        Lens particle glaucoma occurring 15 years after cataract surgery

        A 60-year-old man visited our clinic with a sudden blurred vision and ocular pain in his right eye occurring 15 years after cataract surgery. The intraocular pressure (IOP) was 55 mmHg in the right eye and gonioscopy revealed a wide open angle with white cortical lens material in the inferior angle. Since the IOP was unable to be controlled with medical therapy, removal of the lens material was performed by irrigation and aspiration. Following surgery, the IOP was decreased to 18 mmHg without medication and the patient’s vision recovered to 20/20. The pathology of the aqueous humor showed macrophages with engulfed lens particles.

        <P>Parry-Romberg syndrome, or progressive facial hemiatrophy, is a unilateral, slowly progressive atrophy that affects skin, subcutaneous tissues, muscles, and bones. The authors describe an 8-year-old girl with Parry-Romberg syndrome, amblyopia, recurrent exotropia, and torticollis, and stress that the association between this syndrome and amblyopia, recurrent exotropia, and torticollis should be recognized.</P>

      • Myasthenia Mimicking Monocular Elevation Deficiency

        <P>Monocular elevation deficiency is the inability to elevate the eye in abduction and adduction. Myasthenia gravis could mimic any pattern of ophthalmoplegia. However, myasthenia gravis mimicking monocular elevation deficiency appears to be very rare. A 12-month-old girl presented with the inability to elevate the left eye and blepharoptosis from 3 months of age. A neostigmine test showed a weakly positive response. Repetitive stimulation of the left orbicularis oculi produced no decremental response. Three assays for acetylcholine receptor antibodies were negative. Placing ice over the lids definitely improved left blepharoptosis. In conclusion, myasthenia gravis could mimic elevation deficiency. An ice test could be very helpful in making the diagnosis. Myasthenia gravis should be included in the differential diagnosis of monocular elevation deficiency even at a young age.</P>

        근시가 험프리 매트릭스 30-2 검사에 미치는 영향

        `Purpose: To investigate the effect of myopic optical defocus on Humphrey Matrix 30-2 test. Methods: Twenty-nine myopic eyes of 29 patients underwent 2 consecutive tests with the Humphrey Matrix 30-2 threshold program. The first and second tests were performed with and without correction for myopia. Differences between the mean deviation (?MD) and the pattern standard deviation (?PSD) were calculated, and a correlation between the spherical equivalent (SE) and ?MD and ?PSD was investigated. The influence of optical defocus according to the visual field (central and peripheral) and the severity of glaucomatous visual field damage (area with total deviation plot of ‘P<0.1%’ or ‘P<0.5%’ and the other area) were also evaluated. Results: The correlation between SE and ?MD was significant (p=0.02, r=0.62). However, the correlation between SE and ?PSD was not significant (p=0.81, r=0.15). The differences in the mean total deviation printout value of the central and peripheral visual field groups between the two tests were 5.85±4.26dB and 5.66±3.56dB, respectively (P=0.86). The differences in the mean total deviation printout value of severely damaged and less damaged areas between the two tests were 3.16±3.39dB and 6.62±4.62dB, respectively (P<0.01). Conclusions: Myopic optical defocus has a significant effect on the results of the Humphrey Matrix 30-2 test. There is no difference in the effect of myopia between the central and peripheral visual fields, and the effect is decreased in severely damaged visual fields.`

        단안성 시야장애를 보이는 정상안압 녹내장에서의 개인내 양안간 비교

        `Purpose: To investigate whether there are differences in ocular risk factors reportedly associated with the development of normal tension glaucoma (NTG) between glaucomatous eyes and normal visual field (VF) eyes of monocular NTG patients. Methods: Thirty-six patients who had NTG in one eye and normal RNFL and VF in the fellow eye were included in this hospital-based cross-sectional retrospective survey. We analyzed the difference between glaucomatous eyes and normal VF eyes according to an intra-individual, inter-ocular comparison of ocular parameters, including intraocular pressures (IOP), central corneal thickness (CCT), optic disc size, myopia, optic disc hemorrhage, and zone β of peripapillary atrophy. Measurements of optic disc size and area and the angular and radial extent of zone β were obtained using a Heidelberg Retina Tomograph. We also evaluated the correlation of each parameter with mean deviation (MD) and pattern standard deviation (PSD). Results: There were no inter-ocular differences of IOP, CCT, optic disc size, or myopia (n=36). Optic disc hemorrhage was found in only three glaucomatous eyes. The area of zone β was greater in glaucomatous eyes than in normal VF eyes (p=0.003). No parameters were correlated with MD or PSD. Conclusions: Except for optic disc hemorrhage and zone β area, all other parameters showed no inter-ocular differences. The asymmetry of disc hemorrhage and zone β area in monocular NTG patients implies that optic disc ischemia might play a role in the pathogenesis of NTG.`

