        중국 의료위생정책 변동과정 분석

        본 연구는 역사적 제도주의의 경로진화 모형을 통해 중국 의료위생정책의 진화경로를 확인해 보는 것에 목적이 있다. 분석 결과 의료위생 정책 변동 패턴은 가겹, 가겹, 표류, 전환이 뒤섞인 역동적 과정을 통해 진화해왔다. 의료서비스 공급부족에 따른 문제를 해결하기 위해 각종 조치들이 추가되는 과정과 시장화에 따른 규제개혁의 조치는 가겹으로 해석된다. 이후 합작의료가 와해되고 자비 의료가 증가되는 표류, 공공성 강화를 위한 각종 조치들이 시행되는 전환의 과정을 거친 것이다. 행위자들의 관점에서 정책변동은 단순한 환경 변화만을 뜻하지 않는다. 이들은 전략적 행동변화를 통해 자신들의 이익을 최대화 하고, 이 과정에서 예기치 못한 정책 표류의 현상이 나타날 수 있음을 확인했다. The object of this study is to examine the progress of evolution in Chinese medical and sanitation service policies through the path evolution model of historical institutionalism. The result of the analysis shows that the variation pattern of medical and sanitation service policy has been evolved with dynamic progress including layering, drift and conversion. Both the process of various measures are added in order to solve the problem derived from deficient medical supply and regulatory reform according to marketization could be interpreted as layering. Policy has been gone through the drift which refers to the increase of medical treatment that patients pays by themselves after cooperative medical treatment and conversion that various kinds of measures were executed to enforce the public nature. From the actors' point of view, fluctuation in policy does not mean only the environmental change. They verified that through the strategic change in behavior, they could maximize their benefits, and also drift of policy could occur in this process.

        Evaluation of the susceptibility of coal to spontaneous combustion by a TG profile subtraction method

        −It is imperative to have an in-depth understanding of the intrinsic reaction between coal and oxygen during low-temperature oxidation, as the reaction is the main source responsible for the self-heating and spontaneous combustion of coal. As low-temperature oxidation of coal involves a series of physical and chemical process and many parallel reactions, it is difficult to directly investigate the intrinsic reaction between coal and oxygen by conventional analytical method. Thermogravimetric analysis (TGA) was used to investigate the intrinsic reaction between coal and oxygen based on the mass change. By means of the subtraction analysis method of TGA, the TG-subtraction curves were obtained by subtracting the TG-N2 curves from the TG-air curves. The results indicate that a TG-subtraction curve can better reflect the intrinsic reaction of coal oxidation than a TG-air curve by eliminating the influence of evaporation of water and thermal decomposition of inherent oxygen-containing groups. In terms of the rate of mass increase, the intrinsic reactions can be divided into three stages: slow oxidation stage, advanced oxidation stage and rapid oxidation stage. The activation energy at each of the stages, obtained by Coats and Redfern’s model, can be used to as a technical parameter to evaluate the proneness of coal spontaneous combustion. The optimum experiment conditions were also developed to study low-temperature coal oxidation with the subtraction method of TGA.

