        Because many centers jointly select the subject, multi-center clinical study can promptly recruit various patient groups into the research, thus being advantageous in terms of generalization. However, prospective clinical study has difficulties in implementation, because it takes a lot of efforts to trace a large scale of subjects. The purpose of this study is to develop a web database system suitable for multi-center prospective clinical study. This study has presented three basic strategies-accurate data input, easy data tracing, and real-time data verification-to supplement the demerits of prospective clinical study. The developed system has been applied in the clinical study proceeded by the study group for Respiratory failure which is affiliated to The Korean Academy of Tuberculosis and Respiratory Diseases's Association for Research on Respiratory Failure and has been verified of its performance.

      • 영도구의 생식보건사업 성과분석 : 남자중학생을 대상으로 한 성교육 효과 분석을 중심으로 the Effects of Sex Education on Middle School Students

        The purpose of this study was to reduce the sexual problems of middle school students in Youngco-Gu, Busan. The survey was conducted by content analysis which was related with reproductive healty services such as budget, manpower, etc. This study was also conducted with sex education as the object and to measure the effects of sex education. Questionnaires from 203 students(7th-9th graders) in a male middle school were surveyed on Sept. 12-14, 2001. The statistical analysis used were t-test, paired t-test, one-way ANOVA and χ^2 test by using SPSS/PC+ WIN 8.0 program. The results of this study are summarized as follows: First, 66.2% of the students among the subjects had a religion, and of those 50% were buddhists, 34.4% christians, 9.5% catholic. Religion of sexual related matters showed no significant effect. Of these students 68.1% showed medium interest in sexual related matters, 18% little interest or none and 13.5% high interest. 8th graders seemed to show more interest in sexual related matters than other grades. 41.8% of Students seemed to get information on sexual matters from friends and 27.1% from the internet. 30.9% of the respondents talked about their boy/girl friends with their parents compared to 43.3% who chose to talk to their friends about their sexual related problems. Second, 56.7% logged into lascivious-sites on the internet. Of those who logged onto lascivious-sites 46.1% logged in once or twice a month, 31.9% once or twice a week, and 2.7% logged in daily, 69.2% Felt curiosity, 23.4% felt indecent and 7.5% felt like engaging in a sexual act after visiting one of these sites. Third, the effects of sex education on experimental groups of 7th graders showed a 2.1 point increase(p<0.1), 4 point increase for 8th graders(p<.001), and 9th graders increased by 3.3 points(p<.001). Therefore this showed that it was advantageous for students to have sex education. Fourth, the effects of sex education on the students attitude showed only a little more improvement than before in the controlled groups whereas the experimental groups showed a large increase in good attitude in a good direction. Therefore, the experimental groups showed better results in sexual attitude compared to the controlled groups(p<.01).

        NIHSS 활용을 통한 뇌졸중 환자 간호의 질향상 전략

        문제: 뇌졸중 환자를 돌보는 간호현장에서는 급성기 뇌졸중 환자의 증상악화에 대한 조기사정과 적절한 대처가 매우 중요한 문제이다. 그러나 임상현장에서는 이를 객관화하고 수치화하여 모든 의료진이 의사소통할 수 있는 유용한 도구를 사용하고 있지는 못하는 실정이다. 이러한 문제를 해결하기위해 NIHSS(National Institute of Health Stroke Scale)를 신경과 진료의사를 중심으로활용하고 있지만 아직 간호현장에서는보편적으로사용하지 않고있다. 이에 뇌졸중환자간호에 NIHSS를 활용하여 뇌졸중 증상악화의 조기사정과 빠른 대처로 궁극적으로는 뇌졸중환자 간호의 질을보다 향상시키기 위하여 본활동이 시도되었다. 목적: 첫째, 간호사의 NIHSS 측정 신뢰도의 현수준을 점검하고 이를향상시키기 위한 프로그램을 개발·적용한다. 둘째, 간호사가급성기 뇌졸중환자에게 NIHSS 측정을 통하여 뇌졸중악화를 얼마나 예측할 수 있는지 분석하고 이를 향상시킬수있는 방법을 모색한다. 셋째, 궁극적으로 급성기 뇌졸중 환자의 증상악화의 조기 발견과치료를위한의료진의 올바른 대처지침을 마련하여 이를 적용한다. 의료기관: 경기도에 소재한 대학병원의 뇌졸중 집중치료실 질 향상 활동: 첫째, 간호사의 NIHSS 측정 신뢰도 향상을 위해 「NIHSS 간호사 교육 프로그램」을 마련하였다.

