        척수손상·비손상인의 신체부위별 신체구성 및 골밀도 비교

        This study was designed to compare segmental body composition and bone mineral density in spinal cord-injured(SCIS; n=9, SCIA; n=9) and non-injured control(AB; n=9) subjects. Dual-energy X-ray absorptiometry(DEXA) was used to measure %body fat, body fat mass, lean body mass, bone mineral contents, and bone mineral density in total and various body regions(arms, legs, trunk). The present results showed that body fat mass and %body fat were significantly higher and lean body mass was significantly lower in the SCIS group compared to AB group, while there was no difference between the SCIA and AB group in those of parameters. In BMC, there was no difference among groups and in BMD, SCI group was significantly lower that AB group, but there was no difference between SCIA group and AB group. Based on these results, in can be suggested that long-term regular exercise can reduce the negative changes in body composition, bone mineral density by spinal cord injury.

      • 93년도 대전지역에서 유행한 홍역 질환의 임상적 및 혈청학적 고찰

        1993년 11월부터 1994년 1월까지 대전지역 4개 종합병원에서 홍역으로 진단된 환아 133예를 대상으로 홍역 예방접종 유무와 시기에 따라 15개월에 예방접종을 받았던 환아군은 GroupⅠ, 15개월 이전에 예방접종을 받았던 환아군은 GroupⅡ, 6-9개월과 15개월에 2회 예방접종을 받았던 군은 GroupⅢ, 예방접종을 받지 않았던 군은 GroupⅣ로 분류하여 각군에서 발병연령, 임상증상, 합병증, 홍역에 대한 IgM과 IgG 특이 항체 등을 비교 분석하여 다음과 같은 결과를 얻었다. 1) 대상환아 133예중 예방접종을 받았던 환아는 71예(53.4%)이고 예방접종을 받지 않았던 환아는 62예(46.6%)이었다. 연령에 따른 빈도는 1세 이하가 39.1%, 5-6세가 12.0%, 1-2세가 11.2%이었다. 각 군에서 많이 발생하였던 환아의 연령분포는 GroupⅠ은 6-8세 사이가 64.7%, GroupⅡ는 2세이하가 70%, GroupⅢ은 6세 이상이 80%, GroupⅣ는 1세이하가 80.6%이었다. 2) 모든 환아에서 발진과 발열이 나타났고, 출현 시기에 따른 발진과 발열증상의 상호 관계에서 발열은 환아의 60예(45.1%)에서 발진 출현 2일전에 발생하였으며 발열은 43예)32.3%)에서 발진과 함께 발생하였다. 전체 환아의 46예(34.6%)에서 발열기간이 3일 이하이었다. 3) 전체 환아에서 기침증상이 있었고, 전체 환아의 80예(60.2%)에서 10일 이상 기침이 지속되었다. 예방접종을 받지 않았던 군에서 62예중 42예(67.7%)에서 기침이 10일 이상 지속되었으며 이중 24예(57.1%)에서 합병증이 있었다. 예방접종을 받았던 군에서는 71예중 38예(53.5%)에서 기침이 10일 이상 지속되었으며 이중 17예(44.7%)에서 합병증이 있었다. 4) 합병증은 88예(66.1%)에서 관찰되었으며 예방 접종을 받지 않았던 GroupⅣ(71.0%)에서 가장 많이 발생하였다. 