      • 칼만증후군(Kallmann's Syndrome) 1례

        Kallmann's syndrome is the most common form of isolated gonadotropin deficiency, characterized by hypogonadotropic hypogonadism due to GnRH deficiency, delayed puberty and smelling difficulty. It occurs sporadic or familial pattern, and the mode of inheritence has not been fully documented. The defect in patient of Kallmann's syndrome occurs at suprapituitary level involving mechanism that regulate GnRH synthesis or release, so this syndrome classified as a secondary hypogonadotropic hypogonadism. The gonadotropin or pulsatile GnRH administration enable successful stimulation of spermatogenesis and fertility. We have experienced 1 patient with Kallmann's syndrome and presented with the review of the literature.

      • 승모판 일탈증 환자에서 발생한 감염성 심내막염과 진균성 동맥류 파열에 의한 뇌졸중 2례

        Mitral valve prolapse(MVP) is defined as displacement of some part of mitral valve into left atrium over the mitral valvular anulus during systole by numerous etiology. It is detected common in young women, and its symptom exhibits so variable from asymptomatic to fatigue, palpitation as well as autonomic nervous system symptoms. When it is complicated with mitral regurgitation, mitral valve prolapse may cause serious complications such as infective endocarditis(IE). If MVP complicates with infective endocarditis, variable neurologic complications developed relatively common (40-50%) and in case developed into mycotic aneurysm, critical complication such as subarachnoid hemorrhage(SAH) and stroke can occur. We have experienced the two cases of MVP in young women complicated with IE and mycotic aneurysm rupture causing cerebrovascular accidents(CVA). The diagnosis was confirmed by blood culture, echocardiogram, brain CT and cerebral artery angiogram. And they have the dismal outcome despite of the the meticulous medical and surgical treatments. So we suggest that MVP is a potential risk factor of CVA, especially in young patient with stroke who without definitive risk factors.

      • The Free Radical Scavenger NecroX-7 Attenuates Acute Graft-versus-Host Disease via Reciprocal Regulation of Th1/Regulatory T Cells and Inhibition of HMGB1 Release

        <P>Graft-versus-host disease (GVHD) is a major complication associated with allogeneic hematopoietic stem cell transplantation. Despite the prominent role of the adaptive immune system, the importance of controlling the innate immune system in the pathogenesis of GVHD has recently been rediscovered. High-mobility group box 1 (HMGB1) is a crucial damage-associated molecular pattern signal that functions as a potent innate immune mediator in GVHD. In the present study, we investigated treatment of experimental GVHD through HMGB1 blockade using the compound cyclopentylamino carboxymethylthiazolylindole (NecroX)-7. Treated animals significantly attenuated GVHD-related mortality and inhibited severe tissue damage. These protective effects correlated with the decrease in HMGB1 expression and lower levels of reactive oxidative stress. Additionally, NecroX-7 inhibited the HMGB1-induced release of TNF and IL-6, as well as the expression of TLR-4 and receptor for advanced glycation end products. We also observed increased regulatory T cell numbers, which may be associated with regulation of differentiation signals independent of HMGB1. Taken together, these data indicate that NecroX-7 protects mice against lethal GVHD by reciprocal regulation of regulatory T/Th1 cells, attenuating systemic HMGB1 accumulation and inhibiting HMGB1-mediated inflammatory response. Our results indicate the possibility of a new use for a clinical drug that is effective for the treatment of GVHD.</P>

        Characterizations of High Early-Strength Type Shrinkage Reducing Cement and Calcium Sulfo-aluminate by Using Industrial Wastes

        In this study, the utilization of the by-products of various industries was examined using raw materials of CSA high-functional cement such as coal bottom ash, red mud, phosphate gypsum, etc. Technology to improve energy efficiency and reduce $CO_2$ was developed as part of the manufacturing process; this technology included lower temperature sintering ($150{\sim}200^{\circ}C$) than is used in the OPC cement manufacturing process, replacement of CSA cement with the main raw material bauxite, and a determination of the optimum mix condition. In order to develop CSA cement, a manufacturing system was established in the Danyang plant of the HANIL Cement Co. Ltd., in Korea. About 4,200 tons of low purity expansion agent CSA cement (about 16%) and about 850 tons of the lime-based expansion agent dead burned lime (about 8%) were produced at a rate of 60 tons per hour at the HANIL Cement rotary kiln. To improve the OPC cement properties, samples of 10%, 13%, and 16% of CSA cement were mixed with the OPC cement and the compressive strength and length variation rate of the green cement were examined. When green cement was mixed with each ratio of CSA cement and OPC cement, the compressive strength was improved by about 30% and the expansibility of the green cement was also improved. When green cement was mixed with 16% of CSA cement, the compressive strength was excellent compared with that of OPC cement. Therefore, this study indicates the possibility of a practical use of low-cost CSA cement employing industrial wastes only.