        GIS-based Spatial Analysis Method to Analyze the Characteristics of Fire Services and Identify Vulnerable Areas

        본 연구의 목적은 GIS 공간분석기법을 활용하여 청주시 행정구역별 소방서비스 공급특성을 분석하고, 소방서비스 공급 취약지역을 도출하는 것이다. 이를 위해 먼저, GIS 공간분석기법의 하나인 Kernel 밀도 분석을 통해, 청주시 내의 불법 주차 및 정차 발생 지점의 밀도를 분석하여 불법 주정차 고밀도 발생 위험 지역을 파악했다, 다음으로 GIS 네트워크 분석을 통해 소방서 출발 기준 5분, 10분 내 소방차 접근 취약 권역을 도출했다. 연구 결과, 청주시에는 적절한 소방 서비스가 공급되지 못하고 있으며, 중심 지역과 외곽 지역의 소방 서비스 취약 지역의 비율에 차이가 있음을 확인하였다. 이러한 서비스 불균형을 극복하기 위해 외곽지역에 적절한 소방 서비스가 배치되어야 하며, 중심 지역에는 불법 주차 단속 등의 조치가 필요하다. 본 연구의 결과는 소방서비스 취약지역을 위해 소방대 배치의 기본 자료로 활용될 수 있다. This study analyzed the characteristics of fire services in Cheongju City and identified vulnerable areas using GIS spatial analysis method. It employed a kernel density analysis to identify the areas with high density of illegal parking and stopping, and the GIS network analysis to calculate fire protection accessibility for fire trucks to reach the destination within both 5 minutes and 10 minutes from the departure at the fire station. This study found that adequate fire service has not been provided for Cheongju City and there is a difference in fire service vulnerability in the outskirt of Cheongju City. To solve this problem, it is recommended that a proper fire-fighting arrangement needs to be provided in the vulnerable areas. The results of this study can be used as the basic data for targeted arrangement of fire-fighting services in Cheongju City.

        Influences of Chronic Shoulder Pain on Muscle Tone Changes in Trunk Muscles

        The purpose of this study was to examine the influences of chronic shoulder pain on the muscle tone in trunk muscles. The study's subjects were 40 men and women in their 30 to 50s, which were divided into two groups. A chronic shoulder pain group consisted of 20 subjects who had been diagnosed with chronic shoulder pain by doctors, and a painless group consisted of 20 subjects who had experienced no such pain. An analysis was performed using electromyography on the muscle tone in the rectus abdominalis, external oblique, internal oblique, and erector spinae muscles under the same conditions between the two groups. The analysis results were as follows. The chronic shoulder pain group exhibited an overall high level of trunk muscle tone than the painless group, along with a statistically significant difference in the rectus abdominalis(p<.05). Moreover, the chronic shoulder pain group showed differences in the trunk muscle tone depending on the affected side. The chronic left shoulder pain group yielded higher levels of muscle tone in the right-side trunk muscles. In particular, the group revealed statistically significant differences in the rectus abdominalis and internal oblique(p<.05). The chronic right shoulder pain group exhibited higher levels of muscle tone in the left-side trunk muscles with a statistically significant difference in the internal oblique(p<.05). The above results suggested that chronic shoulder pain influences increases in the muscle tone in the trunk muscles on the opposite side to the affected shoulder.

        Effects of Lax Vox Voice Therapy on Respiration and Phonation in Patients with Parkinson’s Disease

        배경 및 목적: 파킨슨병은 운동기능에 영향을 미치는 중추신경계의 퇴행성 질환으로 자신의 음성을 자가 모니터링을 하지 못하여 음성의 강도와 음도 조절에 어려움을 보이는 감각장애이다. 반폐쇄성도훈련 중 Lax Vox는 튜브를 물에 넣어 발성하는 것으로 호흡, 공명, 발성의 상호작용에 초점을 맞추어 bio-feedback을 제공하는 총체적인 기법이다. 본 연구는 Lax Vox 음성치료가 파킨슨병 환자의 호흡 및 발성에 미치는 영향을 알아보고자 한다. 방법: 파킨슨병 환자 10명(남자 4명, 여자 6명)을 대상으로 Lax Vox 음성치료를 주 1회 간격으로 6주간 총 6회 실시하였다. 치료효과를 살펴보기 위해 음성치료 전후 공기역학적, 음향학적, 청지각적 평가, 전기성문파형 검사 및 한국어판 음성장애지수-10을 실시하였다. 결과: 음성치료 후 공기역학적, 음향학적 및 청지각적 평가 측정치와 음성장애지수가 유의하게 개선되었다. 특히, 폐활량, 음성 강도, 최대발성지속시간이 증가하였으며, 성문하압이 유의미하게 감소하였다. CPP와 음성범위프로파일, 발화 시 평균 강도가 유의하게 증가하였으며, sPPQ, vF0, shimmer, APQ, sAPQ, vAm가 감소하고, 한국어판 음성장애지수-10의 총점이 낮아졌다. 논의 및 결론: Lax Vox 음성치료는 파킨슨병 환자의 호흡 및 발성 능력에 긍정적인 효과가 있음을 확인하였으며, LSVT와 더불어 Lax Vox 음성치료는 파킨슨병 환자의 효과적인 치료법이 될 수 있음을 시사하였다. Objectives: Parkinson’s disease (PD) is a degenerative disease of the central nervous system that mainly affects sensory and motor function. PD patients have difficulty in controlling their vocal pitch and loudness, and poor self-monitoring due to their sensory disorder. Among semi-occluded vocal tract exercises, Lax Vox is a holistic method that provides bio-feedback by focusing on the interaction of breathing, resonance, and phonation by tube phonation in water. The purpose of this study was to investigate the effect of Lax Vox voice therapy on respiration and phonation in patients with PD. Methods: Ten patients with PD (4 males and 6 females) underwent Lax Vox voice therapy six times a week for 6 weeks. To assess the effectiveness of Lax Vox voice therapy, aerodynamic, acoustic evaluation, GRBAS scale, electroglottography, and Korean Voice Handicap Index (KVHI)-10 were performed before and after voice therapy. Results: Significant improvement in aerodynamic, acoustic, auditory-perceptual rating measures, and KVHI-10 score was yielded following Lax Vox voice therapy. After treatment, significantly increased vital capacity (VC) and maximum phonation time (MPT); and reduced subglottal pressure (Psub) were observed. Cepstral peak prominence (CPP) was significantly increased and smoothed pitch perturbation quotient (sPPQ), vF0, shimmer, amplitude perturbation quotient (APQ), smoothed APQ (sAPQ), peak-amplitude variation (vAm) were significantly decreased after treatment. Moreover, Voice Range Profile (VRP) was extended and KVHI-10 scored significantly lower. Conclusion: Current outcomes confirmed that significantly positive effects were found in respiration and phonation change following Lax Vox voice therapy for patients with PD. Therefore, Lax Vox voice therapy, in addition to Lee Silverman Voice Treatment, can be useful for an effective treatment option in patients with PD.