      • 노년 청각손상자와 청각장애의 삶의 질 관련성 연구

        The hearing impairment happened extensively in the elderly and often resulted in the hearing handicap. A lot of study confused the hearing impairment with the hearing handicap and replaced the score of quality of life by the hearing handicap score to calculate the quality adjusted life years and analyzed the cost-utility of aural rehabilitation. In this study, the confusion of hearing impairment and hearing handicap was clarified, and the relevance between hearing handicap and quality of life was explored in order to examine the fitness of replacing the score of quality of life by the hearing handicap score. The result of this study indicated that the correlation between the hearing handicap and the quality of life. The fitness of correlating model of the hearing handicap and the quality of life was not accepted. So, it was not suggested that replacing the score of quality of life by the hearing handicap score. The inventory of disease-specific quality of life should be developed. Secondly, the poor fitness of correlating model of the hearing handicap and the quality of life could be resulted from the other variables that shall be included in the model. It was proposed to include relevant variables into the model in the future. 청각손상은 노년기에 광범위하게 발생하는 문제이다. 심각한 청각장애를 자주 일으킨다. 많은 연구는 청각손상과 청각장애를 혼돈하는 것 뿐 만아니라 자주 청각장애 검목표의 득점으로 피실험자의 생활 품질을 대표한다. 심지어 청각장애 검목표의 득점으로 생활 품질 조정년을 계산한다. 이것으로 청각 재활의 성공 효과를 분석한다. 본 연구는 청각 손상과 청각장애의 다른 점을 밝히는 것 외에도 주요 목적은 청각장애와 생활 품질의 관련성을 이해하고자 하는 것이다. 이것으로 청각장애 검목표를 생활 품질을 삼는 정당성을 관찰한다. 연구결과에 나타나는 것은 청각 표현과 생활품질의 관련성은 높지 않은 결과를 얻었다. 이 두 가지의 관련 양식은 일반화 하기는 제한점을 갖는다. 그래서 이론적 개념에서 청각 표현과 생활품질은 각자 다른 내용을 갖추고 있다. 자료 통계의 분석 결과도 이 두 가지의 관련 양식에 대해서 유의한 결과를 얻지 못했다. 그래서 청각표현 능력으로 피실험자의 생활 품질을 대신할 수 없다. 마땅히 청각 손상자에게 맞는 적합한 생활 품질 설문지를 발행해야한다. 그 다음에 두 가지의 관련 양식의 관련도 적다. 아마도 청각 손상자의 청각 표현과 생활 품질 간에는 많은 변인의 영향이 존재하기 때문이다. 미래의 연구는 더 많은 상관 관련 인소를 귀납해서 이 이론구조를 검증해야한다는 것을 건의한다.

        한국인 및 중국 한족 정신분열병 환자의 5-HT2A 수용체 유전자 -1438A/G 다형성

        Objectives : The purpose of the present study was to investigate the association between -1438A/G polymorphism of 5-HT2A receptor gene and schizophrenia in Korean and Han Chinese population. Methods : A sample of 184 Korean patients with schizophrenia and 96 Korean healthy normal controls and 96 Han Chinese patients with schizophrenia and 96 Han-Chinese healthy normal controls were genotyped for a single nucleotide polymorphism with in 5-HT2A receptor gene (promoter region, A-1438G) by Msp I Resthction Fragment Length Polymorphism (RFLP). Results : There was no difference in allelic frequencies and genotype frequencies of -1438A/G polymorphism between Korean schizophrenics and controls (p=0.13) and Han Chinese schizophrenics and controls (p=0.40). Also, -1438A/G Poly-morphism did not show ethnical difference between Korean and Han Chinese controls. The Scale for the Assessment of Negative Symptoms (SANS) scores showed no significant differences between genotypes of -1438A/G polymorphism in both of Korean and Han Chinese schizophrenics. Conclusion : These results suggest that -1438A/G polymorphism of the 5-HT2A receptor gene is not causally related to the development of schizophrenia in Korean and Han Chinese population, and there no ethnic difference between Korean and Han Chinese population.

        Functional Connectivity Change Associated With Apolipoprotein E Allotypes Precedes Structural Connectivity and Neurodegeneration in Cognitive Normal Older Adults Without Cerebral Aβ Deposition

        Objective Apolipoprotein E (APOE) gene is known to influence cerebral functional connectivity (FC) in Alzheimer’s disease continuum. We investigated association between APOE allotypes and FC, structural connectivity, and cortical thickness in amyloid-PET negative cognitive normal older adults (CN).Methods A total of 188 CN (37 had ε2/ε2 or ε2/ε3 [ε2 group], 113 had ε3/ε3 [ε3 group], and 38 had ε3/ε4 or ε4/ε4 [ε4 group]) were recruited. Voxel-based morphometry and cortical thickness analysis were used to investigate differences in cortical thickness between three APOE allotypes. To investigate integrity of structural connectivity, we analyzed diffusion weighted imaging using fractional anisotropy and mean diffusivity. In terms of FC, differences of FC in default mode network (DMN) among APOE allotypes were measured using functional magnetic resonance imaging.Results There were no significant differences in age, sex, education, cerebral beta-amyloid (Aβ) deposition severity, or neuropsychological profiles. No significant differences were found in cortical thickness and structural connectivity among the APOE allotypes. However, FC within the DMN was significantly lower in ε4 and ε2 carriers compared to ε3 homozygotes.Conclusion This study suggests that both ε4 and ε2 exhibit APOE-associated DMN FC changes before Aβ deposition, structural changes, and neurodegeneration.

