        뉴질랜드 흰 토끼를 이용한 추간판의 퇴행성 변화 모델 형성

        Study Design: An experimental animal study. Objectives: To create a more appropriate disc degeneration model which shows how Interleukin 1α may induce the activation of metalloproteinases within the nucleus pulposus. Summary of Literature Review: There are few disc degeneration models wherein there is activation of metalloproteinases within the nucleus pulposus without structural destruction of the intervertebral disc. Materials and Methods: Three consecutive intervertebral discs in New Zealand White Rabbits were exposed. Each disc was injected with 0.1ml of saline (Saline group), 0.1ml of 1μg/ml (IL-1 group), 0.1ml of 10μg/ml (IL-10 group) of IL-1α through a 30-gauge needle. The lumbar spine was harvested 12 weeks after operation. We then analyzed radiographic findings and histological changes. Results: There was no difference in the radiological disc height index among the three groups; 0.071 in saline group, 0.045 in IL-1 group and 0.058 in IL-10 group (p=0.194). The histological cellularity of the nucleus pulposus revealed a decrease in the number of cells (p=0.0001,1.42 in saline group vs. 3.00 in IL-10 group; p=0.001, 2.00 in IL-1 group and 3.00 in IL-10). The histological matrix of the nucleus pulposus was 1.42 in saline group and 2.42 in IL-10(p=0.007), which meant that there had been condensation of the extracellular nucleus pulposus matrix. Conclusions: The results of this study demonstrate that interleukin-1α may contribute to degradation of the nucleus pulposus. This is useful for future study into the effects of the cytokine inhibitor on matrix regeneration and cellularity in the nucleus pulposus in intervertebral disc disease. 뉴질랜드 흰 토끼를 이용한 추간판의 퇴행성 변화 모델 형성소광영 • 최용수 • 윤대현 • 류지훈 • Brian Johnstone*, Jung U Yoo*광주기독병원 정형외과, Oregon Health and Science University*연구계획: 실험 동물 연구목적: 섬유 연골의 퇴행성 변화가 추간판 세포에서 생성된 matrix metalloproteinases의 활성에 따른 보고를 근거하여 토끼의 추간판에 수핵내metalloproteinase를 활성화하기 위한 Interleukin 1α를 기계적 손상 없이 주사하여 추간판의 퇴행성 변화를 생성하고자 하였다. 선행문헌의 요약: 추간판의 퇴행성 변화에 대한 병태 생리를 밝히기 위해 다양한 분야에서 접근하고 있다. 그러나 추간판의 구조적 이상 없이 추간판의자연 경과에 따른 병태 생리를 설명하는 동물 실험 모델은 아직 드물다. 대상 및 방법: 뉴질랜드 흰 토끼를 전방 복부 도달법으로 세 개의 연속된 추간판을 노출 시킨 후 상위 추간판에 생리 식염수 0.1ml(식염수군), 중간 추간판에 Interleukin 1α의 1μg/ml 농도 0.1ml(IL-1군), 하위 추간판에 Interleukin 1α의 10μg/ml 농도 0.1ml(IL-10군)를 추간판의 기계적 손상을 방지하기 위해 30게이지 주사침을 이용하여 중앙에서 주사하였다. 주사 후 12주에 방사선학적 및 조직학적 변화를 조사하였다. 결과: 12주째 방사선학적 추간판 높이지수는 식염수군 0.071, IL-1군 0.045, IL-10군 0.058로 추간판 높이 변화에 차이가 없었다(p=0.194). 조직학적 수핵내 세포는 식염수군 평균 1.42, IL-1군 2.00, IL-10군 3.00의 결과로 식염수군과 IL-10군(p=0.001), IL-1군과 IL-10군(p=0.0001)의 두 군간에 유의한 차이를 보였다. 조직학적 수핵내 조성은 식염수군 평균 1.42, IL-1군 2.00, IL-10군 2.42의 결과로 식염수군과 IL-10군(p=0.007) 군의 비교에서만 차이를 보였다. 조직학적 수핵의 변화 결과는 IL-10군이 수핵내 세포 및 조성의 퇴행성 변화를 유발함을 알 수 있었다. 결론: 토끼의 추간판에서 Interleukin 1α가 수핵의 세포와 기질변화에 관여함을 시사하였다. 토끼 모델을 이용한 퇴행성 추간판 모델 형성은 사이토카인차단제 등의 수핵에서 기질 재생이나 세포 활성을 확인하는 추간판 질환의 실험을 위해 유용하게 적용될 수 있으리라 사료된다.

