      • [논문]해석방법에 따른 사각 RC 라멘교의 거동

        본 연구에서는 사각(900°~450°) RC 라벤교를 2차원 보해석과 3차원 판해석으로 해석하였다. 연구 결과로부터 다음과 같은 결론을 얻었다. 슬래브 지점부의 단면력 집중에 따라,3차원 판해석에 의한 단면력은 2차원 보해석의 단면력보다 1.015~1.830배 크게 나타났다. 따라서 슬래브 지점부의 철근 보강 영역은 경사폭의 0.11~0.30배로 나타났다. 슬래브 최대 정모멘트 발생각과 라멘교 사각과의 상관관계를 살펴보면, 라벤교 사각 900° ~45°에 대하여 최대 정모벤트 발생각은 93° ~ 51°로 나타났다. 따라서 라벤교의 주인장 철근 이음장 위치는 최대 정모맨트 발생 구역 밖에들도록 해야한다.

      • 스프레드시트와 Green 정리를 이용한 철근 콘크리트 단주의 해석 및 설계

        구조물 설게에서, 압축과 2축 휨을 동시에 받는 기둥 설계를 자주 접하게 된다. 이러한 부재의 해석과 설계는, 모멘트 평형에 따른 여러 변수들의 값을 동시에 구하기 위하여 반복적인 작업이 필요하기 때문에 많은 시간을 요구한다. 본 연구에서는 압축과 2축 휨을 받는 철근 콘크리트 단주를 해석 및 설계할 수 있는 컴퓨터 프로그램 COL2를 개발하였다. 프로그램 COL2는 스프레드시트 프로그램 중의 하나인 엑셀을 이용한 사용자 위주 환경의 프로그램으로 개발되었다. Green정리를 사용하여 임의 형상의 역학적 특성을 계산하였다. In structural design, columns subjected to combined compression and biaxial bending are frequently encountered. The analysis and design of such members, are both time consuming and demanding because an iterative procedure is reuqired in order to simultaneously evaluate a number of variables to achieve moment equilibrium. A computer program COL2 is developed to analyze and design reinforced concrete short columns subjected to combined compression and bizxial bending in this study. The program COL2 has been developed for user-friendly environment using MS-Excel one of the spreadsheets program. The mechanical properties of generic shapes was calculated by Green's theorem.

        A report of 22 unrecorded bacterial species in Korea in the phyla Bacteroidetes and Rhodothermaeota

        A total of 22 bacterial strains belonging to the phylum Bacteroidetes were isolated primarily from aquatic environments such as seawater, freshwater, lagoon and tidal flat. One of these 22 strains was isolated from ginseng soil. Phylogenetic analyses based on 16S rRNA gene sequences revealed that 21 strains showed the high sequence similarities(${\geq}98.7%$) to the closest type strains and formed robust phylogenetic clades with closely related species in the phylum Bacteroidetes. One strain, which had been previously classified as Balneola vulgaris in the phylum Bacteroidetes, was identified as a member of the newly described phylum Rhodothermaeota. These strains had not been previously reported in Korea. Here, we report 21 species of 13 genera in the phylum Bacteroidetes and one species in the phylum Rhodothermaeota which were not reported in Korea. Morphological, biochemical, and physiological characteristics, isolation sources, and NIBR deposit numbers are described in the species descriptions.

        What is the better surgical treatment option for recurrent common bile duct stones?

        Purpose: Repeating endoscopic retrograde cholangiopancreatography (ERCP) in patients with recurrent common bile duct (CBD) stones is problematic in many ways. Choledochoduodenostomy (CDS) and choledochojejunostomy (CJS) are 2 surgical treatment options for recurrent CBD stones, and each has different advantages and disadvantages. The aim of this study was to compare the 2 surgical options in terms of the recurrence rate of CBD stones after surgical treatment. Methods: This retrospective multicenter study included all patients who underwent surgical treatment due to recurrent CBD stones that were not effectively controlled by medical treatment and repeated ERCP between January 2006 and March 2015. We collected data from chart reviews and medical records. A recurrent CBD stone was defined as a stone found 6 months after the complete removal of a CBD stone by ERCP. Patients who underwent surgery for other reasons were excluded. Results: A total of 27 patients were enrolled in this study. Six patients underwent CDS, and 21 patients underwent CJS for the rescue treatment of recurrent CBD stones. The median follow-up duration was 290 (180–1,975) days in the CDS group and 1,474 (180–6,560) days in the CJS group (P = 0.065). The postoperative complications were similar and tolerable in both groups (intestinal obstruction; 2 of 27, 7.4%; 1 in each group). CBD stones recurred in 4 patients after CDS (4 of 6, 66.7%), and 3 patients after CJS (3 of 21, 14.3%) (P = 0.010). Conclusion: CJS may be a better surgical option than CDS for preventing further stone recurrence in patients with recurrent CBD stones.

      • An Approach to Measurement of Water Quality Factors and its Application Using NOAA satellite Data

