During the 2019 domestic measles outbreak in Korea, measles occurred in healthcare workers with two doses of the measles, mumps and rubella vaccine, and the strict application of the Occupational Safety and Health Act required medical institutions to identify healthcare workers' immunity to measles and vaccinate the susceptible pockets. In response to the frontline medical institutions' request to review the measles recommendations and guidelines, the Korean Society of Infectious Diseases held a roundtable discussion on the causes of measles outbreak, timing of vaccinations, antibody tests, and booster vaccinations for healthcare workers, and financial support from the government and municipality as well as response strategies against the outbreak in healthcare settings. In Korea, the seroprevalence of measles is decreasing in the vaccine-induced immunity group during the maintenance of measles elimination over several years. The susceptible group against measles is in their 20s and 30s, and this may be because of waning immunity rather than non-response considering Korea's vaccine policy. The risk of measles nosocomial infection from community increases as these susceptible pockets actively engage in medical institutions. Thus, data on the immunity of low seroprevalence group in Korea are needed, further discussion is needed on the booster vaccination based on the data. Especially, antibody testing and vaccination in healthcare workers may be necessary to prevent the spread of measles in medical insutitutions, and further discussion is needed regarding specific testing methods, and the timing and frequency of test and vaccination.
Infectivity of Coronavirus Disease 2019: A Prospective Cohort Study in the Korean Metropolitan Area

Background: Although several characteristics of coronavirus disease 2019 (COVID-19), an ongoing pandemic disease, have been identified, data on the infectivity of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) are limited. Methods: This prospective cohort study was conducted to analyze the infectivity of SARSCoV-2 based on data of all patients diagnosed with COVID-19 confirmed using real-time polymerase chain reaction test from January to April 2020 in Gyeonggi-do, the largest province in Korea. Results: Of the 502 patients, 298 consisting of 106 clusters with 5,909 contacts were included. Of these, 277 (93.0%) were symptomatic, and the most common symptoms were cough, fever, sputum, sore throat, and headache. A total of 94 patients (31.5%) had pneumonia, while 8 (2.7%) died during the follow-up period. The secondary attack rate (SAR) in the study population was 3.5% (204/5,909). In exposure settings, the SAR was higher in religious gathering (13.5% [95% confidence interval, 10.7–16.8%]), workplaces (8.49% [95% CI, 6.08–11.74%]), and schools (6.38% [95% CI, 3.39–11.69%]) than in health care facilities (1.92% [95% CI, 1.45–2.55%]). Sore throat at any period, dyspnea at diagnosis or any period, lower cycle threshold value in the lower respiratory tract samples, leukocytosis, and higher bilirubin levels were associated with higher infectivity of COVID-19. The presence of symptoms was not related to the infectivity. Conclusion: In establishing the infection control strategies for COVID-19, the variables associated with high infectivity may be considered.
A case of vulva hematoma with rupture of pseudoaneurysm of pudendal artery

Vulvar hematomas are relatively uncommon outside the obstetric population but may be the result of trauma to the perineum. It most often present with low abdominal pain and urologic, neurologic symptoms. The vulva presents rich vascularization, supplied by the pudendal artery, a branch of the anterior division of the internal iliac artery. We describe a rare case of a 15-cm sized vulvar hematoma with suggested rupture of pseudoaneurysm of left pudendal artery without trauma injury. A 14-year-old girl presented with sudden-pain and swelling in her left labium and successfully treated with selective arterial embolization and surgical evacuation. The literature is reviewed and managements discussed.

<P>Harvesting sufficient progenitor cells from bone marrow (BM) for pediatric patients is a challenging process, especially from smaller donors. Growth factor administration to donors prior to harvest results in an enrichment of the graft and leads to early engraftment. A total of 41 patients received a human leukocyte antigen-identical sibling transplantation using granulocyte colony-stimulating factor (G-CSF)-primed BM. All donors received G-CSF 10?μg/kg/day for 2?days prior to harvest. The median weight difference between donor and recipient was 3.9?kg (range, -29.8 to 32?kg), and the median number of CD34(+) cells harvested was 4.16??10(6)/kg (range, 1.17-31.9??10(6)/kg). The median time to neutrophil engraftment was 12?days (range, 10-27 days), and the time for platelet engraftment was 20?days (range, 12-64 days). The cumulative incidence of acute grade 2 to 3 graft-versus-host disease (GVHD) and chronic GVHD was 4.9% and 5.1%, respectively. An analysis according to the weight difference between donor and recipient showed there was no significant difference in harvested CD34(+) cell dose and in time required for engraftment between smaller and heavier donor recipients. G-CSF-primed BM allows successful engraftment and provides a valuable alternative to unstimulated BM and peripheral blood stem cells with good engraftment and tolerable GVHD even in patients with smaller donors.</P>

Background :We compared the outcomes of patients with Burkitt lymphoma and French-American- British (FAB) L3 acute lymphoblastic leukemia treated using Lymphoma Malignancy B (LMB) or other treatment protocols. Methods :Thirty-eight patients diagnosed between July 1996 and December 2007 were treated using LMB 96, and 22 patients diagnosed between January 1991 and May 1998 (defined as the early period) were treated using the D-COMP or CCG-106B protocols. We retrospectively reviewed their medical records and analyzed cumulative survival according to the treatment period by using Kaplan-Meier analysis. Results :There were no intergroup differences in the distribution of age, disease stage, or risk group. The median follow-up period of the 33 live patients in the LMB group was 72 months (range, 36-170 months). Overall survival (OS) and event-free survival (EFS) of patients treated using LMB 96 were 86.8%±5.5% and 81.6%±6.3%, respectively, whereas OS and EFS of patients treated in the early period were 72.7%±9.6% and 68.2%±9.9%, respectively. In the LMB 96 group, OS of cases showing non-complete response (N=8) was 62.5%±17.1%, and OS of relapsed or primary refractory cases (N=6) was 33.3%± 19.3%. Central nervous system (CNS) disease, high lactate dehydrogenase levels at diagnosis, and treatment response were significant prognostic factors. Conclusion :Survival outcome has drastically improved over the last 2 decades with short-term, dose-intensive chemotherapy. However, CNS involvement or poor response to chemotherapy was worse prognostic factors; therefore, future studies addressing this therapeutic challenge are warranted.

