        Synthesis and thermodynamic analysis of Mg-doped calcium phosphate-based porous scaffolds

        A strategy using the sucrose evaporation technique for producing highly macroporous ceramic scaffolds with magnesium (Mg)-doped multiphasic or biphasic calcium phosphates was proposed. X-ray diffraction was performed to identify the ceramic phases, and thermogravimetric analysis and simultaneous differential scanning calorimetry (TGA/DSC) were used to analyze thermal behavior under different sintering temperatures. Product compositions with Ca/P molar ratios between 1.5 and 1.67 were synthesized and thermally treated up to 1050-1400 o C. Results showed that controlling the sintering temperature enabled various adjustable biphasic and multiphasic calcium phosphates to be prepared. Hydroxyapatite (HA) was obtained by increasing the sintering temperature of the reactants with sucrose and multiphasic á-tricalcium phosphates (TCP) and β- TCP were stabilized by the addition of MgO. The spaces occupied by the sucrose (~ 200 μm) produced non-uniform pores after sucrose vaporized. These pores had dimensions ranging from several micrometers to hundreds of micrometers in the final fabricated scaffolds with an interconnected network. Considering phase decomposition and transfer into TCP, the phase amount and grain size of HA decreased with increased sintering temperature. Calculation of the Arrhenius equation revealed that the grain size with Mg doping was relatively larger and had reduced activation energy and smaller exothermic enthalpy, indicating that the formation of β-TCP was stabilized by the MgO additive.

      • Particle Size and Loading Effects on Particle-Laden Flow with Mono-sized Droplets

        Turbulence modulation is an interaction between phases that occurs in multiphase flow, and turbulence intensity traditionally can be represented as an indicator of turbulence modulation. Many researchers paid attention on the subject, but no coincident conclusion can be proposed. A general cognition is that larger particles induced wake to enhance the fluid turbulence intensity and smaller particles suppressed the fluid turbulence intensity since the fluid eddy imparted energy to the smaller particle. Here, the authors orderly changed particle size of dispersed phase and mass loading ratio (LR) to study the change on fluid turbulence intensity. Mono-dispersed droplets with a narrow size distribution were used to load into a homogeneous turbulence to investigate the turbulence modulation. Particle Dynamic Analyzer (PDA) was used to measure the particle (droplets and tracers) dynamic information. The particle concentration effect can be significantly observed. After coupling particle concentration and particle wetting area (πd2), a linear relationship can be obtained and it represents the turbulence modulation dominated the dilute two-phase flow as our expectation.

        An 8-Week Low-Intensity Progressive Cycling Training Improves Motor Functions in Patients with Early-Stage Parkinson’s Disease

        Background and Purpose The effects of high-intensity cycling as an adjuvant therapy forearly-stage Parkinson’s disease (PD) were highlighted recently. However, patients experience difficultiesin maintaining these cycling training programs. The present study investigated the efficacyof cycling at a mild-to-moderate intensity in early-stage PD. Methods Thirteen PD patients were enrolled for 16 serial cycling sessions over a 2-month period. Motor function was assessed using the Unified Parkinson’s Disease Rating Scale part III(UPDRS III) and Timed Up and Go (TUG) test as primary outcomes. The Montreal CognitiveAssessment (MoCA), modified Hoehn and Yahr Stage (mHYS), total UPDRS, Falls EfficacyScale, New Freezing of Gait Questionnaire, Schwab and England Activities of Daily Living, 39-item Parkinson’s Disease Questionnaire, Patient Global Impression of Change, and gait performancewere assessed as secondary outcomes. Results The age and the age at onset were 59.67±7.24 and 53.23±10.26 years (mean±SD), respectively. The cycling cadence was 53.27±8.92 revolutions per minute. The UPDRS III scoreimproved significantly after 8 training sessions (p=0.011) and 16 training sessions (T2) (p=0.001) in the off-state, and at T2 (p=0.004) in the on-state compared to pretraining (T0). TheTUG duration was significantly shorter at T2 than at T0 (p<0.05). The findings of MoCA, totalUPDRS, double limb support time, and mHYS (in both the off- and on-states) also improvedsignificantly at T2. Conclusions Our pioneer study has demonstrated that a low-intensity progressive cycling exercisecan improve motor function in PD, especially akinesia. The beneficial effects were similarto those of high-intensity rehabilitation programs.

        BRCA1/2 mutation status in patients with metachronous breast and ovarian malignancies: clues towards the implementation of genetic counseling

        Objective: The characteristics of patients with metachronous breast and ovarian malignancies and the pathogenic role of BRCA1/2 mutations remain poorly understood. We investigated these issues through a review of hospital records and nationwide Taiwanese registry data, followed by BRCA1/2 mutation analysis in hospital-based cases. Methods: We retrospectively retrieved consecutive clinical records of Taiwanese patients who presented with these malignancies to our hospital between 2001 and 2017. We also collected information from the Data Science Center of the Taiwan Cancer Registry (TCR) between 2007 and 2015. Next-generation sequencing and multiplex ligation-dependent probe amplification were used to identify BRCA1/2 mutations and large genomic rearrangements, respectively. When BRCA1/2 mutations were identified in index cases, pedigrees were reconstructed and genetic testing was offered to family members. Results: A total of 12,769 patients with breast cancer and 1,537 with ovarian cancer were retrieved from our hospital records. Of them, 28 had metachronous breast and ovarian malignancies. We also identified 113 cases from the TCR dataset. Eighteen hospital-based cases underwent BRCA1/2 sequencing and germline pathogenic mutations were detected in 7 patients (38.9%, 5 in BRCA1 and 2 in BRCA2). All BRCA1/2 mutation carriers had ovarian high-grade serous carcinomas. Of the 12 patients who were alive at the time of analysis, 5 were BRCA1/2 mutation carriers. All of them had family members with BRCA1/2-associated malignancies. Conclusions: Our results provide pilot evidence that BRCA1/2 mutations are common in Taiwanese patients with metachronous breast and ovarian malignancies, supporting the clinical utility of genetic counseling.

