        Antiproliferative and Antitumorigenic Activity of Toona sinensis Leaf Extracts in Lung Adenocarcinoma

        Toona sinensis is a traditional Chinese herb, and the extracts of T. sinensis leaf possess a variety of biological functions. This study attempted to test the antiproliferative effect of TSL-1 (a bioactive fraction of T. sinensis) in H441 cells (lung adenocarcinoma). The data showed that the antiproliferative effect of TSL-1 on H441 cells is prominent using the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assay. TSL-1-induced apoptosis was confirmed by cell morphology, sub-G1 peak accumulation, cleavage of poly(ADP)-ribose polymerase, and propidium iodide-annexin V double staining. Furthermore, decreased Bcl-2 accompanied by increased Bax (in western blotting) was found with TSL-1 treatment of H441 cells. TSL-1 treatment-induced G1 arrest was concurrent with the down-regulation of protein levels of cyclin D1 and cyclin-dependent kinase 4 in H441 cells. Peroral and intraperitoneal administrations of TSL-1 were performed to evaluate the therapeutic efficacy, and peroral administration of TSL-1 was also used to elucidate the therapeutic efficacy in the H441 cell xenograft model in vivo. The data revealed that TSL-1 treatment inhibited H441 tumor growth in both therapeutic and preventive experiments. Taken together, these results demonstrate that TSL-1 possesses the capability of preventing and alleviating lung cancer proliferation in vitro and in vivo with proven nephrological and hepatic safety and has the potential to be developed as an anti–lung cancer drug.

        Design Customization of Respiratory Mask based on 3D Face Anthropometric Data

        This study presents a machine learning based method for design customization of a 3D respiratory mask. A parametric model of a3D human face was constructed from an anthropometric database consisting of 495 facial models. An AdaBoost.R algorithm wasapplied to identify a set of measurable parameters most related to the facial geometry. The correlation between parameters wasestimated using principal component analysis and linear regression. With those parameter values as input, the parametric modelgenerates 3D meshes of a human face that serve as a design reference for the construction of a customized respiratory mask of agood fit. We conducted a series of experiments with 10-fold cross-validation to validate the effectiveness of the proposed method.

      • Successful Resection for Huge Combined Hepatocellular- Cholangiocarcinoma after Portal Vein Embolization - A Case Report

        Aims: Combined hepatocellular-cholangiocarcinoma (cHCC-CC) is a rare primary hepatic cancer with poor prognosis. Aggressive surgical planning with complete resection of cHCC-CC plays an important role in treatment. We presented one case of successful resection of huge cHCC-CC after portal vein embolization (PVE). Methods: This is a 46-year-old man with alcoholic liver cirrhosis. CT scan showed one 10cm infiltrative tumor in right liver with right anterior portal venous tumor thrombosis enhanced in arterial phase and washed out in portal venous phase. Regional enlarged lymph nodes is also noted. Under the preoperative diagnosis of HCC, right hepatectomy is planned. CT volumetry showed future liver remnant(FLR) is 480ml, 38.5% of standard liver volume. The ideal percentage of FLR in cirrhotic liver is more than 40% in our institution. Because of tumor thrombosis with total occlusion in right anterior portal pedicle, we performed PVE to right posterior portal vein for inadequate FLR. Two weeks after PVE, left liver enlarged from 480ml(38.5%) to 620ml(49.7%). There was no complication during PVE. Results: Right hepatectomy and regional lymph node dissection were performed two weeks after PVE. The post-operative course was smooth without any evidence of hepatic insufficiency. Pathology reported combined hepatocellular-cholangiocarcinoma but no malignant lymph nodes. There is no evidence of recurrence in follow-up CT scan 17 months after the operation until now. Conclusions: Aggressive surgical planning with PVE is effective for patient with cHCC-CC without adequate FLR even in cirrhotic liver. Complete resection may provide longer overall survival in this disease with dismal prognosis.

