Effectiveness of online versus in-person structured training program on arterial blood gas, electrolytes, and ventilatory management of critically ill patients

        Gaurav Jain,Bhavna Gupta,Priyanka Gupta,Sagarika Panda,Sameer Sharma,Shalinee Rao 대한중환자의학회 2021 Acute and Critical Care Vol.36 No.1

Background: Due to the risk of viral transmission during in-person training, a shift towardonline platforms is imperative in the current pandemic. Therefore, we compared the effectivenessof an in-person interactive course with a structurally similar online course designedto improve cognitive skills among clinical health professionals in arterial blood gas analysis, management of electrolyte imbalances, and approaches to mechanical ventilation in criticallyill patients. Methods: In an observational, outcome assessor-blinded, cohort trial, group A included participantsenrolled prospectively in an online course, while group B included those who tookpart in an in-person course (retrospective arm). The primary objective was comparison ofcognitive skills through a pre and post-test questionnaire. Statistical analysis was performedusing Student t-test. Results: In total, 435 participants were analyzed in group A, while 99 participants were evaluatedin group B. The mean pre-test score was 9.48±2.75 and 10.76±2.42, while the meanpost-test score was 11.94±1.90 (passing rate, 64.6%) and 12.53±1.63 (passing rate, 73.3%)in groups A and B, respectively. Group B scored significantly higher in both pre-test (P=0.001)and post-test evaluations (P=0.004). The improvement in post-test score was significantlygreater (P=0.001) in group A (2.46±2.22) compared to group B (1.77±1.76). The medicalspecialties fared better in group B, while surgical specialties scored higher in group A. Thepre-test vs. post-test scores exhibited a moderate correlation in both groups (P<0.001). Thefeedback survey showed a Likert score >3.5 for most points in both groups. Conclusions: The online teaching module exhibited a significant benefit in terms of participantsensitization and knowledge sharing.

Influence of Alkali and Silane Treatment on the Physico-Mechanical Properties of Grewia serrulata Fibres

        JAIN, Bhupesh,MALLYA, Ravindra,NAYAK, Suhas Yeshwant,HECKADKA, Srinivas Shenoy,PRABHU, Shrinivasa,MAHESHA, G.T.,SANCHETI, Gaurav The Korean Society of Wood ScienceTechnology 2022 목재공학 Vol.50 No.5

Grewia serrulata fibres were chemically treated with 3%, 6%, and 9% NaOH for the duration of 4 h. Additionally, the NaOH-treated fibres were also treated with 3 - (trimethoxysilyl) propyl methacrylate (silane). Properties such as density and tensile strength of the treated fibres were compared against the untreated fibres. The highest density was obtained in the case of 9% NaOH + silane treated fibres, which was 26.47% higher than untreated fibres, implying effective removal of hemicellulose. Likewise, the highest tensile strength was also obtained in the case of 9% NaOH + silane treated fibres. The increment observed in the tensile strength of the natural fibres was related to the removal of impurities, hemicellulose, and stress-raisers as well as deposition over the fibre surface that smoothed it. These observations were further validated by estimating changes in chemical constituents due to chemical treatment along with characterization techniques such as scanning electron microscopy and thermogravimetric analysis.

      • Design and Implementation of Router for NOC on FPGA

        Gaurav Verma,Harsh Agarwal,Shreya Singh,Shaheem Nighat Khanam,Prateek Kumar Gupta,Vishal Jain 보안공학연구지원센터 2016 International Journal of Future Generation Communi Vol.9 No.12

In today’s technological era, SOC has undergone rapid evolution and is still processing at a swift pace. But due to this explosive evolution of semiconductor industry, the devices are scaling down at a rapid rate and hence, SOC today have become communication-centric. However, the existing bus architectures comprising of wires for global interconnection in SOC design are undergoing design crises as they are not able to keep up with the rate of scaling down of devices. To overcome bottleneck of communication system, NOC is an upcoming archetype. In on-chip network, router is considered as an important component. This paper proposes router, its components and parameters which affects the entire design. Thus, to validate the functioning of NOC on hardware, router has been designed in VHDL and simulated in Xilinx ISE 14.1 targeting Xilinx XC5VLX30-3 FPGA.

Bendamustine in combination with ifosfamide, etoposide, and vinorelbine (VIBE) is an effective salvage regimen for heavily pre-treated patients with relapsed or refractory Hodgkin lymphoma: a single-center experience

        Gaurav Prakash,Arihant Jain,Kamalkant Sahu,Amanjit Bal,Charanpreet Singh,Rajender Basher,Harmandeep Singh,Kundan Mishra,Aditya Jandial,Deepesh Lad,Alka Khadwal,Radhika Srinivasan,Ashim Das,Neelam Varm 대한혈액학회 2021 Blood Research Vol.56 No.3

Background This study evaluated the outcomes of patients with refractory/relapsed Hodgkin lymphoma (RRHL) treated with a bendamustine-based regimen in combination with ifosfamide, etoposide, and vinorelbine (VIBE). Methods Consecutive RRHL patients who were treated with the VIBE regimen were identified and studied for clinicopathologic characteristics, response to VIBE regimen, event-free survival (EFS), and feasibility of an autologous stem-cell transplant (autoSCT). Results In total, 24 patients received the VIBE regimen, and a median of 3 cycles were administered. In this cohort, 80% of the patients had received ≥2 prior lines of therapy. The overall and complete response rates with VIBE were 79% and 42%, respectively. After a median follow-up (following VIBE regimen) of 14 months (range, 3‒76), the 3-year EFS and OS were 46% and 74%, respectively. Of the eligible patients, 92% underwent successful AutoSCT. The mean CD34+ cell count in the autograft was 5.5×106 /kg (SD 2.07). Neutropenia was the commonest hematologic toxicity and it was observed in 42% of the patients. However, only 9% of the patients developed grade III/IV febrile neutropenia. Chemotherapy-induced nausea and vomiting were the second most common grade III/IV toxicities in our cohort of patients. Conclusion In this retrospective analysis, the combination regimen, VIBE, has shown good efficacy in heavily pre-treated patients with RRHL without compromising stem cell collection. These encouraging results provide a rationale for further development of this regimen.

