A case of ectopic opening of common bile duct in duodenal bulb

Ectopic opening of common bile duct (CBD) in duodenal bulb is very rare, and it can be missed at endoscopy because of small size of the orifice that lacks typical feature of major duodenal papilla and duodenal deformity that makes endoscopic approach difficult. However, it should not be overlookedbecause of critical complications such as recurrent duodenal ulcer or cholangitis. Here we report one case of ectopic opening of CBD in duodenal bulb which was managed with endoscopic retrograde biliary drainage (ERBD) through endoscopic retrograde cholangiopancreatography (ERCP). The patient visited us for fever and abdominal pain. There were some significant findings in studies, which included stricture of duodenal bulb with ulcer and one loose slit-like opening in duodenoscopy, in addition to the tapered distal CBD with dilated proximal part in abdominal CT and ERCP. He treated with two times of ERBD insertion which improved duodenal ulcer and cholangitis, and discharged without any biliary related symptom. The common feature of ectopic opening of CBD isknown to be the absence of normal sphincteric structure. Since it allows free movement of bile and duodenal contents between CBD and duodenal bulb without sphincteric control, this disease frequently accompanies recurrent duodenal ulcer and cholangitis that can cause irreversible deformity. Another is the tapered distal CBD (so-called “hook shaped CBD” in several reports) with dilated proximal part in imaging studies. This is assumed to contribute to bile stasis that promotes bacterial growth or lithogeniccharacteristics of bile, which can lead to biliary disease. These complications can be managed with conservative way, but sometimes need surgical procedure, then, these situations need further accurate diagnosis because of risk for accidental biliary tract injury during surgery due to abnormal location of bile duct orifice. In conclusion, if we encounter recurrent duodenal ulcer or cholangitis in the field, we have to consider the possibility of ectopic opening of CBD.

Objective: This study was conducted to evaluate changes in nutritional value and in situ dry matter (DM) degradability of oak and pine wood before and after steam-digestion process (60 min/160℃/6 atm) and feeding effect of the oak roughage on performance and behavior of Hanwoo steers. Methods: Chemical composition and tannin concentration were analyzed for oak and pine trees before and after the pretreatment. In situ DM and effective degradability of these samples were assessed using a nylon bag method. In vivo trial was performed to estimate animal performance and behavior, using steers fed total mixed ration (TMR) diets containing 0% (control), 25% (OR-25), and 50% (OR-50) of the oak roughage. Eighteen steers were allocated into nine pens (2 steers/pen, 3 pens/treatment) for 52 days according to body weight (BW) and age. Results: By the steam-digestion treatment, the neutral detergent-insoluble fiber was decreased from 86.5% to 71.5% for oak and from 92.4% to 80.5% for pine, thereby increasing non-fiber carbohydrate. In situ DM degradability of treated oak reached 38% at 72 h, whereas that of untreated oak was only 11.9%. The 0 h degradability of the treated pine increased from 5.9% to 12.1%, but the degradability was unchanged thereafter. Animal performance including BW, average daily gain, DM intake, and feed conversion ratio was not different among control and oak treatments. No differences were detected in animal behavior such as lying, standing, rumination, drinking, and eating, except walking. Walking was higher in control than oak treatments with numerically higher eating and lower lying times, probably due to bulkier characteristics of rice straw in the diet. Conclusion: This study demonstrates that the oak roughage can be substituted for 50% of total forage or 100% of rice straw in TMR diets at early fattening stage of Hanwoo steers.

Objective: To evaluate whether administration of intravenous immunoglobulin G (IVIG) combined with low dose aspirin in women with unexplained recurrent pregnancy loss has clinical efficacy. Methods: We retrospectively analyzed the medical records of 113 patients diagnosed with unexplained recurrent pregnancy loss and treated with IVIG and low-dose aspirin between January 1998 and March 2020. We evaluated the characteristics and live birth rate of patients, and compared the pregnancy outcomes in natural pregnancies and pregnancies assisted reproductive technologies (ART). In addition, patients who checked percentage of natural killer (NK) cells were analyzed. Results: In the study population, the rate of total live birth was 79% (89/113), and there was no significant difference in the rate of live birth between natural pregnancy (81%) and ART (74%, P = .46). Most of the patients delivered at term (90%, n = 80); nine patients delivered preterm, seven of whom were ART cases. None of the newborns had an identifiable congenital anomaly. A total of 25 patients were checked percentage of NK cells, with an average of 13.76 ± 6.9 % (range, 1.0 - 28.4%). The relationship between the increase in percentage of NK cells and the miscarriage was not significant (Odds ratio, 0.81; 95% confidence interval, 0.61-1.06; P = .13). Conclusion: IVIG combined with low-dose aspirin showed good outcomes in both natural pregnancies and those assisted by ART, and was safe for treating unexplained recurrent pregnancy loss. Moreover, the treatment was effective regardless of the percentage of NK cells.