        Protoplast of carrot were isolated and purified from leaves, roots, and callus of root cambium cultured on MS medium. in order to investigate some basic characteristics of the carrot protoplasts. 1. During enzyme treatment. non-shaking brought about higher yield of protoplasts than shaking by which many protoplasts were destroyed. 2. The sizes of the isolated protoplasts were variable, root protoplasts were largest and leaf protoplasts smallest 3. No color particle was observed in callus protoplasts. but green color in leaf and orange in root. 4. Protoplast densities of leaf and callus were slightly increased after 2-day incubation with the treatment of kinetin and BA. while decreased with the treatmnent of GA. NAA and IAA. Cell wall formation was obserbed from each treatment of growth regulator after 2-day incubation. 5. The protoplast fusions of leaf-root. and leaf-callus were easily recognized in virtue of their typical color and size. The degree of protoplast fusion obtained by PEG eluting method was 1-5%. higher than by the centrifugal method.

        궤양성 대장염에 병발한 괴저성농피증 1 예

        Pyoderma gangrenosum is an uncommon, chronic, painful, ulcerating skin disease originally described by Brusting and colleagues in 1930. The lesions have a distinctive clinical appearance of necrotic ulceration with a mucopurulent base and a violaceous underminded border surrounded by a peripheral zone of erythema. Pyoderma gangrenosum typically occurs most frequent- Iy on the lower Iimbs, and there is often a history of preceding minor trauma. Many adult patients have associated systemic abnormalities: arthritis, inflammatory bowel disease, and monoclonal gammapathy being the most common. Recently, we experienced a case of 18-year old woman with pyoderma gangrenosum associated with ulcerative colitis. Treatment with corticosteroids, sulfasalazine improved the symptoms of ulcerative colitis and the skin lesions. The case is presented with the review of literatures.

        14C 추적자법(追跡子法)에 의한 감귤의 당과 유기산에 관한 연구 : C1- 과 SO 의 엽면처리효(葉面處理效果)과 Effects of Foliar Application of C1- and So

        The effects of spraying Cl^- and SO₄^(2-) solutions on sugar, organic acid and amino acid levels in citrus(C. unshiu Marc.) fruit juice were examined using the ^(14)C-tracer method. Each ^(14)C-labelled compound under investigation was measured during the three weeks after ^(14)CO₂ assimilation. The total carbon compound, which was measured as the specific activity(cpm/㎖) of ^(14)C, was transported more into the fruit juice within one week after ^(14)CO₂ assimilation with the SO₄^(2-) treatment than with the Cl-treatment, The ^(14)C-specific activity changes of the sugar fraction were very similar in both Cl- and SO₄^(2-) treatments although the organic acid fraction was reduced more by SO₄^(2-). The specific activity ratio of the sugar fraction to organic acid was increased rapidly when treated with Cl-. This indicates that SO₄^(2-) plays a role in raising the ratio of sugar to organic acid. It was found that the ^(14)C-specific activity in the amino acid fraction was higher when Cl- was applied.