      • Affective state-dependent changes in the brain functional network in major depressive disorder

        <P>In major depressive disorder (MDD), as a network-level disease, the pathophysiology would be displayed to a wide extent over the brain. Moreover, the network-wide changes could be dependent on the context of affective processing. In this study, we sought affective state-dependent changes of the brain functional network by applying a graph-theoretical approach to functional magnetic resonance imaging data acquired in 13 patients with MDD and 12 healthy controls who were exposed to video clips inducing the negative, neutral or positive affective state. For each affective condition, a group-wise brain functional network was constructed based on partial correlation of mean activity across subjects between brain areas. Network parameters, global and local efficiencies, were measured from the brain functional network. Compared with controls’, patients’ brain functional network shifted to the regular network in the topological architecture, showing decreased global efficiency and increased local efficiency, during negative and neutral affective processing. Further, the shift to the regular network in patients was most evident during negative affective processing. MDD is proposed to provoke widespread changes across the whole brain in an affective state-dependent manner, specifically in the negative affective state.</P>

        3D 프린팅이 디자인 비즈니스에 미치는 영향에 관한 연구

        3D프린팅 기술이 일반화되면서 산업 환경과 소비문화에 많은 변화가 기대되는 가운데 디자인 환경에도 큰 변화가 예상된다. 제조업에서 3D프린팅을 활용한 맞춤형 소량생산 시스템은 소비자들의 개인 취향을 반영한 맞춤형 제품생산을 가능하게 하여 상당한 경제적 파급효과를 만들어 낼 것으로 보인다. 특히 모바일시대를 맞아 언제 어디서나 검색을 통해 스스로 원하는 상품에 접근이 가능해지면서 ‘틈새상품’이 중요해지는 새로운 경제 패러다임, 즉 ‘롱테일 법칙’이 점차 사회적 공감대를 형성하고 있다. 이러한 상황에서 시장수요를 충족시킬 수 있는 생산시스템과 서비스플랫폼 구축은 매우 중요하고, 그에 따라 3D프린팅과 디자인의 연관성을 이해할 필요성이 높아지고 있다. 본 연구는 3D프린팅 기술의 내용과 활용사례 분석을 토대로 이 기술이 향후 디자인 비즈니스에 어떠한 영향을 줄 수 있는지 밝히는데 그 목적이 있다. 연구방법은 문헌연구, 사례분석, 방향 제시로 구성된다. 첫째, 문헌연구를 통해 3D프린팅의 개념, 기술현황 등을 파악하였으며, 이론적 고찰을 통해 3D프린팅이 만들어갈 새로운 맞춤형 소량생산 시스템에 대한 이해를 전개시키고 있다. 둘째 3D 프린팅의 활용사례를 제조업을 중심으로 디자인프로세스, 생산영역, 소비·유통영역으로 구분하여 살펴보았다. 마지막으로 3D프린팅과 디자인의 관계성을 고찰함으로써 새로운 디자인비즈니스의 방향을 예측제시하고 있다. 최근에 3D프린팅을 활용한 복제서비스, 셀프제작소, 3D프린팅 서비스와 같은 맞춤형 서비스가 강화되면서 기존 제조업[생산 → 유통 → 소비]의 생산체계를 [소비 → 생산 → 유통]의 순서로 바뀌어 선(先)주문, 후(後)생산이라는 새로운 비즈니스모델로 전환하는 환경변화에 주목하여야 한다. 향후 3D프린팅 서비스산업은 3D프린터(장비), SNS 서비스(플랫폼), 콘텐츠(디자인)가 융합된 다양한 유통 플랫폼을 개발하여 디자인콘텐츠 유통, 3D프린팅 제작물 유통, 개발 컨설팅에 크게 기여할 것이다. 이에 대비하여 출력할 수 있는 3D디자인 콘텐츠 개발과 시스템 구축, 그리고 관련저작권 보호제도 등 3D프린팅과 디자인융합을 촉진시키는 디자인 생태계를 만들어 다양한 디자인 비즈니스 모델을 활성화시켜야 한다. The technique of 3D printing is being popularized and as a result it is expected that there will be a lot of great changes in the fields of environment and consumption culture. A small quantity production system utilized in manufacturing has made the spelling production of goods meet the taste of the consumers, which will bring large economic benefits. Now is the mobile times, and it is possible whenever and wherever to find through search the wanted products and the new economic paradigm of the crack commodities, i.e. the Long Tail will be popular trend in the society. Therefore it is very important to build production system and service platform meeting the needs of the market. The connection between 3D printing and design is more and more important. The purpose of this study is analysis of the technical contents of the 3D printing and examples of utilization and clarification to how the technique will affect the design business. This study is made of study of reference documents, analysis of utilization examples and orientations. First, through theoretical observations it has been possible to learn the concept of 3D printing and its present situation etc., and through theoretical observation it is possible to realize the new small quantity production system, which will be done by 3D printing. Second, the paper has studied the active examples of 3D printing, which are divided into design process, production, consumption, circulation etc. Finally, the study has tried to find the changes of relation between 3D printing and design and predicted the orientation of the new design business. With regards to recent rapid development of reassembling such as reproduction service, self-manufacturer, 3D printing etc. it is necessary to pay attention to the change of business environment where the existing system of production-circulation consumption is being changed into a new model of consumption-production-circulation. In the future the 3D printing service industry will develop the multiple circulation platform of combination of 3D printer (equipment), SNS service (platform), and contents (design), which will make great contribution to circulation of design contents, circulation of 3D printing products and development consulting. In view of this it is necessary to activate a multiple design business model creating a design environment which will promote combination of 3D printing and design contents with capability of output, construction of system as well as protection of related copyrights.

