      • Radiosynthesis and <i>in vitro</i> evaluation of 1‐(tetrahydro‐5‐hydroxy‐6‐(hydroxymethyl)‐2H‐pyran‐3‐yl)‐5‐[<sup>125</sup>I]iodouracil: A new potential agent for HSV1‐tk

        <P><B>Abstract</B></P><P>Synthesis, radiolabelling, and <I>in vitro</I> evaluation of a new <SUP>125</SUP>I‐labelled iodouracil hexitol nucleoside analogue are reported. The target compound was successfully synthesized by an iodination–destannylation method and then purified by reverse phase HPLC. The radiochemical purity of the product was >99% with decay‐corrected yields of 48±3%. <I>In vitro</I> cellular uptake testing was carried out using MCA and MCA‐tk cell lines for comparison of compound 1 with [<SUP>18</SUP>F]FHBG. The newly synthesized compound 1 showed higher accumulation in herpex simplex virus type 1 thymidine kinase (HSV1‐tk) gene expression cell line (MCA‐tk cell line) than in the wild type MCA cell line compared with [<SUP>18</SUP>F]FHBG. The MCA‐tk to MCA cellular uptake ratio for compound 1 was higher than that of [<SUP>18</SUP>F]FHBG from 2 h after incubation. The radioiodine‐labelled compound 1 (I‐125, <I>t</I><SUB>1/2</SUB>=59.37 days) has a longer physical half‐life than F‐18‐(<I>t</I><SUB>1/2</SUB>=110 min) labelled FHBG. Radioiodine‐labelled compound 1 could be used for monitoring gene expression for a long time. The selectivity for MCA‐tk cell line makes compound 1 a promising imaging agent for HSV1‐tk expression.       • 간세포암환자에서 간동맥 화학 색전술 후 발생한 리피오돌에 의한 폐렴 1예

        Hepatocellular carcinoma(HCC)is one of common causes of cancer-related death in Korea where the majority of HCC patients were Hepaitc B virus(HBV)carriers and have cirrhosis. Transarterial chemoembolization(TACE)is commonly applied to the treatment of multinodular HCC in Korea and careful selection of candidate is important for the risk of various side effects. Besides common side effects as fever, nausea, abdominal pain and elevation of liver enzyme, TACE may predispose to hepatic failure, ischemic cholecystitis, pulmonary embolism, cerebral embolism and pneumonitis. In previous studies, some cases of pulmonary and cerebral embolism cases were reported but lipiodol pneumonitis after TACE was rarely reported. A 65-year-old woman with a multinodular HCC associated with HBV infection, was treated with TACE. Seven days after the procedure, nonspecific respiratory symptoms such as dyspnea and dry cough developed. Chest X-ray and chest computed tomography showed diffuse ground glass opacities in whole lung fields, suggestive of lipiodol pneumonitis. After several days of supportive care with steroid administration, radiologic abnormalities and subjective symptoms were much improved, considered that the disease was compatible with lipiodol pneumonitis.

      • Highly selective in-vivo imaging of tumor as an inflammation site by ROS detection using hydrocyanine-conjugated, functional nano-carriers

        Previously, the optical imaging of chitosan-functionalized, Pluronic-based nano-carriers by Cy5.5 conjugation revealed a good tumor targeting characteristic of the nano-carriers in vivo [J. Control. Release, 147 (2010) 109-117]. However, in spite of the relatively strong signal from tumor site, they also showed strong fluorescence signals from other organs, especially liver. Thus, for the detection of pathological sites, the direct use of the Cy5.5-conjugated nano-carriers is limited due to significant background signals associated with non-specific delivery of the probes. To overcome this limitation, in this study, we prepared hydrocyanine-conjugated and chitosan-functionalized Pluronic-based nano-carriers (Hydrocyanine-NC) that can detect ROS in pathological sites. The reduction of cyanine to hydrocyanine of the nano-carriers resulted in complete disappearance of fluorescence emission, and the fluorescence could be recovered by ROS-induced re-oxidization. Hydrocyanine-NC could detect various ROS including superoxide anion (O<SUB>2</SUB><SUP>-</SUP>) and hydroxyl radical (OH<SUP>-</SUP>) in a dose-dependent manner. Hydrocyanine-NC was also stable in serum-containing media and did not show acute cytotoxicity. Hydrocyanine-NC developed strong fluorescence by the intracellular ROS formation in LPS-stimulated macrophage cells in vitro. As an in-vivo inflammation site imaging, SCC7 tumor-bearing mice were optically monitored after the i.v. injection of the dye-conjugated nano-carriers. When non-reduced, cyanine-conjugated and chitosan-functionalized Pluronic-based nano-carriers (Cyanine-NC) were injected, strong fluorescence emission was observed from the abdominal area as well as from the tumor site, and it remained over 2days. In contrast, in the case of Hydrocyanine-NC, the initially very weak fluorescence emission from the abdominal area disappeared over time whereas the fluorescence emission from the tumor site was similar to that of Cyanine-NC. Therefore, the re-oxidation of Hydrocyanine-NC by ROS in vivo specifically eliminated the background signals from non-specific delivery of the probes, but it produced fluorescence emission strong enough to monitor the target inflammation site selectively.

