Background/Aims: Immunoglobulin A nephropathy (IgAN) is a generally progressive disease, even in patients with favorable prognostic features. In this study, we aimed to investigate the antiproteinuric effect and tolerability of lowdose valsartan (an angiotensin II receptor blocker) therapy in normotensive IgAN patients with minimal proteinuria of less than 0.5 to 1.0 g/day. Methods: Normotensive IgAN patients, who had persistent proteinuria with a spot urine protein-to-creatinine ratio of 0.3 to 1.0 mg/mg creatinine, were recruited from five hospitals and randomly assigned to either 40 mg of valsartan as the low-dose group or 80 mg of valsartan as the regular-dose group. Clinical and laboratory data were collected at baseline, and at 4, 8, 12, and 24 weeks after valsartan therapy. Results: Forty-three patients (low-dose group, n = 23; regular-dose group, n = 20) were enrolled in the study. Proteinuria decreased significantly not only in the regular-dose group but also in the low-dose group. The change in urine protein-to-creatinine ratio at week 24 was -41.3% ± 26.1% (p < 0.001) in the regular- dose group and -21.1% ± 45.1% (p = 0.005) in the low-dose group. In the lowdose group, blood pressure was constant throughout the study period, and there was no symptomatic hypotension. In the regular-dose group, blood pressure decreased at weeks 8 and 12. No significant change in glomerular filtration rate, serum creatinine level, or serum potassium level was observed during the study period. Conclusions: Our results suggest that low-dose valsartan can significantly reduce proteinuria without causing any intolerability in normotensive IgAN patients with minimal proteinuria.

Purpose: This study investigated the factors influencing sleep quality in adults according to age groups. The escriptive study was conducted in metropolitan areas of South Korea to control for possible sleep differences between adults living in rural areas and those living in cities. Participants were 450 adults between 19 and 65 years old. Methods: Self-administered questionnaires consisting of including the Korean Modified Sleep Leeds Evaluation Questionnaire, the International Physical Activity Questionnaire and the Perceived Stress Scale were used to assess the quality of adults’ sleep. Results: Sleep quality was poor across the all age groups Stress and subjective health status were influencing factors of sleep quality among participants in their 20s; subjective health status and circadian rhythm (evening-type) were influencing factors of those in their 30s; subjective health status, alcohol consumption, and work days of less than 8 hours were influencing factors among participants in their 40s; and stress, circadian rhythm (evening-type), subjective health status, work days of over 11 hours, past smoking habit, and light physical activity were influencing factors of those aged 50 years and over. Conclusion: The quality of sleep was low across the all adult age groups in South Korea. Although the factors influencing quality of sleep differed by age groups, they in general indicate the need to focus on maintaining and controlling health status in sleep interventions.

Background Amniotic-fluid-derived stem cells and amniocytes have recently been determined to have wound healing effects, but their mechanism is not yet clearly understood. In this study, the effects of amniotic fluid stem cells and amniocytes on wound healing were investigated through animal experiments. Methods On the back of Sprague-Dawley rats, four circular full-thickness skin wounds 2 cm in diameter were created. The wounds were classified into the following four types: a control group using Tegaderm disc wound dressings and experimental groups using collagen discs, amniotic fluid stem cell discs, and amniocyte discs. The wounds were assessed through macroscopic histological examination and immunohistochemistry over a period of time. Results The amniotic fluid stem cell and amniocyte groups showed higher wound healing rates compared with the control group; histologically, the inflammatory cell invasion disappeared more quickly in these groups, and there was more significant angiogenesis. In particular, these groups had significant promotion of epithelial cell reproduction, collagen fiber formation, and angiogenesis during the initial 10 days of the wound healing process. The potency of transforming growth factor-${\beta}$ and fibronectin in the experimental group was much greater than that in the control group in the early stage of the wound healing process. In later stages, however, no significant difference was observed. Conclusions The amniotic fluid stem cells and amniocytes were confirmed to have accelerated the inflammatory stage to contribute to an enhanced cure rate and shortened wound healing period. Therefore, they hold promise as wound treatment agents.
관절염 대상자의 건강상태 조사 : 일개 도ㆍ농 복합시를 중심으로

The Purpose of this study was to investigate the health status and to examine the relationship between health related variables of the clients with arthritis. This descriptive-correlational study was measured physical health status(pain, fatigue, physical disability, joint flexibility) and psycho-social health status(social function disability, self-efficacy, depression) . The clients were consisted of 127 arthritis clients who were randomly selected from health center, 2 sub health center and 6 primary health care center in Wonju City. The data was collected from November 2000 to January 2001. The results of the study analyzed using a SPSS win were as follows : 1) The subjects' average age was 64.81 years old(female : 86.6%, male : 13.4%). 2) In physical health status, the average score of fatigue was 63 scores and pain was 69.77 Physical disability score was higher than total disability score. In psycho-social disability, housekeeping management disability score was higher than social function disability score. In psycho-social health status, the average score of self-efficacy was 70.86 and the subjects have higher level of depression, more than 20 scores. 3) On Pain sites, almost of subjects complained of a knee joint pain. The waist, wrist, and shoulder joint Pain were followed. The numbers of Pain site were 8.3 Per individuals 4) The flexibility of knee and shoulder joint were lower than another joints. 5) Correlation with fatigue, pain, physical disability, social disability, self-efficacy and depression was revealed highly significant. We suggest that health Promotion program considering our research results should be provided for arthritis clients in community level .
