We have carried out dierential scanning calorimeter (DSC) and nuclear magnetic resonance (NMR) measurements in order to investigate the change in the molecular dynamics accompaning by the phase transition at Tc = 343 K. We found that drastic decrease in the line width of the 1H-NMR absorption line was observed at Tc. Moreover, a decrease in the 1H spin-lattice relaxation time that accompanied the phase transition at 343 K was also observed. These results indicate that a re-orientational motion of the (C4H5N2)+ ion appears above Tc. In addition, an anomalously large thermal anomaly (10.6 kJ/mol) was observed at Tc. From these results, we deduced that the phase transition at 343 K was caused by the appearance of a re-orientational motion of the (C4H5N2)+ ion.

Background: Acute exacerbation of chronic fibrotic idiopathic interstitial pneumonia (AE-IIP) is associated with a high mortality rate. In 2016, the International Working Group classified the etiology of AE-IIP into idiopathic and triggered. Several factors can trigger AE-IIP; however, the triggers associated with the worst prognosis have not been identified. The aim of this study was to investigate the prognosis of patients with various types of AE-IIP, particularly infection-triggered, non-infection-triggered, and idiopathic AE-IIPs. Methods: We retrospectively collected data for 128 patients with chronic fibrotic IIP (CF-IIP) who were hospitalized because of respiratory failure during the period between April 2009 and March 2019 at Juntendo University Hospital. There were 79 patients who developed AE-IIP, and 21 patients who developed bacterial pneumonia combined with CF-IIP. AE-IIP was classified into idiopathic, infection-triggered, and non-infection-triggered. We analyzed differences in patient characteristics, examination findings, and prognosis among the types. Finally, we evaluated risk factors for early death due to AE-IIPs. Results: Idiopathic, infection-triggered, and non-infection-triggered AE-IIPs were diagnosed in 34, 25, and 20 patients, respectively. The survival time was significantly longer for bacterial pneumonia combined with IIP than for AE-IIP. Moreover, the survival time was significantly longer for infection-triggered AE-IIP than for idiopathic or non-infection-triggered AE-IIP. The mortality rate was significantly lower with infection-triggered AEIIP than with other types of AE-IIP. Finally, a multivariate analysis revealed that radiological findings at the time of onset of AE-IIPs and AE-IIP patterns were independent risk factors for early death. Conclusion: Our results suggest that patients with infection-triggered AE-IIP may have a better prognosis than those with other types of AE-IIP.

Objective: The purpose of the present study was to clarify the relationship between white matter tracts and cognitive symptoms in children with high-functioning autism spectrum disorder (ASD). Methods: We examined the cognitive functions of 17 children with high-functioning ASD and 18 typically developing (TD) controls and performed diffusion tensor imaging (DTI) tractography. We compared the results between the groups and investigated the correlations between the cognitive scores and DTI parameters within each group. Results: The Comprehension scores in the ASD group exhibited a positive correlation with mean diffusivity (MD) in the forceps minor (F minor). In the TD group, the Comprehension scores were positively correlated with fractional anisotropy (FA) in the right inferior fronto-occipital fasciculus (IFO) and left anterior thalamic radiation (ATR), and negatively correlated with MD in the left ATR, radial diffusivity (RD) in the right IFO, and RD in the left ATR. Additionally, a positive correlation was observed between the Matching Numbers scores and MD in the left uncinate fasciculus and F minor, and RD in the F minor. Furthermore, the Sentence Questions scores exhibited a positive correlation with RD in the right inferior longitudinal fasciculus. Relative to TD controls, the specific tract showing a strong correlation with the cognitive scores was reduced in the ASD group. Conclusion: Our findings indicate that white matter tracts connecting specific brain areas may exhibit a weaker relationship with cognitive functions in children with ASD, resulting in less efficient cognitive pathways than those observed in TD children.

Richard Samuels book, Machiavelli's Children: Leaders and Their Legacies in Italy and Japan raises a number of important issues concerning political leadership and the role individual leaders can play in a nations history. This is a roundtable involving four critical essays and the authors response. Discussion centers on the book, its methods, its broader applicability and the ways in which it dovetails with other intellectual concerns, particularly as these apply to contemporary East Asia.

Background/Aims: While Helicobacter pylori (HP)-negative gastric cancer is frequently reported, little is known about the predictors for detecting HP-negative early gastric cancer (EGC). We aimed to evaluate the predictors for the detection of HP-negative EGC. Methods: We retrospectively reviewed 13,477 consecutive asymptomatic cases where upper endoscopy was performed by nine physicians from April 2017 to March 2019 and analyzed the detection rate of high-risk lesions (HRLs), including EGC, tubular adenoma, and lymphoma, according to the status of HP infection. The observation time was corrected for multiple regression analyses. Results: For all physicians, the average observation time for screening HP-eradicated and -naïve patients was shorter than that for screening HP-positive patients (p<0.05). Multiple regression analyses revealed that the observation time in the three groups was an independent predictor for detecting HRLs in HP-eradicated patients (p=0.03106, 0.01263, and 0.02485, respectively), while experience of endoscopy was an independent predictor for detecting HRLs in HP-naïve patients (p=0.02638). Conclusions: While observation time during screening endoscopy was a quality indicator for detecting HRLs in HP-eradicated patients, experience of endoscopy was a quality indicator for detecting HRLs in HP-naïve patients.

Endoscopic ultrasound (EUS)-guided intervention has been established as a safe, effective and minimally invasive procedure for various diseases in adults, but there have been limited reports in pediatric patients. Herein, we report our experience with successful EUS-guided drainage of an intraabdominal abscess in a 1-year-old infant concomitant with disseminated intravascular coagulation. The abscess was punctured via the stomach using a standard, convex-type echoendoscope, and the patient’s condition improved after naso-cystic catheter placement. Although the clinical course was complicated by delayed hemorrhage from the puncture site, the bleeding was successfully managed by endoscopic hemostasis using a standard forward-viewing endoscope. Bangle (Zingiber purpureum) is a tropical ginger that is used as a spice in Southeast Asia. Phenylbutenoid dimers isolated from Bangle have exhibited neurotrophic effects in primary cultured rat cortical neurons and PC12 cells. Furthermore, chronic treatment with phenylbutenoid dimers enhances hippocampal neurogenesis in olfactory bulbectomized mice. In this study, we investigated the effects of Bangle extract on behavior and hippocampal neurogenesis in vivo. SAMP8 mice, which are an established model for accelerated aging, with age-related learning and memory impairments, were given a Bangle-containing diet for 1 month, and subsequent behavioral tests and immunohistochemistry for Ki67, a proliferating cellmarker, were performed. We found that the Bangle-containing diet improved spatial learning and memory deficits in the Morris water maze and significantly increased the numbers of Ki67-positive cells in the dentate gyrus of the SAMP8 mice. In addition, the Bangle extract exhibited a neurotrophin-like activity as indicated by the induction of neurite sprouting in PC12 cells. Our results suggest that Bangle is beneficial for the prevention of age-related progression of cognitive impairment.