      • 뇌경색증의 급성기에 발생한 변비에 대한 연구

        Objectives: The prevalence of constipation is high after stroke. Multiple factors such as long-term bed-ridden status, comorbidity, medical illnesses are combined in this condition. However, the change of bowel movement during the early stage of stroke remainsuncertain. Methods: Patients with first stroke who admitted in a single institute from 2008 to 2009 were reviewed retrospectively and 36 patients were enrolled. As a control group, 47 consecutive orthopedic patients needing bed rest without surgery in the same period were enrolled. Data of stroke associated factors, frequency of BM (bowel movement) during 5 weeks, use of gastrointestinal medications, and outcomes were collected from the medical records. Results: The cumulative incidence of decreased (<3/week) and severely decreased BM (<1/week) were 80.6% and 69.4% in stroke patients and 53.2% and 14.9% in control group (P<0.05). The cumulative incidence of increased BM (>3/day) was 38.9% in stroke and 14.9% in control group. In acute stroke, 58% of patient showed severely decreased BM in first week, and the proportion was rapidly decreased below 15% from second week. However, laxative use increased with hospital days. The occurrence of severely decreased BM in stroke patients was associated with National Institutes of Health Stroke Scale (NIHSS; P=0.004). Severely decreased BM was not associated with poor outcomes including death, morbidity, and stroke recurrence. Conclusion: Decreased BM is common in acute stroke. The occurrence of severely decreased BM is associated with NIHSS. However, the severe manifestation occurred mainly in the first week, and considered to be well controlled by laxatives.

