        Assessment of the mechanical properties of the muscle-tendon unit by supersonic shear wave imaging elastography: a review

        This review aimed to describe the state of the art in muscle-tendon unit (MTU) assessment by supersonic shear wave imaging (SSI) elastography in states of muscle contraction and stretching, during aging, and in response to injury and therapeutic interventions. A consensus exists that MTU elasticity increases during passive stretching or contraction, and decreases after static stretching, electrostimulation, massage, and dry needling. There is currently no agreement regarding changes in the MTU due to aging and injury. Currently, the application of SSI for the purpose of diagnosis, rehabilitation, and physical training remains limited by a number of issues, including the lack of normative value ranges, the lack of consensus regarding the appropriate terminology, and an inadequate understanding of the main technical limitations of this novel technology

        Hyaline Cartilage Regeneration Using Mixed Human Chondrocytes and Transforming Growth Factor-β<sub>1</sub>- Producing Chondrocytes

        <P>The purpose of this study was to investigate the efficacy of cartilage regeneration when using a mixture of transforming growth factor-beta1 (TGF-beta1)-producing human chondrocytes (hChon-TGF-beta1) and primary human chondrocytes (hChon) ('mixed cells'), compared with either hChon-TGF-beta1 or hChon cells alone. Specifically, mixed cells or hChon cells were first injected intradermally into the backs of immune-deficient nude mice to test the feasibility of cartilage formation in vivo. Both the mixed cells and the hChon-TGF-beta1 cells alone induced cartilage formation in nude mice, whereas hChon cells alone did not. To further test the efficacy of the cells in generating cartilage, an artificially induced partial thickness defect of the femoral condyle of a rabbit knee joint was loaded with hChon-TGF-beta1 cells with or without mixing additional untransduced hChon cells, and hyaline cartilage regeneration was observed at 4 or 6 weeks. The efficiency of complete filling of the defect and the quality of tissue generated after implanting were evaluated on the basis of a histological grading system modified from O'Driscoll et al. (J. Bone Joint Surg. 70A, 595, 1988). Significantly, mixed cells (14.2 +/- 0.9) produced significantly better results than hChon-TGF-beta1 (9.0 +/- 1.7) or hChon (8.0 +/- 1.8) cells alone. Histological and immunohistochemical staining of the newly repaired tissues produced after treatment with either mixed cells or hChon-TGF-beta1 cells alone showed hyaline cartilage- like characteristics. These results suggest that the implantation of mixed cells may be a clinically efficient method of regenerating hyaline articular cartilage.</P>

        Combined Laminoplasty and Posterior Fusion for Cervical Spondylotic Myelopathy Treatment: A Literature Review

        Study Design: A literature review. Purpose: To explore the utility of laminoplasty in combination with instrumented fusion, with a focus on neurological outcomes and changes in kyphotic deformity. Overview of Literature: Management of cervical spondylotic myelopathy (CSM) to reduce morbidity within the neurosurgical population. Methods: A US National Library of Medicine PubMed search was conducted for manuscripts pertaining to cervical laminoplasty and fusion for the management of CSM. Several relevant studies were shortlisted for review, and the bibliographies of the articles were searched for additional references. The search was limited to human studies, English-language literature, and reports on more than one patient. Results: Combined laminoplasty and fusion was found to provide at least comparable, if not superior, neurological outcomes in specific patient populations with CSM. The Japanese Orthopedic Association scores, local kyphosis, and C2–C7 angle have been reviewed in several manuscripts, and improvement in each of these categories was found with laminoplasty and fusion. Conclusions: The treatment of CSM necessitates an individualized approach based on the pathoanatomical variation. Laminoplasty and fusion can be appropriately used for patients with CSM in a setting of local kyphotic deformity, ossification of the posterior longitudinal ligament, associated segmental instability, and the need for strong stabilization.

        Variability of extracorporeal cardiopulmonary resuscitation utilization for refractory adult out-of-hospital cardiac arrest: an international survey study

        Objective A growing interest in extracorporeal cardiopulmonary resuscitation (ECPR) as a rescue strategy for refractory adult out-of-hospital cardiac arrest (OHCA) currently exists. This study aims to determine current standards of care and practice variation for ECPR patients in the USA and Korea. Methods In December 2015, we surveyed centers from the Korean Hypothermia Network (KORHN) Investigators and the US National Post-Arrest Research Consortium (NPARC) on current targeted temperature management and ECPR practices. This project analyzes the subsection of questions addressing ECPR practices. We summarized survey results using descriptive statistics. Results Overall, 9 KORHN and 4 NPARC centers reported having ECPR programs and had complete survey data available. Two KORHN centers utilized extracorporeal membrane oxygenation only for postarrest circulatory support in patients with refractory shock and were excluded from further analysis. Centers with available ECPR generally saw a high volume of OHCA patients (10/11 centers care for >75 OHCA a year). Location of, and providers trained for cannulation varied across centers. All centers in both countries (KORHN 7/7, NPARC 4/4) treated comatose ECPRpatients with targeted temperature management. All NPARC centers and four of seven KORHN centers reported having a standardized hospital protocol for ECPR. Upper age cutoff for eligibility ranged from 60 to 75 years. No absolute contraindications were unanimous among centers. Conclusion A wide variability in practice patterns exist between centers performing ECPR for refractory OHCA in the US and Korea. Standardized protocols and shared research databases might inform best practices, improve outcomes, and provide a foundation for prospective studies.

