with recombinant human TSH is recommended as a screening test (J Kor SOC Endocrinol 18:153-165, 2003).

레이져 비젼 센서를 활용한 로봇 자동 브레이징 시스템에 관한 연구

Recently, the automation of welding process has become the subject of growing research area in visual control systems and robotic intelligent fields that are required exact three-dimensional pose(position and orientation). The objective of this article is to study the flexible automation of brazing process and the optimization of the car body surface's quality. The development of laser-vision sensor and calibration algorithms contribute to the implementation of automatic brazing process and so the flexibility and intelligence of welding robot could be constructed. Generally, the welding process has three-dimensional error due to the transporting and the calibration of robot and sensor. Therefore, the analysis of relative pose of laser senor is needed to determine the solution that provides three-dimensional information of arbitrary target with respect to a robots end. To obtain higher performance of automatic welding through tracking of seam line, it is needed that the exact calibration of laser and robot. The implementation of this methodology will be described. The automatic seam tracking and optimal robot's path is also explained in this article. The experimental results demonstrate the successful operation of the automatic brazing. It shows the validity of the robots calibration equation to a moving laser sensor.

춘천산 연옥을 이용한 옥(玉)광물의 특성에 관한 연구

우리나라에서 옥은 아름답고 귀한 모든 돌을 의미한다. 그러나 전통적인 옥광물로는 주로 경옥, 연옥, 사문석옥만을 지칭한다. 옥광물들은 외관 특징은 유사하지만 서로 다른 화학성분과 물리적 성질을 가지고 있다. 경옥, 연옥, 사문석 옥은 비중, 굴절율, 경도 등의 몇몇 간단한 실험을 통해 감정할 수 있다. 춘천산 연옥의 보석학적 및 광물학적 특성을 알아보기 위해 색, 광택, 투명도, 조흔색, 굴절률, 경도, 비중을 측정하였으며, 편광현미경 관찰, 주사전자현미경 관찰 및 화학분석 등의 실험을 하였다. 춘천산 연옥은 화학성분이 Ca₂Mg□(Si₄O□)₂인 tremolite이며, 보석학적 및 광물학적 특성은 일반적인 연옥에 비해 경도가 높아 내구성이 뛰어나다. Ok means beautiful and precious gemstone in Korea. There are jadeite, nephrite and serpentine in traditional Ok. These three kinds are nearly the same in appearance such as color, luster, transparency though they are different in chemical composition and physical character. They can be distinguished by a few simple experiments like specific gravity, refractive index, hardness. To find out gemological and mineralogical character of Chuncheon nephrite, color, luster, transparency, streak color, refractive index, hardness, specific gravity, observation of polarizing microscope, SEM and EDS analysis are examined. As a result of it, Chuncheon nephrite is tremolite composed of Ca₂Mg□(Si₄O□)₂. Compared with gemological and mineralogical character of general nephrite, Chuncheon nephrite is harder and durable than general nephrite.

재배열된 GaAs(001)표면의 전자 구조

We investigated the atomic structure and the density of state of the rearrangered GaAs(001) surface from the surface layer to the third layer. The geometry is calculated by the energy minimization by the described formula. The result is that displacement of the first layer is asymmetric with no twist, the dimer angle is about 7˚.
