      • Systematic Review on Epstein-Barr Virus (EBV) DNA in Diagnosis of Nasopharyngeal Carcinoma in Asian Populations

        Objective: To conduct a meta-analysis to investigate the value of EBV DNA in diagnosis of nasopharyngeal cancer (NPC) in Asian populations, and provide important evidence for screening. Methods: Prospective or respective case-control or cohort studies regarding the detection role of EBV DNA for NPC were included in our study. We conducted a comprehensive literature search in PubMed, EMBASE, and the Chinese Biomedical Database (CBM database between January 1980 and March 2012. Results: A total of 18 studies with 1492 NPC cases and 2641 health controls were included. Almost of the included studies were conducted in China, and only one other conducted in Thailand. The overall results demonstrated that the pooled sensitivity, specificity, positive likelihood (+LR) and negative likelihood (-LR) were 0.73 (0.71-0.75), 0.89 (0.88-0.90), 8.84 (5.65-13.84) and 0.19(0.11-0.32), respectively. The overall EBV DNA detection showed the largest area of 0.932 under the summary receiver operator curve (SROC). The accuracy of detection by plasma for NPC (0.86) was higher than in serum (0.81), with largest areas under the SROC of 0.97 and 0.91, respectively. Conclusion: Our results demonstrated the EBV DNA detection in plasma or serum has high sensitivity and specificity in diagnosis of NPC, especially in Chinese populations with a high risk of cancer.

      • High Expression of Forkhead Box Protein C2 is Related to Poor Prognosis in Human Gliomas

        Background: Increasing evidence has indicated that high Forkhead box protein C2 (FOXC2) level is closely associated with the development, progression, and poor prognosis of a variety of tumors. However, the relationship between FOXC2 and the progression of human gliomas remains to be clarified. The aim of present study was to assess FOXC2 expression and to explore its contribution in human gliomas. Materials and Methods: Realtime quantitative PCR was performed to examine FOXC2 expression in 85 pairs of fresh frozen glioma tissues and corresponding non-neoplastic brain tissues. Associations of FOXC2 expression with clinicopathological factors and prognosis of glioma patients were statistically analyzed. Results: The relative mRNA expression of FOXC2 was significantly higher in glioma tissues than the corresponding non-neoplastic brain tissues (p<0.001). In addition, high FOXC2 expression was significantly associated with advanced pathological grade (P=0.005) and the low Karnofsky performance score (KPS) (p=0.003), correlating with poor survival (p<0.001). Furthermore, multivariate Cox regression analysis showed that high FOXC2 expression was an independent predictor of overall survival (p=0.006). Conclusions: FOXC2 may act as an oncogenic gene and represent a potential regulator of aggressive development and a candidate prognostic marker in human gliomas.

        Ginseng berry polysaccharides on inflammation-associated colon cancer: inhibiting T-cell differentiation, promoting apoptosis, and enhancing the effects of 5-fluorouracil

        Background: Ginseng is a commonly used herbal medicine in treating various medical conditions. Chronic gut inflammation is a recognized factor for the development of colorectal cancer (CRC). In this project, Asian ginseng berry polysaccharide preparations were used to assess their effects on CRC and related immune regulation mechanisms. Methods: Ginseng berry polysaccharide extract (GBPE) and purified ginseng berry polysaccharide portion (GBPP) were used to evaluate their activities on human HCT-116 and HT-29 CRC cell proliferation. Interleukin-8 secretion analysis was performed on HT-29 cells. Naive CD4 cell isolation and T-helper cell differentiation were performed and determined using flow cytometry for Th1 and Treg in addition to cell cycle and apoptotic investigation. Results: GBPE and GBPP significantly inhibited interleukin-8 secretion and cancer cell proliferation, inhibited CD4<sup>+</sup>IFN-γ<sup>+</sup> cell (Th1) differentiation, and decreased CD4<sup>+</sup>FoxP3<sup>+</sup> cell (Treg) differentiation. Compared to the GBPE, GBPP showed more potent antiinflammatory activities on the malignant cells. This is consistent with the observation that GBPP can also inhibit Th1-cell differentiation better, suggesting that it has an important role in antiinflammation, whereas Treg cells hinder the body's immune response against malignancies. Supported by cell cycle and apoptosis data, GBPE and GBPP, at various degrees, remarkably enhanced the anticancer activities of 5-fluorouracil. Conclusion: Data from this project suggested that Asian ginseng berry potentially has clinical utility in managing enteric inflammation and suppressing CRC through immunomodulation mechanisms.