<P><B>Abstract</B></P> <P>Here, we report the results of a prevalidation trial for an <I>in vitro</I> eye irritation test (EIT) using the reconstructed human cornea-like epithelium, MCTT HCE™. The optimal cutoff to determine irritation in the prediction model was established at 35% with the receiver operation characteristics(ROC) curve for 126 substances. Within-lab(WL) and between-lab(BL) reproducibility was tested for 20 reference substances by 3 participating laboratories. Viability data described by mean±SD or ±1/2 difference between duplicate wells, and scatter plots, demonstrated the WL/BL consistency. WL/BL concordance with the binary decision, whether non-irritant or irritant was estimated to be 85–95% and 95%, respectively. WL/BL reproducibility of viability data was further supported by a strong correlation(ICC, <I>r</I> >0.9). WL/BL agreement of binary decisions was also examined by Fleiss' Kappa statistics, which showed a strong level of agreement (>0.78), nevertheless weaker than the reproducibility of the viability. The EIT with MCTT HCE™ exhibited a sensitivity of 82.2% (60/73), a specificity of 81.1% (43/53), and an accuracy of 81.8% (103/126) for 126 reference substances (for liquids; a sensitivity of 100% (47/47), a specificity of 70.6% (24/34), and an accuracy of 87.7% (71/81), and for solids, a sensitivity of 50% (13/26), a specificity of 100% (19/19), and an accuracy of 71.1% (32/45), suggesting that the accuracy is satisfactory but the sensitivity needs improvement, which shall be addressed through correcting the poor sensitivity for solid substances in future full validation trials.</P> <P><B>Highlights</B></P> <P> <UL> <LI> MCTT™ HCE EIT employing water-soluble formazan WST-1, demonstrated a performance comparable to other RhCEs. </LI> <LI> Within- and between-lab reproducibility for 3 labs with 20 chemicals were 85–95% and 95% respectively. </LI> <LI> Predictive capacity for 126 chemicals was sensitivity 82.2%, specificity 81.1% and accuracy 81.8% </LI> </UL> </P>

Primary CNS lymphoma (PCNSL) is a very uncommon disease in children, and usually treated by chemotherapy, combined with focal or craniospinal radiotherapy (RT). However,adverse effects of RT are a concern. We evaluated the outcomes of childhood PCNSL,treated with systemic and intrathecal chemotherapy, but without RT. For fifteen years, six patients among 175 of non-Hodgkin lymphoma were diagnosed as PCNSL in Seoul National University Children’s Hospital and we analyzed their medical records retrospectively. Their male:female ratio was 5:1, and median age was 10.1 yr. The primary sites were the sellar area in three patients, parietal area in one, cerebellum in one, and multiple areas in one. Their pathologic diagnoses were diffuse large B-cell lymphoma in three patients, Burkitt lymphoma in two, and undifferentiated B-cell lymphoma in one. Five were treated with the LMB96 treatment protocol, and one was treated with the CCG-106B protocol. None had RT as a first-line treatment. One patient had a local relapse and received RT and salvage chemotherapy, without success. No patient had treatment-related mortality. Their estimated 5-yr event-free and overall survival rates were both 83.3%. In conclusion, PCNSL is a rare disease in childhood, but successfully treated by chemotherapy without RT.

In order for a novel test method to be applied for regulatory purposes, its reliability and relevance, I.e., reproducibility and predictive capacity, must be demonstrated, Here, we examine the predictive capacity of a novel non-radioisotopic local lymph node assay, LLNA:BrdU-FCM (5-bromo-2`-deoxyuridine-flow cytome-try), with a cutoff approach and inferential statistics as a prediction model. 22 reference substances in OECDTG429 were tested with a concurrent positive control. Hexylcinnamaldehyde 25%(PC), and the stimulation index(SI) representing the fold increase in lymph node cells over the vehicle control was obtained. The optimal cutoff SI(2.7≤cutoff<3.5), with respect to predictive capacity, was obtained by a receiver operating character-istic curve, which produced 90.9% accuracy for the 22 substances. To address the inter-test variability in respon-siveness, SI values standardized with PC were employed to obtain the optimal percentage cutoff(42.6≤Cutoff<57.3%of PC), which produced 86.4% accuracy. A test subastance may be diagnosed as a sensitizer if a statistically significant increase in Si is elicited. The parametric one-sided t-test and non-parametric Wilcoxon rank-sum test produced 77.3% accuracy. Similarly, a test substance could be defined as a sensitizer if the SI means of the vehicle control, and of the low, middle, and high concentrations were statistically significantly different, which was test-ed using ANOVA or Kruskal-Wallis, with post hoc analysis, Dunnett, or DSCF(Dwass-Steel-Critchlow-Fligner), respectively, depending on the equal variance test, producing 81.8% accuracy. The absolute SI-based cutoff approach produced the best predictive capacity, however the discordant decisions between prediction models need to be examined further.
