        한.양방 입원 환자의 비정상 간기능 검사 발생률 비교

        Herbal hepatotoxicity is a one of the most often encountered controversial issues in medical society. Some believe herbs are totally safe while others regard treatment with herbs as a very common cause of hepatotoxicity. But there are no comparative clinical hepatotoxicity studies between herbal and conventional medicines. We investigated incidence of patients with abnormal liver function who were treated with herbal or conventional medicine at Dongguk University International Hospital. Each group consisted of 300 patients, who were admitted for longer than 30 days during the year 2008. We compared albumin, total bilirubin, AST (aspartate aminotransferase), and ALT (alanine aminotransferase) levels, and excludes patients who had abnormal liver function test or liver-related disease when admitted. As a result, incidence of total bilirubin, AST, and ALT elevation in the CT (conventional treatment) group was more frequent than in the TKM (traditional Korean medicine) group (p<0.05) during admission. Albumin level also decreased more frequently in the CT group than in the TKM group (p<0.05). According to these results, herbal medicine is not more hepatotoxic than conventional medicine, and not a main cause of liver injury in inpatients.

        청간해주탕(淸肝解酒湯)이 kupffer cell의 $NF{\kappa}B$ 활성화 및 세포사멸에 미치는 영향

        Objectives : Previous studies showed that treatment with Chungganhaeju-tang prevents hepatic inflammation and apoptosis in alcoholic liver disease. The purpose of our study is to determine if any relations exsists between the transcription factor $NF{\kappa}B$, an orchestrating expression of a large number of genes and inhibitory effects of Chungganhaeju-tang on ethanol induced apoptosis. Materials and Methods : To assess the role of $NF{\kappa}B$, we blocked NFkB activation by delivering to the kupffer cells $I{\kappa}B{\Delta}N$, a dominant negative $NF{\kappa}B$ inhibitor, and investigated if Chungganhaeju-tang still prevented apoptosis. Results : When $NF{\kappa}B$ activation was blocked, there was no inhibitory effect of Chungganhaeju-tang on ethanol induced apoptosis of kupffer cells. Conclusion : This result suggests that Chungganhaeju-tang protects the liver from ethanol induced apoptosis by activating the $NF{\kappa}B$ that plays a key role in porotecting mechanism and reducing inflammatory cytokine gene expression.

        Entecavir와 한의약 치료를 병행한 후 HBV-DNA가 음전된 만성 B형 간염 환자 증례 보고

        The patient presented to the clinic with the complaint of severe fatigue. The patient had been diagnosed with chronic hepatitis B a number of years earlier. Although he had used entecavir, an antiviral agent, for over two years, his HBV-DNA level had not dropped below undetectable levels. The fatigue seemed to be associated with chronic hepatitis B. Traditional Korean medicine (TKM) therapy for chronic hepatitis B was administered in conjunction with entecavir and at the same dose. The excessive fatigue gradually decreased following the treatment. On the 28th day, laboratory tests revealed that the patient's bilirubin level was slightly lower and that his HBV-DNA level had dropped below undetectable levels. The addition of TKM therapy may have contributed to the HBV-DNA clearance. No similar cases have been reported in Korea. Herein, we summarize the patient's progress.

        비알코올성 지방간 세포 모델에 대한 택사, 산사, 구기자, 울금, 단삼, 인진의 효능 비교

        Objectives : We try to compared the efficacy of six herbal medicines, Rhizoma Alismatis (RA), Fructus Crataegi (FC), Fructus Lycii (FL), Radix Curcumae (RC), Radix Salviae Miltiorrhizae (RSM), and Herba Artemisiae Scopariae (HAS), constituting KHchunggan-tang which was previously proven to be hepatoprotective on non-alcoholic fatty liver disease with combined properties of cellular steatosis, ROS production, and cytoprotection. Methods : HepG2 cells were pretreated with aqueous extracts of the six herb medicines at concentrations of 1, 10, 50 and 100 ${\mu}g/ml$ each, and treated with 0.5 mM palmitate consecutively. After 21 hrs, cell viability was assessed using MTT assay, and the percentage of cells with sub-G1 DNA content was measured using fluorescence-activated cell sorting after propidium iodide staining. Results : The first three extracts, RA, FC, and FL restored cell viability reduced by palmitate in MTT assay, and RA, FC, FL and RC inhibited palmitate-induced apoptosis in sub-G1 analysis. FL showed relatively weak potential only at tested maximal dose, and RA showed the greatest higher efficacy on this experimental cellular model of nonalcoholic fatty liver disease. Conclusions : According to this comparative experiment, Rhizoma Alismatis seems to have the most powerful potential among the six herbs constituting KHchunggan-tang, and consecutive further study seems to be required for more standardized and effective clinical application of KHchunggan-tang for treatment of non-alcoholic fatty liver disease.