        비알코올성 지방간의 중증도에 따른 혈중 Ghrelin 농도 변화

        목적: ghrelin은 주로 위에서 분비되는 새로운 peptide로서 음식물 섭취 및 식욕을 증가시키고 최근 인슐린 저항성 및 대사 증후군과의 관련성에 대해 많은 연구 결과가 보고되고 있다. 비알코올성 지방간은 인슐린 저항성 및 대사 증후군이 그 원인으로 잘 알려져 있는바 저자들은 비알코올성 지방간의 중증도에 따른 혈중 ghrelin 농도의 변화를 관찰하여 그들의 관련성에 대해 알아 보고자 하였다. 방법: 정상 성인 남성 65명(나이: 54.3±6.3세, 범위 42~70세)을 대상으로 하여 간 초음파 및 공복 혈중 ghrelin 농도를 측정하였다. 간 초음파 소견상 지방간의 유무 및 중증도에 따라 대상자를 세 군(Group I: 정상 27명, Group II: 경증: 24명, Group III: 중등도 이상의 지방간: 14명)으로 분류하였고, 세 군 간의 혈중 ghrelin 농도의 차이를 ANOVA test로 검정하였다. 또한 인슐린 저항성 지표인 HOMA 및 체질량 지수, 허리 둘레, 엉덩이 둘레, 허리-엉덩이 둘레비 등 인슐린 저항성 관련 인자와 혈중 ghrelin 농도와의 독립적인 상관성을 다중 회귀 분석으로 검정하였다. 결과: 간 초음파 소견상 지방간의 정도가 심해짐에 따라 혈중 ghrelin 농도가 낮아지는 경향을 보였으나 (Group I: 정상 230.9±94.3, Group II: 경증 195.2±97.2, Group III: 중등도 이상 지방간 164.3±71.4 pmol/L) 통계적으로 의미있는 결과를 얻지는 못하였으며(p=0.081), 인슐린 저항성 지표인 HOMA 및 체질량 지수, 허리 둘레, 엉덩이 둘레, 허리-엉덩이 둘레비 등 인슐린 저항성 관련 인자는 혈중 ghrelin 농도와 독립적인 관련성을 관찰할 수 없었다. 결론: 본 연구의 결과로 인슐린 저항성과 혈중 ghrelin 농도의 관련성을 증명하지는 못하였다. 하지만 간 초음파 소견상 지방간의 중증도가 심해질수록 혈중 ghrelin 농도가 낮아지는 경향을 관찰할 수 있었다. 따라서 향후 추가적인 연구로 이들의 관련성에 대한 증명이 필요할 것으로 생각한다. Background: Recently, ghrelin has been reported to be associated with insulin resistance. Nonalcoholic fatty liver disease (NAFLD) is a condition in which insulin resistance relatively plays a pivotal role. The aim of this study was to evaluate the change of serum ghrelin concentration according to severity of hepatosteatosis. Methods: Sixty five apparently normal male adults who underwent health screen examinations were classified into three groups, Group I: normal (27 subjects), Group II: mild (24 subjects) and Group III: moderate to severe fatty liver (14 subjects), according to ultrasonographic findings of liver. We analyzed the association between serum ghrelin concentration and severity of hepatosteatosis by ANOVA test. And the independent correlation between serum ghrelin concentration and insulin resistance related factors, HOMA (homeostatic model assessment), BMI (body mass index), WC (waist circumference), HC (hip circumference), WHR (waist to hip circumference ratio) were analyzed by multiple linear regression analysis. Results: Serum ghrelin concentration tended to decrease according to severity of hepatosteatosis (Group I: 230.9±94.3, Group II: 195.2±97.2, Group III: 164.3±71.4 pmol/L). But this was statistically insignificant (p=0.081). The independent correlation between serum ghrelin concentration and insulin resistance related factors were not observed. Conclusions: Our study did not prove the correlation between insulin resistance related factors and serum ghrelin concentration in NAFLD according to severity of hepatosteatosis. However, we found a tendency to decrease serum ghrelin concentration according to severity of hepatosteatosis. So, further studies are required for certification these correlations.(Korean J Med 71:45-51, 2006)

        Acute Exacerbation of Interstitial Pneumonia Other Than Idiopathic Pulmonary Fibrosis

        <P>BACKGROUNDS: Acute exacerbation (AE) in idiopathic pulmonary fibrosis (IPF) is increasingly recognized as a relatively common and highly morbid clinical event. However, clinical data on AE in non-IPF interstitial pneumonia are sparse. This study was performed to find the frequency, clinical features, and outcome of AE in non-IPF interstitial pneumonia. METHODS: Retrospective analysis of 10 patients who satisfied the modified Akira criteria for AE during follow-up of 74 patients with surgical lung biopsy-confirmed idiopathic nonspecific interstitial pneumonia (I-NSIP) and 93 patients with biopsy-confirmed interstitial pneumonia associated with collagen vascular disease (CVD-IP). RESULTS: AE occurred in six patients with I-NSIP (1-year frequency, 4.2%) and in four patients with CVD-IP (rheumatoid arthritis [RA], n = 3; scleroderma, n = 1), with 1-year frequency of 3.3%. Median age was 58 years (range, 47 to 75); six patients were female. AE occurred in two patients immediately after surgical biopsy. Median duration of acute symptom before hospital admission was 10 days (range, 1 to 30). Median ratio of Pao(2) to the fraction of inspired oxygen (Fio(2)) was 172 (range, 107 to 273), and Pao(2)/Fio(2) ratio was < 200 in six patients. Surgical lung biopsy performed at the time of AE in two patients revealed diffuse alveolar damage superimposed on nonspecific interstitial pneumonia pattern. Four patients with I-NSIP survived to discharge and were followed up for 24 months (range, 6 to 121). CONCLUSION: AE occurred in the patients with I-NSIP with apparently better prognosis. In patients with CVD-IP, AE occurred mostly with RA-usual interstitial pneumonia in our small series with poor outcome.</P>