        Remotely sensed data is regarded as a potentially effective data source for the measurement of water quality and for the environmental change of water bodies. In this study, we measured the spectral reflectance by using multi-spectral image of low resolution camera(LRC) which will be loaded in the OSMI multi-purpose satellite(KOMPSAT) scheduled to be launched on 1999 to use the data in analyzing water pollution. We also investigated the possibility of extraction of water quality factors in water bodies by using remotely sensed low resolution data such as NOAA/AVHRR. In this study, Shiwha-District and Sang-Sam Lake was set up as the subject areas for the study. In this part of the study, we measured the spectral reflectance of the water surface to analyze the radiance of the water bodies in low resolution spectral band and tried to analyze the water quality factors in water bodies by using radiance feature from another remotely sensed data such as NOAA/AVHRR. As the method of this study, first, we measured the spectral reflectance of the water surface by using SFOV( Single Field of View) to measure the reflectance of water quality analysis from every channel in LRC spectral band(0.4-0.9um). Second, we investigated the usefulness of ground truth data and the LRC data by measuring every spectral reflectance of water quality factors. Third, we analyzed water quality factors by using the radiance feature from another remotely sensed data such as NOAA/AVHRR. We carried out ratio process of what we selected Chlorophyll-a and suspended sediments as the first factors of the water quality. The results of the analysis are below. First, the amount of pollutants of Shiwha-Lake has been increasing every year since 1987 by factors of eutrophication. Second, as a result of the reflectance, Chlorophyll-a represented high spectral reflectance mainly around 0.52㎛ of green spectral band, and turbidity represented high spectral reflectance at 0.57㎛. But suspended sediments absorbed high at 0.8㎛. Third, Chlorophyll-a and suspended sediments could have a distribution chart as a result of the water quality analysis by using NOAA/AVHRR data.

        응급실 사망환자

        The effective management of the critical emergency patients is the most urgent issue in the social and medical field of Korea, but still the prehospital care system is primitive in organization and activity. Prior to the settlement of the emergency medical service system, the analysis of the emergency patients and mortality cases would be important. The authors performed a review on the records of 258 cases who died during management after admission to emergency room in Yonsei University YoungDong Severance Hospital during a six year period since opening of the Hospital, April 1983 to March 1989 were as follows : 1) Among total of 258 patients who died, 159 were male(61.6%) and 99 were female. The mortality rate was 0.21% of all 125,648 patients who admitted to the emergency room, dead on arrival was 0.30% (488 cases) during the same period. The ages were ranged from neonate to 93 years, mean age was 42.5 years. 2) 117 patients, 45% of the total, were medical cases and the remainder were surgical patients. Among the surgical patients, more than half of the cases were due to motor vehicle accidents. 3) The patients who arrived at the emergency room within 30 minutes after onset of urgent situation was 53%, but within 10 minutes only 22%. The cases transported by ambulances was only 11%, most of the rest were moved by public or private vehicles. 25% of all cases had no record of the method of the transportation. 4) The time interval between arrival at the emergency room and onset of cardiac arrest was commonly within 10 minutes in 24% of all patients. In 35% of all, pronounced the death within 30 minutes after onset of cardiac arrest. 5) The main problems of the mortality cases were comatous mental state in 42% and low systolic blood pressure, less than 60mmHg in 42%. 6) The initial routine CBC and arterial blood gas analysis were abnormal in most of the mortality cases. From the above, we can conclude that the initial management in our emergency room is still at an early stage of emergency medical service system as compared to western standards. In order to overcome this obstacle the delay in the time from occurance to transport to the emergency room shortened with organization of emergency medical service system. The mortality of trauma cases in emergency room was more than that of medical disease in Korea. We have to prepare the ambulances and experienced personnel in transportation especially of trauma cases, also for critically - illed medical patients at hand. The total beds of intensive care unit must be increased and the emergent care equipment in emergency room must be added. It is important to teach to the lay - public about transportation of the emergency patients.

      • Functional connectivity in fronto-subcortical circuitry during the resting state in obsessive-compulsive disorder

        <P><B>Abstract</B></P> <P>Obsessions and compulsions mediated by cognitive inflexibility might be associated with abnormal resting state functional connectivity in the default mode network (DMN) that represents intrinsically generated neuronal activity. It was hypothesized that decreased functional connectivity in the DMN would occur in components of fronto-subcortical circuits in patients with obsessive-compulsive disorder (OCD). Twenty-two unmedicated OCD patients and 22 age- and sex-matched healthy controls received resting state functional scanning runs. The posterior cingulate cortex (PCC) region was chosen as the seed region for the connectivity analysis. Correlations between temporal connectivity with the seed region and scores on clinical measures and obsessive-compulsive symptom dimensions were also assessed. OCD patients demonstrated less functional connectivity within the DMN in the anterior cingulate cortex, middle frontal gyrus, and putamen compared to controls. The functional connectivity to the PCC seed region in OCD patients was in the direction opposite to that in the prefrontal areas with regard to scores on cleaning and obsessions/checking dimensions of OCD. These data provide evidence for fronto-subcortical dysfunction in OCD. Results from this study also support the notion that OCD is a heterogeneous disorder mediated by distinct circuits.</P>

        Clinical Practice Guidelines for the Endoscopic Management of Peripancreatic Fluid Collections

        Endoscopic ultrasonography-guided intervention has gradually become a standard treatment for peripancreatic fluid collections(PFCs). However, it is difficult to popularize the procedure in Korea because of restrictions on insurance claims regarding the use ofendoscopic accessories, as well as the lack of standardized Korean clinical practice guidelines. The Korean Society of GastrointestinalEndoscopy (KSGE) appointed a Task Force to develope medical guidelines by referring to the manual for clinical practice guidelinesdevelopment prepared by the National Evidence-Based Healthcare Collaborating Agency. Previous studies on PFCs were searched,and certain studies were selected with the help of experts. Then, a set of key questions was selected, and treatment guidelines weresystematically reviewed. Answers to these questions and recommendations were selected via peer review. This guideline discussesendoscopic management of PFCs and makes recommendations on Indications for the procedure, pre-procedural preparations,optimal approach for drainage, procedural considerations (e.g., types of stent, advantages and disadvantages of plastic and metalstents, and accessories), adverse events of endoscopic intervention, and procedural quality issues. This guideline was reviewed byexternal experts and suggests best practices recommended based on the evidence available at the time of preparation. This will berevised as necessary to address advances and changes in technology and evidence obtained in clinical practice and future studies.