        Systolic Arrays for Lattice-Reduction-Aided MIMO Detection

        Multiple-input multiple-output (MIMO) technology provides high data rate and enhanced quality of service for wireless communications. Since the benefits from MIMO result in a heavy computational load in detectors, the design of low-complexity suboptimum receivers is currently an active area of research. Lattice-reduction-aided detection (LRAD) has been shown to be an effective low-complexity method with near-maximum-likelihood performance. In this paper, we advocate the use of systolic array architectures for MIMO receivers, and in particular we exhibit one of them based on LRAD. The "Lenstra-Lenstra-Lov$\acute{a}$sz (LLL) lattice reduction algorithm" and the ensuing linear detections or successive spatial-interference cancellations can be located in the same array, which is considerably hardware-efficient. Since the conventional form of the LLL algorithm is not immediately suitable for parallel processing, two modified LLL algorithms are considered here for the systolic array. LLL algorithm with full-size reduction-LLL is one of the versions more suitable for parallel processing. Another variant is the all-swap lattice-reduction (ASLR) algorithm for complex-valued lattices, which processes all lattice basis vectors simultaneously within one iteration. Our novel systolic array can operate both algorithms with different external logic controls. In order to simplify the systolic array design, we replace the Lov$\acute{a}$sz condition in the definition of LLL-reduced lattice with the looser Siegel condition. Simulation results show that for LR-aided linear detections, the bit-error-rate performance is still maintained with this relaxation. Comparisons between the two algorithms in terms of bit-error-rate performance, and average field-programmable gate array processing time in the systolic array are made, which shows that ASLR is a better choice for a systolic architecture, especially for systems with a large number of antennas.

        Effect of patient decision aids on choice between sugammadex and neostigmine in surgeries under general anesthesia: a multicenter randomized controlled trial

        Background: Shared decision making using patient decision aids (PtDAs) was established over a decade ago, but few studies have evaluated its efficacy in Asian countries. We therefore evaluated the application of PtDAs in a decision conflict between two muscle relaxant reversal agents, neostigmine and sugammadex, and sequentially analyzed the regional differences and operating room turnover rates.Methods: This multicenter, outcome-assessor-blind, randomized controlled trial included 3,132 surgical patients from two medical centers admitted between March 2020 and August 2020. The patients were randomly divided into the classical and PtDA groups for pre-anesthesia consultations. Their clinicodemographic characteristics were analyzed to identify variables influencing the choice of reversal agent. On the day of the pre-anesthesia consultation, the patients completed the four SURE scale (sure of myself, understand information, risk-benefit ratio, encouragement) screening items. The operating turnover rates were also evaluated using anesthesia records. Results: Compared with the classical group, the PtDA group felt more confident about receiving sufficient medical information (P < 0.001), felt better informed about the advantages and disadvantages of the medications (P < 0.001), exhibited a superior understanding of the benefits and risks of their options (P < 0.001), and felt surer about their choice (P < 0.001). Moreover, the PtDA group had a significantly greater tendency to choose sugammadex over neostigmine (P < 0.001). Conclusions: PtDA interventions in pre-anesthesia consultations provided surgical patients with clear knowledge and better support. PtDAs should be made available in other medical fields to enhance shared clinical decision-making.

        As a new nanomaterial, HgWO4 nanofilms are prepared by the self-inventive technique of collodion nanoparticles-dispersal and film-formation. Their photoluminescent properties are enhanced and then controlled through nano-TiO2 doping. These approaches not only ensure film uniformity and fluorescence (FL) enhancement, but also realize a FL increase/decrease conversion effect. The relationships between HgWO4 nanofilms' PL intensities and TiO2 doped amounts are in good agreement with Gaussion function, and maximum PL intensities of HgWO4 nanofilms are located at 3.8% ratio of TiO2 nanoparticles.

        The Influence of Construction Scheme of Asymmetric Three-Cabin Utility Tunnelling on the Surface Settlement Behaviour

        The asymmetric three-cabin structure is one of the favourite choices in urban utility tunnel engineering, as it can provide a higher space utilization and clearer classification for pipelines. However, few work has been done on utility tunnel in such special cross-section during undercutting construction until now. Besides, as the loose soil layers such as sand and silt are often encountered during the utility tunnelling, its influence on surface settlement become one of the main concerns. This study focuses on the influence of the construction scheme on the surface settlement details for an asymmetric three-cabin utility tunnel. A 3D finite element method (FEM) model was established including both the stratum and a utility tunnel based on the Beijing Daxing International Airport (BDIA) Expressway Urban Utility Tunnel project. With this model, the behaviour of the surface settlement with the undercutting construction of the utility tunnel was investigated, and the causes and values of surface settlement in five construction stages were analyzed. By changing the construction spacing and sequence of mid and side cabins, the surface settlement and structural deformation were compared. The numerical results demonstrated a strong correlation between the cross-section shape and surface settlement characteristics. And the symmetry line of the surface settlement trough was offset by 1 m to the side with the larger cross-section. The second (excavation of upper bench in mid-cabin) and fourth (excavation of upper bench in side-cabin) stages caused the largest surface settlement, which were 41.9% and 18.07% of the total settlement, respectively. In addition, after optimizing the field tunnelling scheme, the surface settlement was reduced by 31.9% using the side-tunnel first construction sequence. The proposed numerical model is able to predict the settlement characteristics in each construction stage, which is important and provides a basis for further studies on the surface settlement mechanism and optimal design of the asymmetric three-cabin utility tunnelling scheme.