      • Effects of polysaccharide fractions from phellodendron chinese SCHNEID on tumor progression and immunopontentiation

        In the previous paper (Kim et al., Glycoconjugate Journal 16, 247-252, 1999), heteropolysaccharides from Korean medicinal plant, Phellodendri cortex (Hwangbek) showed a poten B-Iymphocyte-stimulating activity in a system using polyclonal antibody forming cells in C57BL/6XC3H mice at dosages of 2-10 mg. In a series of biolgical active polysaccharides from natural medicinal plants, the polysaccharide fractions were isolated and purified from Phellodendron chinese SCHNEID, and antitumor activities were examined at dosages of 2, 5 and 10 mg/100 g. F-7 and F-8 showed the highest tumor inhibitory activities (inhibition ratio 96.4 and 98.2% in 2 mg/100 g), and in dose of 5 mg/100 g, the inhibitory ratios were 95.3 and 97.5%, respectively. Furthermore, 10 mg/100 g of intraperitoneal (i.p.) injection gave 97.3 and 98.7% of inhibition. In oral administration, the inhibitory activities were not markedly observed, indicating that the polysaccharides are directly acting to immune system. When the effects on TS and TK activities were determined, TS activities in the F-2 and F-7-treated mice were markedly suppressed to 73.7% and 79.5% of that in the control (p<0.01), while there was little difference in TK activity with a slight decrease in F-2 only. However, in i.p. injection, TS activities in the F-2, F-5, F-7 and F-8-treated mice were markedly suppressed to 83% to 85% of that in the control (p<0.01). Furthermore, there was also significant differences in TK activities in F-2, F-5, F-7 and F-8-treated mice (p<0.05). Therefore, polysaccharide fraction F-8 was further purified to active fractions of F-9 and F-11 by gel permeation chromatography using TSK Gel HW50S. The purified polysaccharides of F-9 and F-11 were composed of GlcNAc (47.3%), Gal (24.7%) and Man (28.0%). These results clearly indicated that the i.p. injection is much effective to suppress tumor growth than oral administration.

        In this paper, we proposed a PC-based CNC control system using EtherCAT-based servo drive and I/O device. The default communication of LinuxCNC is a parallel port, and data processing with high bandwidth is impossible. However, it is possible to apply various bandwidth devices through the application of EtherCAT, one of the industrial Ethernet communications with high bandwidth. Therefore, the hardware control method of LinuxCNC was applied through EtherCAT communication from the existing parallel port. Finally, through HAL configuration, I/O device operation check and 3-axis motion control proved the LinuxCNC system with EtherCAT.

        This study was intended to raise solids yield in the sea tangle extract. Sea tangle extracted by additions of acetone, methyl alcohol, isopropyl alcohol and ethyl alcohol. Among four solvents used for sea tangle extraction, the optimal solvent was ethyl alcohol. The most appropriate method of sea tangle extraction was two-stage extraction, that extracted with addition of ethyl alcohol and residue was extracted with water(1:13, w/v). As the optimal conditions of ethyl alcohol were 25℃ and 70%, respectively. The basis of solids yield in sea tangle extract, optimal extracting time was about 1 hour. The yield of soluble solids under above-mentioned conditions was 18.2%. Drinking product was made by adding 0.1% citric acid, 2% sorbitol and 2% oligosaccharide to the sea tangle extract(Brix 10).

        To improve the utilization of sea tangle, the processing conditions of extracts, such as kind of additives, particle size of a sample, sample to solvent ratio, extracting temperature and time were studied. Among five extracting methods used for sea tangle extraction, the optimal method was water extraction. The most appropriate particle size of sea tangle powder, extracting temperature and mixing ratio of sea tangle to water were 5 mesh, 60℃ and 1 to 13(w/v), respectively. The basis of solids yield in sea tangle extract, optimal extracting time was about 2 hours. The yield of soluble solids under above-mentioned conditions was 5.1%.
