        충돌 증후군에서 Ketorolac 국소주사 요법의 단기 효과

        목적: 견봉 하 국소주사 요법으로 ketorolac과 corticosteroid의 효과를 비교 분석하고자 하였다. 대상 및 방법: 견관절 충돌 증후군으로 진단된 환자 20명에게 ketorolac 60 mg을, 또 다른 20명에게 triamcinolone 40 mg을 주사하고, 시간에 따른 시각적 통증 점수(visual analogue scale, VAS), 관절 운동 범위(range of motion, ROM) 및 Constant-Murley score를 측정하여 후향적인 방법으로 비교, 분석하였다. 결과: VAS는 주사 4주 후, 두 군 모두에서 감소된 소견을 보였으나 corticosteroid군에서 ketorolac군에 비하여 유의하게 더 감소된 소견을 보였고(p=0.001), 12주 후에는 두 군 간의 차이가 없었다(p=0.707). Constant-Murley score는 두 군 모두에서 향상되었으며 유의한 차이가 없었다(p=0.677). ROM 역시 두 군 모두 향상되었으며, 외회전의 경우 ketorolac군이 corticosteroid군보다 유의하게 향상되었다(p=0.005). 결론: 당뇨 등의 기저 질환이나 반복되는 주사 치료로 인한 부작용이 우려될 경우, ketorolac은 견봉 하 스테로이드 국소주사 치료에 대한 대안이 될 수 있을 것으로 생각된다. Purpose: The purpose of this study was to compare the clinical results between the subacromial injection of the ketorolac and that of the corticosteroid in patients with subacromial shoulder impingement syndrome. Materials and Methods: Twenty patients with shoulder impingement syndrome received an injection of 60 mg ketorolac and were evaluated in terms of visual analogue scale (VAS), range of motion (ROM) and Constant-Murley score. The outcomes are compared with the data of patients treated by 40 mg triamcinolone injection, retrospectively. Results: There was no significant difference in the demographics, VAS, ROM, and Constan-Murley score between the two groups before the injection. At the 4 weeks follow-up, pain improvement was significantly greater in the corticosteroid group (2.7±1.53) than in the ketorolac group (4.9±2.08; p=0.001). However 12 weeks after the injection, there was no significant difference in pain improvement between the two groups (ketorolac: 2.9±2.32, corticosteroid: 2.6±1.82; p=0.707). The Constant-Murley score at the final follow-up improved from 33.5 to 52.1 in the corticosteroid group, and from 39.0 to 56.6 in the ketorolac group (p=0.677). ROM was increased in both groups, and external rotation was significantly greater in the ketorolac group than in the corticosteroid group at the final follow-up (ketorolac: 29.3°±9.90°, corticosteroid: 20.8°±7.99°; p=0.005). Conclusion: In this study, ketolorac provided an effect equivalent to triamcinolone in the treatment of subacromial shoulder impingement syndrome at 12 weeks after the injection. This result could offer better opportunities to manage patients with diabetes or local and systemic side effects of repetitive use of corticosteroids.

        Antioxidant effect of garlic and aged black garlic in animal model of type 2 diabetes mellitus

        Hyperglycemia in the diabetic state increases oxidative stress and antioxidant therapy can be strongly correlated with decreased risks for diabetic complications. The purpose of this study is to determine antioxidant effect of garlic and aged black garlic in animal model of type 2 diabetes. The antioxidant activity of garlic and aged black garlic was measured as the activity in scavenging free radicals by the trolox equivalent antioxidant capacity (TEAC) assay. Three week-old db/db mice were fed AIN-93G diet or diet containing 5% freeze-dried garlic or aged black garlic for 7 weeks after 1 week of adaptation. Hepatic levels of lipid peroxides and activities of antioxidant enzymes were measured. TEAC values of garlic and aged black garlic were 13.3 ± 0.5 and 59.2 ± 0.8 μ㏖/g wet weight, respectively. Consumption of aged black garlic significantly decreased hepatic thiobarbituric acid reactive substances (TBARS) level compared with the garlic group which showed lower TBARS level than control group (p<0.05). Activities of superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px) of garlic and aged black garlic group were significantly elevated compared to the control group. Catalase (CAT) activity of aged black garlic group was increased compared with the control group. These results show that aged black garlic exerts stronger antioxidant activity than garlic in vitro and in vivo, suggesting garlic and aged black garlic, to a greater extent, could be useful in preventing diabetic complications.