        Minimally invasive search for a missing vibrator

        Objective To report a unique surgical procedure that was utilized to locate a missing vibrator in the pelvis of a patient. Emergency room admissions and surgery secondary to the malfunctioning of devices intended for sexual stimulation are extremely common. Emergency room staff of hospitals in the United States usually are skilled in the detection and removal of these devices. Occasionally, surgical intervention is warranted if the device enters a cavity that cannot safely be explored in the emergency room setting. We report a case of a vibrator that was lost during sexual activity. A flat plate X-ray showed it to be in the abdominal cavity. Careful questioning of the patient revealed that the device had an unusually small diameter. Surgical intervention showed that the device ultimately ended up in the bladder without causing traumatic injury. Methods We created a narrated video to demonstrate the surgical procedure (Canadian Task Force Classification III). Results Laparoscopy and cystoscopy were used to visualize and successfully remove the device. The patient recovered uneventfully. Conclusion Following laparoscopic confirmation of the location of the device, it was removed via cystoscopy. This case demonstrates how background information, such as the size of the missing device in this case, can be critical to providing high quality patient care.

        Economic Impacts of Japanese Defense Spending in 1985

        This paper examines the economic impacts of various forms of defense spending on the Japanese economy for the calendar year 1985. A simple static 79-sector input-output model was used. For each of three categories, personnel, procurement, and maintenance and other, spend- ing is shown for each of the 79 input-output sectors. The model then incorporates this data and estimates the direct and indirect requirements for goods and services needed to support the defense efforts of Japan. Such estimates, to the best of our knowledge, have not been previously available outside of Japan. The paper shows that while defense expen- ditures are very close to one percent of GNP they account for a far greater proportion of the output of certain key industries such as public administration (6.4 percent), other transport equipment (mainly aircraft) (5.2), communication equipment (4.7), shipbuilding (4.4) and semi- conductors (1.5). The impacts of defense spending are also examined by absolute size and by the impact on employment. The paper then examined the import requirements (direct and indirect) of providing defense. Aircraft, not surprisingly, provide the bulk of imports needed for Japanese defense. Secondly the paper examines the economic impacts of defense spend- ing on the shipbuilding industry. The paper concludes that Japanese defense spending is sixth largest in the world, and because it is such a small percent of GNP Japan has a great capacity to increase defense spending in the future should it so choose. Defense spending also has measurable effects throughout the Japanese economy and economic impacts on other countries.

        Cell Division in the Absence of Mitosis: The Unusual Case of the Fucoid Ascophyllum nodosum (L.) Le Jolis (Phaeophyceae)

        Fluorescence and electron microscopy were used to examine epidermal shedding in the fucoid alga, Ascophyllum nodosum. Mature meristoderm cells are ca. 50-100 x 30-40 μm and highly polarized, with a single nucleus and chloroplasts near the base of the cell. Nuclei in these cells undergo mitosis when they are dividing to form a new cortical cell towards the middle of the frond, or anticlinal divisions as part of frond elongation. However, cytokinesis also occurs regularly in these cells when a new periclinal wall is deposited at about 30% of the cell length from the apical end. The newly formed distal cells are anucleate and without chloroplasts. Following cytokinesis the tangential walls then break at the thinnest point. The whole process is synchronous in adjoining epidermal cells across large areas of the frond surface, and this layer dehisces from the thallus. This is the only known plant or algal system in which cytokinesis regularly occurs in the absence of mitosis. We consider this process a novel form of programmed cell death.

        The Posterior Cranial Vertical Line: A Novel Radiographic Marker for Classifying Global Sagittal Alignment

        Objective: To define a novel radiographic measurement, the posterior cranial vertical line (PCVL), in an asymptomatic adult population to better understand global sagittal alignment. Methods: We performed a multicenter retrospective review of prospectively collected radiographic data on asymptomatic volunteers aged 20–79. The PCVL is a vertical plumb line drawn from the posterior-most aspect of the occiput. The horizontal distances of the PCVL to the thoracic apex (TA), posterior sagittal vertical line (PSVL, posterosuperior endplate of S1), femoral head center, and tibial plafond were measured. Classification was either grade 1 (PCVL posterior to TA and PSVL), grade 2 (PCVL anterior to TA and posterior to PSVL), or grade 3 (PCVL anterior to TA and PSVL). Results: Three hundred thirty-four asymptomatic patients were evaluated with a mean age of 41 years. Eighty-three percent of subjects were PCVL grade 1, 15% were grade 2, and 3% were grade 3. Increasing PCVL grade was associated with increased age (p < 0.001), C7–S1 sagittal vertical axis (SVA) (p < 0.001), C2–7 SVA (p < 0.001). Additionally, it was associated with decreased SS (p = 0.045), increased PT (p < 0.001), and increased knee flexion (p < 0.001). Conclusion: The PCVL is a radiographic marker of global sagittal alignment that is simple to implement and interpret. Increasing PCVL grade was significantly associated with expected changes and compensatory mechanisms in the aging population. Most importantly, it incorporates cervical alignment parameters such as C2–7 SVA. The PCVL defines global sagittal alignment in adult volunteers and naturally distributes into 3 grades, with only 3% being grade 3 where the PCVL lies anterior to the TA and PSVL.
