      • The HBV DNA Amounts in Serum Have No relationship with ALT level and Hetergeneous Population Coexits in Chronic Hepatitis B Virus Infection

Proceedings of International Symposium on East-West Medicine, Seoul. 212-230, 1999. -Hepatitis B is caused by hepadnavirus. Hepatitis B virus replicates through 3.5kb pregenomic RNA intermediate which is regulated by core promoter. Pathogenesis of hepatitis B virus has been bilieved the result of host immune response. But recently many studies have reported that high level of viral replication caused by mutation in core promoter might result in severs hepatitis. But these studies were performed in vitro, not in vivo. So there is yet debate about which factor, viral of host factor, is more important in pathogenesis of hepatitis B virus. So we measured real viral replication level in 204 chronic hepatitis B patients by quantifying HBV DNA from sera by our novel PCR-based more sensitive method, and compared these results with ALT level measured from same sera, which indicates liver cell damage. Surprisingly there are no significant correlation between HBV DNA quantity and ALT level. Then we cloned core promoter region. In SSCP, we found that many viral mutants coexist in one patient. Base on SSCP result, we chose main viral core promoter type in each patients, which is thought to determine overall viral replication level in this patient. Main type of core promoter region of each 41 patients were directly sequenced. And with these we measured promoter activity by luciferase assay system and compared promoter activity with on another. We found tha there were some differences in promoter activity according to core promoter sequences. And we constructed replication-competent viral constructs with core promoter from 41 patients and Transfected these into HepG2 cell and measured HBV DNA by southern blot. There were also differences in HBV DNA quantity according to core promoter sequences. On these all results we investigated correlation between the effect of HBV core promoter on viral replication in vitro and HBN DNA quantity, ALT level from sera of each patients. We found there is no significant correlation among them. As a result, we concluded that in determining severity chronic hepatitis B patients, host factors of each patient is more important rather than replicative activity of virus itself.

        Oblique Posterior Interbody Fusion with Bone Plugs by Unilateral Approach for Treatment of Lumbar Instability - Technical Note -

        A new technical method using bone plugs with oblique posterior interbody fusion(OPIF) was designed to increase the fusion rate. One hundred-seventy-five patients were operated for lumbar instability between November 1990 and December 2000. The L5/S1 segment was affected in 129 cases(74%) of the patients. Oblique posteior interbody fusion with bone plugs by unilateral approach was performed in all patients. Eight-nine percent of these patients showed good surgical results. This stable transplant interbody fusion has the several advantages. Foreign substance is not used and this method increases fusion rate and cost-effective. OPIF using bone plugs along with Oh's screw could provide sufficient stability for segmental instability of the lumbar spine including spondylolisthesis of the lumbar spine, with or without concurrent herniated intervertebral disc or spondylotic narrowing.         유용성 도료와 수용성 도료의 유해성 비교에 관한 연구 : 자동차 보수용 도료를 중심으로

        The purpose of this study is to substitute water-based painting materials for the current solvent-based ones used in motor-repairing process to minimize the exposure of organic solvents to the painters. This study assessed the exposure of organic solvents to the painters using water-based and solvent-based painting materials and compared compositions, painting processes and the health hazards of the application of these alternative painting materials. The results of this study are as follows. 1. solvent-based painting materials used in motor-repairing process consist of various organic solvents, which consist primarily of toluene, xylene, ethyl benzene, ethyl methyl bezene, trimethyl bezene, ethyl acetate, butyl acetate, methyl isobutyl ketone, 2-ethoxy ethanol, 2-ethoxy ethyl acetate and toluene-2.4-diisocianate and the others. These organic solvents are know as health-hazardous substances. But water-based painting materials are high-solid, low-solvent ones and consist of such two organic solvents as 2-butoxy ethanol and 2-heptanone and the others. 2. The painters using solvent-based painting materials in motor-repairing process are exposed to various organic acetate, methyl isobutyl ketone, trimethyl benzene, 2-ethoxy ethanol, and 2-ethoxy ethyl acetate. But the painters using solvent-based ones are only exposed to 2-butoxy ethanol and 2-heptanone. 3. By using water-based painting materials in stead of solvent-based painting materials containing health-hazardous organic solvents, the exposure of such organic solvents in the painter's breathing zone can be largely prevented. 4. This study recommends water-based painting materials as substitutes for the current solvent-based ones used in motor-repairing process to minimize the exposure of organic solvents to the painters.