        알코올 의존 제I형 및 II형 환자에서 검지-약지 길이 비의 차이

        Objectives The second to fourth digit ratio (2D : 4D) is known to reflect testosterone level during intrauterine period. Testosterone is essential for masculinization of brain. Cloninger's type II alcoholism is characterized by early age at onset and largely limited to males. Compared with type I alcoholism, type II alcoholism is thought to be masculine. There is a possibility that the digit ratio of type II alcoholism is low compared with type I. We examined digit length ratio in Cloninger's type I and II alcoholism to understand the effects of intrauterine testosterone on the development of alcoholism. Methods The participants were 87 alcohol dependent patients. We divided two subtypes of patients by Irwin's symptom list, based on Cloninger's concept. We captured the images of both palms by the scanner and measured the lengths of digits by the graphic program. Then we analyzed the digit ratios by independent-sample t-test. Results The mean 2D : 4D of type I was 0.942 (right), 0.952 (left). The mean 2D : 4D of type II was 0.927 (right), 0.940 (left). In both hands, 2D : 4D of type II was significantly lower than type I. Conclusions Type II alcoholism showed masculine type of digit length ratio compared with type I. It suggests the exposure of testosterone during intrauterine period might play an important role in determining the course and feature of alcoholism.

        하수오를 복용한 환자의 간기능 검사에 대한 후향적 조사

        Objectives : in recent years, there have been some case reports about liver injuries suspected to be caused by Heshouwu, the root of Polygonum multiflorum. So, I investigated liver function tests of patients who had taken Heshouwu, to know whether Heshouwu was hepatotoxic. Methods : I surveyed whether Heshouwu caused liver injury in patients, who had been admitted and taken Heshouwu at Dongguk University International Hospital, from May 2005 to July 2008. I looked over all the liver function tests of those patients, done during medication and 30 days after. Results : Among 1,449 inpatients, there were 124 patients who had taken Heshouwu. Among those 124 patients, 14 cases whose liver function tests were abnormal before medication were excluded. 69 cases who did not take liver function test 30 days after were also excluded. As a result, 41 patients were enrolled, and their mean dosage was 14.04(8-24) g/day, and the mean medication period was 13.8 (1-89) days. During and after medication, there were no abnormal liver function test results at all, in the enrolled cases. Conclusions : Roughly speaking, according to the above results, it seems that taking Heshouwu for about two weeks as per the commonly taken dosage did not cause hepatotoxicity, though we needs more laboratory and clinical research to reach a more definitive conclusion.

      • IGBT 모듈의 냉각성능 향상을 위한 전력변환장치의 유로 설계

        In the thermal design stage, a design engineer of the power conversion system (PCS) which is used in wind power, photovoltaic, and energy storage system should evaluate whether the design criterion of the junction temperature in a power semiconductor device such as an insulated-gate bipolar transistor (IGBT) is satisfied or not. Here, an engineer designs the airflow passage structure of the PCS in order to exhaust effectively the heat that is generated by electronic components in the PCS. In this paper, it was compared with different airflow passage structures of the PCS using the numerical method to thermal performance indexes that are the airflow rate of the heat sink attached at the IGBT module and the junction temperature of the IGBT module. In order to satisfy the design criterion of the junction temperature of 130℃, the structure of the airflow passage is that the appearance and the dimension of the prototype is maintained in the same manner as the design 2, and the area of supply side louvers is maintained at least 0.3 ㎡ under the rated load condition considering the numerical error of 5 percent.

        자동차용(自動車用) Door guide rail 표면의 마찰마모 특성에 관한 연구

        Door guide rail is the core part which guides window when it moves up and down. But when the method of grease lubrication in the core has been used for a long times, its lubrication performance has been weakened. Therefore the method of the solid lubrication such as teflon(PTFE) coating in the rail has been studied to be increased the performance in these days. In this study the friction wear characteristic the surface with teflon coating of door guide rail for automobile has been researched. In 1,20 Hz frequency friction wear tests the friction coefficient are below 0.188 and 0.213. In indicate test the wear depth is very slight. Therefore the durability to the friction wear in the surface with teflon coating of door guide rail is good. In addition to, the result of this study can make efficient use to a basic study to develop method of test evaluation to door guide rail surface grade. Especially this study will contribute to improve the quality of automobile parts.

        This study is to estimate the structural stability of the machine which is carried to assemble and test a gearshift lever in the automobile at the same time. Because maximum von-Mises stress of 21.6 ㎫ is smaller than its yield stress of 250 ㎫ and the maximum deformation of 0.033 ㎜ is quite small, it is stable statically. And the analysis result of dynamic characteristics shows that the maximum von-Mises stress of 210.8 ㎫ is smaller than its yield stress 250 ㎫. The natural frequencies is not coincided with the driving speed. Therefore it is stable dynamically.

      • 강의실에서 환기시스템과 난방시스템의 풍량에 따른 PMV와 CO₂ 농도 특성 비교

        In this paper, we performed the experimental and numerical study on the thermal comfort(TC) and indoor air quality(IAQ) in the lecture room with a different airflow rate of the ventilation system and heating system for heating season. Through the experimental results, we found out that there was considerably difference of the PMV but there was little difference of CO₂ concentration with a different heating system. From a numerical results, the best operating condition was that discharge airflow rate of SAC is 29 CMM and supply airflow rate of the ventilation system is 1,200 CMH from a viewpoint of TC and IAQ.