        Effect of Active Synthetic 2-Substituted Quinazolinones on Anti-Platelet Aggregation and the Inhibition of Superoxide Anion Generation by Neutrophils

        Quinazolinones, 2-substituted and 3-substituted, mainly synthesized by microwave irradiation, were subjected to anti-platelet aggregation and inhibition of superoxide anion generation assays. Interestingly, 2-phenyl-4-quinazolinone (4) exhibited significant inhibitory activities toward platelet aggregation and neutrophil activation, and it might therefore serve as a prototype lead compound.

        Which Severe Mental Illnesses Most Increase the Risk of Developing Dementia? Comparing the Risk of Dementia in Patients with Schizophrenia, Major Depressive Disorder and Bipolar Disorder

        Objective: Previous studies have shown that certain severe mental illnesses (SMIs) increase the risk of dementia, but those that increase the risk to a greater degree in comparison with other SMIs are unknown. Furthermore, physical illnesses may alter the risk of developing dementia, but these cannot be well-controlled. Methods: Using the Taiwan National Health Insurance Research Database, patients with schizophrenia, bipolar disorder and major depressive disorder (MDD) were recruited. We also recruited normal healthy subjects as the control group. All subjects were aged over 60 years, and the duration of follow-up was from 2008 to 2015. Multiple confounders were adjusted, including physical illnesses and other variables. Use of medications, especially benzodiazepines, was analyzed in a sensitivity analysis. Results: 36,029 subjects (MDD: 23,371, bipolar disorder: 4,883, schizophrenia: 7,775) and 108,084 control subjects were recruited after matching according to age and sex. The results showed that bipolar disorder had the highest hazard ratio (HR) (HR: 2.14, 95% confidence interval [CI]: 1.99−2.30), followed by schizophrenia (HR: 2.06, 95% CI: 1.93−2.19) and MDD (HR: 1.60, 95% CI: 1.51−1.69). The results remained robust after adjusting for covariates, and sensitivity analysis showed similar results. Anxiolytics use did not increase the risk of dementia in any of the three groups of SMI patients. Conclusion: SMIs increase the risk of dementia, and among them, bipolar disorder confers the greatest risk of developing dementia. Anxiolytics may not increase the risk of developing dementia in patients with an SMI, but still need to be used with caution in clinical practices.

      • Clinical Significance of Smudge Cells in Peripheral Blood Smears in Hematological Malignancies and Other Diseases

        Background: It is reported that the percentage of smudge cells in the blood smear could be a prognostic indicator in chronic lymphocytic leukemia. However, the clinical significance of smudge cells in other hematological malignancies, solid tumors or non-malignant diseases is less clear. Hence, this study was conducted to survey the clinical significance of smudge cells in hematological cancers and other disorders. Materials and Methods: From January to November, 2015, the clinical data of patients who received blood examination with differential counts for clinical purpose and were found to have smudge cells in the peripheral blood film in Far Eastern Memorial Hospital were selected. The percentage of smudge cells and patient outcomes were evaluated for further univariate and survival analyses. Results: A total of 102 patients with smudge cells in their blood smears were included. Smudge cells were frequently presented in out-of-hospital cardiac arrest (OHCA; n=30), infections (n=23), hematological cancers (n=23) and solid cancers (n=10). There was no relationship between the percentage of smudge cells and the patient mortality in all diseases (OR: 1.08, 95% CI: 0.47-2.48, P=1.000) as well as the OHCA group (OR: 1.91, 95% CI: 0.38-9.60, P=0.694). It was observed that in patients with all cancers with the percentage of smudge cells less than 50% had a lower mortality rate in comparison with those who had the percentage of smudge cells of 50% or more (OR: 22.29, 95% CI: 2.38-208.80, P<0.001). Additionally, it was seemingly that patients with smudge cells of 50% or more had a lower survival rate than those with smudge cells less than 50% in all cancers with follow-up at 2-month intervals, but without statistical significance (P=0.064). Conclusions: Our survey indicated that in all cancers, those who had higher percentage of smudge cells were prone to have poor outcomes when compared with the subjects with lower percentage of smudge cells. This finding was quite different from the results of previous studies in which the race-ethnicity of most study populations was non-Asian; hence, further investigations are required. Besides, there was no apparent association of the percentage of smudge cells with patient outcomes in all diseases, including OHCA.

      • Constructing a Conversation Key in Three-Party Communications Environment

        This article proposes an efficient, less communication rounds, three-party encrypted key exchange protocol to achieve the authentication requirement. The protocol is provided with (1) no asymmetric encryption algorithm which is adopted to reduce the costs (such as any public-key infrastructure); (2) using pre-shared key to prevent adversaries that masquerade as legal users after guessing attacks; (3) avoiding the variant man-in-the-middle attacks on Diffie-Hellman based protocols; (4) achieving mutual authentication. With these four features, the proposed protocol is suitable for being applied for establishing secure channels between two clients, which are supported with the same trusted server.