        Recently, the use of underground space has been widely expanded for transportation, storage, and human activities. The increased demand of underground space lead to the necessity of reliable and efficient techniques for quality assurance and safety assessment of tunnel structures. This study investigated the feasibility of SASW measurements, which has been successfully used to investigate the geotechnical sites and to evaluate the structural integrity of concrete structures, in the area of safety assessment of tunnel concrete lining. Two approaches were proposed to identify a water proofing membrane in tunnel concrete lining, and the SASW technique was applied to characterize the properties of shotcrete and the original ground behind tunnel concrete lining.

        초내식성 스테인리스강 선재와 브라켓의 내식성 및 금속 이온 용출 특성

        Metallic orthodontic appliances are known to release metal ions in an oral cavity. It is not advisable that toxic corrosion products derived from the appliances is absorbed into the body. Metal ions release from orthodontic appliances is known to have large effects on the result of orthodontic treatment and systemic health. Metal bracket and wire were made of super stainless steel(S32050) with high corrosion resistance. The corrosion resistance and ion release property of S32050 and conventionally used appliances were examined. Four types of wire were ligated in either super stainless steel(SB) or Tomy bracket(TB) with O-ring, and then each group was immersed in an artificial saliva at 37℃ for 12 weeks. Amount of released metal ions was measured with immersion time by graphite furnace atomic absorption spectroscopy and surface morphology was investigated. S32050 showed the low passive current density of about 1 μA/㎠ and the stable passive region to a high potential in artificial saliva. S32050 appliances did hardly release nickel ions in artificial saliva. S32050 wire(SRW) and NiTi wire(NW) didn't show significant difference of Ni ion release with immersion time, meanwhile, Remanium wire(RW) showed significant difference in 12 weeks. Ni ion releasce from the brackets didn't show any significant difference with immersion time and between SB and TB. Amounts of Ni ion released from TB-SRW showed significant difference in 12 weeks, those from TB-RW showed significant difference from 4 weeks later and highest release among all groupsin12weeks. TB-NW released little Ni ion for all immersion time and showed significant difference from 4 weeks later, TB-TW(TMA wire) showed significant difference in 12 weks. SB-SRW didn't show any significant difference and SB-RW signigicant difference in 12 weeks,but lower compared with TB group. SB-NW and SB-TW didn't show any significant difference. Corrosion products didn't be observed in mostly groups during the immersion time, except that in TB-RW, they were observed on the bracket surface between O-ring and wire from 8 weeks later. Super stainless steel wire had good corrosion resistance compared with stainless steel wire, and it is believed that it can be used as orthodontic material for bracket and wire.

        In Korea, more than four hundred earthquakes with medium intensity centered on off-coastal and inland of Korea during the past 20 years and due to the great earthquakes which occurred recently in neighboring countries the importance of the future earthquake preparedness measures in Korea is highly recognized. More than 80 percents of residential buildings are constructed with unreinforced masonry structure, in Seoul. In general, unreinforced masonry structure don't have enough strength against the lateral force. And low rise buildings such as unreinforced masonry structure didn't adopted seismic design. So, a huge damage is expected when an earthquake occurs. It is that the major variables of shear strength are the aspect ratio, the vertical strength and the method of masonry. The objectives of this study are, 1) to find out the shear behavior of masonry wall and the variables affect the shear capacity of masonry wall such as aspect ratio of the unreinforced masonry wall, and the vertical strength, 2) to propose the equation of the shear capacity of masonry wall by regression analysis using the test data.