      • Real Life Experience of Interferon Free Regimens for Hepatitis C Genotype 1b Korean Patients: A Multicenter Cohort Study

        Aims: A real-life study is essential for the investigation of treatment outcomes outside the clinical trials. The aim is to evaluate the efficacy and safety of interferon free regimen for patients with hepatitis C virus (HCV)-1b in real-practice. Methods: 591 HCV-1b patients who received interferon free regimen at the liver unit of the Catholic University of Korea were consecutively recruited and analyzed. Results: Among 591 patients, 518 patients started the daclatasvir and asunaprevir 24 weeks regimen, 61 patients ledipasvir-sofosbuvir 12 weeks regimen and 12 patients daclatasvir and sofosbuvir 12 weeks regimen. The sustained virologic response (SVR) 12 was 95.6% (261/273) in the daclatasvir and asunaprevir group, 93.8% (15/16) in the ledipasvir-sofosbuvir group and 100% (4/4) in the daclatasvir and sofosbuvir group. Resistant-associated variant (RAV) of NS5A loci test was performed in 575 patients. Among them, L31 or Y93 was positive in 57 patients (9.9%) and the RAV was “undetermined” in 21 patients (3.7%). In the daclatasvir and asunaprevir group, the SVR12 in the RAV positive patients was 25.0% (1/4) compared to 96.4% (239/248) and 100% (12/12) in those with RAV negative and “undetermined”, respectively (P=0.000). However, there was no difference in SVR12 according to the RAV test results in ledipasvir-sofosbuvir and daclatasvir-sofosbuvir groups. No difference of SVR12 was observed according to prior treatment experience, the presence of cirrhosis and chronic kidney disease (GFR ≤ 60 ml/min/1.73 m2), old age (≥ 70 year-old), and high baseline viral load (≥ 800,000 IU/ml). Overall, all the regimens were well-tolerated but 18 (3.0%) patients discontinued treatment due to possible-drug related side effects and breakthrough. Conclusions: Interferon free regimens in genotype 1b HCV infected Korean patients resulted in high SVR rates. This multicenter cohort study represents the efficacy and tolerability of DAA treatment in genotype 1b HCV infected Korean patients in real clinical settings.

      • Optimization of medium composition for enhanced cellulase production by mutant Penicillium brasilianum KUEB15 using statistical method

        Penicillium brasilianum KUEB15 was utilized for cellulase production and medium optimization was investigated. The optimal compositions of the variables for maximum CMCase production by central composite rotatable design were 12.39g/L of corn stover, 0.48g/L of urea, and 6.12g/L of ammonium sulfate. The predicted CMCase activity by the model was 1.207Unit/ml, and the observed CMCase activity which was produced in optimal media was 1.180Unit/ml. The activity of CMCase from P. brasilianum KUEB15, which was fermented in optimal media by RSM, was found to be 1.61-fold increased than that in the Mandel's medium (0.731Unit/ml).

      • Frequent outpatient visits prevent exacerbation of chronic obstructive pulmonary disease

        Background: Chronic obstructive pulmonary disease (COPD) is a chronic inflammatory airway disease requiring frequent outpatient visits and lifelong management. We aimed to evaluate the roles of frequent outpatient visits in prognosis of COPD. Methods: We used claims data in the national medical insurance review system provided by the Health Insurance Review and Assessment Service of Korea from May 1, 2014 to April 30, 2015. A definition of COPD was used based on the diagnosis code and medication. Frequent visitors were defined as subjects who visited the outpatient clinic for COPD three or more times per year. Results: Among 159,025 subjects, 117,483 (73.9%) were classified as frequent visitors. Frequent visitors underwent pulmonary function tests and used various inhalers more often than did infrequent visitors. The rates of COPD exacerbation requiring admission to a general ward, emergency room, or intensive care unit were significantly lower in frequent visitors than in infrequent visitors. In multivariable analysis, frequent visits were identified as an independent factor preventing COPD exacerbation that required admission to a ward (odds ratio [OR], 0.387), emergency room, (OR, 0.558), or intensive care unit (OR, 0.39) (all P<0.001). Conclusions: In conclusion, we showed frequent outpatient visits reduce the risk of COPD exacerbation by 45%-60%.
