Purpose: VEGF, a potent angiogenic factor, increases vascular permeability and induces the fluid accumulation in the peritoneal and pleural cavities. This study is designed to know whether the VEGF concentration are associated with the formation of cystic fluid of thyroid nodules and the results of treatment. Methods: To know whether thyroid follicular cells produce VEGF, we measured the VEGF concentration in the conditioned medium after primary culture of the normal thyroid follicular cells (NT 1.0). Thirty seven patients, who had visited Ajou University Medical Center, were divided into two groups; group A (n=19) is patients whose cystic nodules disappeared completely with 1 or 2 times fine needle aspirations. In Group B (n=18), the cystic nodules recurred rapidly and required aspiration more than 3 times. Clinical records of patients were analyzed and compared with TSH and VEGF levels in cystic fluids. Results: The VEGF concentration in basal conditioned medium were 8.2⁑1.2 ng/ml in NT 1.0 and 8.2⁑0.6 ng/ml in thyroid cancer cell line (FTC-133). NT 1.0 was not affected by TSH stimulation. Mean concentration of TSH of all cystic fluids was 4.36μIU/ml (0.13∼21.7) and there was no difference between 2 groups. Mean concentration of VEGF of all cystic fluids was 140.7 ng/ml (11.1∼688.8). The VEGF concentration (287.9⁑289.6 ng/ml) in group B was significantly higher than that (104.2⁑97.1 ng/ml) in group A (P<0.05). Conclusion: This study suggests that thyroid follicular cells produce and secrete VEGF, and VEGF is related with the accumulation of cystic fluid in degenerative cysts of thyroid. Cysts with high VEGF concentration promote rapid reaccumulation of the cystic fluid and possibly necessitate operation in selected patients. (Korean J Endocrine Surg 2002;2:25- 30)

The aim of this study was to isolate microorganisms from half milk samples of dairy goats by California mastitis test (CMT) during the lactation period and to further investigate the susceptibility of isolated organisms to antimicrobial drugs. From a total of 235 half milk samples with CMT scores of 2 or above from 366 dairy goats distributed throughout Jeonnam province, microorganisms were isolated from 198 (83.5%) samples either singly (99.0%) or in combination (1.0%). The most prevalent microorganism was the coagulase-negative Staphylococcus spp., (44.4%, n=88) followed by Staphylococcus aureus (24.2%, n=48), Escherichia coli (11.1%, n=22) and Streptococcus spp. (7.6%, n=15). Isolated bacteria also included Bacillus spp. (2.5%, n=5), Pseudomonas spp. (2.5%, n=5), Micrococcus spp. (1.5%, n=3), Corynebacterium spp. (1.5%, n=3), Enterococcus facium (1.0%, n=2), Morganella morganii (0.5%, n=1) and Streptococcus agalactiae (0.5%, n=1). During the summer season, a high prevalence of all micro-organisms were observed in which Staphylococcus spp. (30.8%), Escherichia coli (8.6%), and Streptococcus spp. (5.6%) were among the most prevalent bacteria isolated. Staphylococcus spp. was also shown to be high in the winter (21.7%). In most samples, the presence of bacterial pathogens in goat milk led to the increase in the total somatic cell count (SCC). Most of the half milk samples of dairy goats with bacterial contamination showed SCC of ≥1×10(6) cells/ml (90.4%). Minor pathogens (11.4%) were more detected from milk samples with SCC of <1×10(6) cells/ml than major pathogens (4.1%), while the major pathogens tended to be higher from samples with SCC of ≥3×10(6) cells/ml. Susceptibility of these bacteria to 12 antimicrobial agents was tested by the Kirby-Bauer disc diffusion method. Results indicated that more than 90% of bacteria isolated from CMT 2+ dairy goat half milk samples were susceptible to trimethoprim/sulfamethoxazole, amoxicillin/clavulanic, enrofloxacin and cephalothin while they were resistant to tetracycline (44.7%).
추황배(Pyrus pyrifolia Nakai cv. Chuhwangbae) 과피로부터 1종의 Sterol과 3종의 배당체 화합물의 단리·동정