      • Effect of granite fines on mechanical and microstructure properties of concrete

        Jain, Kishan Lal,Sancheti, Gaurav Techno-Press 2022 Advances in concrete construction Vol.13 No.6

Solid waste management is of great concern in today's world. An enormous amount of waste is generated from various industrial activities. Concrete production utilizing some of the potential waste materials will add to the benefit of society. These benefits will include reduction of landfill burden, improved air quality, riverbed protection due to excessive sand excavation, economical concrete production and much more. This study aims to utilize waste granite powder (GP) originating from granite industries as a sand replacement in concrete. Fine GP was collected in the form of slurry from different granite cutting industries. In this study, GP was added in an interval of ten percent as 10%, 20%, 30%, 40% and 50% by weight of sand in concrete. Mechanical assets; compressive strength, flexural strength and splitting tensile strength were prominent for control and blended mixes. Modulus of elasticity (MoE) and abrasion tests were also performed on control and blended specimens of concrete. To provide a comprehensive clarification for enhanced performance of GP prepared concrete samples, scanning electron microscopy (SEM) and X-ray diffraction (XRD) were performed. Results indicate that 30% replacement of sand by weight with GP enhances the mechanical assets of concrete and even the results obtained for 50% replacement are also acceptable. Comprehensive analysis through SEM and XRD for 30% replacement was better than control one. The performance of GP added to concrete in terms of abrasion and modulus of elasticity was far better than the control mix. A significant outcome shows the appropriateness of granite fines to produce sustainable and environmentally friendly concrete.

      • Image Encryption Schemes : A Complete Survey

        Yamini Jain,Ritesh Bansal,Gaurav Sharma,Bhuvnesh Kumar,Shailender Gupta 보안공학연구지원센터 2016 International Journal of Signal Processing, Image Vol.9 No.7

Advancement in digital technologies has resulted in increased data transfer over internet in recent years. As a result, security of images/data is one of the biggest concern of many researchers. Therefore several cryptographic schemes have been proposed for image/data encryption. An efficient cryptographic scheme is one that have high brute force search time, low execution time complexity and should be able to provide good security. In this paper, several ciphers (traditional as well as modern) for images are compared based on various parameters such as: Time complexity, Peak Signal to Noise Ratio (PSNR), Number of Pixels Change Rate (NPCR), Unified Average Changing Intensity (UACI) and Entropy. In addition, the paper also shows the shortcomings of traditional ciphers that were used for text and how modern ciphers overcome this limitations. The analysis of simulation result shows that chaotic encryption schemes are most efficient and better than others.

        Arihant Jain,Aditya Jandial,Thenmozhi Mani,Kamal Kishore,Charanpreet Singh,Deepesh Lad,Gaurav Prakash,Alka Khadwal,Reena Das,Neelam Varma,Subhash Varma,Pankaj Malhotra 대한혈액학회 2024 Blood Research Vol.59 No.1

Background The standard dose (SD) of horse anti-thymocyte globulin (hATG) ATGAM (Pfizer, USA) or its biosimilar thymogam (Bharat Serum, India) for the treatment of Aplastic Anemia (AA) is 40 mg/kg/day for 4 days in combination with cyclosporine. Data on the impact of hATG dose on long-term outcomes are limited. Here, we describe our comparative experience using 25 mg/kg/day (low-dose [LD]) hATG for 4 days with SD for the treatment of AA. Methods We retrospectively studied patients with AA (age > 12 years) who received two doses of hATG combined with cyclosporine. Among 93 AA patients who received hATG, 62 (66.7%) and 31 (33.3%) patients received LD and SD hATG with cyclosporine, respectively. Among these,seventeen(18.2%) patients also received eltrombopag with hATG and cyclosporine. Overall response rates [complete response (CR) and partial response (PR)] of LD and SD hATG groups at 3 months (50% vs. 48.4%; p = 0.88), 6 months (63.8% vs. 71.4%; p = 0.67), and 12 months (69.6% vs. 79.2%; p = 0.167) were comparable. The mean (Standard Deviation) 5-year Kaplan–Meier estimate of overall survival and event-free survival was 82.1 (4.6)% and 70.9 (5.5)% for the study population. The mean (standard deviation) 5-year Kaplan–Meier estimate of overall survival and event-free survival of those who received LD hATG versus SD hATG dose was 82.9 (5·3)% versus 74.8 (10·3)% (P = 0·439), and 75.2 (6.2)% versus 61.4(11.2)% (P = 0·441). Conclusion Our study revealed that the response rates of patients with AA and LD were similar to those of patients with SD to hATG combined with cyclosporine in a real-world setting.