        Kahweol induces apoptosis by suppressing BTF3 expression through the ERK signaling pathway in non-small cell lung cancer cells

        <P>Kahweol, a diterpene molecule, has antiproliferative effects on several types of human cancer cells, but whether it has apoptotic effect in non-small cell lung cancer (NSCLC) is not known. To explore this possibility, we incubated cells from two NSCLC cell lines, NCI-H358 and NCI-H1299, with different concentrations of kahweol and used the MTS assay, DAPI staining, propidium iodide staining, Annexin V staining, immunocytochemical test, and western blot analysis to characterize this molecule and the signaling pathway underlying its effects. The kahweol-treated cells showed significantly decreased cell viability, increased nuclear condensation, and an increased number of Annexin V-positive NSCLC cells. Suppression of basic transcription factor 3 (BTF3) was followed by apoptosis induced by kahweol via the ERK-mediated signaling pathway in a dose- and time-dependent manner. In addition, kahweol modulated the protein expression of BTF3 genes involved in cell-cycle regulation and apoptosis-related proteins, resulting in apoptotic cell death. Our results collectively indicated that kahweol inhibited the proliferation of NSCLC cells through ERK-mediated signaling pathways and the downregulation of BTF3.</P>

      • Current status of pediatric umbilical cord blood transplantation in Korea: A multicenter retrospective analysis of 236 cases

        <P><B>Abstract</B></P><P>We report the outcome of 236 pediatric umbilical cord blood transplantations (UCBT) performed in Korea. Given that the sources of the grafts were mostly unrelated donors (<I>n</I> = 226; 95.8%), only the results of unrelated UCBT were included for all statistics. The most frequent primary disease was acute leukemia (<I>n</I> = 167). In total, 91.7% of recipients were seropositive for cytomegalovirus (CMV). The median doses of nucleated cells and CD34+ cells were 4.84 × 10<SUP>7</SUP>/kg and 2.00 × 10<SUP>5</SUP>/kg, respectively. The median times to neutrophil (>0.5 × 10<SUP>9</SUP>/L) and platelet recovery (>20 × 10<SUP>9</SUP>/L) were 18 and 45 days, respectively. Grade 2–4 acute graft‐versus‐host‐disease (GVHD) and chronic GVHD developed in 41.1 and 36.1% of cases, respectively. Forty‐five patients developed CMV disease. The 5‐year overall and event‐free survival were 47.5 and 36.9%, respectively. Multivariate analysis revealed that adverse factors for survival of the whole cohort were total body irradiation‐based conditioning (<I>P</I> = 0.007), salvage transplant (<I>P</I> = 0.001), failure to achieve early complete chimerism (<I>P</I> < 0.0005), and CMV disease (<I>P</I> = 0.001). The outcomes of the single‐ and double‐unit UCBT (<I>n</I> = 64) were similar, while double‐unit recipients were heavier (<I>P</I> < 0.0005) and older (<I>P</I> < 0.0005). We conclude that double‐unit UCBT is a reasonable option for older or heavier children and that the thorough surveillance of CMV infection and the development of an effective CMV therapeutic strategy may be especially important for Korean children, whose CMV seroprevalence exceeds 90%.         꿩(Phasianus colchicus) 비만세포의 조직화학적 특징과 미세구조