        The purpose of this study was to create plan of Green Sport for all in Pusan toward 21st century. The plan of Green Sport for all in Pusan toward 21st century was conducted by theoretical analysis based upon an advanced countries' sport for all state and Pusan's sport for all state. The result of this study are as follows; 1. To set plan for sport for all in Pusan, it should consider the environment of sport for all for citizen's social welfare. It is more important to use the present facilities effectively with the good management than expand sport for all facilities. To set budget for all facilities to expand it is expected from 2002 Pusan Asian Games's profit. 2. The public sport facilities and sport for all facilities should be open for citizen after build sport for all facilities at the near river and sea side. It should give permission to build green sport for all facilities in the zone of protect development. The regulation of entrance fees at the public sport center such as basketball gymnasium, soccer stadium, and baseball stadium. To enter public sport center, it is recommended free for athletes to do competition or exercise and for citizen the lowest fee is recommended during whole year. The officials should be consider very good quality of service for citizen. Also, school's sport facilities are recommended to open for citizen after school off. 3. It is recommended to build one sport center at every district(gu) in Pusan like Sport for all Pusan association (The memorial olympic hall). The olympic park should protect forever, it should not be allowed for other purpose whenever. 4. The government should set budget for labor's sport clubs. Also, the government try to encourage to expand labors' sport facilities in the company as the law promotion for labors' sport. 5. The government and the government of Pusan city should lead every company to keep the law of promotion for labors' sport. Also, the government leads company to promote sport for labors at each company by the reduction of taxation for company such as th incorporation tax and a real estate acquisition tax. For example, the government let them reduce company's tax for company's private sport facilities(center) so give them chance to invest for sport facilities. 6. KOC should keep remain neutral to promote territory's sport, university team in Seoul should get away from the victory oriented idea then they might participate as amateur at the national tournament. 7. To reduce traffic at the center of the Metropolis, the energy saving purpose, the effective using of the center of the Metropolis, and to increase sport for all people, the green sport for all underground facilities are recommended at the foot of Youngdo park and the public elementary, junior, high school's ground without the nature damage. And then it will be help a lot to citizen to stress out and to use effective leisure time for themselves. 8. It was suggested that the green sport for all facilities might build in the sea near the yacht race center at Soo young, Haeundae bathing resort and Kwangan ni bathing resort, with minimizing nature damage. The green cultural resport and ocean sport facilities were recommended at the sea side to play yacht, wind surfing, water ski, and water bicycle for touring and sport for all purpose. 9. The green winter sport resort and sport resort were recommended at Seungji park and Daeshin park for citizen. After build these facilities, it should be open for citizen every day.

      • 자궁경부 상피내병변과 침습성 편평세포 암종에서의 bcl-2 발현양상

        Background and Objectives: Squamous cell carcinomas (SCC) of the uterine cervix arise from the carcinoma in situ developed in a field of atypical squamous cell proliferation. So, clinical significance of the cervical intraepithelial neoplasm (CIN) are widely accepted. The purpose of this study is to investigate the oncoprotein changes in the sequence of low-grade squamous intraepithelial lesion (LSIL), high-grade squamous intraepithelial lesion (HSIL), keratinizing type of squamous cell carcinoma (K-SCC) and nonkeratinizing type of squamous cell carcinoma (NK-SCC). Materials and Methods: Immunohistochemical stains for anti-bcl-2 oncoprotein to 120 conized cervical tiussues or resected uterus specimens diagnosed as squamous cell lseions including 30 cases of LSIL, 30 cases of HSIL, 30 cases of K-SCC, and 30 cases of NK-SCC were done. The evaluation of the bcl-2 immunostaining was based on the percentages of the positive neoplastic cells. And statistical analysis of data for the relation between each lesion was performed using the Student t-test. Results: The average positive cell rate was 2.0%, 4.44%, 6.25% and 13.57% in LSIL, HSIL, K-SCC, and NK-SCC, respectively. Statistically, immunoreactive positive cell rate of NK-SCC was significantly higher than those of LSIL (p=0.007) and HSIL (p=0.031). But, there was no significant difference of immunoreactive cell rate between K-SCC and HSIL. Conclusion: These results suggested that bcl-2 may play a role in a cervical tumorigenesis and prognostic relationship with NK-SCC.

      • 성상세포종에서 등급에 따른 p53 단백 발현의 차이

        Background and Objectives: Mutation or inactivation of the p53 is important mechanism in a wide variety of human tumors. The purpose of this study is to investigate the relationship between expression amount of p53 protein and the grade of astrocytic gliomas. Materials and Methods: This study analyzes 40 paraffin-embedded astrocytic glioma, including 10 cases of low grade, 20 cases of high grade, and 10 cases of maligant glioma (glioblastoma multiforme) were used to analyze the status of abnormal accumulation of wild type of p53 protein. The evaluation of the immunostaining result was based on the percentage of positive neoplastic cells. Results: Average positive cell rate of p53 protein were 4.2%, 35.4% and 16.2% in low grade astrocytoma, high grade astrocytoma and glioblastoma multiforme, respectively. Conclusion: p53 gene mutations were correlated with histologic grade of glial tumors. High grade and glioblastoma multiforme showed higher positive cell rate than low grade astrocytoma. These results suggest that the high positive rates of p53 protein is related to the poor prognosis.