        Equal Z standard-setting method to estimate the minimum number of panelists for a medical school’s objective structured clinical examination in Taiwan: a simulation study

        Purpose Undertaking a standard-setting exercise is a common method for setting pass/fail cut scores for high-stakes examinations. The recently introduced equal Z standard-setting method (EZ method) has been found to be a valid and effective alternative for the commonly used Angoff and Hofstee methods and their variants. The current study aims to estimate the minimum number of panelists required for obtaining acceptable and reliable cut scores using the EZ method. Methods The primary data were extracted from 31 panelists who used the EZ method for setting cut scores for a 12-station of medical school’s final objective structured clinical examination (OSCE) in Taiwan. For this study, a new data set composed of 1,000 random samples of different panel sizes, ranging from 5 to 25 panelists, was established and analyzed. Analysis of variance was performed to measure the differences in the cut scores set by the sampled groups, across all sizes within each station. Results On average, a panel of 10 experts or more yielded cut scores with confidence more than or equal to 90% and 15 experts yielded cut scores with confidence more than or equal to 95%. No significant differences in cut scores associated with panel size were identified for panels of 5 or more experts. Conclusion The EZ method was found to be valid and feasible. Less than an hour was required for 12 panelists to assess 12 OSCE stations. Calculating the cut scores required only basic statistical skills.

        Production performances and antioxidant activities of laying hens fed Aspergillus oryzae and phytase co-fermented wheat bran

        Objective: Wheat bran (WB) was co-fermented with Aspergillus oryzae and phytase (Phy) to determine whether co-fermentation improve WB phosphorus and fiber utilization in Isa-brown layers. Methods: A total of 112 Isa brown layer were randomly divided into 7 treatments with 8 replicates per a treatment and 2 hens per a replicate. The treatments included basal diet (control), basal diet supplemented with 250 unit/kg Phy (control+Phy), diet with 10% WB (10% WB), diet with 5% WB and 250 unit/kg Phy (5% WB+Phy) diet with 10% WB and 250 unit/kg Phy (10% WB+Phy), diet with 5% fermented WB supplemented with molasses and phy (PCFWH) and 125 unit/kg Phy (5% PCFWH), and diet with 10% PCFWH (10% PCFWH). The intestinal microbial population, intestinal morphology, serum antioxidant enzyme activities, and excreta phosphorus content were assessed. Results: In PCFWH, spore counts, protease activity, xylanase activity, and ferulic acid were 8.50 log/g dry matter (DM), 190 unit/g DM, 120 unit/g DM, and 127 μg/g, respectively. Xylobiose and xylotriose were released in PCFWH, while they were not detectable in WB. Antioxidant capacity was also enhanced in PCFWH compared to WB. The 10% WB+Phy and 10% PCFWH groups produced higher egg mass, but hens fed 5% WB+Phy had the lowest amount of feed intake. Eggs from 10% PCFWH had better eggshell weight, eggshell strength, and eggshell thickness. Birds fed with 10% PCFWH also had higher serum superoxide dismutase and catalase activities. Compare to control, 10% PCFWH significantly reduced excreta phosphorus content. Conclusion: Diet inclusion of 10% PCFWH improved egg quality, antioxidant status, and excreta phosphorus content of laying hens.         A Genetic Algorithm for Directed Graph-based Supply Network Planning in Memory Module Industry

        A memory module industry"s supply chain usually consists of multiple manufacturing sites and multiple distribution centers. In order to fulfill the variety of demands from downstream customers, production planners need not only to decide the order allocation among multiple manufacturing sites but also to consider memory module industrial characteristics and supply chain constraints, such as multiple material substitution relationships, capacity, and transportation lead time, fluctuation of component purchasing prices and available supply quantities of critical materials (e.g., DRAM, chip), based on human experience. In this research, a directed graph-based supply network planning (DGSNP) model is developed for memory module industry. In addition to multi-site order allocation, the DGSNP model explicitly considers production planning for each manufacturing site, and purchasing planning from each supplier. First, the research formulates the supply network"s structure and constraints in a directed-graph form. Then, a proposed genetic algorithm (GA) solves the matrix form which is transformed from the directed-graph model. Finally, the final matrix, with a calculated maximum profit, can be transformed back to a directed-graph based supply network plan as a reference for planners. The results of the illustrative experiments show that the DGSNP model, compared to current memory module industry practices, determines a convincing supply network planning solution, as measured by total profit.