합병증으로는 폐렴(46.6%), 중이염(7.5%), 급성위장염(6.8%), 간염(6.8%) 등이 있었다. 5) 예방접종을 받았던 환아 71예중 64예에서 혈청검사를 실시한 결과 55예에서 홍역에 대한 IgM 특이항체가 양성으로 나타나 일차 면역 실패(primary vaccine failure)는 전체 환아의 85.9%에서 있었고, IgM은 9예에서 음성으로 나타나 이차 면역 실패(secondary vaccine failure)는 전체 환아의 14.1%에서 나타났다. 예방접종을 받지 않았던 환아 62예중 혈청검사를 시행한 53예에서 IgG 음성이 20예(37.7%) 이었으며 IgG가 음성으로 나타난 연령은 6개월에서 14개월로 고루 분포하였다. 결론적으로 홍역 유행시 예방접종을 받았던 환아에서 발생한 홍역질환의 주원인은 일차 백신 실패이었으며 예방접종을 받지 않았던 환아에서 모체로 부터의 수동 항체가 생후 6개월부터 소실되는 소견이 관찰되므로써 홍역 이환의 위험이 높은 경우에는 조기에 예방접종을 시행하여야 할 것으로 사료된다. A clinical study was made on 133 case of measles patients during epidemics in Taejeon from Nov. 1993 to Jan. 1994. The patients were divided into four groups by vaccination history. We evaluated the clinical manifestations, complications, and serological antibody of measles (IgM & IgG) in each group. The results were summerized as follows. 1) Out of 133 patients, 71 patients(53.4%) were the vaccinated group and 62 patients (46.6%) were the unvaccinated group. Peak ages of measles in each group were 5-8 year old(64.7%) in GroupⅠ, less than 2 year old (70%) in GroupⅡ, more than 6 year old (80%) in GroupⅢ, and less than 1 year old(80.6%) in GroupⅣ. 2) Fever developed before 2 days of rash appearence(45.1%) and at the same time(32.3%), Duration of fever was less than 3 days in 46cases(34.6%). 3) All patients had cough and 80 cases(60.2%) had cough more than 10 days. Cough continued more than 10 days in 42 cases(67.7%) of 62 unvaccinated cased and 38 cases(53.5%) of 71 vaccinated cases. 4) Complications such as pneumonia, otitis media, gastroenteritis, and hepatitis were noted in 66.1% of all patients and most common in GroupⅣ. 5) In the 64 patients of vaccinated group carried out serologic test, measles specific IgM antibodies were positive in 85.9% (primary vaccine failure) and negative in 14.1%(secondary vaccine failure). In the 53 patients of unvaccinated group carried out serologic test, measles specific IgG antibodies were negative in 37.7%, less than 14 month old. In conclusion, primary vaccine faliure (85.9%) was the main cause of measles during epidemics in the vaccinated group. maternal measles IgG antibodies were not detected in 37.7%, less than 14 month old.