        A Newly Improved Technique for Transscleral Fixation of a Foldable Posterior Chamber Intraocular Lens

        Purpose: The authors present a newly improved technique for scleral fixation of a foldable posterior chamber intraocular lens. Methods: Intraocular lenses (IOLs) were fixed into sclrera in three patients. The both Prolene needles entered into the external sclera beneath flap 1.5 mm away from the limbus, passing through the pupil, and out through the peripheral clear cornea (ab externo approach) Two loops of sutures were externalized through the clear corneal incision, and anchored to the both haptics. IOL were folded and then inserted. Results: All three patients showed stable position of IOL to final follow-up. Best corrected visual acuities were more than 20/40 in all patients. Conclusions: This approach is easily acquired even by the inexperienced surgeon, reduces operative time and places the lens more precisely in the sulcus. This procedure is considered promising for postoperative early visual rehabilitation.

        한국인에서의 연령 및 성별에 따른 안압-단면적 연구

        `Purpose: To assess the effect of age and gender on intraocular pressure (IOP) in a large Korean population. Methods: This cross-sectional study included 102,218 healthy Koreans who were aged between 20 and 79 and had no preexisting ocular conditions that could affect the IOP. All the subjects had undergone a physical check up between 1996 and 2005, and their medical records were reviewed retrospectively. Anthropometric measurements, blood tests, noncontact tonometry, and fundus examination were performed on all participants. Subjects were grouped according to decade of age. For all subjects and each age group, age and systemic variables were analyzed by multiple regression analysis on their relationship with IOP. Results: A significant trend of decreasing IOP was observed in the 40s, 50s, and 60s in men, while a significant trend of increasing IOP was found in the 50s, 60s, and 70s in women. Multiple regression analysis revealed different IOP trends with age between age groups in both men and women. In general, the IOP had a significant positive correlation with systolic blood pressure, body-mass index (BMI), hematocrit, and serum cholesterol, especially with BMI in men and hematocrit in women. Conclusions: In a multiple regression analysis, the IOP trend in each age group was quite different from each other in a large Korean population, and it was suggested that women may have a steeper increasing slope (or less steep decreasing slope) of IOP with age than men. Further investigations with longitudinal study would be required to clarify the age- and gender-related physiologic changes of IOP.`

      • Expression-associated polymorphisms of <i>CAV1</i> - <i>CAV2</i> affect intraocular pressure and high-tension glaucoma risk

        <P><B>Purpose</B></P><P>The human <I>CAV1</I>-<I>CAV2</I> locus has been associated with susceptibility to primary open-angle glaucoma in four studies of Caucasian, Chinese, and Pakistani populations, although not in several other studies of non-Korean populations. In this study with Korean participants, the <I>CAV1</I>-<I>CAV2</I> locus was investigated for associations with susceptibility to primary open-angle glaucoma accompanied by elevated intraocular pressure (IOP), namely, high-tension glaucoma (HTG), as well as with IOP elevation, which is a strong risk factor for glaucoma.</P><P><B>Methods</B></P><P>Two single nucleotide polymorphisms (SNPs) were genotyped in 1,161 Korean participants including 229 patients with HTG and 932 healthy controls and statistically examined for association with HTG susceptibility and IOP. One SNP was rs4236601 G>A, which had been reported in the original study, and the other SNP was rs17588172 T>G, which was perfectly correlated (<I>r</I><SUP>2</SUP>=1) with another reported SNP rs1052990. Expression quantitative trait loci (eQTL) analysis was performed using GENe Expression VARiation (Genevar) data.</P><P><B>Results</B></P><P>Both SNPs were associated with HTG susceptibility, but the rs4236601 association disappeared when adjusted for the rs17588172 genotype and not vice versa. The minor allele G of rs17588172 was associated significantly with 1.5-fold increased susceptibility to HTG (p=0.0069) and marginally with IOP elevation (p=0.043) versus the major allele T. This minor allele was also associated with decreased <I>CAV1</I> and <I>CAV2</I> mRNA in skin and adipose according to the Genevar eQTL analysis.</P><P><B>Conclusions</B></P><P>The minor allele G of rs17588172 in the <I>CAV1</I>-<I>CAV2</I> locus is associated with decreased expression of <I>CAV1</I> and <I>CAV2</I> in some tissues, marginally with IOP elevation, and consequently with increased susceptibility to HTG.</P>