        High-Throughput Screening Protocol for the Coupling Reactions of Aryl Halides Using a Colorimetric Chemosensor for Halide Ions

        <P>Mercury complex of 4-(2-pyridylazo)resorcinol (PAR-2Hg(2+)), a halide-ion chemosensor, was prepared and its efficiency as a tool for high-throughput screening (HTS) of transition-metal-catalyzed coupling reactions was investigated. It showed a high selectivity for halide ions. When the PAR-2Hg(2+) complex was used in the Suzuki coupling reaction and C-H activated coupling reaction with aryl bromides, the quantitative and qualitative conversions of aryl halides were obtained from the reaction mixture color change.</P>

        Pyrogenic transformation of <i>Nannochloropsis oceanica</i> into fatty acid methyl esters without oil extraction for estimating total lipid content

        <P><B>Abstract</B></P> <P>This study fundamentally investigated the pseudo-catalytic transesterification of dried <I>Nannochloropsis oceanica</I> into fatty acid methyl esters (FAMEs) without oil extraction, which was achieved in less than 5min via a thermo-chemical pathway. This study presented that the pseudo-catalytic transesterification reaction was achieved in the presence of silica and that its main driving force was identified as temperature: pores in silica provided the numerous reaction space like a micro-reactor, where the heterogeneous reaction was developed. The introduced FAME derivatization showed an extraordinarily high tolerance of impurities (<I>i.e</I>., pyrolytic products and various extractives). This study also explored the thermal cracking of FAMEs derived from <I>N. oceanica</I>: the thermal cracking of saturated FAMEs was invulnerable at temperatures lower than 400°C.         Circulating Tumor DNA in a Breast Cancer Patient's Plasma Represents Driver Alterations in the Tumor Tissue

        Tumor tissues from biopsies or surgery are major sources for the next generation sequencing (NGS) study, but these procedures are invasive and have limitation to overcome intratumor heterogeneity. Recent studies have shown that driver alterations in tumor tissues can be detected by liquid biopsy which is a less invasive technique capable of both capturing the tumor heterogeneity and overcoming the difficulty in tissue sampling. However, it is still unclear whether the driver alterations in liquid biopsy can be detected by targeted NGS and how those related to the tissue biopsy. In this study, we performed whole-exome sequencing for a breast cancer tissue and identified PTEN p.H259fs*7 frameshift mutation. In the plasma DNA (liquid biopsy) analysis by targeted NGS, the same variant initially identified in the tumor tissue was also detected with low variant allele frequency. This mutation was subsequently validated by digital polymerase chain reaction in liquid biopsy. Our result confirm that driver alterations identified in the tumor tissue were detected in liquid biopsy by targeted NGS as well, and suggest that a higher depth of sequencing coverage is needed for detection of genomic alterations in a liquid biopsy.

        성인 대상 한국인을 위한 식생활지침 실천 및 순응도 평가 항목 개발

        본 연구에서 개정된 한국인을 위한 식생활 지침을 기반으로 식품 및 영양소 섭취, 식생활 습관, 환경을 고려한 식생활 문화를 평가할 수 있는 항목을 개발하였다. 예비 문항을 확보하기 위하여 국내·외 평가도구 및 최신 문헌을 조사하여 항목을 조사하였고, 전문가 11명 대상 타당도 평가를 통해 70개의 항목을 선정하였다. 선정된 후보 항목 평가를 위해 서울·수도권 지역 중심으로 성인 332명을 대상으로 온·오프라인 조사를 시행하였고, 하루 총 에너지 섭취가 500 kcal 미만 또는 5,000 kcal 이상으로 섭취한 대상자 2명을 제외한 330명의 조사 자료를 활용하여 수렴타당도 분석 및 탐색적 요인분석을 진행하였다. 분석 결과 최종 선정된 문항은 총 34개로 식품 및 영양소 섭취 영역 11개, 식생활 습관 영역 9개, 식생활 문화 영역 14개로 구성되었다. 본 연구에서 개발한 평가 항목은 2021년 개정된 식생활지침을 기반으로 영역별로 성인의 전반적인 식사의 질과 지속가능한 식생활을 동시에 평가하는 데 용이하다는 점에서 건강한 식생활 개선을 위한 평가 도구로써 활용이 기대된다. Purpose: With the 2021 revision of dietary guidelines in Korean emphasizing environmentally sustainable dietary practices, this study attempted to develop an evaluation tool to comprehensively evaluate the degree of practice and compliance with food and nutrient intake, dietary habits, and dietary culture guidelines based on the revised dietary guidelines. Methods: The candidate evaluation items were collected by reviewing 934 literature reviews on domestic and foreign dietary evaluations. Fifty-nine candidate items were derived by selecting the items corresponding to dietary guidelines. The content validity ratio (CVR) cutoff point evaluation was conducted with 11 experts to delete 11 items that did not meet the CVR standard. Fifty-five pilot survey candidate items were selected through revision and additional process according to expert opinion. Seventy final candidate items were selected by adding 15 questions for validity verification and reference. A pilot survey was conducted online and offline on 332 adults aged 19–64 in Seoul and the metropolitan area, and exploratory factor analysis was used to verify the construct validity of the evaluation items. Through exploratory factor analysis, the Kaiser–Meyer–Olkin, Bartletts sphericity test, variance explained, and Cronbachs alpha criteria were confirmed in each process. Results: Exploratory factor analyses derived three criteria: food and nutrient intakes (11 items), eating behaviors (9 items), and dietary culture (14 items), consisting of 34 evaluation items. Conclusion: The present scale was validated and can be used for comprehensive evaluations of the dietary guidelines for Korean adults in a simple way.