      • AN EXPLORATIVE STUDY OF INNOVATIVE ENTREPRENEURSHIP IN A HOSPITALITY STARTUP CONTEXT

        Introduction Due to fierce competition in the marketplace, globalization and an explosion of technology in recent years, innovation and differentiation are considered as a necessity for every company (Tajeddini & Trueman, 2008a). At the same time, to achieve market success and sustain a competitive advantage, businesses need to exploit new opportunities, develop new products and/or services and markets (Berthon, McHulbert, & Pitt, 2004) as well as place customer orientation at the heart of the firm’s competitiveness (Deshpande, Farley, & Webster, 1993). Hospitality entrepreneurship is a pivotal factor in the development and growth of many national economies (Hospitality Standards Institute, 2012), and in Taiwan, a significant proportion of hospitality businesses are small owner-operated outlets (). Therefore, understanding the nature of, and challenges faced by hospitality entrepreneurs is an important issue for researchers, as well as current and future entrepreneurs, financial institutions, local authorities, and government. This article reports on an exploratory study of qualitative data collected through open-ended questions, understanding of the many factors influencing entrepreneurs to start their businesses. Literature As noted by Middleton (2001), small businesses form a seedbed for the entrepreneurial and enterprise culture on which much of the profit and employment prospects of big businesses ultimately depend. This observation clearly indicates the difference between small business owners and entrepreneurs. But although support for this viewpoint in the literature is universal, some researchers do not draw such a fine line between the two terms when discussing related issues; also, studies of small business firms are usually found in the entrepreneurship literature, where sometimes the two terms are used interchangeably. The existing entrepreneurship literature shows that several critical factors influence the birth and growth of start-up firms. These factors include both macro-level environmental and micro-level, or personal level, forces. Studies focusing on the former often examine the influence of politics, culture, society, economics, competition, and demographics on a person’s decision to start or grow a new business in a specific setting; this in turn can take place at different levels, such as the company, city, region, or nation (Jogartnam, 2002; Ramos-Rodriguez et al., 2012). Studies on the latter investigate the influence of personal characteristics, such as subculture, social factors (e.g., role, status, reference group), personal factors (e.g., age, gender, education, life cycle, personality, self-perception, lifestyles, values), and psychological factors (e.g., motives, beliefs, attitudes, perceptions, learning, risk-taking propensity, etc. (Littunen, 2000; Ramos-Rodriguez et al., 2012). This study adopted Chen and Elston’s (2013) entrepreneurial activity model for classifying and modeling these factors affecting firm start-ups and growth. Fig. 1 captures this process and highlights the critical factors influencing the entrepreneurial process and their various relationships. In this model, the macro-environment mix influences not only a person’s personal characteristics but also this person’s entrepreneurial process. These personal characteristics in turn also influence the entrepreneurial process. The research methodology was conducted with qualitative research method. Firstly, we interviewed the experts with regard to this industry, and then used content analysis for the interview records. After the in-depth interviews, a grounded theory was adopted to analyze the interview data with repeated coding and reading, and then innovative entrepreneurship were coded into fundamental categories with diverse descriptions (Corbin & Strauss, 1990). These descriptions represent the idea and meaning squeeze from the transcripts of interview. As for the research reliability, we read the biographies of participants and collect relative information such as their provided books, documents and reports from the news. To guarantee the validity of this study, we also improve the research content with researchers’ interview and reflection notes. Those notes were observation on innovative entrepreneurship process in this study. Therefore, these systematic methods could conclude a theory for a specific phenomenon (Hsieh & Shannon, 2005). Results The results of our qualitative analysis demonstrated the opinions of experts with regard to innovative entrepreneurship in hotel industry. Based on Chen and Elston’s (2013) entrepreneurial activity model, the characteristics of each concept were grounded with four dimensions: (1) Macro-environment mix (2) Personal characteristics (3) Entrepreneurial Proces (4) Major entrepreneurial outcome. The experts’ critical viewpoints are also presented by typical quotations. (1) Macro-environment mix ●? Shifting Market “Now the entire consumer market is shifting, everyone booking via the internet.”(sample 1) “When you book a hotel or a flight, people like the price competition. So you may have to spend a lot of time.” (sample 9) ●? The old technology “We are still using the technology more than a decade ago. Taiwan is still running the hotel in a very traditional way. It is when I open a hotel , I am looking for some people, and then ... I think I will make money. In fact, a lot of know how they do not know.”(sample 1) ●? Price competition “We think the outbound market has entered the Red Sea. The inbound market has not been fully developed and has potential for development. Therefore, we would not like to see the inbound market enter the price competition.” (sample 6) ●? In the future, we would integrate them.” (sample 7) Discussion and conclusions This study examined the innovative entrepreneurship with start up in the hotel industry. Using content analysis method to access the research conclusions, the four key components of entrepreneurial activity: Macro-environment mix, personal characteristics, entrepreneurial process and major entrepreneurial outcome (Chen and Elston, 2013) were adopted to set up a theoretical framework. The results of our research were provided through a systematic approach to interpret and summarize experts’ professional opinions. These data were collected by in-depth and semi-structured interviews which give participants opportunities to express their opinions based on their experience in innovative entrepreneurship of hospitality. Meanwhile, the concepts of experts’ comprehensions were written into different characteristics with logical explanations. Most important of all, our research contributes to the entrepreneurship literature. Though some previous studies have discussed Chen and Elston’s (2013) entrepreneurial activity model, none of them tried to integrate this perspective with start up company in hotel industry. Therefore, the results of this study filled the gap between theory and practice, and provide a distinct example for future innovative entrepreneurship research in hospitality. To sum up, as most previous research focus on theoretical or practical perspective to interpret entrepreneurship, the result of this research further contribute the development of innovative entrepreneurship especially in start up. These findings offered essential framework and required knowledge of strategies for current and future trends in this field.

