An experimental study has been performed on thermal transport from an aluminum foam heat sink under a confined impinging air jet. Three kinds of aluminum foam heat sinks with 10, 20 and 40 PPI and a conventional pin-fin heat sink are tested in the present study. The jet Reynolds number is varied in the range of Re=667~5672 The effect of the confinement disk diameter and the distance between the confinement disk and the heater surface on the averaged Nusselt number is investigated in detail. The results are also compared with those of the unconfined impinging air jet. The critical distance, at which thermal performance shows a minimum compared to the unconfined jet impinging, will be described in terms of the Reynolds number and the pore density of the aluminum foam.

저자들은 위암 2기(T2N1M0)로 근치적 수술을 시행 받은 후 추적 관찰 중 5개월 만에 높은 AFP수치(227,325 IU/mL)와 다수의 간 내 다발성 종괴 및 복강내 종양소견이 관찰되어 시행한 간 조직 검사와 면역조직화학염색을 통해 간세포양 샘암종으로 진단된 증례를 경험하였기에 보고하는 바이다. Hepatoid adenocarcinoma is a rare variant of adenocarcinoma that can occur in the stomach. This neoplasm has a poor prognosis and it frequently presents at an advanced stage of disease; it is clinically characterized by increased serum levels of alpha-fetoprotein (AFP) in most cases. We experienced a case of hepatoid adenocarcinoma of the stomach with multiple liver metastases that mimicked germ cell tumor in a 43-year-old man. He had incidentally detected gastric adenocarcinoma via gastrofibroscopy and he underwent subtotal gastrectomy. The postoperative pathologic stage was II (T2N1M0). He complained of epigastric pain. The serum AFP was markedly increased (227,325 IU/mL) and abdominal CT revealed multiple liver masses. We considered these as extragonadal germ cell tumors that occurred in the liver because of the markedly elevated AFP, the short duration of follow-up and early stage of stomach cancer at the that time. To confirm the diagnosis, biopsy of liver mass was done and he was diagnosed with hepatoid adenocarcinoma.(Korean J Med 73:107-111, 2007)
말기 암 환자의 생존기간 예측: 혈액학 검사와 PPI에 대한 전향적 연구

Background/Aims: The palliative prognostic index (PPI) was designed to predict life expectancy based on clinical symptoms. In this study, a PPI was constructed and used with other biological parameters to predict 3-week survival in patients with advanced cancer. Methods: The study included 222 patients. The PPI was constructed with five variables (performance status, oral intake, edema, dyspnea at rest, and delirium). PPI scores were grouped as follows: 4 (group 1); > 4 and ≤ 6 (group 2); and > 6 (group 3). At admission, seven biological variables (white blood cell count, lymphocyte, C-reactive protein [CRP], bilirubin, albumin, creatinine, and lactate dehydrogenase) were measured. Results: The overall survival duration was 50 days in group 1, 22 days in group 2, and 14 days in groups 3. Using the PPI, a survival of < 3 weeks in group 3 was predicted with a sensitivity of 76.5% and a specificity of 65.4%. The important factors significantly affecting the 3-week survival rate were a PPI score > 6 and increases in serum bilirubin and CRP levels. Furthermore, the 3-week survival rate in patients with hepatopancreatobiliary cancer was more accurately predicted using a combination of the PPI, CRP, and serum bilirubin levels. Conclusions: Although a PPI has limitations, it can be quickly applied to determine survival duration in patients admitted to hospice and accurately predicts 3-week survival. Furthermore, bilirubin and CRP are useful factors for predicting 3-week survival in patients with gastrointestinal cancer, including hepatopancreatobiliary cancer. (Korean J Med 2011;81:359-365)

This paper focused on the indoor thermal comfort improvement by the wind free floor-standing type air conditioner. Evaluate the indoor thermal comfort and air quality to meet the growing demands of the occupants, a circulator was applied to the air conditioner. There are various parameters which are influenced thermal comfort, Especially, Air flow is the most essential parameter to achieve comfort condition. This study presents the thermal comfort improvement method by using a circulator to the air conditioner. Air flow rate increased 20% higher by the circulator. Circulator fan spread cold air wide and achieve comfort condition fast. When the occupant zone reached to the comfort condition air flow can be changed to the wind free mode to reduce cold draft. To investigate the effect of the circulator fan, we measured the air flow velocity, temperature distribution and compared to general cooling mode.

This study presents an improvement method to reduce cold draft using hybrid flow path in the inverter driven wall mount split type residential air conditioner. To investigate the effect of the hybrid flow path, we measured the air flow velocity, temperature distribution and PMV, PPD under wind-free mode and compared to general cooling mode. Under wind-free mode, occupant zone was formed in still air condition(under 0.15m/s) and maintained uniform temperature distribution. Therefore, PMV and PPD were improved to meet the ASHRAE comfort zone

Long and indirect air flow have been required for 4-way ceiling type cassette. In this paper, We show how to make long and comfort wind free air flow considering cold draft by using hybrid flow for 4-way ceiling type indoor units. To investigate the effect of the hybrid flow path, we measured the air flow velocity, distance, temperature distribution and PMV, PPD. The effect of the hybrid air flow path air flow distance was two times further and under wind-free mode, occupant zone was formed in still air condition(under 0.15m/s) and maintained uniform temperature distribution. 운동거리이상(dysmetria)은 운동경로, 속도 및 힘에 대한 조절 능력이 감소되는 것으로서 운동 기능에 심각한 지장을 초래할 수 있다. 본 논문에서는 운동거리이상을 과학적으로 계측하고 분석함으로써 환자의 운동 조절능력을 정량적으로 평가할 수 있는 도구를 개발하였다. 이를 위하여, 터치 스크린상에 그려진 수직 및 수평선이 모든 위치좌표를 추출하기 위한 거북이 알고리즘을 제안하였으며, 이를 이용하여 운동 경로와 운동성을 분석함으로써 상지의 운동실조를 정량적으로 측정할 수 있는 알고리즘을 제안하고 구현하였다.