      • Nodular Fasciitis of the Parotid Gland, Masquerading as Pleomorphic Adenoma

        <P>It is difficult to distinguish nodular fasciitis (NF) from other neoplasm of the parotid gland, especially pleomorphic adenoma (PA) by fine needle aspiration cytology. A 39-year-old female noticed a mass in the parotid region. The aspirate material showed cohesive parts composed of the cells that had oval or spindle-shaped nuclei and relatively abundant cytoplasm and some cells with plasmacytoid features. The background substance was fibromyxoid. PA was diagnosed based on the cytologic findings. Subsequently, parotidectomy was performed and NF was diagnosed based on histologic and immunohistochemical findings. NF in the parotid region is rare and may be misdiagnosed as other benign or malignant tumors of the parotid gland. The clinical history of rapid growth and the presence of mitoses and inflammatory cells help to distinguish NF from PA. In addition, immunohistochemical stains for smooth muscle actin and CD68 are useful to confirm the diagnosis of NF.</P>

        Origin of lactic acid bacteria in mulkimchi fermentation

        The assortment of endophytic lactic acid bacteria (LAB) in kimchi derives from its raw vegetables, which include Chinese cabbage, radish, welsh onion, onion, garlic, red pepper, and ginger. These vegetables were examined during mulkimchi fermentation using gene-specific multiplex polymerase chain reaction and 16S ribosomal RNA sequence analysis. Sixteen species from five LAB genera (Leuconostoc, Lactobacillus, Lactococcus, Pediococcus, and Weissella) appeared in the raw kimchi materials. Interestingly, nine LAB species were identified in mulkimchi on fermentation day 0 as follows: Leuconostoc carnosum, Leuconostoc citreum, Leuconostoc gelidum, Leuconostoc inhae, Leuconostoc mesenteroides, Lactobacillus plantarum, Lactobacillus sakei, Lactococcus lactis, and Weissella confusa. Seven additional LAB species were present in mulkimchi at fermentation day 9 as follows: Leuconostoc gasicomitatum, Leuconostoc kimchii, Lactobacillus brevis, Lactobacillus curvatus, Lactobacillus pentosus, Pediococcus pentosaceus, and Weissella koreensis. These species corresponded completely with the LAB in kimchi vegetables. Wei. confusa was the predominant LAB during early fermentation (pH 6.20 to 4.98 and acidity 0.20 to 0.64%), while Lac. sakei, Lac. plantarum, and Wei. koreensis became dominant later in fermentation (pH 4.98 to 3.88 and acidity 0.64 to 1.26%). These results collectively demonstrate that the LAB involved in mulkimchi fermentation originates from the raw vegetables examined.

      • 원위 대퇴골의 고립성 골연골종에서 발생한 속발성 연골 육종 1례 보고

        연골육종은 비교적 서서히 자라며 늦게 전이하는 악성 연골 종양으로 알려져 있다. 골 연골종에서 발생한 속발성 연골육종은 발생빈도는 드물며 타부위로의 전이가 적고 악성도가 낮은 경우가 많아 적합한 수술적 치료로서 좋은 예후를 보이는 것으로 보고되고 있다. 본 동국대학교 정형외과학 교실에서는 우측 대퇴골 원위부 골간단부에 생긴 고립성 골 연골종에서 이차적으로 발생한 연골육종을 치험한 바 문헌고찰과 함께 보고하는 바이다. Secondary chondrosarcomas arising from a solitary osteocartilaginous exostosis are rare and those have a better prognosis than other chondrosarcomas, and they rarely metastasize. We present our experience with a case of secondary chondrpsarcoma arising in solitary osteochondroma of distal metaphysis of right femur with clinical details.