        국내 트리티케일 품종의 품질 및 이화학적 특성

        본 연구에서는 국내산 트리티케일 3품종(신영, 조성, 신조성)의 이화학적 특성을 조사하고 이를 국내 품종의 밀(백강, 새금강) 및 호밀(씨드그린)과 비교하였다. 트리티케일은 7.77-9.69%의 단백질을 함유하여 밀과 호밀의 단백질 함량보다 다소 낮았지만, 전분함량이 높아(81.57-85.25%) 에너지원 또는 전분 기반 소재에 활용도가 높을 것으로 생각된다. 신영과 조성의 탄산나트륨 보유능은 각각 72.5와 82.0%로 이는 새금강과 백강과 비슷한 수준이었으며 신조성은 97.6%로 트리티케일 중 제분에 의한 전분 손상이 가장 많이 발생한 것을 알 수 있다. 50% 자당 보유능에서는 조성과 신조성은 신영보다 펜토산에 의한 수화 능력이 높았지만, 트리티케일의 젖산 보유능은 밀과 호밀보다 현저하게 낮아 글루텐 형성에는 취약함을 알 수 있다. 이는 wet gluten의 결과에서 트리티케일의 글루텐형성능이 밀가루의 30-50% 수준이었던 것과도 상응하며 트리티케일 중 신조성이 다른 품종보다 높은 GPI와 wet gluten을 나타내 양적 및 질적으로 가공에 유리할 것으로 생각된다. 조성과 신영은 peak viscosity가 200cp 미만으로 빵용 밀인 백강보다 낮은 반면, 신조성은 백강과 새금강의 중간 수준의 점도를 나타내었고 최종적으로 조성과 신영은 저점도의 페이스트 특성을 보였지만 신조성의 경우 다른 두 품종보다 5배 높은 점도를 가지고 있었다. 트리티케일은 낮은 호화 피크와 호화 엔탈피 값이 관측되었다. α-amylase에 의한 전분 가수분해 특성에서 트리티케일은 밀과 호밀보다 초기 20분 동안 가수분해가 더 많이 진행되었으며 120분이 경과하였을 때 밀과 호밀은 38.2-42.2% 수준인 반면 트리티케일은 47% 이상으로 가수분해 정도가 현저히 높았다. 국내산 트리티케일 품종과 특성에 대한 기초연구와 이를 기반으로 한 가공 연구를 다각적으로 수행한다면 국내 식량작물로부터 새로운 식품소재 창출과 더불어 식량안보문제 해결에도 기여할 수 있을 것이라 생각된다. This study investigated the physicochemical properties of domestic triticale flour and compared it with domestic wheat and rye flour. The triticale flour had a low protein content (7.7-9.7%) but a high total starch and amylose content compared to that of the wheat and rye flour. Shinjoseong showed the highest solvent retention capacity for four solutions (water, Na₂CO₃, lactic acid, and sucrose) among the triticale flour. The wet gluten content of triticale ranged from 13.6- 20.4%, which is lower than that of wheat flour (35.7-39.0%). Joseong showed low viscosity behavior in rapid visco analyzer (RVA), and Shinjoseong showed ranges between Baekkang and Saekeumkang flour. The differential scanning calorimetry (DSC) results showed that Shinyoung had a higher gelatinization temperature and enthalpy compared to that of the other triticale flours. All the triticale flour was hydrolyzed over 50% by α-amylase solution for 120 min, which is higher than wheat and rye flour. This study is expected to provide basic information regarding domestic triticale for the food industry and expand its utility by identifying the physicochemical properties.
