<P>Although many cases of interindividual variation in the metabolism of CYP2C19 drugs are explained by the CYP2C19*2, *3, and *17, a wide range of metabolic variation still occurs in people who do not carry these genetic variants. The objectives of this study were to identify new genetic variants and to characterize functional consequences of these variants in metabolism of CYP2C19 substrates. In total, 21 single-nucleotide polymorphisms including three new coding variants, V394M, E405K, and D256N, were identified by direct DNA sequencing in 50 randomly selected subjects and in individuals who exhibited an outlier phenotype response in the omeprazole study. Recombinant proteins produced from the coding variants V394M, E405K, and D256N were prepared by using an Escherichia coli expression system and purified. Metabolism of S-mephenytoin and omeprazole by V394M was comparable with that of the wild-type protein. E405K showed a moderate decrease in metabolism of the substrates. However, D256N exhibited a significantly decreased activity in S-mephenytoin metabolism, resulting in 50 and 76% decreases in V(max) and intrinsic clearance, respectively, compared with the wild type. This variant also exhibited a significant decrease in omeprazole metabolism in vivo. CYP2C19 D256N and E405K were assigned as CYP2C19*26 and *2D, respectively, by the Cytochrome P450 Nomenclature Committee. In summary, this report characterizes the allele frequency and haplotype distribution of CYP2C19 in a Korean population and provides functional analysis of new coding variants of the CYP2C19 gene. Our findings suggest that individuals carrying CYP2C19*26 would have lower activity for metabolizing CYP2C19 substrate drugs.</P>

In this study, comprehensive metabolic profiles of mulberry fruits (Morus alba Linnaeus) at various maturation stages were determined using GC-MS and HPLC. In total, 48 compounds, including 3 alcohols, 16 amino acids, 7 organic acids, 2 sugars, 4 phenolics, 2 terpenes, 3 vitamins, 9 fatty acids, and 2 cyanidins were identified in the mulberry samples. Levels of chlorogenic acid, cryptochlorogenic acid, neochlorogenic acid, ascorbic acid, and ${\delta}-tocopherol$, and total fatty acid content were significantly higher in the semi-matured mulberry fruits. Furthermore, levels of glycerol, citrate, fructose, glucose, 3-O-glucoside, and cyanidin-3-O-rutinoside were significantly higher at the fully matured stage than at the other stages. Twelve biosynthetic pathways were suggested as major pathways involved in mulberry fruit maturation. The information obtained in this study will provide a basis for future investigations toward quality control or metabolic engineering for development of mulberry fruits possessing commercially valuable characteristics.
A Preliminary Study on Uncovering Medical Students' Unprofessional Behaviors from YouTube Videos

Creation of guidelines and education on digital professionalism have been sluggish despite the ever-increasing use of social media by digitally native medical students, who are at risk of blurring the line between their professional and personal lives online. A qualitative thematic analysis was applied on 79 videos extracted from 70,154 YouTube videos uploaded by Korean medical students between March and April 2020. We found 20% contained at least one concerning behavior themed under ‘failure to engage,’ ‘disrespectful behaviors,’ or ‘poor selfawareness.’ Professional lapses identified were classified into seriousness levels. Mostly were “controversial’ or ‘concerning’ but some ‘highly concerning’ contents were also found. This is the first study on digital professionalism behavior on medical students' YouTube videos. The potential negative impact on the medical profession of the easily accessible public online videos cannot be ignored and thus we suggest the need for them to be taken more seriously.

<P>The use of peptides as gatekeepers for payloads of mesoporous silica nanoparticles would allow triggering the release of guests by various biological stimuli. We investigated the effect of peptide conformation on their gatekeeping capability by employing two model peptides with a turn or a random structure. The conformation-dependent gatekeeping properties provided an opportunity to utilize the conformational conversion of peptides as a valuable motif for stimuli-responsive gatekeepers. Based on that investigation, we demonstrated that Fmoc-CGGC-SS-Si, which exhibited a zero-release property without any stimuli due to a turn-like conformation induced by the intramolecular disulfide bond, can be triggered to release guests by converting its conformation to a random structure, induced by reduction of the disulfide bond upon addition of glutathione.