        Bostrycin production by agro-industrial residues and its potential for food processing

        Bostrycin, a red antibacterial agent produced by Nigrospora sp. no. 407, is considered for meat processing. To optimize production, the culture conditions of submerged fermentation (SmF) and solid-state fermentation (SSF) were investigated. The optimal SmF conditions were a medium containing 1.0% cane molasses and incubation at 30 C and 150 rpm for 6 days. In SSF, other than bostrycin, less pigment was produced and the optimal ratio of bagasse to water was 1:2 for 10 days. The production and recovery rate of bostrycin by SmF were 120 mg/L and 40%, respectively. Bostrycin exhibited thermostable, pHdependent color change and dose-dependent antibacterial activity against Clostridium botulinum. Bostrycin-modified meat turned strong red for at least 24 h and could not be removed by washing; bostrycin maintained its antibacterial activity with a bacteriostasis rate of 91% on Staphylcoccus aureus. This is an easy and inexpensive means of acquiring bostrycin from molasses and sugarcane.

        Concomitant Acromioclavicular and Coracoclavicular Ligament Reconstruction with a Duo-Figure-8 Autogenic Graft Wrapping Technique for Treating Chronic Acromioclavicular Separation

        Background: Coracoacromial ligament transfer is the traditional procedure for treating chronic acromioclavicular separation, but it is significantly inferior to ligament reconstruction according to biomechanical and clinical studies. However, ligament reconstruction carries the risk of complications of graft loosening and peri-tunnel fractures. Currently, there is no ligament reconstruction procedure optimal for preventing such complications. The purpose of this study was to describe and retrospectively analyze the clinical and radiological outcomes of a “duo-figure-8” autogenic graft wrapping technique, which was used to concomitantly reconstruct the acromioclavicular and coracoclavicular ligaments. Methods: Preoperative, immediate postoperative, and final follow-up oputcomes were evaluated in 10 enrolled patients. Radiographic outcomes were indicated by the bilateral difference of the coracoclavicular distance (CCD) and overlapping length of the acromioclavicular joint (OLac). Quality of reduction was classified into 4 grades according to bilateral CCD difference into overreduction (< 0 mm), anatomic reduction (0–4 mm), partial loss of reduction (4–8 mm), and recurrent dislocation (> 8 mm). Clinical outcomes were evaluated using the American Shoulder and Elbow Surgeons (ASES) and Constant scores. Results: The mean side-to-side differences for CCD were 11.9 mm (preoperative), −0.1 mm (immediate postoperative), and 3.4 mm (final follow-up); those for OLac were 9.4 mm (preoperative) and 2.7 mm (final follow-up). CCD and OLac outcomes significantly improved at final follow-up (p < 0.05). At the immediate postoperative stage, 6 and 4 patients had overreduction and anatomic reduction, respectively. At final follow-up, 7 and 3 patients had anatomic reduction and partial loss of reduction, respectively. The magnitude of improvement of ASES scores for patients with anatomic reduction and partial loss of reduction (p = 0.20) was 18.1 and 20.0, respectively. The magnitude of improvement of Constant scores in patients with anatomic reduction and partial loss of reduction (p = 0.25) was 19.9 and 22.3, respectively. Conclusions: The technique yielded acceptable functional outcomes in patients with anatomic reduction or partial loss of reduction. The “duo-figure-8” wrapping method—a single autogenic tendon graft passing beneath the coracoid process with a tendonknot fixation over the distal clavicle and looping around the acromion intramedullary—did not increase the risk of peri-tunnel fractures over the clavicle, coracoid process, or acromion.
