        Bibliographical Characteristics of North Korean Medical Journals and Articles

        The aim of our work was to analyze the bibliographical characteristics of 9 North Korean medical journals and articles. All journals show only ten pieces of information, such as publication dates and table of contents, and do not provide submission guidelines and journal policy. Most of the journals were published four times a year. Editorials often share government policy. Research articles are very short and compressed. The journals often publish articles on oriental medicine and medical information useful to the public. In conclusion, the journals were published in their own unique style and format, which are far from the global standard.

        Frequently covered diseases in North Korean internal medicine journal Internal Medicine [Naegwa]—Secondary publication

        Purpose This study aimed to investigate the distribution of research fields and diseases in the North Korean internal medicine journal by using a content analysis and a frequency analysis method. Methods All 2,092 articles published in Internal Medicine [Naegwa], a North Korean medical journal, from the first issue of 2006 to the last of 2015, were searched and classified by subspecialty of internal medicines, diseases, and classification codes of the Korean Standard Classification of Diseases version 6. Results In total, 1,392 out of the 2,092 articles were classified into the internal medicine field, with the remaining 700 classified as basic medicine, family medicine, or anesthesiology. Among the articles on internal medicine, most concerned the digestive system, followed by circulatory, respiratory, renal, and endocrine systems. Among the 700 articles in other fields, articles on gynecology were most common. According to the analysis of diseases, the most commonly studied in internal medicine were gastrointestinal diseases, hypertension, respiratory infectious diseases, glomerular diseases, and diabetes. Meanwhile, cerebrovascular diseases, herpes zoster, mental and behavioral disorders, and urinary tumors were most covered in the other fields. In the distribution by classification code of the Korean Standard Classification of Diseases version 6, circulatory and digestive diseases accounted for 42.4% of articles. Conclusion The results of this study are expected to be exploited to estimate the disease distribution and disease burden in North Korea.

        Underestimated Burden: Non-Communicable Diseases in North Korea

        There remains a misperception that non-communicable diseases (NCDs) are predominantly an issue for developed countries andare not major public health problems in low-income countries. North Korea is also often considered as a country with a diseasestructure that is typical of a low-income country. Infectious diseases and nutritional problems, including tuberculosis and pneumonia,are still major sources of medical discussion. The authors challenge this misperception by reviewing empirical data on epidemiologicand demographic transitions of North Korea and show that the current NCD burden is the main public health issue inNorth Korea. In result, it can be said that epidemiologic transition and demographic transition of North Korea preceded prior toeconomic hardship. It is necessary for the international community, including South Korea, to advance in a new direction of medicalsupport for North Korea.

        북한 내과 학술지 논문에서 다루어진 질환들의 분포

        The purpose of this study was to investigate the distribution of research fields and diseases in internal medicine by using contents analysis and frequency analysis with 2,092 articles of ‘Internal Medicine’ from volume 1 of 2006 to volume 3 of 2015. We extracted three key information items (internal medicine divisions, disease name and major classification code of the Korean Standard Classification of Diseases version 6) and three other information items (bibliographic information, type of research and presence or absence of Korean oriental medicine) from the articles. After cross-reviewing the extracted items, data was created for analysis. 1,392 out of 2,092 articles were classified into internal medicine divisions and, the remaining 700 were classified into other medical divisions such as basic medicine, family medicine and anesthesiology. In other medical divisions, neurology was the most common. The major diseases studied in divisions of internal medicine were gastroduodenal diseases, hypertension, respiratory infections, glomerular diseases and diabetes. In other medical divisions, cerebrovascular disease, shingles, mental and behavioral disorders and urological tumors have been studied extensively. Classification of the Korean Standard Classification of Diseases version 6 resulted in 42.4% of circulatory system diseases and digestive system diseases. Through this study, we observed the distribution of research field in North Korean internal medicine. The results of this study are expected to be used to estimate disease distribution and disease burden in North Korea. 본 연구의 목적은 북한의학학술지 ‘내과’ 2006년 1호부터 2015년 3호까지 게재된 전체 논문 2,092편을 대상으로 내용분석과 빈도분석 방법을 활용하여 내과에서 다루어진 연구 분야와 질환들의 분포를 분석하는 것이다. 연구자들은 논문들에서 핵심 정보 3개 항목들-내과 분과명, 질환명, 제6차 한국표준질병․사인분류표의 대분류 코드-과 기타 정보 3개 항목들-서지정보, 연구종류, 고려의학여부-을 추출하여 교차 검토 후 자료화 하였다. 내용적으로 내과 분과에 해당하는 논문은 2,092건 중 1,392건이었고 기초의학, 가정의학과 마취통증의학을 포함, 기타과로 분류할 수 있는 논문은 700건이었다. 내과 분과에 속한 논문들은 소화기, 순환기, 호흡기, 신장, 내분비 순으로 많았고 기타과에서는 신경과가 가장 많았다. 질환별 분석에서 내과 분과에서는 위십이지장질환, 고혈압, 호흡기감염성질환, 사구체질환과 당뇨병이 많이 연구 되었고 기타과에서는 뇌혈관 질환, 대상포진, 정신 및 행동 장애와 비뇨기 종양이 많이 연구되었다. 제6차 한국표준질병․사인분류표 대분류 코드별 분포에서는 순환계통의 질환과 소화계통의 질환이 42.4%를 차지하였다. 본 연구를 통해서 북한 ‘내과‘ 학술지 연구 분야 분포를 확인해 볼 수 있었다. 본 연구 결과는 북한의 질병 분포와 질병 부담정도를 추정하는데 활용될 수 있을 것으로 기대한다.

        The Status and Distinct Characteristics of Endocrine Diseases in North Korean Articles Published between 2006 and 2015

        Background: Past decades of division have led to substantial differences in medical environments between South and North Korea. However, little is known about North Korea’s medical status and research field, especially regarding endocrinology. In this study, we report the characteristics of North Korea’s articles regarding endocrine-related diseases. Methods: Among the nine medical journals, articles published in Internal Medicine between 2006 and 2015 were reviewed. A total of 2,092 articles were included; among them, 96 articles were associated with endocrinology. We analyzed these articles according to the disease categories they focused on and evaluated their features. Results: Articles related to diabetes mellitus accounted for 55.2% (n=53) and those to thyroid disease accounted for 28.1% (n=27). Other disease categories, including adrenal gland (n=1), pituitary gland (n=1), and osteoporosis (n=3), comprised minor portions. Regarding diabetes mellitus, more than half the articles (n=33) focused on treatment and complications. Experimental studies were conducted with old hypoglycemic drugs or natural substances for the treatment of hyperglycemia. Regarding thyroid disease, articles related to hyperthyroidism were the most common (51.9%, n=14), followed by thyroid nodule/cancer (18.5%, n=5). Unique article features were short length, no figures, and less than five references. Conclusion: North Korea’s endocrinology articles mainly focused on diabetes mellitus and thyroid disease. Persistent studies have been carried out in North Korea with dedication despite the poor medical environment. We hope that this study will be the beginning of mutual medical exchange and collaboration between North and South Korea.