Objective: We developed a Motor-Assisted Rowing Machine (MARM) for Spinal Cord Injury (SCI), by modification of the Concept II rowing machine, so that the seats could be operated automatically in a backward and forward direction by a motor. Design: Case report. Methods: Motor rowing consisted of a chair with inclination control, a motor system, control button, monitor, program, leg supporter, safety belt, and seat. The patients were 2 men rowing athletes with SCI, classified as American Spinal Injury Association class B, participated in the study. Level of thoracic injury ranged from T8 to T10. The subjects rowed at a self-selected stroke rate with 50 watts. Two different rowing methods (static rowing without movementof the seat, dynamic rowing using MARM) were assigned to each participant during 10 minutes; 34 reflective markers were attached to their full bodies. Kinematic data were collected using the Vicon motion analysis system. Based on the full body model provided as a default by the equipment. In the rowing exercise, the rowing motions were divided into Drive Phase and Recovery Phase. Results: The two rowing methods differ in handle range, seat range, handle and seat ratio, handle velocity, and seat velocity during static and dynamic rowing. The rowing exercise using a rowing machine developed MARM increased tendency to the range of motion in the dynamic method compared to the static method. Conclusions: The newly developed MARM could be a useful whole body exercise for people with SCI.

Objective: This study aimed to investigate the reliability and validity of a personal computer-based muscle viewer (PC-BMW) compared with that of a portable ultrasound (P-US) for measuring upper trapezius (UT) and transversus abdominis (TrA) muscle thickness at rest and during contraction. Design: Observational inter-rater reliability study. Methods: Fifty-five healthy participants (25 men, 30 women) participated in this study. PC-BMW and P-US were randomly measured at the UT and TrA muscles. Two examiners randomly obtained the images of all participants in 3 test sessions lasting 2 days. Intra-class correlation coefficients (ICCs), standard error of measurement, contraction ratio, and correlation were used to estimate reliability and validity. Pearson`s correlation coefficients were used to analyze the relationship between muscle thickness measures taken from PC-BMW and P-US. Results: The intra-rater reliability ICCs of UT and TrA muscle thickness for the PC-BMW were >0.995, indicating excellent reliability. Inter-rater reliability ICCs for the PC-BMW ranged from 0.963 to 0.987. The P-US also exhibited high reliability. A high correlation was found between the measurements of the two muscles in PC-BMW and P-US (p<0.01). Conclusions: PC-BMW provides clear and excellent images, is pocket-sized and less expensive than a conventional ultrasound imaging system. PC-BMW can be utilized variously and has the advantage of rehabilitative ultrasound imaging. More research is needed to evaluate the utility of PC-BMW for rehabilitation.
Heshouwu (Polygonum multiflorum Thunb.) Extract Attenuates Bone Loss in Diabetic Mice

This study investigated the effects and mechanism of Heshouwu (Polygonum multiflorum Thunb.) water extract (HSW) on diabetes-related bone loss in mice. HSW was orally administered (300 ㎎/㎏ body weight) to high-fat diet and streptozotocin-induced diabetic mice for 10 weeks. HSW significantly alleviated mouse body weight loss and hyperglycemia compared with the control group, and elevated serum levels of insulin, osteocalcin, and bone-alkaline phosphatase. HSW supplementation also significantly increased the bone volume/tissue volume ratio and trabecular thickness and number, and decreased the bone surface/bone volume ratio and trabecular structure model index in the femur and tibia. Moreover, HSW significantly increased femoral bone mineral density. In addition, HSW down-regulated osteoclastogenic genes, such as nuclear factor of activated T-cells, cytoplasmic 1 and tartrate-resistant acid phosphatase 5 (TRAP), in both the femur and tibia tissue, and reduced serum TRAP level compare to those of control mice. These results indicate that HSW might relieve diabetes-related bone disorders through regulating osteoclast-related genes, suggesting HSW may be used as a preventive agent for diabetes-induced bone loss.

Regulation of proper gene expression is important for cellular and organismal survival, main- tenance, and growth. Abnormal gene expression, even for a single critical gene, can thwart cellular integrity and normal physiology to cause diseases, aging, and death. Therefore, gene expression profiling serves as a powerful tool to understand the pathology of diseases and to cure them. In this study, the difference in gene expression in Flammulina velutipes was compared between the wild type (WT) mushroom and the mutant one with clogging phe- nomenon. Differentially expressed transcripts were screened to identify the candidate genes responsible for the mutant phenotype using the DNA microarray analysis. A total of 88 genes including 60 upregulated and 28 downregulated genes were validated using the real- time quantitative PCR analysis. In addition, proteomic differences between the WT and mutant mushroom were analyzed using two–dimensional gel electrophoresis and matrix- assisted laser desorption/ionization-time of flight (MALDI-TOF). Interestingly, the genes iden- tified by these genomic and proteomic analyses were involved in stress response, transla- tion, and energy/sugar metabolism, including HSP70, elongation factor 2, and pyruvate kinase. Together, our data suggest that the aberrant expression of these genes attributes to the mutant clogging phenotype. We propose that these genes can be targeted to foster normal growth in F. velutipes.