        Increased Tolerance to Furfural by Introduction of Polyhydroxybutyrate Synthetic Genes to Escherichia coli

        Polyhydroxybutyrate (PHB), the most well-known polyhydroxyalkanoate, is a bio-based, biodegradable polymer that has the potential to replace petroleum-based plastics. Lignocellulose hydrolysate, a non-edible resource, is a promising substrate for the sustainable, fermentative production of PHB. However, its application is limited by the generation of inhibitors during the pretreatment processes. In this study, we investigated the feasibility of PHB production in E. coli in the presence of inhibitors found in lignocellulose hydrolysates. Our results show that the introduction of PHB synthetic genes (bktB, phaB, and phaC from Ralstonia eutropha H16) improved cell growth in the presence of the inhibitors such as furfural, 4-hydroxybenzaldehyde, and vanillin, suggesting that PHB synthetic genes confer resistance to these inhibitors. In addition, increased PHB production was observed in the presence of furfural as opposed to the absence of furfural, suggesting that this compound could be used to stimulate PHB production. Our findings indicate that PHB production using lignocellulose hydrolysates in recombinant E. coli could be an innovative strategy for cost-effective PHB production, and PHB could be a good target product from lignocellulose hydrolysates, especially glucose.

        MiR-30a-5p and miR-153-3p regulate LPS-induced neuroinflammatory response and neuronal apoptosis by targeting NeuroD1

        Neurogenic differentiation 1 (NeuroD1) is an essential transcriptionfactor for neuronal differentiation, maturation, and survival,and is associated with inflammation in lipopolysaccharide (LPS)-induced glial cells; however, the concrete mechanisms are stillambiguous. Therefore, we investigated whether NeuroD1-targetingmiRNAs affect inflammation and neuronal apoptosis, as wellas the underlying mechanism. First, we confirmed that miR-30a-5pand miR-153-3p, which target NeuroD1, reduced NeuroD1 expressionin microglia and astrocytes. In LPS-induced microglia,miR-30a-5p and miR-153-3p suppressed pro-inflammatory cytokines,reactive oxygen species, the phosphorylation of c-JunN-terminal kinase, extracellular-signal-regulated kinase (ERK),and p38, and the expression of cyclooxygenase and induciblenitric oxide synthase (iNOS) via the NF-κB pathway. Moreover,miR-30a-5p and miR-153-3p inhibited the expression of NOD-likereceptor pyrin domain containing 3 (NLRP3) inflammasomes,NLRP3, cleaved caspase-1, and IL-1β, which are involved in theinnate immune response. In LPS-induced astrocytes, miR-30a-5pand miR-153-3p reduced ERK phosphorylation and iNOS expressionvia the STAT-3 pathway. Notably, miR-30a-5p exertedgreater anti-inflammatory effects than miR-153-3p. Together,these results indicate that miR-30a-5p and miR-153-3p inhibitMAPK/NF-κB pathway in microglia as well as ERK/STAT-3 pathwayin astrocytes to reduce LPS-induced neuronal apoptosis. This study highlights the importance of NeuroD1 in microgliaand astrocytes neuroinflammation and suggests that it can beregulated by miR-30a-5p and miR-153-3p.