본 연구에서는 배의 유용성 증명을 위한 일환으로 배의 화학성분을 분자수준에서 밝히고자 하였다. 이에 배 과피 MeOH 추출물을 용매분획하여 얻은 EtOAc-산성 분획과 EtOAc-중성분획을 대상으로 Sephadex LH-20, silica gel, 그리고 ODS colmn chromatography와 HPLC를 이용하여 정제 및 단리하였다. 그 결과, EtOAc-산성 분획과 EtOAc-중성 분획으로부터 각각 2종씩의 화합물을 단리하였다. 단리된 화합물 1-4는 1H- 및 13C-NMR 분석을 통하여 각각 (S)-(+)-2-cis-abscisic acid O-β-D-glucopyranosyl ester (화합물 1), 1-[4-O-β-D-glucopyranosyl]phenyl ethanone(piceoside, 화합물 2), β-sitosterol (화합물 3), 그리고 β-sitosteryl 3-O-β-D-glucopyranoside (화합물 4)로 동정되었다. 단리된 3종의 배당체 화합물(화합물 1, 2, 4)들은 본 연구에 의해 배로부터 처음 동정되었으며, 화합물 3은 추황배로부터 처음 동정되었다. 본 연구결과가 배 함유 성분연구는 물론 배의 기능성 해명 연구에도 추후 중요한 기초자료로 활용되길 기대한다. We isolated and identified antioxidants from acidic and neutral ethyl acetate fractions of the peel of pear (Pyrus pyrifolia N. cv. Chuhwangbae). We isolated 4 compounds from the methanol extract, by using 3 different types of column chromatography (Sephadex LH-20, silica gel, and octadecylsilane) and preparative HPLC. We identified the isolated compounds as (S)-(+)-2-cis-abscisic acid O-β-D-glucopyranosyl ester (compound 1), 1-[4-O-β-D-glucopyranosyl]phenyl ethanone (picroside, compound 2), β-sitosterol (compound 3), and β-sitosteryl 3-O-β-D-glucopyranoside (compound 4) by nuclear magnetic resonance analysis. We are the first to report the identification of compounds 1, 2, and 4 from pear.

Two diastereomeric chiral stationary phases (CSPs) were applied to the liquid chromatographic resolution of various racemic α-amino methyl esters, α-amino N,N-diethylamides and α-amino N-propylamides. The CSP incorporating (R)-3,3’-diphenyl-1,1’-binaphtyl and (R,R)-tartaric acid unit as chiral barriers did not show any chiral recognition. In contrast,the CSP incorporating (R)-3,3’-diphenyl-1,1’-binaphtyl and (S,S)-tartaric acid unit as chiral barriers was found to show excellent chiral recognition especially for the two enantiomers of α-amino N-propylamides. Some of α-amino methyl esters and α-amino N,N-diethylamides were also resolved on the CSP incorporating (R)-3,3’-diphenyl-1,1’-binaphtyl and (S,S)-tartaric acid unit. From these results it was concluded that the two chiral units composing the diastereomeric CSPs can show “matched” or “mismatched” effect on the chiral recognition according to their absolute stereochemistry.
갑상선암의 내시경 갑상선 절제술에서 근전절제술 및 전절제술의 임상적 비교

Purpose: The aim of this study was to evaluate and compare the surgical outcomes of endoscopic total and near-total thyroidectomies in patients with thyroid cancer. Methods: Between February 2000 and January 2009, among 387 patients who underwent endoscopic thyroidectomy in our hospital, we evaluated 50 patients who underwent endoscopic total or near-total thyroidectomy. Thirty- five and 15 patients underwent endoscopic total thyroidectomy and near-total thyroidectomy, respectively. We analyzed the patients' clinicopathologic characteristics and post-operative complications between the two groups. Results: The mean size of tumors was 1.08 cm (range, 0.1∼3.5 cm) and the mean operative time was 192 minutes. Forty-nine tumors were papillary cancers and 1 tumor was a follicular cancer. Two patients in the endoscopic total thyroidectomy group underwent re-operation because of bleeding. In the near-total thyroidectomy group, one patient had transient hypocalcemia and one patient had temporary recurrent laryngeal nerve palsy. In the total thyroidectomy group, nine patients had transient hypocalcemia and one patient had temporary recurrent laryngeal nerve palsy. There was no statistical difference in the post-operative complicationrates between the two groups (P=0.254 for hypocalcemia and P=0.470 for injury to the recurrent laryngeal nerve). Conclusion: Based on our experience, endoscopic total thyroidectomy is a safe and feasible alternative to endoscopic near-total thyroidectomy. (Korean J Endocrine Surg 2009; 9:201-205)

In patients with advanced non-small cell lung cancer harboring epidermal growth factor receptor (EGFR) mutations, the epidermal growth factor receptor-tyrosine kinase inhibitors (EGFR-TKIs) are used as first treatment option. Because adenosquamous carcinoma (ASC) is a rare histologic subtype, evidences about EGFR-TKIs as first treatment option for advanced ASC are lacking. We report a case of an advanced ASC patient with the EGFR mutation, who showed good responses during 4-month treatment with gefitinib. And we will review about a necessity of EGFR mutation test and efficacy of EGFR-TKIs in ASC patients from the recent studies.