        There is growing evidence that the coronavirus disease 2019 (COVID-19) caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is associated with increased risks of psychiatric sequelae. Depression, anxiety, cognitive impairments, sleep disturbance, and fatigue during and after the acute phase of COVID-19 are prevalent, long-lasting, and exerting negative consequences on well-being and imposing a huge burden on healthcare systems and society. This current review presented timely updates of clinical research findings, particularly focusing on the pathogenetic mechanisms underlying the neuropsychiatric sequelae, and identified potential key targets for developing effective treatment strategies for long COVID. In addition, we introduced the Formosa Long COVID Multicenter Study (FOCuS), which aims to apply the inflammation theory to the pathogenesis and the psychosocial and nutrition treatments of post-COVID depression and anxiety.

        Two-Dimensional ZnO Nanowalls for Gas Sensor and Photoelectrochemical Applications

        Zinc oxide (ZnO) nanowalls were vertically grown on a glass substrate through a fast, low-temperature, catalystfree process in a tube furnace. The morphology and microstructure of ZnO nanowalls were characterized by field-emission scanning electron microscopy, x-ray diffraction analysis, transmission electron microscopy, and photoluminescence measurements. The ZnO nanowall films showed strong UV emission and preferential c-axis orientation, with a hexagonal structure. Potential applications of the ZnO nanowalls were further investigated through experiments. The sensitivity of ZnO-based carbon monoxide gas sensors increased with increasing temperature and reached a maximum value at 300°C. In photoelectrochemical experiments, direct photoelectrolysis of water to generate hydrogen was performed using the ZnO nanowalls as the working photoelectrode. For an external bias of 1.2 V, the photocurrent densities reached 0.037 A/cm2 under Xe lamp illumination.