        꿩(Phasianus colchicus) 비만세포(mast cell)에 관한 조직화학적 특징과 미세구조를 밝히기 위하여 본 연구를 실시하였다. 꿩의 혀(tongue)와 앞위(proventriculus), 돌창자(ileum), 파브리시우스주머니(fabricius bursa)의 조직을 10% 중성포르말린 용액 또는 카르노이 (Carnoy) 용액, 절반 농도의 카르노브스키 (half-strength Karnovsky) 용액으로 각각 고정한 다음, 툴루이딘청 (toluidine blue) 또는 알시안청 (alcian blue), 콩고적 (congo red), 알칼리김자(alkaline Giemsa) 염색을 각각 실시하여 광학현미경으로 비만세포를 관찰하였다. 또한 비만세포의 미세구조를 관찰하기 위하여 위와 같은 부위를 절취하여 절반 농도의 카르노브스키 용액과 1% 오스뮴산으로 고정한 다음 통상적인 방법에 따라 염색을 실시하여 전자현미경으로 관찰하였다. 10% 중성포르말린으로 고정된 모든 조직에서 4가지 염색으로 비만세포들을 관찰할 수 없었다. 카르노이 용액으로 고정된 일부 조직에서 비만세포들을 관찰할 수 있었으나 세포의 바깥 윤곽은 뚜렷하지 않았다. 절반 농도의 카르노브스키 용액으로 고정된 모든 조직에서 염색방법에 관계없이 비만세포와 세포질내 과립들이 잘 관찰되었다. 비만세포들은 대부분 난원형이었으나 삼각형, 방추형, 부정형도 관찰되었고 이들은 손가락모양의 세포질돌기를 가지고 있었다. 전자현 미경적 관찰에서 일부 비만세포에서 세포소기관들은 잘 발달되어 있었고, 세포질내 과립들은 원형 또는 부정형이었고 막으로 둘러 싸여 있었다. 과립안은 전자밀도가 높거나 낮은 망상형 또는 입자형, 스폰지형, 혼합형 구조들이었다. 꿩의 비만세포를 관찰하기 위 하여는 10% 중성포르말린이나 카르노이 용액을 이용한 고정 보다 절반농도의 카르노브스키 용액을 이용한 고정방법이 좋을 것으로 생각된다. This study describes the histochemical characteristics and ultrastructure of mast cells from tongue, proventriculus, ileum and fabricius bursa, in pheasant (Phasianus colchicus) by light and electron microscopy. We compared the stainability of 4 different methods, toluidine blue, alcian blue, congo red and alkaline Giemsa, to stain mast cell granules from fixed pheasant organs in three different fixatives, 10% neutral buffered formalin, Carnoy’s solution or half-strength Karnovsky’s solution. Mast cells in all experimental organs were not stained with 4 different staining methods after fixation in 10% neutral buffered formalin but well stained in fixed organs with half-strength Karnovsky’s solution. The mast cells had many metachromatic granules stained with toluidine blue or alkaline Giemsa and orthochromatic granules stained with alcian blue or congo red in tissues fixed in half-strength Karnovsky’s solution. In electron microscopy, pheasant mast cells were oval, triangular, spindle-like or irregular and had a few finger-like cytoplasmic processes. There were the membrane-bounded secretory granules and the well-developed organelles in mast cells. Internal large granules were oval or irregular, and had variable shape; some higher or lower electron density with homogeneous appearance; some had a particular appearance, and a few showed reticular or spongy-like structure. This indicates that 10% neutral buffered formalin or Carnoy’s fixation may be inadequate for detection of mast cells in pheasant, whereas the half-strength Karnovsky’s fixation provides metachromatic or orthochromatic staining of mast cell granules.

        NUDT15 genotype distributions in the Korean population

        <P>Thiopurines have a narrow therapeutic range because of frequent toxicity (i.e. marrow suppression), which is only partly explained by TPMT genetic polymorphisms, especially within Asian populations. Recent studies have identified NUDT15 variation as another important factor affecting thiopurine metabolism. In this study, a total of four NUDT15 coding variants (p.Arg139Cys, p.Arg139His, p.Val18Ile, and p.Val18_Val19insGlyVal) were genotyped in 920 Korean individuals using direct sequencing of NUDT15 for the first time in a Korean population. The allele frequencies were 86.7% for NUDT15*1, and 4.4, 6.9, 0.4, 1.1, and 0.50% for *2, *3, *4, *5, and *6, respectively. The NUDT15 phenotypes based on diplotypes included normal activity (n=692), intermediate activity (n=209), and low activity (n=19), occurring in 75.2, 22.7, and 2.1% of the population, respectively. This study was the first to report NUDT15 variants other than NUDT15*3 in the Korean population and more individuals who were categorized as having intermediate or low NUDT15 activity in our study than in previously reported studies in the Korean population (24.8 vs. 19.4%, P<0.05). This study is useful for future clinical studies on thiopurine pharmacogenetics and dosage adjustment in the Korean population.</P>

        Gouty arthritis is a metabolic disorder characterized by hyperuricemia, deposition of monosodium urate crystal in the joints, and recurrent episodes of acute inflammatory arthritis. Depending on the actual causes of hyperuricemia, gout is classified as primary or secondary gout. In myeloproliferative neoplasms, a turnover of nucleic acids is greatly augmented and therefore the blood concentration of uric acid may be markedly increased. But an acute attack of gout is extremely rare in patients with myeloproliferative neoplasms. Essential thrombocythemia, one of the myeloproliferative neoplasms, is characterized by megakaryocytic hyperplasia in bone marrow and marked thrombocytosis. We report a case of secondary gout in a 66-year-old man with essential thrombocythemia.