        가바와 비당체 이소플라본이 증가된 Lactobacillus brevis 발효 콩-분말 두유의 생리활성 증진 효과

        본 연구는 Lactobacillus brevis 균주들을 이용하여 제조된 발효콩-분말 두유(fermented soy-power milk, FSPM)의 기능성물질함량(GABA 및 isoflavone)과 항산화 활성 및 소화효소 저해활성을 비교 측정하였다. 김치로부터 가바 생성 유산균 10종을 분리하였고 16S rRNA 유전 분석 결과 L. brevis와 96-99% 상동성을 나타내었다. GABA 전환율은 MSG, 활성 글루텐과 콩 단백질에서 각각 66.96-93.51%, 63.76-84.58%과 57.05-69.75% 있었다. FSPM은 콩-분말 두유(SPM)보다 pH와 GA 함량은 낮았으나, 산도와 GABA 함량은 높았다. BMK484 균주 FSPM의 GABA 전환율은 69.97%로 가장 우수하였다. 한편, SPM은 배당체 이소플라본의 함량(1290.93 μg/g)이 높았으나, FSPM는 비당체 이소플라본의 함량(287.27-501.9 μg/g)이 높았다. BMK484균주 FSPM의 경우 비당체 이소플라본인 daidzein, glycitein과 genistein 함량이 각각 240.2, 61.24과 200.45 μg/g으로 가장 높았다. BMK484 균주 FSPM의 DPPH, ABTS와 hydroxyl 라디칼 소거활성 및 α-glucosidase와 pancreatic lipase 저해활성은 각 60.31, 88.10와 61.25 및 52.71와 39.37%로 비교적 우수하였다. 따라서 본 연구의 L. brevis들은 발효 콩-분말 두유에서 GABA 및 비당체 이소플라본을 동시에 강화할 수 있는 소재로서 사용될 수 있다. The research was aimed to analyze the functional constituents (GABA and isoflavone), radical (2,2-diphenyl-1- picrylhydrazyl (DPPH), 2,2-azino-bis (3-ethylbenzothiazoline-6- sulfonic acid) (ABTS), and hydroxyl) scavenging activities and enzyme (α-glucosidase and lipase) inhibitory effects of soypowder milk (SPM) and fermented soy-powder milk (FSPM) with varied Lactobacillus brevis. Ten γ-aminobutyric acid (GABA) producing lactic acid bacteria that showed 96-99% similarity with L. brevis, according to 16S rRNA gene sequence analysis, were isolated from fermented kimchi. The conversion rates of GABA were obtained 66.96-93.51, 63.76-84.58, and 57.05-69.75% in monosodium glutamate, gluten and soy protein, respectively. The levels of pH and glutamic acid of FSPM were found lower than those of SPM, but the acidity and GABA contents were higher. The GABA conversion rate of FSPM with BMK484 strain was attained the highest 69.97%. The contents of isoflavone glycoside (1290.93 μg/g) was higher in SPM, but the content of isoflavone aglycone (287.27-501.9 μg/g) was higher in FSPM. The levels of isoflavone aglycone such as daidzein, glycitein and genistein, were found as the highest 240.2, 61.24 and 200.45 μg/g, respectively, when FSPM was made with BMK484 strain. The DPPH, ABTS and hydroxyl radical scavenging and α-glucosidase and pancreatic lipase inhibitory activities of FSPM made with BMK484 strain were the relatively higher 60.31, 88.10, 61.25, 52.71, and 39.37%, respectively. Therefore, the L. brevis can be used as a material capable of simultaneously enhanced GABA and isoflavone aglycone in FSPM.

        Structural, Optical, and Chemical Properties of Cadmium Phosphate Glasses

        In this study, we prepared cadmium phosphate glasses with various compositions, given by $xCdO-(100-x)P_2O_5$ (x = 10-55 mol%), and analyzed their Fourier transform infrared spectra, dissolution rate, thermal expansion coefficient, glass transition temperature, glass softening temperature, and optical band gap. We found that the thermal expansion coefficient and dissolution rate increased while the glass transition temperature and glass softening temperature decreased with increasing CdO content. These results suggest that CdO acts as a network modifier in binary phosphate glass and weakens its structure.

        <P>Hypersensitivity pneumonitis (HP) is mediated by Th1 immune response. NKT cells regulate immune responses by modulating the Th1/Th2 balance. Therefore, we postulated that NKT cells play a critical role in the development of the HP by modulating the Th1/Th2 response. To address this issue, we explored the functional roles of NKT cells in Saccharopolyspora rectivirgula (SR)-induced HP. In CD1d(-/-) mice, the HP was worse in terms of histological changes, hydroxyproline levels, the CD4:CD8 ratio in bronchoalveolar lavage fluid, and SR-specific immune responses than in control mice. CD1d(-/-) mice showed elevated IFN-gamma production in the lung during the HP, and this was produced mainly by Gr-1+ neutrophils. The blockade of IFN-gamma in CD1d(-/-) mice attenuated the HP, whereas the injection of rIFN-gamma aggravated it. Moreover, the depletion of Gr-1+ neutrophils reduced CD8+ T cell numbers in bronchoalveolar lavage fluid during the HP. The adoptive transfer of IL-4(-/-) mouse NKT cells did not attenuate the HP, whereas wild-type or IFN-gamma(-/-) mouse NKT cells suppressed the HP. In conclusion, NKT cells producing IL-4 play a protective role in SR-induced HP by suppressing IFN-gamma-producing neutrophils, which induce the activation and proliferation of CD8+ T cells in the lung.</P>
